과민성방광의 비수술적 치료 : 방광의 저장용적을 증가시키는 치료법 Therapy to Increase Bladder Storage Capacity

Overactive bladder including frequency, nocturia, urgency ate common and Its cause is decreased bladder storage capacity. Non-pharmacologic therapy to facilitate bladder storage capacity are behavioral, bladder overdistention or electrical stimulation. For behavioral therapy, patient education about lower urinary tract function, information about lifestyle changes or dietary modification, bladder training which includes instituting intervals of timed voiding and gradually increasing these intervals, pelvic floor physiotherapy with or without biofeedback are necessary. For physically or mentally challenged individuals, schedulled toileting or prompted voiding is necessary, too. Bladder overdistention, electrical stimulation or neuromodulation are also used for the treatment of overactive bladder.

Hemospermia is not an uncommon disease and may affect the sperm motility. But the research about sperm motility in hemospermia is rare. So we studied the change of sperm motility between control group and patients with hemospermia and also studied the change of sperm motility between patients with hemospermia and those whose hemospermia was improved, the change of sperm motility between control group and artificially induced hemospermia group. We observed that the sperm motility in patients with hemospermia was decreased than that of control group, and that, as hemospermia being better, sperm motility was improved. We also observed that sperm motility in artificially induced hemospermia was decreased. The results provide that hemospermia has an effect on decreasing hemospermia motility.
흰쥐 방광에 이식된 superparamagnetic iron oxide labelled human mesenchymal stem cells의 조직학적 추적관찰

Purpose: Molecular magnetic resonance (MR) imaging technique using the superparamagnetic iron oxide nanocrystals (SPIO) are developed for the monitoring the stem cells noninvasively. This study was performed to monitor the transplanted SPIO labelled human mesenchymal stem cells (hMSCs) with the molecular MR imaging and histologically in the rat bladder. Materials and Methods: SPIO (Feridex; AMI, Cambridge, MA, USA) were labelled to hMSCs using GenePORTER. The labeling viability, efficiency of the SPIO labelled hMSCs were examined with tripan blue, prussian blue staining. SPIO labelled hMSCs are transplanted to the bladder in rats and MR images of them were examined using 1.5 T MR. Histologic examination of the SPIO labelled hMSCs transplanted bladder was performed with H&E and prussian blue staining. Results: The viability, efficiency of the SPIO labelled hMSCs were good. MR signal intensity at the areas of the SPIO labelled hMSCs in the bladder decreased after transplantation of 1x106 SPIO labelled hMSCs and were confined locally. Intracellular SPIO were confirmed in the hMSCs transplanted bladder up to 5 weeks. Conclusion: The SPIO labelled hMSCs in the bladder can be monitored using the molecular MR imaging and SPIO labelled hMSCs were found in the transplanted bladder. (J. 성공적인 발기부전 치료법의 개발에도 불구하고, 근본적으로 발기부전을 치료할 수 없고, 심한 발기부전 환자에서는 효과적이지 않다는 한계점을 지니고 있으므로 새로운 치료법의 필요성이 대두되었다. 줄기세포 치료는 줄기세포가 분화하여 손상된 조직이나 세포의 재생을 촉진하며, 줄기세포 자체에 의한 효과뿐 아니라 줄기세포를 매개로 한 유전자 전달을 통하여 유전적으로 변형된 세포를 안정적으로 전달할 수 있고, 자가세포를 이용할 경우 면역거부반응을 유발하지 않으며, 국소염증반응을 일으키지 않는다는 장점이 있다.배아줄기세포를 신경분화시킨 신경배아줄기세포 치료는 해면체신경 손상 후, 축색돌기 신장을 돕는 세포기질을 제공하며, 신경수초화를 촉진하고, nurofilaments 생성을 통하여, 손상된 신경세포 재생을 촉진하여 발기능을 개선시켰다. 중간엽줄기세포 단독 혹은 중간엽줄기세포 매개 유전자치료는 노령 발기부전 흰쥐의 음경해면체에 투여 후, 중간엽줄기세포가 내피세포와 평활근 세포로 분화하고, 음경해면체 조직에서 eNOS 단백 표현과 NOS 활성 및 cGMP의 을 증가시켜 발기능을 개선시켰다. 횡문근유래줄기 세포치료도 해면체신경 손상 후 음경해면체에 투여하면 손상된 신경의 회복을 촉진하여 발기능을 개선시켰다. 태아줄기세포, 양수유래줄기세포 및 제대혈줄기세포는 다능하여 이들을 이용한 세포치료법도 시도되고 있다.줄기세포 단독 및 줄기세포 매개 유전자치료법은 발기부전을 근본적으로 치료하여 자연스러운 성생활의 회복을 가능하게 할 수 있고, 노령이나 기존 치료에 반응하지 않는 심한 발기부전 환자 및 기존치료의 적용이 불가능하였던 환자들에서 새로운 치료법을 제시할 수 있을 것으로 기대한다.
