Sixty-eight consecutive patients have been subjected to total corrective surgery of tetralogy of Fallot at the Hanyang University Hospital between August 1975 and December 1984. Of these, the pressure of chambers and great arteries were measured, before and immediately following an operation in 39 of the survived group and 11 of the expired group. The values of postoperative peak pressure were correlated with the outcome of the operation. The result are as followings; 1] Compared to the average peak pressure of right atrium in the survived group, 14.46*4.16 mmHg, that in the expired group, 19.36*8.19 mmHg, was significantly lower, 2] Compared to the average systolic pressure of right ventricle in the survived group, 56.72z16.37 mmHg, that in the expired group, 70.45*15.26 mmHg, was significantly lower. 3] Compared to the average systolic pressure of pulmonary artery in the survived group, 33.26*12.95 mmHg, that in the expired group, 37.55*11.63 mmHg, was higher. But the difference was not significant. 4] Compared to the average peak pressure of left atrium in the survived group, 17.41*6.90 mmHg, that in the expired group, 31.18*12.47 mmHg, was significantly higher. 5] Compared to the average systolic pressure of left ventricle in the survived group, 103.82~12.83 mmHg, that in the expired group, 90.55*20.02 mmHg, was significantly lower. 6] Compared to the average value of the right ventricle-pulmonary artery systolic pressure gradient [RV-PA] in the survived group, 24.95a15.44 mmHg, that in the expired group, 35.09*17.01 mmHg, was significantly higher. 7] Compared to the average value of the right ventricle to left ventricle systolic pressure ratio [RV/LV] in the survived group, 0.55*0.15, that in the expired group, 0.80a0.20, was significantly higher. 8] Compared to the average value of the pulmonary artery-left atrium peak pressure gradient [PA-LA] in 15.85x12.29 mmHg that in the expired group, 4.18x6.00 mmHg, was significantly lower. It is, therefore, suggested that in the operating room, PA-LA is as valuable as RV - PA, RV/LV in making important surgical decision and predicting the prognosis.

The development of extracardiac valved conduits for establishment of RV-PA continuity has permitted repair of previously uncorrectable congenital heart anomalies and has facilitated the repair of other complex lesions. But the distressing problem of neointimal peel formation with eventual conduit obstruction in patients with Dacron valved conduits has led to the need for premature replacement in many patients. Therefore in the absence of pulmonary hypertension, hypoplastic pulmonary arteries, significant right ventricular dysfunction or unrepaired tricuspid regurgitation, preference of a valveless conduit to a valved conduit gives excellent results and may diminish the need for late reoperation. In our Hanyang University Hospital, the Rastelli operation was performed for the repair of pulmonary atresia with a valveless Gore-Tex conduit. The patient was operated on with good result. The CVP after operation was 8-13 mmHg at POD #0 and 4-6 mmHg from POD #3-4. Postoperatively the patient was acyanotic and had improved physical capacity compared with his preoperative status.

Between 1977 and 1987, 8 patients underwent excision of cardiac myxomas at the Hanyang University Hospital. All had a left atrial myxoma. There were 4 female and 4 male patients ranging from 15 to 62 years of age. Preoperative findings consist of symptoms and signs of congestive heart failure except one. Diagnosis was confirmed by echocardiography[8 cases] and angiography[2 cases], preoperatively. A biatrial operative approach was utilized in all but 2, who were small sized. Complete excision of the tumor with a cuff of normal tissue[1 was atrial wall and 7 were atrial septum] was performed. all heart chambers were carefully explored for presence of multicentric myxomas or tumor debris. There were no operative deaths or intraoperative embolization. Follow-up has been 1/3 to 10 years. There has been 1 late death, due to recurrence and 1 patient had reoperation for mitral regurgitation due to dilatation of the annulus by a huge tumor mass. Surgical excision of the myxoma can be performed with low morbidity, and it provides excellent and sustained symptomatic relief. The recurrence rate is low, but long-term follow-up and serial echocardiography are advisable.

현대 테러리즘은 발생양상이 개별국가나 단체를 초월하여 초국가적 성격을 띠고 있다. 특히 9.11 테러 사태와아프간‧ 이라크 전은 국가안보와 전쟁의 패러다임을 새롭게 변화시켰다. 테러리즘의 개념과 양상의 변화 추이와더불어 테러리스트의 커뮤니케이션 수단 역시 시대적으로 많은 변화를 가져왔다. 카세트 테이프와 비디오 테이프시대를 거쳐 오늘날 인터넷을 통하여 더욱 정교한 옵션을 선택할 수 있는 시대에 와있다. 작금, 과학기술의 발달과 함께 인터넷은 테러조직에게 신경조직으로써 활용성은 급격히 증대하고 있다. 테러조직들은 인터넷으로 의사소통을 하는 것은 물론 선전활동과 자금조달, 그리고 가상공간에 성전학습소를 설치하여 원격학습을 하는 등 다양하게 사용되고 있다. 그들은 인터넷을 통한 심리전으로 지지자들을 더욱 교화시키고 적을 혼란스럽게 하며 대중에게 공포감을 심어준다. 이러한 현상은 9.11 테러 이후로 급증하여 인터넷은 초국가적 테러리즘에 있어서 중요한 양상이 되었다. 그러나 테러리스트들이나 극단주의자들의 인터넷 활용성 증대에 비하여 정책결정자들은 그에 대한 인식과 대처방안이 상대적으로 현저히 낙후되어 있는 실정이다. 따라서 테러리스트들의 인터넷을 활용한커뮤니케이션 실태를 정확히 인식하고 세계화의 흐름과 더불어 UN 등 국제기구들의 보다 적극적인 역할을 촉구한다. 또한 개별국가들은 사사로운 자국의 이해논리에 국한하기보다 국제법적 기초를 토대로 인류평화 유지 차원에서 대비책을 강구하도록 적극 노력하고 협조하는 자세가 필요하다.

Effect of Amiloride to Retinal Toxicity Induced by Tissue Plasminogen Activator

Purpose: The effects of amiloride on cellular toxicity caused by tissue plasminogen activator (tPA) in mouse primary retinal cells were investigated. Methods: Primary retinal cell cultures were maintained using glial conditioned medium. Commercial tPA and L-arginine were added, and the level of cyclic guanosine monophosphate (cyclic-GMP) in the culture supernatant was assessed using an ELISA assay. We measured the cell viability of cultured retinal cells pretreated with three different concentrations of amiloride (1, 10, and 100 μm) in addition to commercial tPA or L-arginine treatment. Results: After exposing the cultured mouse retinal cells to tPA plus L-arginine or L-arginine alone, cyclic-GMP concentrations were 61.9 ± 5.1 pmole/mL and 63.1 ± 6.1 pmole/mL, respectively. However, the control group had a significantly lower concentration of cyclic-GMP (37.2 ± 3.4 pmole/mL, p < 0.01). The cyclic GMP-dissolved solution did not cause retinal cell death. In the control group and the group treated with 1 μm amiloride and tPA containing L-arginine, the cell viability was 43.7% and 44.5%, respectively. However, cell viability increased to 70.6% with 10 μm amiloride and 78.4% with 100 μm amiloride (p = 0.015). Conclusions: L-arginine increases intracellular cyclic-GMP and may give rise to retinal cells through this mechanism. In addition, amiloride in concentrations greater than 10 μm protects against L-arginine-induced retinal cell death.
