의료서비스에서 환자 및 소비자 참여 – 영국적 상황에서의 비판적 분석

연구배경: 선진국에서는 공공서비스 관리에 있어 시민참여가 일반적이다. 전국민을 대상으로 무상의료를 제공하는 영국의 경우에는, 그 예산이 일반세금에서 수립되기 때문에 의료소비자의 참여가 더욱 중요한 문제라 할 수 있다. 때문에 영국에서는 의료 정책 수립 및 서비스 제공에 있어 의료소비자들의 참여(Patient Public Involvement: PPI)가 증가하고 있으며 그 필요성 또한 더욱 강조되고 있다. 연구목적: 이 논문은 영국 내 의료환경에서 소비자들이 참여하게 된 정치적 맥락과 가정(assumption)과 한계를 ‘사회과학적’ 시각에서 비판적으로 논의하고자 하였다. 연구방법: 먼저 이 논문은 영국적 의료환경에서 소비자가 참여할 수 있게 된 정치적 맥락을 다루었다. 다음으로 의료서비스 발전을 위한 소비자 참여를 정치적 가정과 경제적 가정을 들어 분석하였다. 논의는 영국에서 발행된 정부정책 계획서 및 보고서들을 토대로 하였다. 연구결과: 이 논문은 의료서비스에 있어서 소비자 참여에 대한 맥락과 한계를 비판적으로 분석함으로써 한국에 비해 의료 정책 수립에 있어 소비자참여가 활발한 영국의 현 상황을 논의하였다. 이 논문에서 영국의 사례를 드는 이유는 환자 및 소비자 참여와 관련된 개념적 정의의 문제나 나타난 문제 및 그러한 문제의 해결방안을 구체적이고 축약적으로 제시함으로써 한국의 의료계나 관련단체가 참조할 수 있도록 하기 위함이다
드라마에 나타난 의사 이미지와 의사와 환자간 커뮤니케이션 유형에 대한 연구

The research analyzed how medical doctors were portrayed in Korean television dramas and what kinds of communication types between the doctor and patient were mainly appeared. Top-rated five dramas from each decade (1990's, 2000's, and 2010's) were selected for the content analysis. The results showed that 79 doctors in the 15 dramas appeared and 79.7% were males. Over 80% were in the age range of late-twenties and thirties, indicating that the general image of the doctors was the young-middle male. The results on appearance characteristics of doctors showed that most doctors in dramas were described as thin and tall, attractive, active and self-regulating. The most frequently-appeared doctor was general surgeon, explaining 26.3% of the 79 doctors. The communication styles between the doctors and patients were significantly changed on the time periods. Doctor-focused professional communication type was appeared mainly in the 1990s, but significantly decreased from the 2000s. From the time period, doctor-patient cooperation communication type and friendly communication type were sharply increased. Doctor-patient cooperation communication type was the most-appeared one in the 2010s' dramas. The results can be interpreted as Korean dramas reflect the current social reality that interactive communication between the doctors and patients are getting more important in medical institutions and services.

The purpose of this study is to identify the problems of the current curriculum for improving medicalcommunication skills in Korean medical schools. After investigation of the state of education of medicalcommunication in curriculum of each medical school some general problems were identified which couldmake education of medical communication difficult. The concrete problems of method and content ofmedical communication curriculum were described. Some suggestions were made to solve the problems.

Medical consultation have a lot of limitations due to the peculiarity of medical practice. However,broadening of understanding between medical institutions and their customers, the role of medicalconsultations is important. Futhermore, direct and indirect contacts of medical consultation as a new mediais going to prevent distortion of medical information. In this regard, video medical consultation is likelyto be settled as a means of long-term consultation method. This project conducted two steps analysis ofthe demonstration to check the possibility of video consultation system. In conclusion, to overcome thetechnical limitation turned out to be the most important things. It is expected that future telecommunicationinfrastructure will continue to improve the system. So the future prospect of video medical consultationwould be even brighter. Because this system use existing technologies rather than the new ones. Videomedical consultation are expected to make significant contributions for understanding between medicalinstitutions and customer. So ongoing efforts will be made for video medical consultation to be settled asthe new communication channel.
의과대학생의 영화예술 융합교육 기반 의료현장에 대한 인식 분석

Background : The need for interdisciplinary integrative education in medical humanities has been increasingly emphasized to cultivate holistic physicians. This study aims to develop a learner-centered curriculum by applying an arts-based integrative approach. Methods : To this end, a qualitative study was conducted to investigate and analyze the perceptions of medical students through in-depth interviews following art-based indirect experiential learning. The subjects of this study were 21 medical students at K University. The study examined students’ perceptions of medical themes presented in films and explored how the arts and interdisciplinary education can impact the content and methodology of medical humanities education. Results : Following the indirect experiential learning, students’ perceptions in the medical field were categorized into four domains: physicians’ communication, medical ethical issues, physicians’ self-management, and medical education for clinical setting. A total of 12 themes were identified within these domains. Conclusion : Based on the results of this study, the following points of discussion arise in medical education: First, both direct and indirect experiential learning, facilitated by arts-based, learner- centered education, are necessary. Second, the shared learning experiences based on reciprocity are important. Third, integrative education is needed to be composed within the integrated medical curriculum. Fourth, experiential and reflective education is needed for cultivating professionalism. The conclusion of this study suggests the professionalism education as a form of integrative education within the integrated medical curriculum, based on the points of discussion. The integrative educational approach to medical humanities, proposed in this study, may offer diverse ideas for the development and implementation of an integrated medical curriculum.

의료커뮤니케이션 수업을 위한 교육용 소프트웨어 도구의 개발

To understand the process of medical interviewing with patients, skill-based education should be stressedin teaching medical communication. The purpose of this study is to develop a educational aid tool whichcan make various educational materials (audio-vidual, textual, and presentation materials) organized veryeasily and used effectively in medical communication education. In this study we designed GUI by usingSWT (Standard Widget Kit) for the sake of convenient use of the educational software tool. This softwarecan be used in all the systems supporting Java of PC or UNIX systems, and prompt use of GUI can bepossible. The educational software tool can make educational contents presented according to the processof teaching (introduction, main course, and conclusion). In the introduction stage, objectives of learning, planof lesson, and educational materials to facilitate students' participation are presented. In the main coursecore contents of learning according to the subjects, audio-vidual clips to facilitate teaching, and educationalmaterials for the students to participate in experiences are presented. In the conclusion part, summary ofthe lesson, brief quiz to help the students memorize the core knowledge are presented. The developededucational software tool in this study can be used as a medium for the sake of effective progress of lessonin medical communication teaching. And it can facilitate collaboration of teachers for developing educationalmaterials, and also make possible the development of faculties who will take charge of medicalcommunication teaching.

This study explores the effect of communication class to students' communication self-efficacy in thepresent university period, in the future nurse life, and in the future relation between nurse and patients. The analysis result of the data gathered from the first grade students who took the communication classshows significantly positive influence of communication class to three variables. In particular, studentsresponded that they will have the most powerful self-efficacy in the future relation between nurse andpatients. In addition to that, this study also investigated test performing correlate of communicationself-efficacy. But we cannot examine it. This suggests that the test record will not predict the communicationself-efficacy.
