        COVID-19로 인한 일상생활 변화와 주관적 스트레스의 관련성

        Objectives: This study aimed to describe association between change in daily life and perceived stress by using nationwide representative data to identify socially vulnerable population during the coronavirus 2019 (COVID-19) pandemic. Methods: The Korea community health survey 2020 was used to address the research questions considering complex sample design effects. To analyze the relationship between change in daily life and perceived stress, binary logistic regression was employed. Results: Change in daily life was positively associated with perceived stress after controlling demographic, socioeconomic and health-related factors (OR=0.12, p<.0001). Conclusions: In unpredictable and uncertain situation, restrictive measures to control COVID-19 and its spread have increased anxiety, worry, and negative mental health in the individuals and communities. The difference in resilience to stressful situations may be due to the differences in the individuals' defense mechanisms against internal and external situations. The results of this study indicate the need to support vulnerable populations for a smooth return to daily life during the COVID-19 pandemic and post COVID-19 era.

        Downward bias of conductivity based point-of-care hemoglobin measurement compared with optical methods

        Background: Point-of-care (POC) arterial blood gas analysis (ABGA) is widely used for checking hemoglobin (Hb) level. However, there is the tendency of downward bias of conductivity-based POC ABGA Hb measurement compared with optical methods. Authors tried to correct that bias by linear regression equation. Methods: We retrospectively collected a total of 86 Hb result pairs during surgeries. Hb measured by the Sysmex XE-2100 in the laboratory was set as the gold standard and was compared with that measured by the GEM Premier 3500. Data were compared using the Bland-Altman analysis, the reliability of transfusion decision was assessed using three-zone error grid. The linear regression analysis was performed to find out the relation between the Hb results of POC ABGA and those of laboratory based test. Results: The bias of the Hb measured between Sysmex XE-2100 and GEM Premier 3500 was −0.9 g/dl (P < 0.001, 95% confidence interval, −1.038 to −0.665 g/dl). The percentage error was 16.4%. According to error grid methodology, zone A, B and C encompassed 89.5%, 10.5% and 0% of data pairs. After adjusting the POC ABGA Hb values, the bias of the Hb measured by two methods was 0 g/dl (P = 0.991). The percentage error was 18.2%. The zone A, B and C encompassed 91.9%, 8.1% and 0% of data pairs. Conclusions: Hb measurements obtained with reference to conductivity via a POC ABGA were significantly lower than those obtained via optical methods. This bias may deserve attention of anesthesiologists when POC ABGA Hb level is used as a transfusion guideline.

        Recent results have supported the cancer stem cells hypothesis in which tumors originate from tissue stem cells or their early progenitors and, as a result, produce tumors that retain stem cell properties. These properties include selfrenewal that drives tumorigenesis and differentiation that contributes to cellular heterogeneity. The number of these cells is very small, and is tightly controlled by the self-renewal pathway and the signals of their environment (niche). Evidence for the existence of cancer stem cells has been reported for a number of human cancers including leukemia, cancers of the breast, brain, and colon. Although our understanding of the biology of these cancer stem cells remains rudimentary, the existence of these cells has implications for current conceptualization of malignant transformation and targeted therapy for the treatment of cancer.

        군집분석을 활용한 지역별 건강격차 연구: 주관적 건강수준을 중심으로

        Background: Personal socio-economic abilities are crucial as it affects health inequalities. These multidimensional inequalities acrossthe regions have been structured and fixed. This study aimed to analyze health vulnerabilities by regional cluster and identify regionalhealth disparities of self-rated health, using nationally representative cross-sectional data. Methods: This study used personal and regional data. Data from the Community Health Survey 2021 were analyzed. K-means clusteranalysis was applied to 250 si-gun-gu using administrative regional data. The clusters were based on three areas: physicalenvironment, health-related behaviors and biological factors, and the psychosocial environment through the conceptual frameworkfor action on the social determinants of health. And binary logistic regression analyses were conducted to examine the differencesin self-rated health status by the regional clusters, controlling human biology, environment, lifestyle, and healthcare organizationfactors. Results: The most vulnerable group was group 3, the moderate vulnerable group was group 1, and the least vulnerable group wasgroup 2. The group 2 was more likely to have high self-rated health status than the moderate vulnerable group (odds ratio [OR],1.023; p<0.001). And the group 3 showed low self-rated health status than the moderate vulnerable group (OR, 0.775; p<0.001). However, the moderate vulnerable group had significantly higher self-rated health status than the most vulnerable group (group 2:OR, 1.023; p<0.001; group 3: OR, 0.775; p<0.001). Conclusion: These results demonstrate that community members’ health status is influenced by regional determinants of health andindividual levels. And these contribute to understanding the importance of specific and differentiated interventions like locallytailored support programs considering both individual and regional health determinants.

        유방암 선별검사: 선별 유방촬영술에 대한 한 기관에서의 10년간 의학적 감사

        Purpose: Screening for breast cancer has constantly been increasing since the benefit of screening for breast cancers was established. The purpose of this study was to investigate the efficacy of annual breast cancer screening at one institution for 10 years by conducting a medical audit. Methods: From March 1995 to July 2004, we performed 110,588 annual clinical examinations and mammographies on 58,024 women, who wanted to undergo breast cancer screening. Two hundred fourteen breast cancers were detected during screening, and one hundred sixty one of these patients were operated on. We then compared these results with the ideal rates for medical audits. Results: Of the 110,588 cases that were screened, the recall rate for further examination was 12.1% (n=13,423). The biopsy rate was 1.01% (n=1,116). Two hundred fourteen breast cancers were detected for a detection rate of 0.19%. The percent of stage 0 cancer among all the cancer was 23.6%, stage I was 40.4%, stage IIa was 19.9%, stage IIb and IIIa were a combined 6.2%, stage IIIc was 3.1%, and stage IV was 0.6%. The positive predictive value (PPV) based on the abnormal findings on the screening examinations was 1.6% (PPV1). The PPV when a biopsy or surgical consultation was recommended was 15.1% (PPV2). The percent of tumor found as stage 0 or I was 64% (103/161). The tumor found as minimal cancer (stage 0 or tumor lesser than 1 cm) was 38.5% (62/161). There were 38 cases of axillary lymph node metastasis (23.6%). The number of cases of cancers found per 1,000 cases was 1.7. The prevalence of cancer found per 1,000 first examinations was 2.3. The incidental cancer found per 1,000 follow-up examinations was 1.2. The recall rate for further evaluation was 12.1%. These results were compatible with the ideal rates for medical audits, except for the recall rate, the PPV1, the PPV2, and the cancers found per 1,000 cases. Conclusion: On the base of these results, breast cancer screening was properly performed in this institution. Breast cancer screening using a clinical examination and a mammography is effective for the early detection of breast cancer.

        한국판 밀론 다축 임상성격검사 4판의 신뢰도와 타당도

        이 연구는 한국판 밀론 다축 임상성격검사(Millon Clinical Multiaxial Inventory-IV, MCMI-IV)의 신뢰도와 타당도를 알아보는데 목적이있다. 층화 표집을 통해 지역사회에 거주하는 성인 및 환자 990명을 대상으로 자료를 수집하여 분석하였다. 한국판 MCMI-IV의 신뢰도를검증하기 위해 내적 합치도와 검사-재검사 신뢰도 분석을 실시한 결과 내적 합치도와 검사-재검사 신뢰도 모두 양호하였다. 탐색적 요인분석을 실시하여 한국판 MCMI-IV 척도의 요인구조를 확인한 결과 일반적 부적응, 정서적 불안정성, 반사회적 행동/충동성의 3요인이 있는것으로 밝혀졌다. 또한 타당도를 검증하기 위해 PDQ-4+, PDDS, K-BDI-II, K-BAI, K-BHS, K-BSS 간 상관 분석을 실시한 결과 개념적으로상응하는 척도들은 높은 정적 상관을 나타낸 반면 반대되는 척도들은 부적 상관을 보였다. 이러한 결과는 한국판 MCMI-IV가 성격과 임상증후군을 신뢰롭고 타당하게 측정하는 것을 의미한다. MCMI-IV는 다양한 임상 장면에서 활용될 수 있을 것으로 기대된다.
