부모의 직업과 선천성 기형, 사산, 저체중, 초생아 사망 등 불량한 임신결과와의 관련성을 알아 보기 위해 환자-대조군 연구를 시도하였다. 연구대상은 2년간(1991년 4월 1일 부터 1993년 3월 31일까지) 대구시내 5대 병원에서 대구시내에 거주하는 산모로부터 출산된 선천성 기형아, 사산아, 심한 저체중아(1,500mg 미만)와 초생아 사망 중 전화면담이 가능했던 422명을 환자군으로 하고, 환자의 출생병원에서 환자 출생일에 가장 가까운 날에 출생한 신생아로서 산모의 연령, 교육수준, 출산순위 그리고 신생아의 성이 같고 출생시 체중이 2,500mg 이상인 정상아를 1:1로 짝을 지어 대조군을 선정한 총 422쌍, 844명이었다. 인적 사항을 비롯한 기본 자료는 산모의 의무기록지에서 얻었고 전화면담을 통해 부모의 직업력, 음주 및 흡연력 등 필요한 자료를 수집하였다. 분석은 EGRET program을 이용하여 conditional logistic regression으로 직업별 교차비(odds ratio: OR)를 구하고, 임신 중 합병증 및 산모의 만성질환을 조정하여 조정교차비를 구하였다.
선천성 기형, 사산, 저체중, 초생아 사망의 임신결과를 합해서 환자군으로 하여, 사무직을 기준으로 한 임신전 1년 동안의 남편의 직업별 교차비를 구한 결과 플라스틱 및 합성물 제조업(OR=13.69, 95% confidence interval(CI) 1.62-115.50), 건설공사 종사자(OR=3.16, 95% CI 1.51-6.59), 염색업(OR=3.15, 95% CI 1.20-8.30), 금속공업(OR=2.73, 95% CI 1.50-4.98)이 5% 유의수준에 도달하였다. 임신중 합병증 및 만성질환을 조정한 교차비는 플라스틱 및 합성물 제조업(OR=55.78, 95% CI 1.82- 1700), 염색업(OR=4.53, 95% CI 1.39-14.78), 자동차 및 기계정비업(OR=3.18, 95% CI 1.02-9.91), 금속공업(OR=2.49, 95% CI 1.25-4.98)이 높았다.
산모의 임신 전 1년 이전의 직업에 대해서 선천성 기형, 사산, 출생시 저체중, 초생아 사망을 합해서 환자군으로 하여 가정주부를 기준으로 직업별 교차비를 구한 결과 염색업 4.64(95% CI 0.54-39.74), 서비스업 2.38(95% CI 0.82-6.91), 전자,전기 및 전산업 1.47(95% CI 0.42-5.21)로 높았으나 5% 유의수준에는 미달하였다. 임신중 합병증 및 만성질환에 대한 조정 후에도 염색업 13.05(95% CI 0.31-548.60). 서비스업1.66(95% CI 0.39- 6.93)으로 1보다 큰 교차비를 나타냈으나 5% 유의수준에 도달하지는 못하였다.

To examine the association of parental occupation and poor reproductive outcomes, a case-control study was conducted on 422 matched pairs. Cases were selected from the women who were residents of Taegu city and delivered a baby with congenital anomaly, or birthweight under 1,500gm, or a stillbirth, or gave a livebirth but died within the first week after birth at one of the five major hospitals in Taegu between 1 April 1991 and 31 March 1993. There were 869 women who met these criteria but telephone interview was successful for 422 women. Controls were selected by 1:1 pair-match with the women who delivered a normal baby of birth-weight over 2.500gm. Matching variables were the hospital of delivery, maternal age and educational level, birth order, and baby sex. Basic demographic and medical data were obtained from the hospital record and detailed occupational and other medical history were obtained by a telephone interview. Odds ratio(OR) of each occupational category for the reproductive outcome and the OR adjusted for the pregnancy complications and maternal chronic diseases were obtained by the conditional logistic regression of EGRET program.
For all poor reproductive outcomes including congenital anomalies, stillbirth, very low birth-weight, and early neonatal death, paternal work during one-year period before the index pregnancy in plastic and synthetic works(OR=13.69, 95% CI 1.62-115.50), construction works(OR=3.16, 95% CI 1.51-6.59), dyers(OR=3.15, 95% CI 1.20-8.30), and metal workers(OR=2.73, 95% CI 1.50-4.98) had largest ORs. The largest ORs adjusted for pregnancy complications and maternal chronic diseases were 55.78(95% CI 1.82-1700) for plastic and synthetic workers, 4.53(95% CI 1.39-14.78) for dyers. 3.18(95% CI 1.02-9.91) for vehicle and machine maintenance workers, and 2.49(95% CI 1.25-4.98) for metal workers.
The maternal occupations prior to one-year before the index pregnancy that showed an increased risk for all poor reproductive outcomes were dyeing(OR=4.64, 95% CI 0.54-39.74), service works(OR=2.38, 95% CI 0.82-6.91), and electronic/electric/computer works(OR=1.47, 95% CI 0.42-5.21). Adjusted ORs that were greater than 1 were dyeing(OR=13.05, 95% CI 0.31-548.60) and service works(OR=1.66, 95% CI 0.39-6.93).
