        어린이 음료수가 레진관과 지르코니아 기성관의 색조에 미치는 영향

        The purpose of this study was to evaluate the effects of exposure to various children’s drinks on the color stability of anterior primary teeth and different esthetic restorative materials clinically used in pediatric dentistry. Exfoliated maxillary primary central incisors that had been unaffected by caries were chosen as control group. Maxillary primary central incisor-shaped specimens made of strip crown and Nu-smile zirconia crowns were chosen as test groups. Polished strip resin crowns were additionally prepared to compare with unpolished strip resin crown. Each specimen and teeth were divided into 4 groups in which the test samples (n=5 each) were immersed in 4 different drinks (distilled water, cola, grape juice, jelly juice) for 6 days. In all 4 drink groups, all specimens generally showed increasing ΔE * value (color difference) with time. Polished resin strip crown had higher ΔE * value than the unpolished in cola, grape juice and jelly juice groups. ΔE * value of zirconia crown in cola, grape juice and jelly juice groups were significantly different (p < 0.05). In conclusion, dietary control of children’s drinks is required for preventing discoloration of restorative materials. 이 연구는 유전치와 그 심미 수복에 사용되는 전장관의 어린이 음료수에 대한 색조 안정성을 평가하였다. 우식 및 변색 없이 자연탈락된 20개의 상악 우측 유중절치와 상악 우측 유중절치 모양의 resin strip crown 40개 및 Nu-smile zirconia crown 20개를 대상으로 하였다. 복합 레진의 연마 여부에 따른 색조 안정성을 비교하기 위해 40개의 resin strip crown 중 20개는 Sof-lex™ Contouring and Polishing Discs를 이용하여 연마하였다. 이 후 각각의 시편을 5개씩 나누어 증류수, 콜라, 포도주스, 젤리주스에 6일간 담갔다. 그 결과네 음료수 군에서 모든 시편의 색조 변화가 있었으며, 연마한 resin strip crown은 연마하지 않은 군에 비해 색조 변화가 컸고 콜라, 포도주스, 젤리주스에서 통계적으로 유의하였다(p < 0.05). Zirconia crown의 경우 포도주스와 콜라에서의 색조 변화 값은 통계적으로 유의하지 않았으나 젤리주스와 나머지 두 음료수 사이에서는 유의하였다(p < 0.05). 따라서 소아 유전치 수복에 사용하는 수복재료들도어린이 음료수에 노출될 경우 색조 변화가 있을 수 있으므로 어린이 음료수에 대한 식이 조절이 필요하다.

        유전치 기성 크라운의 형태 및 치질 삭제량 비교

        The purpose of this study was to obtain instructions for size selection of prefabricated crown and tooth reduction by 3-dimensional analysis of the size and shape of the maxillary primary central and lateral incisors and prefabricated crowns (celluloid strip, resin veneered stainless steel, and zirconia crowns). The maxillary primary central and lateral incisors of 300 Korean children was scanned with three types of prefabricated crown to create standard three-dimensional tooth models and prefabricated crowns. The shapes of the prefabricated crowns and natural teeth were compared according to four parameters (mesio-distal width, height, labio-palatal width, and labial surface curvature coefficient) and calculated the amount of tooth reduction required for each prefabricated crown. The size 2 resin veneered stainless steel crown, size 1 zirconia crown, and size 2 celluloid strip crown were most similar in shape to the primary central incisor. The size 3 rein veneered stainless steel crown, size 2 zirconia crown, and size 3 celluloid strip crown were most similar to the primary lateral incisor. The amount of tooth reduction was similar in both maxillary primary central and lateral incisors. The incisal reduction was greatest for the zirconia crown. At the proximal surface, the zirconia and celluloid strip crowns required a similar amount of tooth reduction, but more than the resin veneered stainless steel crown. The labial surface reduction was greatest for the zirconia crown. The degree of lingual surface reduction was not significant among the three prefabricated crowns. Among the assessment parameters, mesio-distal crown width was the most important for choosing a prefabricated crown closest to the actual size of the natural crown. 이 연구의 목적은 임상에서 많이 사용되고 있는 3종의 기성 크라운 즉, 레진관, 레진피복 금속관, 지르코니아 크라운의 크기와 형태를 3차원적으로 계측하여 자연치와 비교함으로써 기성 크라운의 크기 선택과 치질 삭제량에 대한 지침을 얻고자 하는데 있다. 300개의 유전치 석고모델과 3종의 기성 크라운을 대상으로 3D 스캐너와 컴퓨터 이미지 프로그램으로 3차원적 모델을 만들었다. 3 차원 모델에서 치관의 근원심 너비, 치관 높이, 치관 ratio, 순면의 곡률반경 등 4가지 평가 척도에 의해 각 기성 크라운별로 한국인 자연치 표준모형과 가장 유사한 형태를 고르고 이를 기준으로 치질 삭제량을 비교, 평가하였다. 유중절치의 경우 기성 크라운별로 레진피복 금속관은 2번, 지르코니아 크라운은 1번, 레진관은 2번이, 유측절치의 경우 레진피복 금속관은 3번, 지르코니아 크라운은 2번, 레진관은 3번이 가장 유사한 형태를 보였다. 치아 삭제량은 상악 유중절치와 유측절치에서 같은 양상을 보였다. 절단연에서는 지르코니아 크라운의 삭제량이 가장 컸다. 인접면은 지르코니아 크라운과 레진관이 비슷한 삭제량을 보였으며 레진피복 금속관에 비해서 컸다. 순면 역시 지르코니아 크라운이 유중절치와 유측절치에서 삭제량이 가장 컸다. 설면의 경우 순면에 비해 전체적으로 삭제량이 적었으며 3종류의 기성 크라운 간에 삭제량차이도 크지 않았다. 크라운의 형태를 비교하기 위한 4가지 평가 척도 중 치관의 근원심 너비가 기성 크라운의 크기를 선택하는데 가장 중요한 척도로나타났다.

        COMPARATIVE STUDY ON THE FRACTURE STRENGTH OF METAL-CERAMIC VERSUS COMPOSITE RESIN-VENEERED METAL CROWNS IN CEMENT-RETAINED IMPLANT-SUPPORTED CROWNS UNDER VERTICAL COMPRESSIVE LOAD

        Statement of problem. Fracture of the tooth-colored superstructure material is one of the main prosthetic complications in implant-supported prostheses. Purpose. The purpose of this in vitro study was to compare the fracture strength between the cement-retained implant-supported metal-ceramic crowns and the indirect composite resinveneered metal crowns under the vertical compressive load. Material and methods. Standard implants of external type (AVANA IFR 415 Pre-mount; Osstem Co., Busan, Korea) were embedded in stainless steel blocks perpendicular to their long axis. Customized abutments were fabricated using plastic UCLA abutments (Esthetic plastic cylinder; Osstem Co., Busan, Korea). Thirty standardized copings were cast with non-precious metal (Rexillium III, Pentron, Walling ford, Conn., USA). Copings were divided into two groups of 15 specimens each (n = 15). For Group I specimens, metal-ceramic crowns were fabricated. For Group II specimens, composite resin-veneered (Sinfony, 3M-ESPE, St. Paul, MN, USA) metal crowns (Sinfony-veneered crowns) were fabricated according to manufacturer's instructions. All crowns were temporary cemented and vertically loaded with an Instron universal testing machine (Instron 3366, Instron Corp., Norwood, MA, USA). The maximum load value (N) at the moment of complete failure was recorded and all data were statistically analyzed by independent sample t-test at the significance level of 0.05. The modes of failure were also investigated with visual analysis. Results. The fracture strength of Sinfony-veneered crowns ($2292.7{\pm}576.0N$) was significantly greater than that of metal-ceramic crowns ($1150.6{\pm}268.2N$) (P < 0.05). With regard to the failure mode, Sinfony-veneered crowns exhibited adhesive failure, while metal-ceramic crowns tended to fracture in a manner that resulted in combined failure. Conclusion. Sinfony-veneered crowns demonstrated a significantly higher fracture strength than that of metal-ceramic crowns in cement-retained implant-supported prostheses.

        치과 치료학에서 적용되는 접합기술 연구 ; 스테인리스강 크라운에 접합된 직접용 콤포짓트 레진과 간접용 콤포짓트 레진의 전단결합강도 비교

        This study was performed to compare the shear strength of the bondings between stainless steel crown/direct type composite resin and stainless steel crown/indirect type composite resin. Four groups of bonding conditions were prepared. Two groups of bonding conditions were made by the indirect type composite resin system and the other two groups were made by the direct type composite resin system. The shear strength tests were carried out using universal testing machine, Model 4465 of Instron Co.. It was indicated that the bond strength values of the indirect type composite resins were higher than those of the direct type composite resins. TE-SE group was superior to the TE-ONE in indirect type resin system. These results were thought to be the high degree of the polymerization accompanied with temperature and pressure of the resin of indirect type resin. It was also found that indirect composite resin contains less amount of porosity in resin.

        Fracture resistance of crown-root fractured teeth repairedD with dual-cured composite resin and horizontal posts

        The purpose of this study was to investigate the fracture resistance of crown-root fractured teeth repaired with dual-cured composite resin and horizontal posts. 48 extracted human premolars were assigned to control group and three experimental groups. Complete crown-root fractures were experimentally induced in all control and experimental teeth. In the control group. the teeth (n=12) were bonded with resin cement and endodontically treated. Thereafter, the access cavities were sealed with dual-cured composite resin. In composite resin core-post group (n=12), the teeth were endodontically treated and access cavities were sealed with dual-cured composite resin. In addition, the fractured segments in this group were fixed using horizontal posts. In composite resin core group (n=12), the teeth were endodontically treated and the access cavities were filled with dual-cured composite resin without horizontal posts. In bonded amalgam group (n = 12), the teeth were endodontically treated and the access cavities were sealed with bonded amalgam. Experimental complete crown-root fractures were induced again on repaired control and experimental teeth. The ratio of fracture resistance to original fracture resistance was analyzed with Kruskal-Wallis test. The results showed that teeth in control and composite resin core-post group showed significantly higher resistance to re-fracture than those in amalgam core group (p < 0.05). The resistance to refracture was high in the order of composite resin - post group, control group, composite resin group and bonded amalgam group. Within the scope of this study, the use of horizontal post could be beneficial in increasing the fracture resistance of previously fractured teeth. 이 연구의 목적은 이중 중합복합레진과 수평 포스트를 이용하여 수복한 치관-치근 복합파절 치아의 수직파절 저항성을 알아보기 위한 것이다. 48개의 발치된 사람의 소구치에 실험적으로 치관-치근 복합파절을 일으킨 후 대조군 및 3개의 실험군에 (n=12) 각각 할당하였다. 대조군에서는 치아 파절편을 레진시멘트를 이용하여 접착한 후 근관치료를 시행하고, 근관와동을 이중중합 복합레진을 사용하여 충전하였다. 실험군에서는, 각각의 치아에 근관치료를 시행한 후 레진-포스트 군에서는 근관와동을 수평 포스트 및 이중중합 복합레진을사용하여 충전하였다. 레진 군에서는 근관와동을 포스트 없이 이중중합 복합레진만 사용하여 수복하였으며 아말감 코어 군에서는 근관와동을 접착 아말감을 사용하여 수복하였다. 이렇게 수복된 치아를 처음과 같은 방법으로 다시 치관-치근 복합파절을 유도하여 이때의 파절 저항성을 기록하고 이 값과 처음 파절을 유도하였을 때의 파절 저항성의 비를 계산하고 각 군별로 차이가 있는 지 검증하기 위해 Kruskal-Wallis test를 시행하였다. 수복된 치아의 파절저항성은 레진-포스트군, 대조군, 레진군, 접착 아말감 순으로 높았으며, 이 실험결과로 볼 때,치관-치근 복합파절 치아의 수복 시 수평 포스트를 사용하는 것이 재파절 방지를 위해 효과적 일 것으로 사료된다.

        다양한 조성의 3D 프린팅 임시치관용 레진의 물성 비교

        Conventional dental resins for crown and bridge fulfill ISO 10477 and ISO 10993 before clinical application. Although 3D printing or rapid prototyping (RP) for the fabrication of temporary crown and bridge have been proposed, a little studies were reported for 3D printing resin for temporary crown and bridge. The purpose of this study was to evaluate the physical properties (such as water sorption and solubility, color stability and flexural strength) of the various 3D printing temporary crown and bridge resin following the ISO 10477:2018 and estimate the effect of chemical composition of resin on the physical properties. Four resins approved by KFDA and 4 experimental resins developed by different manufacturer were tested in this study. Samples were prepared with DLP typed 3D Printer (G-Printer) and post-cured using UV-light Cure Unit (Cure M). Proper 3D printing and post-curing conditions were selected for different 3D printing resins. Each test was performed according to the ISO 10477 and results were statistically analyzed using Tukey-multiple comparison test (p=0.05). Only group-B did not satisfied the ISO requirement (< 40 μg/ mm³) in water sorption test, but 3 groups (C, E and F) showed high solubility which exceeds the ISO requirement (< 7.5 μg/mm³). For color stability, the color difference were detected at 5 groups (A, E, F, G and H). Group-D and F showed the low flexural strength and some building direction did not satisfied the ISO requirement (> 60 MPa). Resin components may affect the flexural strength, then user should check the components of 3D printing resin. Deficient degree of polymerization may lead to large water sorption, water solubility and color changes. Further study should be done comparison between specimens printed with specific 3D printer recommended by manufacturer and specimens used in this study.

      • 치과 진료시에 적용되는 접합기술 연구(I) : 치과용 스테인리스강 크라운에 접합된 직접용 Composite Resin과 간접용 Composite Resin의 미세조직 비교 관찰

        This study was performed to compare the microstructures of the bondings of stainless steel crown/direct type composite resin with stainless steel crown/indirect type composite resin. Four groups of bonding conditions were prepared. Two groups of bonding conditions were made by the indirect type composite resin system(TE-SE) and the other two groups were made by the direct type composite resin system(TE-ONE). The extensive microstructural characterization was carried out using stereoscope, optical and scanning electron microscope. The microstructural evaluation indicated that there were sound and unsound bonds for TE-SE group. While there was no unsound bond for the TE-ONE group in indirect type composite resin system. The unsound bond for the TE-SE resin might be the reason for the thicker application of primer bond. These unsound bond might affect the poor bonding properties. It was also found that indirect composite resin contains less amount of pores in resin.

        코어 형태와 시멘트 종류에 따른 전부주조금관의 유지력에 관한 연구

        The purpose of this study was to compare the retention of complete cast crown over amalgam ores, composite resin cores, and cast gold cores when cemented with three different luting agents. Eighteen core specimens each of amalgam(Bestaloy, Dong Myung, Seoul, Korea), composite resin (Z100, 3M Dental product, st. Paul, Minn) and type IV gold alloy (Ba-4, Heesung Engelhard Corp., Korea) were made in a customized milling stainless steel die. A wax pattern with a loop attached to occlusal surface was made for each core and a type II gold alloy casting was fabricated. The castings which had clinically acceptable marginal fit were used as test samples. The following luting cements were used to cement cast crowns on each core material : (1) zinc phosphate cement (Confi-dental Products Co., USA) (2) glass-ionomer cement (Fuji Plus, GC Industrial Corp., Tokyo, Japan) (3) resin cement (Panavia 21, Kuraray Co., USA). All cements were mixed according to manufacturers' instructions. A static load of 5kg was then applied for 10 minutes on the crowns. All specimens were stored in saline solution for 24 hours at $37^{\circ}C$ and thermocycled for 500 cycles. After storage and cycling, the tensile bond strengths were measured by using a universal testing machine (Instron Corp., Canton, Mass.) at a crosshead speed of 0.5mm/min. The results were as follows 1. The retentive strength of resin cement was the highest of alt three types of cement for resin core (p<0.05). 2. There was no statistical difference among the retentive strengths of three cements for amalgam core (p>0.05). 3. The retentive strength of resin cement was higher than that of zinc phosphate for cast core, but there was no difference between the retentive strength of glass ionomer cement and those of rein and zinc phosphate cement. 4. The retentive strength of the zinc phosphate cement for amalgam core was the highest of all type of cores.

        Indirect short resin post Crown을 이용한 유전치부 심미수복

        Children who have severely destructive anterior primary dentition, as in nursing bottle caries, in trauma, in rampant caries and in developmental defects. present the dentist with one of the most perplexing situations in dentistry. Especially. children with managed behaver difficultly is very severly situations. This paper reported a new technique for the utilization of resin post and strip crown to indirect methods on severely destructive primary anterior teeth. We name it "indirect short resin post crown" in this paper. Indurect technique be considered to be a simple, a retentive, retentive, intensive, color-stable and esthetic restoration.

        자가중합 임시치관용 레진의 표면 처리에 따른 색 안정성

        이번 연구는 자가중합 임시치관용 레진의 표면 처리와 음식에 따른 색 안정성 확인을 위해 설계되었으며 ethyl methacrylate형의 SNAP 시편에 denture bur polishing, silicone bur polishing, pumice polishing의 3단계 표면 처리 후 커피와 콜라, 된장과 고추장, 레드 와인과 맥주에 침전한 블럭을 분광광도계로 관찰하여 초기 값과 비교한 ΔE*값을 알아보았다. 모든 시편에서 침전 시간과 표면 처리에 따라 나타난 착색의 차이는 시간의 변화에 따라 색 변화가 관찰되었다(p<0.001). 커피는 1일(2.59±1.08), 5일(3.89±0.83), 7일 (4.53±0.22) 모두 pumice polishing이 색 변화가 가장 많았다. 콜라는 1일(2.53±0.55)과 5일(4.75±0.94) pumice polishing이색 변화가 가장 많았고, 7일(5.28±0.25)은 denture bur polishing이 색 변화가 가장 많았다. 고추장은 1일(6.38±0.61)과 5일(10.4±7.58) silicone bur polishing이 색변화가 가장 많았고 7일(12.44±2.82)은 denture bur polishing이 색 변화가 가장 많았다. 반면 pumice polishing에서는 1일(6.09±1.03), 5일(4.9±2.37), 7일(5.33±0.64) 모두 색 변화가 가장 적게 나타났다. 된장은 1일(1.37±0.35)과 7일(4.98±0.55)은 pumice polishing이 색 변화가 많았으며, 5일(2.61±0.38)은 denture bur polishing이 색 변화가 가장 많았다. 레드 와인은 denture bur polishing이 1일(4.78±1.10), 5일(12.69±3.06), 7일(13.48±2.08) 모두 색 변화가 가장 많았고, pumice polishing에서 1일(2.66±0.39), 5일(8.56± 0.16), 7일(8.77±0.22) 모두 색 변화가 가장 적게 나타났다, 맥주는 pumice polishing이 1일(4.15±0.47), 5일(4.12±0.37), 7일(4.53±0.89) 모두 색 변화가 가장 많았으며 반면 silicone bur polishing이 1일(1.27±1.1), 5일(2.3±0.32), 7일(2.48±0.46) 모두 색 변화가 가장 적게 나타났다. 이와 같이 자가중합 레진의 임시치관 색 안정성 제공을 위해 음식 섭취에 대한 주의가 필요하다. 선호 음식의 종류에 따라 표면처리 방법을 선택할 수 있으며, 장기간의 임시치관 사용 시 주기적인 재제작도 고려할 필요가 있겠다. In this study, the surface treatment of a self-cured temporary crown was polished using a denture bur, silicone bur, or pumice. The color stability of the temporary crown resin specimen was evaluated by immersing it in coffee, and cola, wine, beer, red pepper paste, or soybean paste. Two-hundred eighty-five identical resin specimens with six types of staining solution and three types of surface treatment were placed in a shaking incubator at 37oC. The degree of discoloration was observed using a time-lapse recording of days 1, 5, and 7. L*, a*, and b* were measured using a spectrophotometer, which shows the quantitative value of discoloration, and statistically processed after calculating ΔE*. The results show that as time passed, all the specimens showed a color change (p<0.001). The amount of color change was the greatest in in crowns with denture bur polishing on the day 1, 5, and 7. As the precipitation time increased, the ΔE* value was also increased. Of the specimens immersed on day 1, the greatest color change in crowns polished with denture bur was observed in those immersed in red pepper paste, while the smallest color change was observed in those immersed in cola. On days 5 and 7, the greatest color change in crowns polished with denture bur was observed in those immersed in red wine. Crowns polished with silicone bur and immersed in soybean paste exhibited the smallest color change. Based on the results, compared to pumice polishing, silicone bur polishing results in better color stability, saves time and money, and is recommended for patients with temporary crowns.
