      • 동시적 의약품 사용평가(cDUR) 시스템 구축 및 적용 사례 연구

        Medical errors such as adverse drug event, improper transfusion, wrong-site surgery, mistaken patient identity and so on commonly occur at health care practice. Information technology, like Drug Utilization Review(DUR) system which reviews, analyzes, and interprets medication data when prescribing, can play a key role in reducing such medical errors and improving patient safety. Korean Government has guided all hospitals to implement concurrent DUR(cDUR) system, which is the first case worldwide in that all healthcare providers have to use cDUR system when prescribing. This paper introduced a case study that a tertiary hospital has integrated the cDUR system into its comprehensive Hospital Information System(HIS) and analyzed the whole prescription data during a week right after adoption of cDUR system. Considering technical strength and weakness, the cDUR system was integrated into the HIS, using Broker Servers for minimizing doctors’ anxiety. As the quantitative analysis of the whole prescription data, DUR conflict events, which mainly included duplicate medications and contra-indicated drug interactions for outpatients, were 2.77%. Although only 0.7% is for the contraindicated drug interactions, it will be greatly devoted to achieve the purpose of DUR such as improving patient safety.

        The Design of mBodyCloud System for Sensor Information Monitoring in the Mobile Cloud Environment

        Recently, introduced a cloud computing technology to the IT industry, smart phones, it has become possible connection between mobility terminal such as a tablet PC. For dissemination and popularization of movable wireless terminal, the same operation have focused on a viable mobile cloud in various terminal. Also, it evolved Wireless Sensor Network(WSN) technology, utilizing a Body Sensor Network(BSN), which research is underway to build large Ubiquitous Sensor Network(USN). BSN is based on large-scale sensor networks, it integrates the state information of the patient's body, it has been the need to build a managed system. Also, by transferring the acquired sensor information to HIS(Hospital Information System), there is a need to frequently monitor the condition of the patient. Therefore, In this paper, possible sensor information exchange between terminals in a mobile cloud environment, by integrating the data obtained by the body sensor HIS and interoperable data DBaaS (DataBase as a Service) it will provide a base of mBodyCloud System. Therefore, to provide an integrated protocol to include the sensor data to a standard HL7(Health Level7) medical information data.

        병원행정직원의 병원정보시스템 이용만족도에 영향을 미치는 요인

        This study was designed to identify satisfaction with Hospital Information System (HIS) and the factors affecting them in hospital administrators. The data was collected for 24 days from Mar. 02 to 25, 2011. Among a total of 82 cases of the questionnaires, only 76 cases were used. From the survey, it was made known that there was statistically significant difference in speed, preciseness and stability of environmental factors depending on the type of hospitals while significant difference was found in stability by number of beds. Among the factors of system operation, the administrational method affected speed, preciseness, stability and convenience statistically significantly whereas, however, preciseness, stability and convenience became significantly different depending on the number of staff members. As for the factors that could exercise influence upon satisfaction with administrative service of the HIS, it was revealed that statistically considerable impact could be caused under management of own EDPS expert as well as under comanagement by the medical institution and professional business entity in case of hospital.

        처방전달체계에 대한 사용자 만족도에 영향을 미치는 요인

        Objectives : To identify the factors affecting users' satisfaction with the Order Communicating System(OCS) and to highlight the factors important for the successful establishment of OCS. Methods : A Users Satisfaction survey was sent to 4,513 people, consisting of 1,503 doctors, 2,379 nurses, 255 pharmacists and 370 administrative workers in 16 hospitals which had introduced OCS. The response rate was 63.9%. Measurement of users' satisfaction was peformed with the instrument which was used in Doll's study. Some aspects of Doll's instrument were adjusted according to the aims of this study. The classifying sections of this survey included age, job and status classification, computer experience, OCS education, duration of daily OCS use, type of order entering, number of personnel in the Hospital Information System's department, cost of OCS, problem frequency, proportion of work managed by hand, OCS type, and Hospital establishment type. Results : There was a positive correlation between satisfaction level and managerial status throughout all job classifications. Irrespective of the importance of OCS education as a factor relating to users' satisfaction, the additional work load caused by OCS lowered users' satisfaction. Different factors affected users' satisfaction according to job and status classification. The composition of factors affecting the pharmacist and administrative worker satisfaction levels was simpler than that of the doctor and nurse levels. There were no statistically significant differences between the actual computer experience duration of daily OCS use and users' satisfaction with OCS. Conclusions : There was an understandable relationship between users' attitude to OCS and factors affecting users' satisfaction. The results of this study could be used as a basis for the successful expansion of the operation of OCS. But more detailed studies on users' satisfaction and further improvements of methodologies are required for the successful establishment of OCS.

        병원고객관계관리 시스템 도입에 영향을 미치는 요인 연구

        Due to the high level of information supportive function of Internet, the management environment change is accelerating. The Patients are able to have the chances of Hospital- and Doctor-shopping. So the Customer Relationship Management skill or Patient -oriented management is becoming one of the important management method of Hospital. To adapt these trend, many Hospital invest the money and efforts for the implementation of Hospital CRM system. But the Performance of HCRM is not satisfactory for the investor from many reasons. In this paper we try to find out which kinds of factors have the Influences on the Implementation of HCRM, and how is the relation between these factors. We use the Structural Equation Model to clarify these relationships. The result of this paper will contribute in the decision making of the implementation of HCRM in the field, and in providing the knowledge base. 병원 산업계의 경영관리 환경 변화에서 인터넷의 정보 제공 수준이 높아짐으로써 고객의 의료 지능이 높아지고, 병원선택(Hospital Shopping)과 의사선택(Doctor Shopping)을 스스로 하고 있어서, 병원의 경영도 고객(환자)접점 관리가 사업성과를 중요한 결정 요인으로 작용하고 있다. 이러한 결과로 환자와의 접점관리를 위한 고객관계관리(CRM) 시스템을 구축하고 운영하는 병원이 점차 늘어가고 있다. 그러나 이러한 병원CRM시스템 투자 노력에도 불구하고 그 성과는 미미한 수준에 머물고 있거나 대부분의 CRM 시스템이 실패함에 따라 CRM의 적극적인 도입의지가 줄어들고 있다. 본 연구에서는 병원CRM시스템의 도입 의도에 어떤 영향 변수들이 주요한 변수로 작용하고 있는지에 대하여 연구하였다. 이러한 문제를 해결하기 위해 병원CRM시스템의 구축에 있어서 영향을 많이 끼치는 분야로 병원정보시스템을 선택했고, 이를 위해 구조방정식모형을 이용하여 연구 가설을 검증하고 연구 모형의 유효성을 증명한 독립변수의 측정 변수를 도출하여 향후 CRM 도입을 하고자 하는 병원에게 지식정보를 제공하고자 하였다.
