        심장호흡물리치료의 비전

        Purpose : This study is designed to discuss the vision of the cardiorespiratory physical therapy for the future. The vision of the cardiorespiratory physcial therapy will be achieved with the enthusiasm and interests of physical therapists in Korea. Currently, academic conference for the cardiorespiratory physcial therapy has been received a great deal of attention from the medical societies. The vision of the cardiorespiratory physical therapy will be realized with various programs for the research and training in the cardiorespiratory physical therapy. Korean cardiorespiratory physical therapists’ mission is to improve public health by enhancing clinical excellence in physical therapy practice through credentialing clinical specialist. The Korean academy of cardiorespiratory physical therapy(KACRPT) needs to move toward the vision of cardiorespiratory physcial therapy for the public health promotion.

        큰가슴근에 대한 수동 신장이 폐 기능에 미치는 영향

        Purpose: This study aimed to investigate the effects of passive stretching of the pectoralis major muscle on pulmonary functions. Methods: The participants were 24 male university students. They were divided randomly into the experimental group (n = 12) and the control group (n = 12). They did not have any other of neurological, muscular, and cardiopulmonary dysfunctions. A digital pulmonary function measuring device (Pony FX, COSMED Inc., Italy) was used to test pulmonary functions. The experimental group performed three sets of passive stretching on the pectoralis major muscle. One set included three bouts of passive stretching. The control group did not perform any other exercise. After passive stretching, pulmonary functions were measured again. SPSSWIN (ver 27.0) package program was used to analyze data. Results: The forced vital capacity (FVC), forced expiratory volume in one second (FEV1) of maximal effort vital capacity (MEVC), and maximal voluntary ventilation (MVV) were significantly improved in the experimental group after passive stretching. However, no change was observed in the control group. The changes in FVC, FEV1, and MVV were higher in the experimental group than in the control group. Conclusion: The results show that passive stretching of the pectoralis major muscle can improve pulmonary function parameters, namely, MEVC and MVV.

        둥근 어깨 자세 교정이 호흡 및 집중력에 미치는 효과

        Purpose: The study aimed to investigate the effect of round shoulder posture correction on respiration and concentration. Methods: A total of 34 college students were included in this study. The participants were divided into three groups: group 1, stretching and stability (n = 12); group 2, stretching and strengthening (n = 12), and group 3, control (10). The exercise interventions were conducted for 4 weeks. Group 1 performed warm-up, stretching, muscle strengthening, and cool down for 30 min. Group 2 performed warm-up, stretching, stabilization, and cool down for 30 min. Group 3 did not receive any intervention. All groups performed round shoulder posture tests, lung capacity tests to determine respiratory function, stroop test, and concentration ability test to determine the concentrations before and after intervention. Results: Significant differences were found in the round shoulder posture on the left between before and after interventions over time (p < 0.05) and between groups (p < 0.05). However, no interaction was observed. The post-hoc test showed that the interventions in group 2 were more effective than those in group 3. A significant difference was noted in the round shoulder posture on the right between before and after intervention over time (p < 0.05). No significant difference and interaction were found between groups. No significant difference was noted in lung capacity. A significant difference in the stroop test (time and number of wrong answers) was found between before and after intervention over time (p < 0.05). No significant difference and interaction were found between the groups. No significant difference was found in the concentration ability test. Conclusion: Our results reveal some evidence that stretching and strengthening exercise causes a decrease in round shoulder posture. However, the intervention period was short and did not allow investigation of the effects on respiratory function and concentration. Continued research on this subject could provide benefits to physiotherapy as it relates to cardiopulmonary treatment.

        심부전 환자의 신체적 수행 능력에 따른 도구적 일상생활 수준과 인지 수준의 연관성

        Purpose: This study aimed to investigate the level of physical functional ability in patients with heart failure and to suggest the correlation between cognitive level and instrumental daily activity level with physical function ability. Methods: The study included individuals who were diagnosed with heart failure and those who were not diagnosed with stroke. Among 66 patients who were tested for weakness at S Hospital located in Gyeonggi-do from June 2021 to April 2022, data from 56 patients were selected and analyzed. Results: In the correlation between MMSE-K and SPPB, balance (r = .381, p < .01), gait speed (r = .368, p < .01), chair stand (r = .245, p >.05), and total score (r = .387, p < .01) showed a positive correlation except for the chair stand. In the correlation between K-IADL and SPPB, balance (r = −.532, p < .01), gait speed (r = −.644, p < .01), chair stand (r = −.391, p < .01), and total score (r = −.600, p < .01) showed a negative correlation. The higher the simple physical function, the higher the cognitive level and the lower the level of need for help in the level of instrumental daily activities. Conclusion: A correlation was found between the level of physical function and the level of cognitive and instrumental daily activities in patients with heart failure; thus, its evaluation is essential.

      • 세라밴드운동과 노인의 심장 자율신경 활동에 대한 PSA 평가

        Purpose: The purpose of this study is to find out the effect of Thera-Band exercise on cardiovascular autonomic nerve system activity with the Heart Rate Variability Power Spectral Analysis(HRV PSA) by using “The Biopac and Program”. Metods: The old men were divided into two groups, Thera-Band exercise group(EG, n=13) and control group(CG, n=8).They participated in pre-test and after 8 weeks’ Thera-Band exercise. The Biopac System Test conducted for 5 minutes. Results: Heart Rate(HR) significantly effected after Thera-Band exercise. But no significant changes in CG. R-wave significantly effected after Thera-Band exercise. But no significant changes in CG. The present observations provide new information(PS form) on the contribution of the cardiovascular autonomic function from Power Spectral Analysis(PSA) to the exercise responses. High frequency(HF, 0.15-0.4Hz), as the marker of parasympathetic nerve activity tone, is the distributed power 10 -15 ~10 -17. Low frequency(LF, 0.04-0.15Hz), as the marker of sympathetic nerve activity tone, is distributed power 10 -12~10 -10. The common frequency doma- in(0.135-0.3Hz) of PSN․SN, as the marker of parasympathetic and sympathetic nerve activity tone. is the distributed power 10 -16~10 -11. Conclusion: From the discussions above results, we can conclude that Thera-Band exercise in old men induce the effect of autonomic nerve activity. These results are associated with increased and decreased HR and R-wave after Thera-Band exercise. Finally, in order to Normalized for the Power Spectrum, the further experimental work is needed to under-stand the possible mechanistic link between the cardiovascular autonomic function and the exercise responses.

        태권도 선수들의 성장 기간에 따른 심박수, 산소섭취량 및 심폐기능의 비교

        Purpose: This study aimed to investigate the characteristics of male taekwondo players by evaluating their heart rate, oxygen intake, and cardiorespiratory function according to the growth period. Methods: This study enrolled 34 male middle school, high school, and university taekwondo players belonging to D city. Their heart rate, oxygen intake, and cardiorespiratory function were evaluated according to the growth period. Results: The resting heart rate was significantly different between the groups, and the values in high school athletes were lower than those in middle school athletes and university athletes. A significant difference was found between the groups in the maximum oxygen intake, with values higher in high school athletes than in middle school athletes and university athletes. The respiratory rate at the anaerobic threshold was significantly different between the groups, with values higher in middle school players than in high school players. Conclusion: This study revealed significant differences among the groups, and results will guide in creating a systematic training program for growing taekwondo players.

        Post COVID-19 환자들의 재활

        Purpose: Data on clinical and prognosis of patients post coronavirus disease-2019 (COVID-19) and interest in rehabilitation are still insufficient. Contagious isolation causes respiratory, physical, and psychological dysfunction. Therefore, we synthesized and analyzed the effect of rehabilitation on patients with COVID-19 studied abroad. Methods: Randomized controlled trials on the rehabilitation of patients with COVID-19 were searched using four international databases: CINAHL, Embase, MEDLINE, and PEDro. For rehabilitation, a protocol including breathing training and physical activity was selected, and usual care or prior values were used for the control group. The quality of randomized controlled trials was assessed using the risk-of-bias tool provided by the Cochrane Library. Analysis was performed using RevMan 5.4, and data were weighted and pooled with a random-effects model. Results: Of the 129 studies identified, 4 were finally selected. A total of 227 patients with COVID-19 were evaluated, and significant improvements were seen in forced expiratory volume in one second (SMD = 0.75; 95% confidence interval [CI] 0.10-1.40; overall effect [Z = 2.25]) and depression (SMD = 3.46; 95% CI, 1.21-5.71; overall effect [Z = 3.02]), excluding forced vital capacity (SMD = 0.28; 95% CI −0.09 to 0.65; overall effect [Z = 1.48]). Conclusion: Rehabilitation showed improvement in respiratory function through breathing training, but a more effective protocol is required for tele-rehabilitation. In addition, interventions involving physical activity will have a beneficial effect on muscle loss and depression caused by quarantine because of coronavirus infection.

        가슴우리 확장운동이 가슴우리 확장정도, 호흡기능 및 기능적 활동에 미치는 효과

        Purpose: The study aimed to analyze the effect of chest-expansion exercise studies on chest expansion, respiratory function, and functional activity over the past 8 years (2015-2022). Methods: Previous studies were electronically searched using the PubMed, PEDro, KISS, and RISS4U databases. A total of five studies were selected according to the PRISMA guidelines, and the PEDro scale was used for the qualitative analysis. Results: Five studies met the criteria. Two studies scored 6 of 10 and three scored 5. Four studies had an intervention of 20 and 30 min, each at a frequency of 4-5 times per week, for a total of 4-8 weeks, except for one study that performed 20 min of intervention at a time, which demonstrated an immediate effect. The results indicated that chest-expansion exercises effectively improved chest expansion, respiratory function, and functional activity. Conclusion: Chest-expansion exercises exert positive effects on chest expansion, respiratory function, and functional activity.

        딥벨트의 가로막 수축압력과 초음파영상의 가로막 움직임 및 폐활량과의 상관성

        Purpose: This study was designed to determine the correlation between the diaphragmatic contraction pressure using diaphragmatic pressure belt (DiP Belt) and the ultrasound imaging of diaphragmatic motion and forced vital capacity. This study was conducted to confirm the usefulness of assessing the diaphragmatic function using the DiP Belt. Methods: This study included 41 healthy subjects (13 males and 28 females). The diaphragmatic motion was measured using sonography, and the forced vital capacity (FVC) was measured using a portable digital spirometer device. After 2 weeks, the diaphragmatic contraction pressure was measured during maximal inspiration using the DiP Belt. Results: The mean diaphragmatic contraction pressure (MDCP) and peak diaphragmatic contraction pressure (PMCP) using the DiP Belt were not significantly correlated with sonography imaging of diaphragmatic motion during quiet breathing but were significantly positively correlated with the diaphragmatic motion during deep breathing. Both the MDCP and PMCP were significantly positively correlated with FVC, forced expiratory volume in 1 second (FEV1), and peak expiratory flow (PEF). However, diaphragmatic motion was significantly positively correlated with FVC, FEV1, and PEF in only deep breathing. Conclusion: The Dip Belt can provide valuable information about the diaphragmatic function, particularly ultrasound imaging of diaphragmatic motion and lung capacity.

        운동 종료 후 호흡보조 도수기법 적용이 심장박동회복에 미치는 즉각적인 효과

        Purpose: This study was designed to investigate the effects of the application of the manual assistive breathing technique on heart rate recovery. Methods: In this study, 20 subjects were randomly assigned and divided into an experimental group and a control group. The experimental group received manual breathing assistance by a therapist immediately upon completion of exercise. In contrast, the control group rested without intervention after exercise. In this study, the heart recovery rate, rate of perceived exertion (RPE), and oxygen saturation were measured. Results: The experimental group who used the manual assistive breathing technique showed a significant difference in heart rate recovery and RPE at 30 s compared with the control group. However, no significant difference in oxygen saturation was observed between the two groups. Therefore, the manual assistive breathing technique was effective in heart rate recovery within a short time. Conclusion: In conclusion, the manual assistive breathing technique is recommended for subjects who must recover their heart rates quickly.
