        전신성 광택태선에 대한 임상적 연구

        Background: Generalized lichen nitidus is a rare subtype of lichen nitidus characterized by multiple, flesh-colored, shiny papules occurring over the entire body. Objective: We investigated the clinical features of generalized lichen nitidus in patients followed up at our institution. Methods: The patients with generalized lichen nitidus diagnosed clinically and histopathologically between 1998 and 2007 were reviewed. Results: Twelve patients (9 males and 3 females) were enrolled in this study and the mean age at onset was 10.3 (range: 4~27) years. Of the 12 patients, 5 (41.7%) experienced pruritus and 4 (33.3%) had atopic dermatitis. None of the patients had a family history of lichen nitidus. Nine patients were treated with topical steroids, systemic steroids, oral antihistamines, or narrow-band UVB. We propose that narrow band UVB is an effective treatment modality for generalized lichen nitidus. The mean duration to clearance was 37.7 (range: 7~120) months and the disease was relapsing in 2 patients. The duration to clearance was correlated to the age at onset (p=0.0005) and to the duration at the first visit (p=0.0000), whereas it was not related to the sex of the patient, pruritus or the accompanying atopic dermatitis. Conclusion: Generalized lichen nitidus is often associated with varying degrees of pruritus and the clinical course of the disease tends to be chronic. We propose that onset at an early age and the short duration of disease at the first visit are good prognostic factors, and that narrow band UVB is an effective treatment modality for generalized lichen nitidus.         Lichen nitidus is a chronic inflammatory skin disease characterized by a large number of asymptomatic skin-colored or erythematous millimetric papules. Topical or systemic steroids, topical tacrolimus, PUVA and NB-UVB may yield various results. Also, retinoids are thought to have anti-inflammatory and antiproliferative effects and there are a few reported cases of lichen nitidus treated with oral retinoids. However, there was no reported case that tried topical retinoid to treat lichen nitidus. A 51-year-old man presented with asymptomatic tiny flat-topped, flesh-colored papules clustered on both sides of abdomen for 20 years. Histopathologic examination showed typical features of lichen nitidus which is well circumscribed lymphocytic infiltrate closely confined to a widened dermal papillae, so called “claw clutching a ball” pattern. From these findings, we diagnosed this case as lichen nitidus. We tried topical retinoid on the right side and topical steroid on the left side of the abdomen to compare the therapeutic effects between two topical agents. After 3-months, the retinoid side had better response than steroid side, although flattening of papules and pigmentation showed on both side. Herein, we present a man with lichen nitidus who was successfully treated with topical retinoid.

        광택태선의 임상적 및 병리 조직학적 고찰

        N/A Background : Lichen nitidus is a rare condition of unknown cause, characterized by minute, flesh-colored, shiny papules occurring on the abdomen, chest, extremities, and genitalia. There have been few studies worldwide about the clinical and histopathologic manifestations of lichen nitidus. Objective : The purposes of this study were aimed to evaluate the clinical and pathologic presentations of lichen nitidus. Methods : The medical records and histopathological slides of 31 patients with lichen nitidus diagnosed at Korea University Medical Center from April 1983 to March 2002 were reviewed. Results : 1) Onset age of skin lesions ranged from 1 year to 34 years and 17 cases(63%) were developed in the patients younger than 9 years old. The ratio of male to female was 2.1:1 and the mean duration of the eruption was 2.8years. 2) Twenty four cases(77%) had multifocal lesions, four cases(13%) had generalized lesions, and three cases(10%) had unifocal lesions. The predilection sites were trunk(27.8%), upper extremities(24.1%), lower extremities(19%), face and neck(13.9%), penis(10.1%), and hands(5.1%) in decreasing order. 3) Mild pruritus was noted in 8 cases(26%), and Koebner phenomenon in 9 cases(29%). 4) Atopic dermatitis was noted in 3 cases(10%), lichenplanus in 1 case, urticaria in 1 case, and verruca plana in 1 case. None had a positive family history. 5) The papules were closely grouped but remained discrete except in areas exhibiting the Koebner phenomenon. The color of papules was skin-colored(74%), reddish-brown(13%), and mixed(13%). 6) The frequent histopathologic findings were liquefaction degeneration of the basal layer(100%), edema of the dermis within the areas of the infiltrate(100%), clawlike rete ridges(96.8%), epidermal atrophy(87.1%), perivascular inflammatory cell infiltrate(87.1%), and absent or thinned granular layer(77.4%). 7) We divided lichen nitidus into three stages, early stage, developed stage and late stage, according to the predominant cell types of the inflammatory infiltrates in the dermal papilla. There was a tendency of increasing number of infiltrated dermal papillae as the lesion progressed to late stage. It was interpreted as the lesions tend to be grouped clinically. Conclusion : In our study, we were able to obtain the clinical and histopathological features of lichen nitidus which help to understand lichen nitidus. (Korean J Dermatol 2003;41(7) : 857~868)

        Generalized lichen nitidus is a rare subtype of lichen nitidus, and the former condition is characterized by multiple, shiny flesh-colored pin-head sized papules that occur over the entire body. Generalized lichen nitidus is often associated with various degrees of pruritus and the clinical course tends to be chronic in contrast with the other forms of lichen nitidus. We report here on a case of a 39-year-old man who was diagnosed with generalized lichen nitidus that developed after a subcutaneous insulin injection. He was treated with topical corticosteroid.         Perforating lichen nitidus is an unusual variant of lichen nitidus. It is clinically characterized by umbilicated papules and histopathologically characterized by transepidermal elimination of inflammatory debris from granulomatous infiltrates. Most cases of perforating lichen nitidus are treated with topical corticosteroids, but long-term use of topical corticosteroids could provoke several side effects. Herein, we report a case of perforating lichen nitidus treated effectively and safely with topical tacalcitol ointment. We performed a dermoscopic examination for the perforating lichen nitidus lesion and found the characteristic frogspawn dermoscopic feature. (Korean J Dermatol 2011;49(12):1083~1086)

        Lichen nitidus is an uncommon, asymptomatic cutaneous shiny papular eruption and it can rarely show Koebner`s phenomenon, which may clinically resemble lichen striatus. It is known that for such cases, the clinicopathological correlation is the most important factor for making the diagnosis and immunohistochemical staining for CD4, CD8 and CD68 can be helpful to differentiate these two diseases. We experienced a case of Koebner phenomenon, which clinically mimicked lichen striatus, in a patient suffering with lichen nitidus. (Korean J Dermatol 2008; 46(11): 1517∼1520)

        손바닥에 발생한 광택태선 1예

        Lichen nitidus is an uncommon chronic inflammatory disease characterized by multiple minute discrete flesh-colored dome shaped papules. Its distribution is frequently localized on trunk, arm or penis. There are a few cases reported to occur on palm and sole. We report a case of lichen nitidus which presents erythematous or brownish colored hyperkeratotic plaques, composed of 1-2 m sized small papules on the right palm. Its histopsthologic findings were consistent with a diagnosis of lichen nitidus. (Korean J Dermatol 2003;41(3) : 383∼384)

        Clinical and Histopathologic Study of Lichen Nitidus-Like Papular Eruptions; Eruptive Idiopathic Papulosis

        Background: A large number of patients present with multiple papular skin lesions at the outpatient department of dermatology. In practice, however, the clinical presentation and skin histology has often been difficult to classify as a specific disease group. Objective: The purpose of this study was to analyze the clinical and histopathologic characteristics in patients at a single center with papular eruptions that looked clinically similar to lichen nitidus. Finally, we propose a new disease category with the series of characteristics we found. Methods: We retrospectively reviewed and analyzed the medical records, clinical photographs, and histopathologic slides of 22 patients who underwent skin biopsies for the diagnosis of lichen nitidus-like papular lesions. Results: There were 16 men (72.7%) and 6 women (27.3%) included with a mean age at onset of 13.3 years. There were 17 children (77.3%) and 5 adults (22.7%). Eleven patients (50%) had pruritic symptoms. Fifteen patients (68.2%) had generalized eruptions. The localized eruptions predominantly involved the upper extremity (31.0%), especially the dorsum of the hands (14.1%). The most frequent histopathologic changes were superficial perivascular inflammatory cell infiltration (100%) and epidermal acanthosis (95%). Conclusion: Our study was an overall review of flat or dome-shaped papular eruptions. The data are expected to contribute to the identification of the characteristics of this cutaneous disease that has been difficult to categorize. (Korean J Dermatol 2021;59(2):125∼131)

        Perforating Lichen Nitidus

        A healthy 18-year-old woman presented with an asymptomatic tiny papular eruption on the hands, forearms, legs and trunk. A skin biopsy from the left forearm revealed a typical lichen nitidus lesion associated with an adjacent transepidermal perforating lesion. We report a case of perforating lichen nitidus, and briefly review the relevant literature of perforating lichen nitidus.

        Lichen planus is an unique, common inflammatory disorder that affects the skin, mucous membranes, nails and hair. Oral lichen planus is among the more common mucosal conditions with an unknown etiology. Lichen nitidus is an uncommon, usually asymptomatic cutaneous eruption that is characterized by the presence of multiple, small, flesh colored papules. Perforation in lichen nitidus is a rare phenomenon. Lichen nitidus is distinguished from lichen planus on both clinical and histologic grounds. However, the possibility that lichen nitidus represents a micropapular variant of lichen planus has never been definitively excluded. Herein, we report on a case of perforating lichen nitidus associated with oral lichen planus. (Korean J Dermatol 2010;48(11):1012∼1015)
