청심연자탕(淸心蓮子湯)으로 호전된 뇌내출혈에 고혈압이 동반된 태음인 환자 치험 1례

■ Objectives The purpose of this case study is to report that a hypertension with intracerebral hemorrhage of tae-eumin was treated with 'Cheongsim Yeunja-tang' and then the symptoms were improved and blood pressure was decreased. ■ Methods Although the patient who had hypertension with intracerebral hemorrhage took antihypertension drugs, intermittent increase of blood pressure was shown. We diagnosed him as Tae-eumin and treated with Cheongsim Yeunja-tang. We daily checked blood pressure and evaluated the symptoms. ■ Results After the treatment with Cheongsim Yeunja-tang, blood pressure was decreased and symptoms were improved. ■ Conclusion The result shows Cheongsim Yeunja-tang has antihypertensive effect and improve hypertension symptoms of Tae-eumin patient with intracerebral hemorrhage.

소아 IgA 신병증 환자에서 미토콘드리아 DNA 돌연변이 분석

Purpose: The association of mitochondrial DNA (mtDNA) mutations, deletions and copy number with progressive changes in patients with some glomerular disease and end-stage renal disease have been reported. In this study, we performed mtDNA mutation analysis in children with IgA nephropathy to investigate its role in progressive clinical course. Methods: Seven children with IgA nephropathy were involved in this study. MtDNA isolated from platelet was amplified by PCR and sequenced entirely. Results: The mean age at renal biopsy was $11.5{\pm}2.2$ year and the mean age at latest evaluation was $17.9{\pm}3.2$ year. The mean follow-up period were $7.8{\pm}3.1$ years. Patients was divided into 2 groups according to the amount of proteinuria at presenting manifestation. Group 2 patients were nephrotic syndrome. Renal function reveals within normal range in all patients. In group 2 patients, the mean serum albumin level was significantly lower than those of group 1 ($3.7{\pm}0.6g/dL$ vs. $4.7{\pm}0.2g/dL$, P=0.0241) and the mean total cholesterol level was significantly higher than those of group 1 ($222.7{\pm}35.7mg/dL$ vs. $148.3{\pm}29.1mg/dL$, P=0.0283). In Group 2 patients, total amount of protein of 24 hour collected urine also significantly higher than those of group 1 ($1,466.0{\pm}742.5mg$ vs. $122.5{\pm}48.1mg$, P=0.0135). Pr/Cr ratio in random urine sample was also higher in group 2 than those of group 1 but the statistical significance was not noted ($1.8{\pm}1.6$ vs. $0.2{\pm}0.2$, P=0.0961). Deletion of mtDNA nt 8272-8281 were observed in two patients, one patient in each groups, respectively. This is noncoding lesion. No patients demonstrated the mtDNA mutations. Conclusions: We have identified a deletion of mtDNA nt 8272-8281 in two children with IgA nephropathy. Further studies are needed to clarify the role of mitochondrial function in the progressive change of IgA nephropathy.

수면중 이갈이에 의해 발생한 턱관절 통증 환자에 대한 FCST 활용 1예 보고

A 27 year-old male patient with sleep bruxism-induced temporomandibular pain was managed by appliance of FCST with 3 weeks of duration, combined with acupuncture. After being treats for 3 weeks, the patient's pain was improved. Assessment was made by self assessment (Numerical Rating Scale, NRS; Korean Oral Health Impact Profile, KOHIP) and clinical observation. An impressive effect was observed and further clinical and biological research on FCST is expected.

Although hypertension doesn't kill itself, its complications can be deadly: increased risk of heart attack and stroke. So hypertension is often called the silent killer, and this is why controlled hypertension is so important. A 51 year-old female stroke patient was diagnosed with hypertension 10 years ago. Since then, though western medication have been used to control her hypertension, it was ineffective. As we know, obesity can cause high blood pressure, so we try weight-loss as alternative way to her hypertension drug. After discharge from hospital, she reduce her weight by 10kg and more by diet only. And her blood pressure became decreased steadily. As maintaining her decreased weight, her blood pressure is now relatively stable without taking any medication. In this case, we recognised that weight-loss by diet could be effective instead of taking hypertension drug for obesity patient who has high blood pressure.

본 증례에서는 발병한지 3개월된 틱장애에 대해 약 7개월 간의 FCST 치료법이 양호한 치료효과가 있는 것을 관찰하였다. 틱장애 증상의 정도는 YGTSS를 통하여 평가하였으며, 1주(${\pm}1$일)간격으로 방문때마다 측정이 이루어졌다. YGTSS 점수가 치료 도중 몇차례 초진 시 측정점수인 24점보다 상승한 적이 있었으나 곧 호전되었으며, 최종 치료 종결시에는 5점으로 초진 점수에 비해 현저하게 호전된 것을 관찰할 수 있었다. 그리고 이 호전된 상태는 추적관찰 중에서 2015년 10월 현재까지도 지속되고 있는 것으로 확인되었다. 이에 저자는 본 증례를 FCST 치료를 통하여 틱장애를 호전시킨 유의한 증례라고 판단하여 보고하는 바이다. A 8 year-old male patient with tic disorder was managed by appliance of Functional Cerebrospinal Therapy (FCST) for 7 months, combined with acupuncture. After being treated for 7 months, the patient's symptoms were improved. Assessment was made by Yale Global Tic Severity Scale (YGTSS) and clinical observation. A positive effect was observed and further clinical and biological research on FCST is expected.

A 67 year-old Taeeumin(太陰人) female patient with Thalamic pain Syndrome(Dejerine-Roussy Syndrome) was treated. She was suffering from rt. side numbness, pain and weakness, rt. facial palsy, dizziness, dysarthria and lassitude. She was diagnosed as Cerebral lacunar infarction(Lt. thalamus) by Brain MRI(Magnetic Resonance Imaging) and adminstered Yeoldahanso-tang(熱多寒少湯). After taking Yeoldahanso-tang(熱多寒少湯), her symptoms had improved. Result suggest that Yeoldahanso-tang(熱多寒少湯) is an effective for Taeeumin Thalamic pain Syndrome. Though, results support a role for oriental medical treatment for Cerebrovascular Accident(CVA) connected Sasang Constitution, further research into the effectiveness of oriental medical treatments for CVA connected Sasang Constitution is needed.

This is a case report on a deep vein thrombosis(DVT) patient whose pain, numbness, and swelling in the left leg have been decreased with Korean Medicine. The 59 year-old male patient who was diagnosed with DVT and showed the classic symptoms of DVT(pain, numbness and swelling in the left leg). Although he was treated with medication(100mg of aspirin, 20mg of rivaroxaban) for five weeks, his symptoms did not improved. Then, he was gaven Sunkihwalhyul-tang three times a day and Jungsongouhyul pharmacopuncture once a day for two months. Conservative therapy including leg elevation and an elastic stocking was co-administered. During our treatment period, the size of the thrombus in the left external iliac vein was decreased according to sonography compared with the pre-hospitals sonography, and the patient felt his symptoms had improved. This case report suggests Korean Medicine may play a role in improving the deep vein thrombosis symptoms.

Objectives This study reports three cases of pressure ulcer treated with Hwangryunhaedok-tang pharmacopuncture. Methods Three patients with pressure ulcers were treated by Hwangryunhaedok-tang pharmacopuncture and Jaungo at the Oriental Medicine Hospital of Daejeon University. Hwangryunhaedok-tang pharmacopuncture and Jaungo was administrated once a day with simple dressing. Thereafter, we observed the pressure ulcers macroscopically. Results and Conclusions After 3 weeks, wound sizes were reduced and tissue regeneration was accelerated. In conclusion, Hwangryunhaedok-tang pharmacopuncture and Jaungo are effective to treat pressure ulcers, but more studies will be required to validate its use in pressure ulcers.

This paper presents the Electronic Ballast combine Boost Converter with Half-Bridge Inverter. A Boost Converter is used to control input power factor and a Half-Bridge Inverter is used to drive the fluorescent lamp. The two stages were unified to simplify the design. This electronic ballast presents simplicity of control, allowing the use of small switches when compared another topologies of ballasts.
