Objective: The purpose of this study introduces a safe, effective method for treating cornual pregnancy. Methods: Between April 2005 and December 2008, seven patients with cornual pregnancy underwent hysteroscopic surgery at a minimally invasive surgery clinic. An 18F Foley catheter was inserted after dilating the cervix; then, the uterine cavity was irrigated with 300 mL of H2O2 solution to prevent excessive bleeding. The cornual gestation was removed by hysteroscopy without complications under ultrasonographic guidance. Results: The serum β-human chorionic gonadotropin levels were monitored postoperatively and decreased optimally in all patients. All tissues removed from the uterus were confirmed to be gestational products. The estimated blood loss was less than 30 mL. All patients were discharged on the first or second postoperative day. Conclusion: Hysteroscopic removal of a cornual pregnancy with H2O2 is very practical and safe.

Two women presented with history of vaginal bleeding and abnormal transvaginal ultrasound findings. Saline infusion sonohysterography was done for preoperative evaluation and this imaging study revealed centrally located uterine mass with internal cystic portion. Our initial impression was submucosal myoma with cystic degeneration and hysteroscopic removal of the mass was performed. During the procedure, dark brownish cystic fluid was drained. The excised submucosal mass was pathologically diagnosed as adenomyosis and clinically as submucosal adenomyotic cyst. Submucosal adenomyotic cyst should be part of the differential diagnosis of submucosal uterine masses with cystic portion. We report two cases of submucosal adenomyotic cyst successfully treated with hysteroscopic resection and brief review on this topic.

Objective : To examine the outcome after operative resectoscopic resection of submucosal myomas in symptomatic women. Methods : Forty eight symptomatic women with submucosal myomas underwent operative resectoscopy for resection of the submucosal myomas. A few months after the myomectomy, I compared postoperaive status of the patients to preoperative status of the patients. Results : No operative or postoperative complications occurred and all patients were discharged within 4 days. The follow-up period was 11.12±6.44 months(mean±SD). The age of the patients was 37.69±9.22 years(mean±SD) at the time of the operation. Twelve women out of 16 conceived(pregnancy rate of 75%)and all of them subsequently delivered at term. Thirty two women(menorrhagia of 28 and backache of 4) had been rid of their chief complaints that they had suffered from preoperatively. Conclusion : The results of this study indicate that operative resectoscopy achieved a pregnancy rate 75%. These results suggest that operative resectoscopy is an effective procedure to resect submucosal myomas in infertile symptomatic women.

Objective The purpose of this study is to assess the impact of various factors of hysteroscopic lysis of intrauterine adhesion on live-birth rate after surgery. Methods Medical records of 54 patients suffering from infertility or recurrent spontaneous abortion were retrospectively analyzed. The independent variables were degree of intrauterine adhesion, surgical technique, use of intrauterine Foley catheter, insertion of intrauterine device, and dose of postoperative estradiol. Multivariate analysis as well as univariate analysis was performed. Results Among the 54 patients, 17 patients gave live-birth. The degree of intrauterine adhesion was found to be a sole factor affecting the live-birth rate by univariate and multivariate analysis. The mechanical dissection with scissors had been resulted a higher live-birth rate against resectoscopic electrosurgery, but the difference was not statistically significant. Conclusion The degree of intrauterine adhesion is the most important factor affecting the live-birth. The uses of intrauterine Foley catheter, insertion of intrauterine device and dose of postoperative estradiol have little impact.

Objective : to evaluate the incidence of complications of hysteroscopic surgery. Methods : A total of 150 patients who was done by hysteroscopic surgery from February 1997 to February 2000 were retrospectively analyzed at Choon Hae Hospital, Pusan, Korea. We classified complications into 6 groups-infection, surgeon-related complication, distension-media related complication, energy source related complication, hemorrhage and others. Results : Overall complication rates were 6% (9 to 150). Surgeon-related complication rate was 1.3% (2 of 150), media-related complication rate was 0.7% (1 of 150), hemorrhage was 4% (6 of 150). And there's no complications of infection, energy-source related, or others. Conclusion : Hysteroscopy is a safe procedure if you know the complications of hysteroscopy & its mechanisms.

In the case of pregnancy complicated by intrauterine device, complications such as ectopic pregnancy, septic abortion, preterm delivery are increased significantly. Especially in relation to intrauterine pregnancy, IUD should be removed to prevent complication. But when the tail is missed, therapeutic abortion has been done as an alternative method conventionally. Recently we experienced a case of hysteroscopic removal of IUD complicated by intrauterine pregnancy of 9th week without any harm to the G -sac or embryo. And she delivered healthy girl weighing 2950 gm. So we report this case with the review of articles related.

Objective: To compare the diagnostic accuracy and acceptability of saline infusion sonohysterography (SIS), and hysteroscopy for detecting intracavitary abnormalities in women with abnormal uterine bleeding. Methods: Seventy-eight patients with abnormal uterine bleeding were selected from the Department of Obstetrics and Gynecology in Chonnam University Hospital. The findings at SIS were compared with the hysteroscopic and histologic findings. The agreement of diagnosis between SIS and hysteroscopy was calculated. Sensitivity, specificity, positive predictive value, and negative predictive value were calculated. Results: SIS had a sensitivity of 84% and a specificity of 85% for a diagnosis of endometrial polyp. Hysteroscopy had a sensitivity of 86% and a specificity of 96% for diagnosis of endometrial polyp. SIS had a sensitivity of 75% and a specificity of 86% for a diagnosis of submucosal myoma. Hysteroscopy had a sensitivity of 100% and a specificity 99% for a diagnosis of submucosal myoma. SIS had a sensitivity of 91% and a specificity of 96% for a diagnosis of submucosal myoma. Hysteroscopy had a sensitivity of 100% and a specificity 91% for a diagnosis of endometrial hyperplasia. Conclusions: The diagnostic accuracy of SIS was significant to that of hysteroscopy in diagnosing intracavitary abnormalities, Moreover, SIS in a safe, convenient, cost effective, easily accessible and acceptable investigative modality.
