        PC를 이용한 자기공명분광 신호처리분석 시스템 개발: 1.5T MR Spectroscopy에서의 정상인 뇌 분광 신호

        목적 자기공명 스펙트럼 데이터의 처리 및 분석을 특정 workstation이 아닌 일반 PC의 windows 운영체제에서 동작할 수 있도록 GUI(Graphical User Interface)기반의 Spectroscopy 분석용 도구를 개발하였다. 대상 및 방법 S/W의 개발은 MATLAB(Mathwork사 미국)을 이용하여 PC의 window운영 체제에서 GUI 기반으로 동작하게 하였다. 시간 영역의 raw data와 주파수 영역의 spectrum data를 동시에 display할 수 있게 하였으며 Zero filling, 여러 종류의 filtering, 위상보정, FFT, peak area 측정 등의 기능을 갖추었다. 또한, 1.5T Gyroscan ACS-NT R6(Philips, Amsterdam, Netherland)의 $^1H$ Spectroscopy 패키지를 이용하여 정상인 뇌의 Parietal white matter, Basal ganglia, Occipital grey matter 영역에서 얻은 $^1$H MRS data를 정성 .정량적으로 분석하여 타 기종에서 얻어 발표된 $^1H$ MRS data와 비교분석하였다. 결과 : 본 연구에서 개발된 S/W를 이용하여 정상인 뇌에서 $^1H$ MRS data를 processing한 결과 NAA/Cr, Cho/Cr, MI/Cr 비율은 TE를 달리하였을 때, 유의수준 5%에서 Parietal white matter(PWM)의 NAA/Cr peak ratio를 제외하고 유의한 차이가 없었다. 그리고 기존에 발표된 문헌과 비교할 때 다른 MR장치의 NAA/Cr, Cho/Cr, MI/Cr 값들에 비해 평균값과 표준편차가 전반적으로 10-50%의 큰 값을 나타내었다. 결론 : 정상인 뇌에서 세 부위에 대하여 $^1H$ MRS를 얻고 이에 대한 정성.정량 분석을 함으로써 MRS를 임상적으로 적용하기 위한 준비를 하였으며 이러한 작업을 PC에서 독립적으로 수행함으로서 MRI system의 작업효율을 향상시킬 수 있었다. 그리고 서로 다른 기종간에는 유의한 차이가 있으므로 정상인에 대한 MRS database를 구축한 후에 $^1H$ MRS를 임상에 적용해야 함을 알 수 있었다. Purpose : The aim of this study is to develope the Magnetic Resonance Spectroscopy(MRS) data processing S/W which plays an important role as a diagnostic tool in clinical field. Materials and methods : Post-processing software of MRS based on graphical user interface(GUI) under windows operating system of personal computer(PC) was developed using MATLAB(Mathwork, U.S.A.). This tool contains many functions to increase the quality of spectrum data such as DC correction, zero filling, line broadening, Gauss-Lorentzian filtering, phase correction, etc. And we obtained the normal human brain $^1H$ MRS data from parietal white matter, basal ganglia and occipital grey matter region using 1.5T Gyroscan ACS-NT R6 (philips, Amsterdam, Netherland) MRS package. The analysis of the MRS peaks were performed by obtaining the ratio of peak area. Results : The peak ratios of NAA/Cr, Cho/Cr, MI/Cr for the different MRS machines have a little different values. But these peak ratios were not significantly different between different echo time MRS peak ratios in the same machine (p<0.05). Conclusion : MRS post-processing S/W based on GUI using PC was developed and applied to the analysis of normal human brain $^1H$ MRS. This independent MRS processing job increases the performance and throughput of patient scan of main console. Finally, we suggest that the database for normal in-yivo human MRS data should be obtained before clinical applications.

      • Development of a QA phantom and protocol for proton magnetic resonance spectroscopy

        <P>The purpose of this study was to develop a special proton magnetic resonance spectroscopy (1H-MRS) phantom, to establish and verify a quality assurance (QA) protocol, and to perform QA for a MR system. For a complete QA protocol, seven principal quality factors (PQFs) were measured. For evaluation of localization and quantification accuracy, two PQFs which are difficult to measure in commercial phantoms, we developed the layered dumbbell-shaped phantom. The performance sensitivity of our phantom was determined as a function of the voxel size on three occasions over ∼1 year. All MRS QA factors were optimized on two different MRI/MRS systems. The correlation between the metabolite signal intensity and the metabolite concentration was verified. The seven principal MRS QA factors studied can be used to evaluate MR spectra as well as to measure the stability of MR systems. © 2009 Wiley Periodicals, Inc.         Quantification of Metabolic Alterations of Dorsolateral Pre-Frontal Cortex in Depression SD Rat by MR Spectroscopy

        Purpose: Contrary to the human study, it has rarely investigated metabolic alterations in the dorsolateral prefrontal cortex (DLPFC) of depressed rats versus age and sex-matched controls using proton magnetic resonance spectroscopy (MRS). Thus, the purpose of this research was to verify the feasibility of metabolic differences between the normal rat and the depression model rat. Materials and Methods: A homogeneous group of 20 SD male rats was used for MRI and in vivo 1H MRS. To induce a depressed status in SD rats, we performed the forced swimming test (FST). Using image-guide, water suppressed in vivo 1H MRS with 4.7 T MRI/MRS system, NAA/Cr and Cho/Cr ratios were mainly measured between depressed rats and normal subjects. Results: In depressed rats, increased Cho/Cr ratio was measured versus control subjects. However, no significant group effect for NAA/Cr was observed between case-control pairs. Discussion and Conclusions: The present 1H MRS study shows significant brain metabolic alterations of dorsolateral prefrontal cortex with experimental depressed status of SD rat induced by FST compared to normal subjects. This result provides new evidence that in vivo 1 H MRS may be a useful modality for detecting localized functional neurochemical markers alterations in left DLPFC in SD rats.

        인체 뇌 대사물질에서의 In vitro 2D-COSY와 2D-NOESY 스펙트럼 분석 평가

        목적 : 본 연구에서는 인체 뇌 대사물질에 대하여 2D MR 기술인 correlation spectroscopy (COSY) 와 nuclear Overhauser effect/enhancement spectroscopy (NOESY)를 직접 적용하고, 데이터를 획득하여 인체 뇌대사물질들간의 스칼라 짝지움 (coupling)과 쌍극자 (dipolar) 상호작용- NOE에 대한 분석을 통하여 결합연결관계 및 공간연결관계에 대한 정보를 획득하고자 하였다. 대상 및 방법 : 모든 2-D (dimensional) MR 실험 (즉, COSY와 NOESY)은 z축 능동차폐 펄스경사자장, 삼중공명 동결탐침기가 장착된 Bruker Avance 500 (11.8 T) 장비에서 298 K에 수행되었다. MRS상에서 뇌 대사물질과 유사하도록만든 희석액을 만들었고, 최종 MRS 샘플은 10% $D_2O$를 이용하였다. 2-D 스펙트라는 2048 복합 (complex) 데이터 포인트로서 총 320개 의 free induction decay (FID)를 평균화 (averaging)하였고, $H_2O$에서 얻어진 스펙트라는 8012 Hz 였다. 반복지연 (repetition delay) 시간은 2초, 각각의 FID는 4개의 평균화를 선택하였다. 얻어진 2D-COSY, 2D-NOESY 데이터는 Top Spin 2.0 소프트웨어에서 후 처리기법 (post-processing)에 의해 분석되었다. 분석 대상 대사물질은 N-acetyl aspartate (NAA), creatine (Cr), choline (Cho), glutamine (Gln), glutamate (Glu), myo-inositol (Ins), lactate (Lac)로서 총 7 가지 화학물로서 주요 목표 피크로 정했다. 결과 : 인체 뇌 대사물질에 대한 대칭형태의 2D-COSY와 2D-NOESY 스펙트럼을 획득하였고, COSY 스펙트럼상에서는 오직 1.0-4.5 ppm 사이에서만 교차피크들이 생성된 반면 NOESY 스펙트럼상에서는 1.0-4.5 ppm 외에도 7.9 ppm에서 공명 교차피크를 발견할 수 있었다. COSY 스펙트럼을 통하여 lactate에서 메틸 프로톤과 CH 프로톤의 COSY 크로스피크가 발견되었고, NAA에서 메틸렌 프로톤들간과 메틸렌 프로톤과 NH프로톤의 크로스피크가 발견되었고, Ins에서 CH 프로톤 들간의 크로스피크가 발견되었다. NOESY 스펙트럼을 통하여 NAA 분자내 NH 프로톤과 메틸 (-CH3) 프로톤과의 NOESY 크로스피크가 발견되었고, lactate에서 메틸 프로톤과 CH 프로톤과의 크로스피크가 발견되었고, Cr에서 메틸 프로톤과 메틸렌 프로톤과의 크로스피크가 발견되었고, Glu에서 메틸렌 프로톤 들간과 또한 메틸렌 프로톤과 CH 프로톤과의 크로스피크가 발견되었고, Gln에서 메틸렌 프로톤과 CH 프로톤과의 크로스피크가 발견되었고, Ins에서 CH 프로톤 들간의 크로스피크가 발견되었다. 결론 : 본 연구에서는 in vitro 상태의 인체 뇌 대사물질에 2D-COSY와 2D-NOESY 기술을 직접 적용하고, 결합연결관계 및 공간연결관계에 대한 정보를 성공적으로 획득하여 분석하여 보았다. 본 연구 결과물은 향후 인체 내 in vivo 2D-COSY를 이용한 뇌 대사물질 연구에 매우 유용하리라 사료된다.

        Analysis of in vitro 2D-COSY on Human Brain Metabolites for Molecular Stereochemistry: The Preliminary Results

        To investigate the 3-bond connectivity of human brain metabolites by scalar coupling interaction through 2D-correlation spectroscopy (COSY) techniques using high field NMR spectroscopy. All NMR experiments were performed at 298K on Unity Inova 500 or 600 (Varian Inc.) equipped with a triple resonance probe head with z-shield gradient. Human brain metabolites were prepared with 10% D2O. Two dimensional 2D COSY spectra were acquired with 4096 complex data points in t2 and 128 or 256 increments in t1 dimension. The spectral width was 9615.4 Hz and solvent suppression was achieved using presaturation using low power irradiation of the water resonance during 2s of relaxation delay. NMR data were processed using VNMRJ (Varian Instrument) software and all the chemical shifts were referenced to the methyl resonance of N-acetyl aspartate (NAA) peak at 2.0 ppm. Total 10 metabolites such as N-acetyl aspartate (NAA), creatine (Cr), choline (Cho), glutamine (Gln), glutamate (Glu), myo-inositol (Ins), lactate (Lac), taurine (Tau), γ-aminobutyricacid (GABA), alanine (Ala) were included for major target metabolites. Symmetrical 2D-COSY spectra were successfully acquired. Total 14 COSY cross peaks were observed even though there were parallel/orthogonal noisy peaks induced by water suppression. Except for Cr, all of human brain metabolites produced COSY cross peaks. The spectra of NAA methyl proton at 2.02 ppm and Glu methylene proton (CH2(3)) at 2.11 ppm and Gln methylene proton (CH2(3)) at 2.14 ppm were overlapped in the similar resonance frequency between 2.00 ppm and 2.15 ppm. The present study demonstrated that in vitro 2D-COSY represented the 3-bond connectivity of human brain metabolites by scalar coupling interaction. This study could aid in better understanding the interactions between human brain metabolites in vivo 2D-COSY study. Also it would be helpful to determine the molecular stereochemistry in vivo by using two-dimensional MR spectroscopy

        Investigation of the Neuroprotective Effects of Bee-venom Acupuncture in a Mouse Model of Parkinson’s Disease by Using Immunohistochemistry and <i>In−vivo</i> <sup>1</sup>H Magnetic Resonance Spectroscopy at 9.4 T

        Neuroprotective therapeutics slows down the degeneration process in animal models of Parkinson’s disease (PD). The neuronal survival in PD animal models is often measured by using immunohistochemistry. However, dynamic changes in the pathology of the brain cannot be explored with this technique. Application of <i>in-vivo</i> <sup>1</sup>H magnetic resonance spectroscopy (<sup>1</sup>H MRS) can cover this shortcoming, as these techniques are non-invasive and can be repeated over time in the same animal. Thus, the sensitivity of both techniques to measure changes in the PD pathology was explored in an experiment studying the neuroprotective effects of the vigilance enhancer bee-venom (BV) in a mouse model of PD. The mice were pre-treated with 0.02-ml BV administered to the acupuncture point GB34 (Yangneungcheon) once every 3 days for 2 weeks. Three groups were classified as control, MPTP-intoxicated PD model and BV-treated mice. Outer volume suppression combined with the ultra-short echo-time STEAM (TE = 2.2 ms, TM = 20 ms, TR = 5000 ms) was used for localized <i>in-vivo</i> <sup>1</sup>H MRS. Based on the <sup>1</sup>H MRS spectral analysis, substantial changes of the neurochemical profiles were evaluated in the three investigated groups. In particular, the glutamate complex (Glx)/creatine (Cr) ratio (7.72 ± 1.25) in the PD group was significantly increased compared to that in the control group (3.93 ± 2.21, P = 0.001). Compared to the baseline values, the Glx/Cr ratio of the BV-treated group was significantly decreased 2 weeks after MPTP intoxication (one-way ANOVA, p < 0.05). In conclusion, the present study demonstrated that neurochemical alterations occurred in the three groups and that the neuroprotective effects of the BV acupuncture in a mouse model of PD could be quantified by using immunohistochemistry and <sup>1</sup>H MRS.

        Quantitative Comparison of 1H-MRS Spectra Depending on the Paramagnetic Gadolinium Contrast Agent(GBCA) Injection

        본 연구는 1H-MRS 검사 시 조영제 사용이 대사물질의 spectrum에 어떤 영향을 미치는지 비교 분석함으로써, 조영제 주입 후 MRS 시행의 가능성을 알아보고자 하였다. 연구방법은 2017년 1월부터 동년 5월까지 뇌 MRI 검사를 시행한 30명을 대상으로, 조영제 주입 전, 후 전두엽 백질 부분의 spectrum을 획득하여 뇌 조직의 대사물질 spectrum을 비교평가 하였다. 연구결과, 조영제를 주입할 경우 각 대사물질의 spectrum은 통계적으로 의미가 없어 조영제 사용이 대사물질의 spectrum에 영향을 미치지 않음을 알 수 있었다. 결론적으로 대조도가 떨어져 병변의 위치파악이 힘든 경우 정확한 위치의 spectrum을 얻기 위해선 조영제를 사용하여 대조도를 높인 다음 MRS를 시행하는 것이 바람직 할 것으로 사료된다. This study evaluated the effect of gadolinium contrast agents on the spectrum of metabolites during 1H-MRS of brain and to investigate whether the contrast agents injected before MR spectroscopy significantly affect the estimated peaks of MRS. From January to May 2017, brain MR spectroscopy was performed on 30 patients to compare the spectrum before and after contrast injection of the brain white matter tissue. As a result, the spectrum of metabolites decreased after the paramagnetic contrast agents injected. However, it was not statistically significant which indicated that the use of contrast agent did not meaningfully affect the spectrum of metabolites. In conclusion, the use of the paramagnetic contrast before the acquisition of the spectroscopy may aid voxel positioning especially when it is difficult to determine the exact location of the lesion or the contrast is low.

        신경베체트병 환자의 양성자(1H) 자기공명분광 영상 2예

        Magnetic resonance imaging (MRI) and computed tomography had been used to detect the neurologic lesions of the Behcet`s disease. However, there are a number of cases which can not be detected by conventional imaging studies in neuro-Behcet`s disease. To overcome this problem, magnetic resonance spectroscopy (MRS) has been introduced in several autoimmune diseases involving central nervous system such as neuropsychiatric lupus, but which was rarely performed in Behcet`s disease. In the present study, we report two cases of neuro-Behcet`s disease that showed normal MRI but abnormal proton MRS (1H MRS) findings. MRS in two cases revealed a reduction of the N-acetyl aspartate (NAA)/creatine (Cr) ratio in the substantia nigra and thalamus, respectively, which were the areas without abnormality on MRI, suggesting a functional defect of neurons in these areas. In a case with thalamic dysfunction, abnormal NAA/Cr ratio was normalized in parallel with clinical improvement after treatment with high-dose steroids and immuno-suppressive agents. These observations suggest that MRS may be useful to early detect the functional abnormality in neuro-Behcet`s disease and to monitor neurologic status after treatment with immuno-suppressive agents.
