        The aim of this study is to evaluate the effect of skin dose and PDD by using wounds protecting gauzes or Vaselinespread gauzes. And it was studied that the possibility to substitute custom bolus into gauzes. 4MV photon (CL600C, varian, US), Polystyrene Phantom (30(W) X30(L) X 30(H)) with Markus chamber(PTW, US) were used for dose measurement. This study was distinguished natural gauzes and spread over Vaseline gauzes. We gave variety to the gauze thickness at 5, 10 and 15 sheets respectively. For comparison between using bolus and not that, we had used 1.0 cm thickness bolus so that analyzed surface dose and PDD at the same conditions above mentioned. When maximum point was defined as reference point, surface dose was measured as $35\%$ in open beam. When the gauzes were attached to surface as 5, 10 and 15 sheets, surface dose were increased as 69, 80 and $91\%$ respectively according to thickness of gauzes. When spread over Vaseline gauzes were attached to surface as 5, 10 and 15 sheets, surface dose were increased respectively as 98, 100 and $98\%$ according to thickness of gauzes. Also when 0.5 cm bolus and 5 sheets gauzes were composed, surface dose was measured as $98\%$. The gauzes that were attached to skin surface in radiation therapy had been scattering material and contributed increasing surface dose without variation of percentage depth dose. However, if we want to delivery much dose to skin surface then we have to apply many sheets of gauzes to skin surface. Although we get easy that result by bolus or spread over Vaseline gauzes, we have to revise percentage depth dose at calculation. Therefore, if we find pertinent conditions based on measured data that are considered skin dose and patient setup efficiency, to replace custom bolus with gauzes will be helpful to efficient treatment.

        재발한 급성 혼합형 백혈병에 동반된 골수괴사 1례의 MRI 소견

        Bone marrow necrosis is a rare complication of a variety of diseases affecting the marrow. The cause and incidence are unknown, and reports of treatment response are rare. We describe a case of relapsed acute mixed type leukemia with bone marrow necrosis. The patient was a 10 year old female diagnosed with acute mixed type leukemia four years ago. She had been on second remission state for 1 year, presented with severe back pain, tenderness in lower extremities, low-grade fever and general weakness. Her level of serum lactic dehydrogenase on admission was increased. Bone marrow aspiration from both posterior iliac crest showed marrow necrosis. Subsequent examination showed the same feature. Hip MRI showed heterogenous low signal intensity in both iliac bone on T-1 weighted image and heterogenous high signal intensity on T-2 wieghted image. Remission induction therapy was started but she expired on 59th hospital day due to the complication of sepsis.

        중풍환자에서 수족냉증과 적혈구변형능의 관련성 연구

        Objectives : Coldness of hands and feet has been regarded to be associated with blood circulation. This study was performed to assess the correlation between erythrocyte deformability and coldness of hands and feet. Methods : 145 patients who had taken an erythrocyte deformability test were selected for this study among hospitalized patients within 4 weeks after their cerebral infarction at the Internal Medical Department at Kyunghee Oriental Medical Center from August 2008 to August 2009. We divided the selected patients into two groups, with and without of coldness of hands and feet groups. The two groups were compared on risk factors for atherosclerosis and examined for variables including erythrocyte deformability. Results : 1. On demographic variables of the patients, only body mass index was significantly lower in the coldness of hands and feet group than the control group. There was no significant difference of the rate of hypertension, diabetes mellitus, hyperlipidemia, ischemic heart disease, smoking, drinking and carotid artery stenosis between the two groups. 2. According to blood test, the coldness of hands and feet group showed significantly lower erythrocyte deformability index than the control group. 3. Body mass index and erythrocyte deformability index showed a close relationship with coldness of hands and feet in multivariate analysis. Conclusions : The correlation between the erythrocyte deformability and coldness of hands and feet in cerebral infarction patients were decreased erythrocyte deformability participated in the mechanism of coldness of hands and feet.

        멀티캐스트 기술은 송신자나 망의 부하를 가중시키지 않으면서도 다수의 수신자들에게 동일 데이터를 동일 시간에 전달할 수 있는 기술이다. 특히나 멀티캐스트 전송 기술을 사용할 경우 수신자 수의 한계를 극복할 수 있다는 큰 장점이 있다. 따라서 이 기술은 생방송형 일-대-다 그룹 통신 서비스에 매우 적합한 기술로 널리 알려져 있다. 최근 IPTV 서비스가 각광을 받음에 따라 멀티캐스트 기술은 생방송형 스트리밍 서비스를 효율적으로 지원할 수 있는 기술로 재조명 받게 되었고, 다양한 국제 표준화 기구에서 활발한 관련 표준화 활동을 추진하는 중이다. 본 고에서는 ITU-T, IETF 및 3GPP와 같은 국제표준기구에서 다루고 있는 멀티캐스트 관련 표준화 기술 동향을 기술한다.

        12개 미래 예측 한의약 정책 과제의 실현 평가 연구

        Objectives : Aim of this study is to contribute to establishment of the Traditional Korean Medicine (TKM) policies in the future. Final assessment for 12 of the forecasting projects was carried out on the TKM policies that deduced by professionals in 1996 whether or not to realize in 2013. Methods : We investigated governmental and private research projects, reports and papers, and laws and systems on the forecasting projects. We reviewed them through the Traditional Korean Medicine Information Portal OASIS (http://oasis.kiom.re.kr), Korean studies Information Service System (KISS) (http://kiss.kstudy.com/) and DBpia (http://www.dbpia.co.kr/), Akomnews(http://www.akomnews.com/), THE MINJOK MEDICINE NEWS(http://www.mjmedi.com/), Ministry of Government Legislation(http://www.law.go.kr/). Results : Of the 12 forecasting projects, five were judged as 'realization', four were as 'partial realization' and three were as 'un-realization', The realization rate was 75.0%. Three un-realized projects included the TKM insurance coverage for various herbal medicines, leadership secure on medical technicians and commercialization of the TKM managing system on senior medicare policy. Realization of the future forecasting TKM policy projects was decided depending on conditions such as the importance, domestic capability levels, principal agents, methods and restrains. Conclusions : Continuous studies and new developed forecasting projects for the TKM policies will be required to realize the projects in the future.

        Cleft Lip and Palate Surgery in Kyrgyzstan - Two Times of Our Experiences in Central Asia

        키르기즈스탄은 옛 실크로드의 한 가운데에 위치한 중앙아시아 국가로서 소련연방이 해체된 뒤 1991년에 독립한 신독립국(Newly Independent States, NIS)의 일원이다. 대한민국과의 공식적인 수교가 1992년에 이루어진 이후 18년이 지났지만 키르기즈스탄 거주 한국인의 숫자는 약 900명에 불과하며 특히 이들 중 대부분은 수도인 비쉬켁(Bishkek)에 살고 있다. 남 키르기즈스탄 제 1의 도시이자 옛 실크로드의 중심지로 유명했던 오쉬(Osh)시에는 2009년 말 기준 약 50여명의 한국인이 살고 있다. 한국얼굴기형환자후원회의 후원으로 구순구개열 자선수술팀은 2002년~2009년까지 11개국, 19회의 자선수술 활동을 다녀왔으나 중앙아시아 국가로의 방문은 키르기즈스탄이 최초였다. 많은 봉사활동국에서 지역적, 문화적 그리고 의료환경적인 어려움이 있었지만 2008년 12월, 2009년 7월 두 번의 키르기즈스탄 오쉬시로의 활동도 예외는 아니었다. 본 논문에서는 최초의 중앙아시아 키르기즈스탄 구순구개열 자선봉사활동에 대한 자세한 보고를, 준비과정에서부터 활동 완료시점까지 서술하고자 한다.

        중환자실 간호사의 도덕적 민감성, 셀프리더십, 피로도와 표준주의지침 수행과의 관계

        본 연구의 목적은 중환자실 간호사의 도덕적 민감성과 셀프리더십, 피로도 및 표준주의지침 수행도의 상관관계를 파악하고 표준주의지침 수행도에 미치는 영향요인을 탐색하기 위함이다. 연구대상은 전국 의료기관의 중환자실 네트워크를 활용하여 15개 종합병원 중환자실 간호사 153명을 대상으로 하였으며, 자료수집은 2019년 3월 5일부터 2019년 3월 10일까지 시행하였다. 자료분석은 Descriptive statistics, Pearson's correlation coefficient, Stepwise multiple regression analysis를 이용하였다. 연구결과, 본 연구대상자의 표준주의지침 수행도에 셀프리더십(r=.17, p=.042)이 통계적으로 유의한 양의 상관관계를 보였다. 연구대상자의 표준주의지침 수행도에 영향을 주는 요인을 확인하기 위하여 단계적 회귀분석을 수행한 결과, 셀프리더십(β=.17, p=.042)이 유의한 요인으로 나타났으며, 설명력은 통계적으로 유의하였다(R<sup>2</sup>=.03, F=4.21, p=.042). 따라서 최근 간호사의 높은 수준의 표준주의지침 수행도를 유지하게 된 시점에 각 병원은 간호사들의 셀프리더십 역량 강화를 위한 노력이 필요하다. The study was done to identify relations among moral sensitivity, self-leadership, fatigue and compliance with standard precautions and to identify the influencing factors on compliance with standard precautions for intensive care nurses. Data were collected from 153 intensive care nurses in March 2019 and were analyzed using Descriptive statistics, Pearson correlation coefficients, and Stepwise multiple regression with SPSS/WIN 24.0 version. The compliance with standard precautions has positive correlation with their self-leadership (r=.17, p=.042). The self-leadership (β=.17, p=.042) was a significant predictor of the compliance with standard precautions. It was statistically significant (R<sup>2</sup>=.03, F=4.21, p=.042). Based on the findings of this study, each institution needs to make efforts to strengthen the capacity of nurses for self-leadership.

        PS 긴장재 긴장력 계측을 위한 PSC 거더 내부 매립용 EM 센서

        In this paper, an embedded EM sensor was researched to estimate prestressing force of PS tendon in PSC girder. Recent methodologies for managing prestressing force loss were staying on verifying a applying prestressing force under construction, namely the loss management can not be controlled after construction. To estimate the tensile force of PS tendon during lifetime of PSC girder, this research proposed a bobbin-type embedded EM sensor that can be embedded in PSC girder is designed and fabricated considering the shape properties of anchorage zone and sheath. To verify the proposed sensor, a small PSC girder test was performed. The embedded EM sensor was connected to a sheath and anchor block, and the concrete was poured. After curing, the change of the permeability of PS tendon under tensile forces of 200, 710, 1070, 1300kN was measured using embedded EM sensor. The permeability of PS tendon had decreased according to the increment of applied tensile force. Also it is confirmed that the change of permeability due to applied tensile force could resolve the applied tensile force values. As a result, proposed embedded EM sensor could be embed into the PSC girder and it could be used to estimate the tensile force variation during lifetime of PSC girder.

        생약연구소의 인삼연구와 약도개성

        The Natural Products Research Institute (NPRI, 生藥硏究所), an institution affiliated with Keijo Imperial University (京城帝國大學), was the predecessor of the NPRI at Seoul National University and a comprehensive research institute that focused on ginseng research during the Japanese colonial era. It was established under the leadership of Noriyuki Sugihara (杉原德行), a professor of the second lecture in pharmacology at the College of Medicine in Keijo Imperial University. Prof. Sugihara concentrated on studying Korean ginseng and herbal medicine beginning in 1926 when the second lecture of pharmacology was established. In addition to Prof. Sugihara, who majored in medicine and pharmacology, Kaku Tenmin (加來天民), an assistant professor who majored in pharmacy; Tsutomu Ishidoya (石戶谷勉), a lecturer who majored in agriculture and forestry; and about 36 researchers actively worked in the laboratory before the establishment of the NPRI in 1939. Among these personnel, approximately 14 Korean researchers had basic medical knowledge, derived mostly from specialized schools, such as medical, dental, and pharmaceutical institutions. As part of the initiative to explore the medicinal herbs of Joseon, the number of Korean researchers increased beginning in 1930. This increase started with Min Byung-Ki (閔丙祺) and Kim Ha-sik (金夏植). The second lecture of pharmacology presented various research results in areas covering medicinal plants in Joseon as well as pharmacological actions and component analyses of herbal medicines. It also conducted joint research with variousinstitutions. Meanwhile, in Gaesong (開城), the largest ginseng-producing area in Korea, the plan for the Pharmaceutical Industry Complex was established in 1935. This was a large-scale project aimed at generating profits through research on and the mass production of drugs and the reformation of the ginseng industry under collaboration among the Gaesong Ministry, Kwandong (關東) military forces, Keijo Imperial University, and private organizations. In 1936 and 1938, the Gyeonggi Provincial Medicinal Plant Research Institute (京畿道立 藥用植物硏究所) and the Herb Garden of Keijo Imperial University (京城帝國大學 藥草園) and Pharmaceutical Factory were established, respectively. These institutions merged to become Keijo Imperial University's NPRI, which wasthen overseen by Prof. Sugihara as director. Aside from conducting pharmacological research on ginseng, the NPRI devoted efforts to the development and sale of ginseng-based drugs, such as Sunryosam (鮮麗蔘), and the cultivation of ginseng. In 1941, the Jeju Urban Test Center (濟州島試驗場) was established, and an insecticide called Pancy (パンシ) was produced using Jeju-do medicinal herbs. However, even before research results were published in earnest, Japanese researchers, including Prof. Sugihara, hurriedly returned to Japan in 1945 because of the surrender of Japanese forces and the liberation of Korea. The NPRI was handed over to Seoul National University and led by Prof. Oh Jin-Sup (吳鎭燮), a former medical student at Keijo Imperial University. Scholars such as Woo Lin-Keun (禹麟根) and Seok Joo-Myung (石宙明) worked diligently to deal with the Korean pharmaceutical industry.
