      • A rare case of multiple cutaneous larva migrans

        Cutaneous larva migrans is caused by larvae of nematodes such as hookworms which can penetrate into the skin. When directly contacted with soil which is contaminated with the larvae, the infestation can occur. The predilection sites are known as the exposed area such as feet and forearms, and clinically presented as irregularly linear, elevated burrow. A 4-year-old boy presented with multiple linear migratory elevated erythematous papuloplaques on both extremities, abdomen and perianal area with itching. Skin biopsy specimens from lesion of left thigh showed traces of hookworm in epidermis, perivascular infiltration with lymphocytes and some eosinophils in dermis. The patient traveled Vietnam 2 weeks before visit and played with his father burying his body in the sand of beach which might be contaminated. He was treated with topical ivermectin and oral albendazole and showed considerable improvement. In our literature review, the parasite can be found 1 to 2 cm ahead of the visible track within a burrow located in the upper layers of the epidermis but often not observed in the biopsies. With typical lesions and contact history of sand, he was finally diagnosed with cutaneous larva migrans. Herein we report a case of cutaneous larva migrans with literature review.

      • A case of cutaneous sparganosis masquerading cutaneous larva migrans

        Sparganosis is a parasitic flatworm infection caused by pleroceroid tapeworm larvae that belongs to the genus Spirometra. Humans are accidental secondary hosts for second-stage larva, and human infection is often associated with dietary consumption or medicinal usage of raw snake and frog tissue preparations. Since sparganum is a visceral larva migrans, the larva usually lodge in muscle or subcutaneous tissue. Therefore, sparganosis usually presents as a soft palpable subcutaneous nodule. A 75-year-old female presented with a non-pruritic, sperpiginous, and erythematous raised track on her right upper arm, a typical appearance of cutaneous larva migrans. She had occasionally eaten raw freshwater fish and had a history of eating dog before. Skin biopsy revealed a parasitid body with a characteristic pattern of noncellular tegument, cellular subtegument, numerous bundles of muscle fibers and excretory canals. The parasite was identified as Sparganum by enzyme-linked immunosorbent assay serologic test. Herein, we report a rare case of cutaneous sparganosis masquerading cutaneous larva migrans in an Asian woman.

        유충 피부 유주증 1예

        A 30-year-old man presented with several intensely prurutic erythematous serpiginous thread-like skin lesions which began as a small papule on the upper back 2 weeks prior to his visit and was migrating to the lower back and buttock area. The laboratory examination showed non-specific findings. Histopathologic examination revealed a pustular burrow containing numerous eosinophils in the epidermis. In the dermis, there was a diffuse and occasionally dense perivascular inflammatory cell inflammation consisting of eosinophils and some lymphocytes. The patient was treated with oral albendazole, antihistamine and topical corticosteroid for 2 weeks without recurrence for 6 months up to now. Multiple histologic sections were made without success in demonstrating parasitic tissue in the epidermis, however, the clinical and histopathologic features were consistent with cutaneous larva migrans.

        A 58-year-old male presented with a pruritic, erythematous hread-like eruption on the right chest, which appeared as a small papule since 6 months prior to his visit. Histopathologic examination showed a burrow containing numerous eosinophiles and a few mononuclear cells in the epidermis. There was focal spongiosis unde the burrow. In the dermis, there was a diffuse and occasionally dense perivascular and periadnx 1 inflammatory cell infiltration consisting of eosinophiles and some lymphocytes. Multiple histologic sections were made without success in deizo strating parasitic tissue in the epidermis, however, the clinical and histopathologic features we onsistent with cutaneous larva migrans.         Dermoscopy for the Diagnosis of Creeping Hair Caused by Ingrowing Hair: A Case Report

        Creeping hair is a rare condition characterized by creeping eruption with a black line at the advancing end, mimicking cutaneous larva migrans. The condition is also referred to as cuta- neous pili migrans, migrating hair, and embedded hair. A total of 52 cases have been reported since 1957 and most cases were published in English. Herein, we report a case in which creep- ing hair occurred in the iliac region and review the literature from 1957 to February 2021. A 35-year-old Chinese female presented with a black moving linear eruption that had migrated from the lower abdomen to the iliac region without causing any symptoms during a 3-year period. Cutaneous examination showed a 6.5-cm-long black linear lesion beneath the skin that was revealed to be a hair shaft. After removal of the hair, the eruption diminished and no re- currence occurred in 3 months of follow-up. The creeping hair that had migrated with its lower end forward was confirmed by observation under dematoscopy and light microscopy. A review of the literature revealed that creeping hair occurs most frequently in young and middle-aged patients and the reported cases are mainly from Asia. The top locations involve the foot. The causative hair includes head hair, beard, pubic hair, body hair, and one case of dog hair. A close- up examination and dermoscopic inspection are helpful for the diagnosis of creeping hair.

        해외 여행 과거력없이 목에 발생한 피부 애벌레이동증 1예

        Cutaneous larva migrans is a characteristic serpiginous creeping eruption that is caused by the migration of the larval form of neomatodes in the skin. It is caused by hookworm larvae, which are present in the feces of infected dogs and cats. The skin lesions progressively evolve at a rate of 2∼3 cm daily. Most cases have a travel history to tropical and subtropical countries, but our patient had no travel history to an endemic area and no contact history with the feces of dogs and cats. We report herein on a case of cutaneous larva migrans on the neck and the patient did not have a travel history to an endemic area and the lesions showed relatively rapid progression.         Cutaneous larva migrans (CLM) is a creeping eruption caused by the skin invasion of larval hookworms, which live in the intestines of pets and shed their eggs via feces. A 41-year-old man with pruritic serpiginous tracks on his lower extremity visited clinic with his 2-year-old son who also presented with creeping eruptions on his thigh. They had traveled to Vietnam 4 months earlier and played on the beach. At that time, the father felt an itching sensation on his ankle and reddish eruptions emerged within 3 days. After 3 months, the same symptoms and skin lesions appeared in his son. They were both diagnosed with CLM and were treated with a single 300-mg and 4 weeks apart 200-mg twice oral dose of albendazole. After 4 weeks later, their symptoms had completely resolved. To our knowledge, no cases of concurrent CLM in family members have been reported. Clinicians should be aware of it and should consider that not all CML cases have a self-limiting course but could potentially result in worsening disease without the use of oral antiparasitic treatments.

        Cutaneous larva migrans (CLM) is an uncommon cutaneous creeping eruption caused by the invasion and migration of nematode larvae into the skin. Burrowing larvae produce intensely pruritic, serpiginous, raised erythematous skin lesions. Most cases of CLM reported in Korea involved travelers returning from endemic areas with tropical or subtropical climates. We report herein a case of CLM with no history of travelling to endemic area and briefly review the relevant literature. (Korean J Dermatol 2010;48(3):250~252)

        Cutaneous larva migrans(CLM) is a skin disease caused by direct penetration of nematode larvae in sand or soil. It is one of the most common dermatoses among travelers returning from tropical countries. Patient with CLM typically presents intensively pruritic serpiginous eruptions on the exposed areas. This is due to intraepidermal migration of animal hookworms which cannot penetrate the basement membrane so that they are only pathogenic to animals. A 32-year-old male with extremely pruritic winding tracks on his buttock, thigh, and arm visited our clinic with his wife, a 30-year-old woman who also presented creeping eruptions on the same area. They had travelled Thailand and the symptoms started a month ago soon after coming back to Korea. The couple said they ate cooked food except salmon. Besides, they do not keep any kind of pets at home. The laboratory findings of them revealed eosinophilia(eosinophil count 770/㎕) in the man but normal(eosinophil count 300/㎕) in his wife. The histopathology examination of the husband showed many inflammatory cellular infiltrations on the epidermis and upper dermis composed of eosinophils and lymphocytes. They were both treated with a single 400-mg oral dose of albendazole. A week later, their itching sensations were subsided and skin lesions were much improved.
