        치위생학과 학생들의 노인에 대한 지식 및 태도가 노인차별주의에 미치는 영향

        연구배경: 우리나라 65세 이상 고령 인구는 2023년 18.4%로, 향후 계속 증가하여 2025년에는 20.6%로 초고령사 회로 진입 전망이다. 임상에서 치과위생사의 노인차별주의는 고령화에 따른 노인 환자의 증가로 인하여 치위생 처치 시 어려움을 경험할 수 있고 이는 업무 부적응과 이직으로도 이어질 수 있기 때문에 예비 치과위생사인 치 위생학과 학생을 대상으로 노인의 이해에 대한 교육이 반드시 필요하고 사료된다. 이에 노인차별주의와 노인의 지식 및 태도의 관련성 연구를 통해 초고령화 사회를 대비하고 대학의 교육과정 변화 및 노인관련 프로그램 개발 을 위해 연구를 실시하였다. 연구방법: D지역 치위생학과 재학생 204명을 대상으로 SPSS/WIN 25.0 프로그램을 이용하여 분석하였다. 대상 자의 노인차별주의, 노인에 대한 지식, 노인에 대한 태도는 평균과 표준편차로 구하였다. 대상자의 일반적 특성 에 따른 노인차별주의 차이를 검증하기 위해 t-test, one-way ANOVA를 실시하였으며, 사후검증은 Scheffe’ test 를 실시하였다. 대상자의 노인차별주의, 노인에 대한 지식, 노인에 대한 태도는 상관관계분석을 실시하였다. 결과: 노인차별주의는 4점 만점에 2.03±0.36점이었다. 노인에 대한 지식은 신체적 영역 0.57±0.15, 사회적 영 역 0.36±0.17, 심리적 영역 0.35±0.20 순이었다. 노인에 대한 태도는 3.86±0.27로 나타났다. 노인에 대한 지 식은 25점 만점에 평균 11.27±3.30점이었다. 노인에 대한 지식 정답률이 가장 높은 문항은 ‘체력은 나이가 들면 서 감소하는 경향이 있다’로 93.1%였다. 연구대상자의 일반적 특성에 따른 노인에 대한 태도는 연령(p=0.009)에 서 노인 차별주의는 성별(p=0.040), 연령(p=0.026), 조부모와 생활경험(p=0.001)유의한 차이를 보였다. 연구 대상 자의 노인 차별주의는 노인에 대한 태도와 지식 모두 음의 상관관계를 보였으며, 노인 차별주의 중에서도 정서회 피(r=.892, p<0.001)와 관련하여 높은 양의 상관관계를 보였다. 결론: 대학생들은 노인 부양의 주도적 역할자이자 고령화 사회문제에 직접적인 영향을 받게 된다. 그러므로 노인 들에 대한 긍정적인 사고 형성과 노인차별을 야기 시키는 부정적 편견을 지양할 수 있도록 국가와 사회, 교육기 관에서 다양한 프로그램의 적용이 필요할 것으로 사료된다. Background: The elderly population aged 65 or older in Korea is expected to continue to increase to 18.4% in 2023, and to enter a super-aged society at 20.6% in 2025. In clinical practice, the elderly discrimination of dental hygienists may experience difficulties during dental hygiene treatment due to an increase in the number of elderly patients due to aging, which can lead to maladjustment to work and turnover, so education on the understanding of the elderly is essential for students in the Department of Dentistry, who are prospective dental hygienists. Accordingly, a study was conducted to prepare for a super-aged society by studying the relationship between elderly discrimination and the knowledge and attitudes of the elderly, and to change the curriculum of universities and develop programs related to the elderly. Method: 204 students enrolled in the Department of Dentistry in D area were analyzed using the SPSS/WIN 25.0 program. The subject’s geriatric discrimination, knowledge about the elderly, and attitude toward the elderly were calculated as the mean and standard deviation. T-test and one-way ANOVA were performed to verify the difference in geriatric discrimination according to the general characteristics of the subject, with a Scheffe’ test applied for post-hoc analysis. Correlation analysis was conducted on the subject’s geriatric discrimination, knowledge about the elderly, and attitudes toward the elderly. Results: Geriatrics scored 2.03±0.36 out of 4. Knowledge about the elderly was categorized as follows: physical domain 0.57±0.15; social domain 0.36±0.17; and psychological domain 0.35±0.20. The attitude toward the elderly was 3.86±0.27. Knowledge of the elderly averaged 11.27±3.30 points out of 25. The question with the highest percentage of correct answers to knowledge about the elderly was ‘physical strength tends to decrease with age’, which was 93.1%. The attitude toward the elderly according to the general characteristics of the study subjects showed significant differences in gender (p=0.040), age (p=0.026), and life experience with grandparents (p=0.001). The elderly discrimination of the study subjects showed a negative correlation in both attitude and knowledge toward the elderly, and among the elderly discrimination, there was a high positive correlation with regard to emotional avoidance (r=.892, p<0.001). Conclusion: College students are the leading players in caring for the elderly and are directly affected by aging social problems. Therefore, it is considered necessary to apply various programs in the state, society, and educational institutions to avoid negative prejudices that lead to positive thinking and discrimination against the elderly.

        요양시설 노인을 위한 구강건강 평가 도구의 타당도와 신뢰도

        Background: The elderly in long-term care facilities tend to have a diminished oral health status, with a high prevalence of dental caries and periodontal disease, as reduced cognitive function, joint mobility, and gait ability hinder the performance of oral hygiene. To improve the oral health of the elderly at long-term care facilities, it is necessary to have precise guidelines for oral health care and assessment; however, such guidelines are not readily available. Therefore, the present study aimed to develop an oral care assessment tool with verified reliability and validity. Methods: The participants in this study were 100 elderly patients at a care facilities and 10 clinical dental hygienists. Collected data were analyzed using the descriptive statistics, content validity index, and inter-rater reliability, as well as the analyses of intra-class correlation coefficient. Results: After a review of relevant literature, a preliminary questionnaire comprised of seven questions related to the evaluation of oral health was formed. After revising and supplementing the questions through a content validity test, a total of nine questions were selected. Conclusion: The novel assessment tool developed for the present study is anticipated to allow analyses of the level of problems related to oral health care before routine and professional care. Moreover, regular oral health status check-ups will enable the early diagnosis and treatment of diseases.

        청소년의 신체활동 정도와 구강건강행태와의 관계

        Background: The purpose of the present study was to confirm the degree of physical activity and oral health behaviors of adolescents, and to investigate the relationship between them. Methods: The present study utilized data from the 15th (2019) Korea Youth Risk Behavior Web-Based Survey data, which included a total of 57,303 adolescents for analysis. Statistical analysis was performed using PASW Statistics 21.0. The significance level for significance was set at 0.05. Results: The rate of toothbrushing before sleeping was 1.152 times higher in the group that performed muscle strengthening exercises more than 3 times a week than in the group that did not (p < 0.001). The rate of use of oral hygiene products was 1.246 times higher in the group that performed physical activity for 60 min or more every day than in the group that did not, and 1.207 times higher in the group that performed muscle strength exercise at least 3 times a week than in the group that did not (p < 0.001). Conclusions: Adolescents who engaged in physical activities had a high rate of practice for some oral health behaviors. These results suggested that the group with positive health behaviors has a higher rate of oral health behavior practice, showing that health behaviors are highly interrelated.

      • 일부 성인의 치주질환에 대한 주관적인 인식정도와 구강환경에 관한 연구

        Periodontal diseases are one of major dental diseases that commonly happen in oral cavity. They tend to start with initial inflammation, and if they are not managed appropriately, they would cause the loss of teeth, which is a chronic disease. Patients should recognize their oral health for themselves. This study is to find out the relationship between the subjective recognition and objective assessment results. Therefore, the purpose of this study is to find out whether the subjective recognition is related to oral environment or not. This study was conducted in from June 2015 to October. Oral health assessment and survey were carried out to the adult subjects of this study who were over 19 years and visited dental clinics located in Seoul and Gyeonggi-do Province. The factors influencing oral environment was analyzed according to the subjects’ characteristics of social demography. In TPI(p<0.001), S-PHP(p<0.001), PMA(p<0.044), CPI(p<0.000), male’ figure is higher than female’ one. In a smoker group, the figure of both male and female is noticeably high in TPI(p<0.001), S-PHP(p<0.004), PMA(p<0.009), CPI(p<0.002). The factors influencing oral hygiene was analyzed according to the subjective recognition about periodontal diseases. For people who have suffered from periodontal diseases, their figures of S-PHP(p<0.010), PMA(p<0.011), CPI(p<0.024) are high, and for people who have suffered from tooth mobility, their figures are high in S-PHP(p<0.008), PMA(p<0.002), CPI(p<0.002). The figures of TPI(p<0.007), PMA(p<0.010), CPI(p<0.002) are noticeably high for the group who do not use dental floss. The factors influencing oral environment was analyzed according to the subjective recognition about oral hygiene care. The unhealthier oral health is, the higher the figure is in S-PHP(p<0.001), PMA(p<0.028), CPI(p<0.004). The less often brushing teeth is performed, the higher the figures in TPI(<0.001), S-PHP(p<0.008), PMA(p<0.033), CPI(p<0.004) are. The poorer ability to manage oral hygiene care the subjects have, their figures are noticeably high in TPI(p<0.001), S-PHP(p<0.001), PMA(p<0.002), CPI(p<0.000). There are 3 relevant factors between subjective recognition about periodontal diseases and oral environment: chewing difficulty(p<0.001) in S-PHP, alveolar bone resoption(p<.0001) in PMA, and tooth mobility(p<0.011) in CPI. As the results are indicated above, adults’ subjective recognition about periodontal diseases is not related to oral environment. Preventive programs effective for oral health care should be developed so that adult-centered dental health education programs can be activated.
