국민건강보험 청구자료를 활용한 의료관련감염 환자안전지표의 검증 = Feasibility of Using Administrative Data to Compare Healthcare-Associated Infection Performance

국문 초록 (Abstract)

환자안전 수준을 파악하고 개선 전략을 수립하는데 이용되는 질 지표는 안정적인 자료 확보를 위해 주로 행정자료를 이용한다. 우리나라도 건강보험심사평가원에서 국민건강보험 청구자료를 활용하여 각종 질 지표의 모니터링 결과를 산출하고 있지만 정확도가 낮다는 지적을 받아왔다. 본 연구에서는 두 단계 검증방법을 통해 행정자료를 통한 환자안전지표의 모니터링 타당성을 확인해 보고자 하였다. 우선 2010년 1년간의 국민건강보험 입원진료비 청구자료 6,592,121건을 활용하여 환자안전지표를 산출하였다. 이후 6개 표본 의료기관을 선정하여 각 지표별 의무기록을 조사하였다. 연구결과, 수술후 패혈증, 카테터 관련 혈류감염, 욕창, 인공호흡기 관련 폐렴의 지표 결과는 청구자료와 의무기록에서 모두 큰 차이를 보였다. 수술후 패혈증과 욕창의 청구자료는 과소 보고되었고, 인공호흡기 관련 폐렴은 과다 보고되었다. 카테터 관련 혈류감염은 낮은 지표값을 나타내 검증이 어려웠다. 결론적으로 청구자료를 활용한 환자안전지표의 산출은 그 타당도가 매우 낮은 것으로 해석된다. 그러나 질 지표의 모니터링 중요성이 높아지는 사회적 요구를 감안할 때 지표별 필요 자료가 청구자료에 포함되어 정확도를 높이거나 다른 유형의 행정자료를 활용하는 방법이 고려되어야 한다.

다국어 초록 (Multilingual Abstract)

To determine the level of national patients` safety and to establish improvement strategies of patient safety through comparison between countries, we used administrative data of the patient safety indicators (PSI). This study conducted a two-step verification method. First, we calculated PSIs using administrative data and then confirmed the validity and applicability of indicators by reviewing the medical records. Four PSIs were calculated using NHI claims data, 6,592,121 cases in 2010; postoperative sepsis, catheter-related bloodstream infection (CRBSI), pressure ulcer and ventilator-associated pneumonia (VAP). A total of six institutions were selected as the sample survey and carried out reviewing medical record. The NHI claims data and medical records survey results had a significant difference for the four PSIs. In conclusion, we didn`t have confidence the validity of PSI utilizing the NHI claims data. However, the importance of health care quality improvement and patient safety activities using quality indicators is increasing. We should explore diverse strategies to increase the accuracy of administrative data and study the different way to utilize the data.
