Objective To investigate changes in lumbar multifidus (LM) and deep lumbar stabilizing abdominal muscles (transverse abdominis [TrA] and obliquus internus [OI]) during transcutaneous neuromuscular electrical stimulation (NMES) of lumbar paraspinal L4-L5 regions using real-time ultrasound imaging (RUSI).Methods Lumbar paraspinal regions of 20 healthy physically active male volunteers were stimulated at 20, 50, and 80 Hz. Ultrasound images of the LM, TrA, OI, and obliquus externus (OE) were captured during stimulation at each frequency.Results The thicknesses of superficial LM and deep LM as measured by RUSI were greater during NMES than at rest for all three frequencies (p<0.05). The thicknesses in TrA, OI, and OE were also significantly greater during NMES of lumbar paraspinal regions than at rest (p<0.05).Conclusion The studied transcutaneous NMES of the lumbar paraspinal region significantly activated deep spinal stabilizing muscle (LM) and the abdominal lumbar stabilizing muscles TrA and OI as evidenced by RUSI. The findings of this study suggested that transcutaneous NMES might be useful for improving spinal stability and strength in patients having difficulty initiating contraction of these muscles.

Background and Objectives Pneumatic injection therapy (PIT) is a needle-free, penetrationenhanced, transdermal method of delivering drugs into the skin. This study evaluated topographically the PIT-induced transcutaneous infiltration of a 20% hypertonic glucose solution in fresh human cadavers and outlined the microscopic patterns and reactions to PIT with a 20% glucose solution in in vivo rat skin. Materials and Methods The penetration depths of a 20% glucose solution by PIT were analyzed macroscopically in 10 hemifaces from fresh cadavers. In addition, the infiltration patterns of PIT with a 20% glucose solution performed in vivo in rats were analyzed through a histology evaluation. Results Pneumatic injections of a hypertonic glucose solution generated PIT zones in the subcutaneous fat and superficial temporal fascia layers (10 out of 10 hemifaces), superficial layer of the deep temporal fascia (DTF; 8 out of 10 hemifaces), and deep layer of the DTF (7 out of 10 hemifaces). In addition, distinctive PIT zones were generated in rat tissues from the upper papillary deep to the muscle and fascia layers. Conclusion The data suggest that PIT-induced, jet infiltration of a 20% hypertonic glucose solution contributes to clinical improvement of skin laxity by stimulating the wound repair processes and increasing collagen production throughout multiple layers of the skin.

Objective: This study was conducted to investigate the effect of high-intensity laser therapy(HILT), transcutaneous electricalnerve stimulation(TENS), and ultrasound(US) treatment on pain, grip strength, and hand function in patients who had undergonecarpal tunnel syndrome surgery. Design: A randomized controlled trial. Methods: Thirty patients who had undergone carpal tunnel syndrome surgery were randomly assigned to receive either TENScombined with HILT, US combined with HILT, or only HILT as the control group. Treatments were applied around the surgicalsite, and pre- and post-treatment changes were evaluated. Pain was assessed using NPRS, hand symptoms using CTS-6 andBCTQ-SSS, grip strength with an electronic dynamometer, and hand function using BCTQ-FSS. Treatments were administeredseven times over two weeks. Results: The pain and symptoms were significantly reduced(p<0.05) and grip strength and hand function were significantlyincreased(p<0.05) after treatment compared to before treatment for all subjects. Pain was significantly reduced(p<0.05) and gripstrength was significantly increased(p<0.05) in the TESN+HILT group and US+HILT group compared to the Control group. Hand symptoms were significantly reduced(p<0.05) and hand function significantly increased(p<0.05) in the TENS+HILTgroup compared to the Control group. Conclusions: TENS combined with HILT was found to be more effective than US combined with HILT in reducing pain andsymptoms and improving grip strength and hand function in patients following carpal tunnel syndrome surgery. These findingssuggest that these treatment modalities can be beneficially applied in clinical practice.
