사회적 배제와 중국 농촌 노년인구 자살률의 관계 연구

국문 초록 (Abstract)

최근 중국사회에서 자살은 가장 큰 사회문제로 꼽을 수 있다. 본 연구는 이러한 자살의 심각성을 감안하여 그동안 노인자살행위에 대한 연구 분야를 보완하고자, 1990년부터 2009년까지 농촌 노년인구의 자살률에 영향을 미치는 사회적 배제요인을 분석하는 데 목적을 두었다. 본 연구는 중국 공공부서에서 매년발표된 데이터를 이용하여 성별과 연령에 따라 자살률을 표준화하였고, 사회적배제의 하위개념인 국가재정에서 민정사업지출이 차지하는 비율, 국가위생총지출에서 정부위생지출이 차지하는 비율 그리고 천 명당 향진 위생원의 침대 수를 계산하였다. 연구결과로는 첫째, 연구기간 동안 국가재정에서 민정사업지출이 차지하는 비율은 농촌 노년인구의 자살률에 부(-)적 영향력을 미쳤다. 이러한 영향력은 농촌 중고령 여성노인에게 가장 강한 예측력이 지니는 것으로 나타났다. 둘째, 국가위생총지출에서 정부위생지출이 차지하는 비율은 농촌 중고령 여성노인의 자살률에만 부(-)적 영향력을 미친 것으로 나타났다. 셋째, 천명당 향진 위생원의 침대 수가 역시 농촌 중고령 여성노인의 자살률에 가장 강한 예측력이 지니는 것으로 나타났다. 본 연구를 통해 공공서비스와 공공복지의 결핍을 비롯한 사회적 배제가 중국 농촌 노년인구의 자살률에 중대한 영향을 끼침을 확인하였다. 그래서 향후 중국에서 자살예방대책을 마련하는데 있어 사회적·의료적 보장 수준을 보다 강화하고 자살사망자의 인구학적 특징을 충분히 고려하여, 특히 농촌 중고령 여성노인을 비롯한 취약집단에게 더 많은 관심과 배려를 기울려야 한다고 판단된다.

다국어 초록 (Multilingual Abstract)

Suicide has been a big problem in recent Chinese society. In this study we try to investigate the effect of sub-concepts of social exclusion on rural senior citizen`s suicide rates over the time period 1990-2009. In this study official data of suicide rates and sub-concepts of social exclusion indexes during 1990-2009 of China were standardized and calculated. The suicide data were categorized by age and gender, and the sub-concepts of social exclusion include the proportion of civil affairs in the state financial expenditure, the proportion of government health expenditure in total expenditure on national health, number of beds in township hospitals(per thousand). The results of this study showed that proportion of civil affairs in the state financial expenditure is a strong predictor of suicide rates for both genders. And the effect is strongest on middle-old female group than other groups. The proportion of government health expenditure in total expenditure on national health and number of beds in township hospitals(per thousand) also have the strongest effect on suicide rates of female middle-old group. Exclusion from social welfare system has strong negative effect on suicide rates for rural elderly. Higher health care and social expenditure appear to have suicide-preventive effects. It is important to improve preventive programs for the most vulnerable group, especially the middle-old female group.
