        방사선사 직무수행을 위한 방사선사 단독법 제정에 대한 요구도

        In order to provide high-quality medical services to the public and contribute to the improvement of public health, it is necessary to enact an independent law according to the work of radiological technologists. Therefore, this study intends to review the regulations related to radiographers in the Medical Service Technologists, etc. Act. and to present opinions and directions for enactment of individual laws for radiological technologists. An online survey was conducted to 15,000 radiological technologists working in medical institutions and education sites in Korea; 1,027 people (6.85%) responded. The questionnaire consisted of 3 questions on demographic characteristics, 5 questions on the scope of work, and 12 questions on the revision of the Medical Service Technologists, etc. Act. and the establishment of the Radiological Technologist Independent Act. Reliability and factor analysis were performed on 9 questions measured on a Likert 5-point scale in “Revision of the Medical Service Technologists, etc. Act. and the establishment of the Radiological echnologist Independent Act” among the questionnaire items. Reliability for the total 9 questions was Chronbach α=0.728. There was a high perception that the regulations related to radiological technologists were insufficient in the current Medical Service Technologists, etc. Act., and the perception that examinations performed by radiological technologists at medical institutions were included in medical practice was high. If the Radiological Technologist Independent Act is enforced, a high percentage of respondents said that they could receive legal protection through the institutionalization of the scope of work, that the status of radiological technologists would be improved, and the scope of work would be expanded. The response that the scope of work of radiological technologists should be included was the highest at 96.6%. In the analysis according to demographic characteristics, it was found that 96.7% of the respondents were agreed regardless of the factors. Radiological technologists will have to work hard to secure the public health by coping with new radiology devices, procedures and treatment methods. Therefore, as the results of this study, it is expected that the enactment and implementation of the Radiological Technologist Independent Act will contribute to the improvement of the quality of treatment for patients and to the public health.

        Restructuring Enrollment Quotas of College for Radiological Technologists

        본 논문에서는 보건의료인의 한 직종인 방사선사의 수요와 공급관련 문헌을 조사하여 방사선사를 양성하는 대학과 전문대학의 방사선(학)과의 입학정원 조정방안을 제시하였다. 오영호 등이 연구한 보건의료인력 중·장기 수급추계연구에서 방사선사는 2015년에 1,134명 ~ 2,052명의 공급과잉 현상이 전망되고, 2030 년에는 12,632명 ~ 14,196명의 공급이 과잉될 것으로 전망되었다. 방사선사 공급과잉문제를 해결하기 위해서는 방사선사 교육기관의 정원 외 신입생을 5%이하로 모집하여 방사선사 응시자를 308명까지 감축하고,3년제 방사선사 양성 교육기관을 4년제 교육기관으로 학제를 일원화하여 매년 방사선사 응시자 387명을 감축하고, 가칭 한국방사선사평가원을 설립하여 대학과 전문대학의 교육여건을 평가하고 최소한의 수준에 미달하는 대학의 방사선사 응시자격을 제한하여야 한다. 이를 위해서는 방사선사 관련단체뿐만 아니라 관련된 보건의료인 단체와의 협력이 필요하다. This study presented a mediation strategy of admittance quota of four year and three year colleges raising radiology technicians by investigating the demand and supply related literature for Radiological Technologists, a type of health and medical services personnel. Oh Yeong‐ho et al.’s study on mid and long term supply system for health and medical services personnel resources, excess supply of 1,134~2,052 Radiological Technologists in 2015 was predicted and excess supply of 12,632~14,196 Radiological Technologists are to be expected in 2030. To settle excess supply of Radiological Technologists, Supernumerary quota over the entrance students of institutions raising Radiological Technologists should be less than 5% to reduce applicants for Radiological Technologists to be 308 people, reduce 387 applicants for radiology technicians every year by unifying three year colleges raising Radiological Technologists with four year colleges into education period unification, evaluate education conditions of colleges to restrict those colleges lacking the minimum requirements from getting eligibility for take the examination for Radiological Technologists by establishing tentatively ‘Korean Institute of Radiologic Education and Evaluation.’ For this, it is necessary to obtain cooperation from health and medical services personnel organizations in pertinent fields as well as related organizations of Radiological Technologists.

        방사선사법 제정의 필요성

        According to the changes of the medical environment of the times, it is necessary to discuss the issues of the doctor s medical guidance and to conduct continuous research so that alternatives can be prepared systematically. Furthermore, in order to enhance the professionalism of radiological technologists and to develop the medical technician system, the new Radiological Technologist Independent Act has been established, which contains the overall contents of the scope of work, professional qualifications, and specialized education of radiological technologists, and provides quality medical services to patients through professional procedures and treatment. In order to increase the level of medical care, the purpose, definition, mission, role, and scope of work specified in the Medical Act, Medical Service Technologists, etc. Act, the Enforcement Decree, and the Enforcement Rules were variously analyzed and new directions were presented. First, the definition of a medical technician should use a generic term so that the factors of conflict and prejudice could be resolved. Second, change the doctor s guide to doctor s prescription; and then legislate the authority to sign and write medical records after examination by radiological technologists, thereby prohibiting unlicensed technicians that seriously endanger patient safety. Third, an accurate definition of radiological technologists roles should be established; not only selection and management of radiological technologists work but also procedures and treatment for each radiology field should be specified to suit the current medical system. Fourth, a professional radiological technologists qualification system and a specialized education system should be established in order to secure human resources that could provide patients trust in procedures and treatment based on professional knowledge and experience in the field of radiology. Fifth, the Education and Evaluation Institute should be operated in Korea education system to educate the professional knowledge and competency for students. In addition, it is necessary to in-depth analysis of foreign cases could be applied to the medical system and education system in Korea; it could strive to nurture systematic human resources.

        의료기사 등에 관한 법률에서 ‘의사 또는 치과의사의 지도’ 문구에 대한 법률 개정 요구도

        The purpose of this study is to check the extent to which “instruction of physician or dentist” defined in the Medical Service Technologists, etc. Act is applied in relation to radiography examination procedures for radiological technologists. In addition, it is intended to present basic data on the requirement to revise the Medical Service Technologists, etc. Act in the radiological technologist s duty area and scope of work, The subjects of this study were radiological technologists with license, and the response data were collected after sending the questionnaire link written on the online questionnaire form. The final number of respondents were 1,018, and the response rate was 6.8%. Most of the negative responses were “I have never received ‘instruction’ for radiologic examination by a physician or dentist, including a radiologist in a medical environment.” There were a high perception that “the professionalism in radiation examination on radiological technologists are higher than that of a physician or dentist.” They answered that the current continuing education has a great impact on maintaining and continuing professionalism and learning new knowledge in the radiology field. In addition, the radiological technologists provide a very high level of education in areas related to radiography procedure ethics such as patient care, patient safety, and patient privacy protection, as well as specialized fields such as radiation-related examination methods, radiography examination dose, and patient exposure dose. Radiological technologists replied that they were receiving it consistently. In conclusion, in the current medical environment, the ‘instruction’ of a physician or dentist cannot be seen as being realistically performed. The phrase ‘instruction’ of a physician or dentist as defined in the Medical Service Technologists, etc. Act is considered inappropriate in respect of the fact that the state recognizes the qualifications of the medical service technologist through a license. It is thought that revision to a new term suitable for the current medical environment is necessary.

        건강권과 방사선사법 제정에 관한 고찰

          「의료기사 등에 관한 법률」에는 다수 직종을 총괄하여 규정하고 있어 방사선사 업무의 전문성 향상과 방사선의료기술에 대한 국민의 건강권을 보장하기 어렵다. 따라서 방사선사에 관련된 법령을 개별적으로 제정하는 것이 요청된다. 개별법으로서 방사선사법은 궁극적으로 국민의 건강증진을 목적으로 하는 방사선사의 책임규제에 관한 법으로 제정되어야 한다. 이를 위해 전문방사선사에 대한 규정을 신설하고 보수교육과 면허관리제도의 강화를 통하여 국민들이 보다 수준 높은 양질의 방사선의료기술을 제공받을 수 있도록 하여야 한다. 또한 방사선사의 역할과 업무를 명확히 규정하여 국민에게 제공되는 의료방사선서비스의 질적 수준을 확보하여야 한다. 정부기관에는 의료방사선정책심의위원회를 두어 의료방사선의 안전관리와 방사선사 인력의 수급, 기타 의료방사선정책에 관한 중요한 사항이 검토되고, 심의되어야 한다. 그밖에 방사선조사선량의 기록, 관리를 통하여 방사선피폭으로부터 국민의 건강보호를 위한 규정도 필요하며, 팀의료의 일원으로서 방사선사의 지위가 보장되어야 한다.   There are the Medical Radiation Health and Safety Act(the Patient Radiation Health and Safety Act, the Radiologic Technologist Act), the Medical Laboratory Technologist Act, the Physical Therapy Practice Act, and the Dental Hygienist Act, etc in America. However, Korea has only one Act for a medical radiologic technologist(including radiation therapy technologist, nuclear medicine technologist), medical laboratory technologist, physical therapist, occupational therapy examiner, dental hygienist, and so on. It is the Medical Technologist Act. Therefore, the Medical Radiation Health and Safety Act for a radiologic technologist(including radiation therapy technologist, nuclear medicine technologist) has to be enacted independently in Korea.<BR>  It is the purpose of this Act to provide for the appropriate certification of persons using radioactive materials, equipment emitting ionizing radiation on humans or performing medical imaging for diagnostic and therapeutic purposes. In Korea, the radiologic technologist is a “fusion technologist” who is a person other than a licensed practitioner as a radiographer, radiation therapist, nuclear medicine technologist, computed tomography technologist, magnetic resonance technologist, mammographer, sonographer, medical dosimetrist, quality management technologist, etc.<BR>  This Act will have some provisions related to the definitions, reserved title, scope of practice, specialized technologist, application for licensure, radiologic technology council, renewal, continuing education , the radiation control advisory commission, etc.<BR>  This Act will ensure that quality radiation therapy treatments are delivered and that quality diagnostic information is presented for interpretation, which will lead to accurate diagnosis, treatment and cure.<BR>  Accurate diagnosis can be provided only when a personnel is properly educated in technique, equipment operation and radiation safety. In the end, this Act will protect the civil right to health.<BR>  By regulating the personnel responsible for performing those procedures, this Act will mean improved care for patients-higher quality images, improved accuracy, and less exposure to radiation.

        방사선검사의 의무기록에 관한 요구도 분석

        The purpose of this study was to discuss the required items and feasibility of medical records of radiological examinations performed by radiological technologists at medical institutions. An online survey was conducted to a total of 10,000 radiation-related workers, of which 1,026 (10.3%) responded. As a research method, self-made questionnaires were used. The online survey was conducted from September 10 to September 20, 2021 for the survey period. For response data, a Chi-square test was performed according to demographic characteristics using SPSS 27.0 version (IBM Inc., Chicago, Ill, USA), and it was judged to be significant when the P value was less than 0.05. The reliability of the questionnaire response was found to be Chronbach α=0.933. More than 90% of the medical records related to radiological examinations are necessary, and they answered that a curriculum, remuneration curriculum, and legal system for medical records should be prepared. More than 90% of the respondents agreed with the proposal of the Radiological Technologist Independent Act for legal preparation, and most of the information required for medical records is currently recorded in DICOM images. According to the demographic characteristics, the medical record requirement for radiological examination, curriculum, continuing education, and legislation were found to be higher with higher education and higher with longer working experience. In addition, most of the radiology departments showed a high demand for medical records, so most of them responded positively to the medical records requirements for radiological examinations. This study analyzed the medical record requirements for radiological examinations, and as shown in the results, medical record requirements for radiological examinations was found that most radiological technologists felt need for the new law and supported it. In addition, if the information recorded in the DICOM image is used, it is considered that medical records could be easily prepared without additional work by the radiological technologists.

        A Scope of Work of Radiological Technologists for Ultrasound Examinations

        한국에 초음파사 자격제도는 없다. 그러나「의료기사 등에 관한 법률」에 방사선사가 초음파 진단기를 취급할 수 있도록 규정하고 있다. 따라서 방사선사가 초음파검사를 시행하는데 허용되는 업무 범위와 한계에 대하여 논란이 있다. 이에 한국 보건복지부의 유권해석과 행정심판 재결, 법원의 판결을 분석해 보았다. 그 결과 유권해석에서는 방사선사가 초음파영상검사를 수행할 때 의사가 방사선사의 촬영영상을 동시에 보면서 실시간으로 진단과 구체적인 지도가 이루어질 수 있다면, 물리적으로 동일한 공간에 있어야 한다는 의미로서 입회 또는 기타 의료기술을 활용한 “의사의 실시간 지도하에 방사선사의 초음파검사 및 촬영”은 가능하며, 이 경우 의사의 지도는 방사선사와 1:1로 이루어져야만 한다’고 풀이하고 있다. 행정심판에서는 초음파 관련 장비의 취급은 방사선사의 업무라고 재결하였다. 법원에서는 의사의 구체적인 지휘·감독 없이 방사선사가 단독으로 초음파검사를 시행하고, 방사선사가 초음파검사에 대하여 의학적 판단을 하는 것은 위법이라고 판결하였다. 그러나 미국, 캐나다, 호주, 일본 등에서는 초음파사(sonographer, 超音波検査士)가 의사의 처방에 따라 독자적으로 환자에 대한 초음파검사를 하고, 그 결과에 대한 예비소견을 작성하여 의사에게 전달하고 있다. 따라서 한국에서도 미국 등과 같이 초음파검사를 시행하는 방사선사가 실시간 지도를 받지 않고 의사의 처방에 따라 초음파검사를 수행할 수 있도록 제도적 개선이 요구된다. There is no qualification system for sonographers in Korea. But the MEDICAL SERVICE TECHNOLOGISTS, ETC. ACT stipulates that radiological technologists can handle ultrasound equipment. However, there is controversy about the scope of the work allowed for radiological technologists to perform ultrasound examinations. Accordingly, the authoritative interpretation of the Ministry of Health and Welfare of Korea, the adjudication of administrative judgment, and the judgment of the courts were analyzed. As a result, the authoritative interpretation expresses that when a radiological technologist performs an ultrasound examination, a diagnosis and specific guidance should be made in real time while the doctor simultaneously watches the radiological technologist s images. In the adjudication of administrative judgment, it was decided that the handling of ultrasound-related equipment was the work of the radiological technologist. The court ruled that it was illegal for a radiological technologist to make a medical judgment on ultrasound examination. In the United States, Canada, etc., the sonographer independently conducts ultrasound examination according to the doctor s prescription, prepares a summary of what they saw and this is passed on to the doctor. Therefore, in Korea, there is a need for institutional improvement so that radiological technologists can perform ultrasound examinations according to doctors prescriptions without real-time guidance.

        자기공명영상장치의 정도관리에 관한 연구

        의료장비의 정도관리는 시스템 성능평가와 시스템의 유지?보수를 지속적으로 수행하고 다양한 측정 및 평가하여 의료정보의 정확성을 나타내기 위해 필요하여 한국의료영상품질관리원은 특수의료장비의 품질관리 교육은 매년 정기적으로 실시하고 있다. 하지만 품질관리교육은 영상의학과 전문의 위주로 행해지고 있으며 의료기관에서도 영상의학과 전문의와 일부 방사선사에게 교육의 기회가 주어지지만 이차적인 내부교육의 실시 또는 내용 전달 등이 미비하여 장비를 직접 다루는 방사선사들은 잘 모르거나 교육을 이수하였더라도 정확히 모르고 있다. 본 연구는 2008년 대한자기공명기술학회에 등록된 전국의 방사선사를 대상으로 설문을 실시하여 2008년 5월부터 9월까지 5개월 동안 자료를 수집하여 분석하였다. 500명을 대상으로 우편 설문조사를 실시하였으며, 설문응답률은 6%(n=180)이었다. 본 연구 결과는 품질관리검사의 필요성은 86.7%가 필요하다고 느끼지만 특수의료장비 관리자 교육이수는 27.8%밖에 되지 않았고, 품질관리검사를 숙지하는 사람도 53.9%로 거의 절반 밖에 되지 않았다. 품질관리교육의 이수는 성별, 나이, 근무지의 규모, 근무지의 방사선사 인원, MR실내의 방사선사 인원, 방사선사 경력 및 MR실내의 경력과 병원에 의공학과의 유무와 상관관계는 없었다. 한국의료영상품질관리원에서 실시하는 품질관리교육 이수자 중 78.0%가 검사숙지를 하고 있는 것(p<0.05)에 비교하여 병원자체의 교육은 43.9%의 병원이 하고 있었지만 검사 숙지자는 54.4%에 이르기 때문에 병원자체교육은 검사숙지에 큰 영향을 미치지 못하였다. 이는 교육의 내용, 교육의 방법, 교육이수에 대한 전반적인 환경요인이 품질관리교육에 얼마나 중요한지를 알 수가 있다. The quality control is needed to ensure the accuracy of medical information and achieved by evaluating the performance of and maintaining the system and practicing various measurements and evaluations. The Korean Institute for Accreditation of Medical Image, therefore, have held educational program for quality control of special medical equipments. The major of programs participants, however, are radiology specialists with only small number of radiological technologists from some hospitals, furthermore, the follow-up education and the share of information between participants and non-participants are insufficient in general, thus, the knowledge level of radiological technologists, regardless of their participation, is relatively low. This study carried out the questionnaire research for the 500 radiological technologists registered in Korean Society of MRI Technology, on the basis of 2008, and performed analysis for five months from May to Oct., 2008. The questionnaires were delivered by post to each radiological technologists and the response rate was 36%(n=180). The results of this revealed that the 86.7% of respondents felt the necessity of inspection on quality management, while only the 27.8% completed the educational program for manager of special medical equipment. and only the half(53.9%) had the knowledge about inspection on quality management. The completion of educational program had no correlations with sex, age, size of occupying hospital, the number of radiological technologists in occupying site and MRI laboratory, career year of general radiologist and in MRI laboratory, and the presence of biomedical engineering department in occupying hospital. The 78.0% of participants at the educational program for quality management held by the Korean Institute for Accreditation of Medical Image had the knowledge about inspection on quality management(p<.05) whereas the 43.9% of the hospitals held such program and the 54.4% of radiological technologists from those hospitals had related knowledge, which indicated that such programs held by hospitals had not effects on the knowledge level of radiological technologists. This indicates also that the contents, methods, and other conditional factors of educational programs are important for the outcome of them.

        The purpose of this study is to investigate various opinions on changes and revisions in the Medical Technician Act, to compare the curriculum of radiological technologist and physicians or dentists, and to compare the definitions and scope of work of radiological technologist in Korea and abroad. From the information, The goal is to review whether the phrase ‘guidance of a doctor or dentist’ specified in the definition of the ‘Act on Medical Technicians, etc.’ is realistically appropriate. radiological technologist receive specialized college education on radioligical science & medical imaging. The training hours for radiolgical science student are greater than medical students. In addition, radiological technologists are continuously developing their competencies for new knowledge and skills through continuing education in clinical fields. In particular, radiological technologist are making steady research efforts to reduce patient exposure and improve medical image quality. As a result of this investigation, it is considered that the term “guided by a doctor or dentist” as currently defined in the ‘Act on Medical Technicians, etc.’ may need to be revised in consideration of the professionalism of the radiological technologist.

        방사선사의 직무 스트레스 측정도구 개발을 위한 요인별 신뢰도 분석

        The present study is designed to analyze reliability and validity of the instruments used to measure job stress of radiological technologists mainly by means of the questionnaires from 'Korean edition of NIOSH check list'. The subjects of this study are 890 radiological technologists active at 44 general hospitals in 16 provinces and cities across the country. The item-total reliability revealed that 10 factors in 6 are the major causes of stress in job life of radiological technologists. The measuring instrument proved to be valid with reliability coefficient of internal consistency by factors being more than 0.7. The factors found to be applicable herewith include limit and authority of job (0.73), cohesion within departments (0.86), resources control (0.81), mental requirements (0.85), job load (0.82), and job stability (0.72). When job autonomy item of causes are modified for proper use to radiological technologists, the measuring instruments are expected to show high reliability. It seems, however, necessary to develop a measuring instrument to analyze and use the validity of related measuring instruments since the reliability here was rated low, with statistical coefficients lower than 0.7 in such factors as job management (0.57), conflicts within departments (-1.13), sense of responsibility for patients (0.66) and usage of education (0.26). As this study was intended to examine reliability and validity of the concepts related to measurement of job stress on the part of radiological technologists, it nay not be proper to apply the results of this study to general use, but is advised to utilize them as basis for developing instruments to verify reliability and validity by comparing with and analyzing other measuring instruments.
