자가 골연골 플러그를 이용한 슬관절 불안정 박리성 골연골염의 치료

목적: 박리성 골연골염의 불안정성 골연골편을 자가 골연골 플러그를 이용하여 고정을 한 치료 방법의 임상적 결과를 평가하자고 한다. 대상 및 방법: 2004년 1월부터 2012년 1월까지 International Cartilage Repair Society 2, 3, 4단계에 해당되는 불안정성의 박리성골연골염을 가진 환자 중 자가 골연골 플러그로 골편 고정이 가능하였고 2년 이상 추시가 가능하였던 19명의 환자를 대상으로, 술 전 및 최종 추시 시의 Lysholm 점수와 International Knee Documentation Committee (IKDC) 주관적 점수를 비교하여 임상적 평가를 하였다. 결과: 환자는 모두 남자였으며, 평균나이는 19.1세였고, 병변은 대퇴내과 외측면이 16예, 대퇴외과 관절면이 2예, 대퇴골 활차부위가 1예였다. 병변의 평균 크기는 5.68 ㎟, 사용된 골연골 플러그의 개수는 평균 4.3개, 평균 추시 기간은 38개월이었다. 수술 전 Lysholm 점수와 IKDC 주관적 점수는 수술 후 유의하게 호전되었다. 결론: 자가 골연골 플러그를 이용한 박리성 골연골염의 불안정성 골편의 고정은 남아 있는 조직을 이용하고 보존할 수 있어 장점이 있는 수술 방법으로 생각된다. Purpose: The purpose of this study was to evaluate the clinical results of fixation of the unstable osteochondral dissecans (OCD) lesion with autologous osteochondral plugs. Materials and Methods: This study was conducted in 19 patients who were relevant to the International Cartilage Repair Society 2, 3, or 4 who were treated with autologous osteochondral plugs and followed-up for more than 2 years from January 2004 to January 2012. Clinical evaluation was performed by comparing the preoperative and last follow-up scores of Lysholm score and subjective International Knee Documentation Committee (IKDC) score. Results: All patients were male and the average age was 19.1 years. Of the 19 cases, there were 16 cases of medial femoral condyle lateral side lesion, 2 cases of lateral femoral condyle articular surface, and 1 case of femoral intercondylar notch lesions. The average size of the lesion was 5.68 ㎟, and average use of osteochodral plugs were 4.3. Average follow-up period was 38 months. Preoperative Lysholm score, IKDC subjective score showed significant improvement. Conclusion: Fixation with autologous osteochondral plugs for unstable OCD uses the remnant tissues therefore conserving it, which is thought to be the positive aspect of this type of operation.
