Objective To assess the intra-rater and inter-rater reliability for measuring tibial torsion measurements by a radiographic method using three-dimensional computed tomography reconstruction (3D-CT) and to compare the physical measures to those of 3D-CT. Method The study included 33 children who presented with intoeing gait. Tibial torsion was measured by 3D-CT. Distal reference point was the bimalleolar axis. Proximal reference points were the transtibial axis and posterior condylar axis. Physical measurements included thigh-foot angle (TFA) and bimalleolar angle (BMA). 3D-CT measurement and physical measurement were performed twice at both lower extremities by each rater. The intra-rater and inter-rater reliability were calculated by intraclass correlation coeffi ciency (ICC). Th e relationship between radiological and physical examination was calculated by Spearman correlation coefficient. Results The 3D-CT measures for tibial torsion were reliable within individual raters and between different raters. However, physical measures for tibial torsion were reliable within an individual rater but not reliable between raters. The 3D-CT measures by any proximal reference axis were more reliable within a rater and between raters than physical measurements. There was no signifi cant impact introduced by the selection of the proximal reference axis. The correlation coeffi ciency between 3D-CT and physical measurement methods was low. Conclusion Because the 3D-CT measurements for tibial torsion are more reliable than physical measurements, we recommend that accurate diagnosis of internal tibial torsion should be detected by using 3D-CT measurements. Also, considering the disadvantages of radiological measurements, physical measurement may be used for short term follow-up by same raters, as intra-rater reliability is relatively good.

Torsion of a mucocele of the vermiform appendix is an extremely rare condition and also a rare cause of an acute abdomen with a clinical presentation that is indistinguishable from acute appendicitis, and thus, the condition is diagnosed during operation. Here, the authors describe the case of a 78-year-old female, who presented with intermittent abdominal pain. The appendix had a pelvic position and the torsion was counterclockwise. In addition, the torsion was associated with mucocele of the appendix, which was considered a secondary factor of torsion. Appendectomy and drainage were performed.

Hydatids of Morgagni are benign, pedunculated cystic structures which originate from mu ¨llerian vestiges in the inferior aspect of the fallopian tube. They are usually asymptomatic unless torsion or ischemia occurs, which is rare. We report on a child case of a bilateral Hydatid of Morgagni with torsion. Hydatids of Morgagni는 양성질환이며 유경성(pedunculated), 낭성 구조물로서 난소술(fimbria) 근처의 나팔관아래위치인 mu ¨llerian vestiges에서 기원한다. 염전이나 허혈이 발생하기 이전에는 대게 임상적인 증상이 없다. Hydatid of Morgagni의 염전은 드물게 보고되는 질환으로 저자들은 소아에서 발생한 양측성 hydatid of Morgagni에 염전이 동반되었던 1예를 보고하고자 한다.

Torsion of a hernia sac is an extremely rare condition that presents as acute scrotum in children. We report a case of a 6-year-old boy who presented with an acute scrotum and was found during surgical exploration to have torsion of an indirect hernia sac associated with hydrocele. Upon scrotal exploration, deterioration of the scrotum due to inflammatory changes was found. A necrotic cyst was recognized within a communicating hydrocele of the scrotum and was twisted at an angle of about 360°. All urologists should be aware of this special condition in the differential diagnosis of acute scrotum.

We report a case of subserosal leiomyoma that was first detected by ultrasonography, and further confirmed as torsion of subserosal leiomyoma by laparoscopic surgery. Ovarian Torsion in a Postmenopausal Woman: A Case Report and Review of Literature

Postmenopausal ovarian masses are not uncommon. Most of them are malignant due to inflammation and adhesion with the surrounding structures; these masses are fixed and torsion is not a usual phenomenon. We present a 73-year-old postmenopausal woman, parity five and five living issues with an acute abdomen. The examination revealed a 28–30-week size abdominal pelvic mass, which was confirmed to be ovarian in origin, with characteristics that indicated torsion on ultrasound Doppler. She underwent an emergency laparotomy with a total abdominal hysterectomy and bilateral salpingo-ophrectomy. Histopathology confirmed the diagnosis of serous cystadenoma. Ovarian torsion is a crucial differential diagnosis in postmenopausal adnexal masses presenting with acute abdomen. A high index of suspicion is required to diagnose and promptly manage women to improve the outcome.
Omental Torsion and Infarction with Right Inguinal Hernia: A Case Report

Omental torsion and infarction are rare causes of acute abdominal pain, and it is often difficult to diagnose these conditions preoperatively. There are several reports showing that computed tomography is useful to diagnose these conditions. Here, we report a case of a 59-year-old man with omental infarction due to secondary omental torsion associated with an untreated inguinal hernia. Preoperative computed tomography showed a fatty mass in the right lower quadrant and a right inguinal hernia containing torsion of the greater omentum. He underwent laparoscopic partial omentectomy and herniorrhaphy, and the postoperative course was uneventful.
