현재 많이 이용되고 있는 동통 수술방법에 대하여 간단히 저술하였으며 이들 방법의 합병증에 대하여 저자들의 경험을 포함하여 기술하였다. 1) Percutaneous radiofrequency trigeminal rhizotomy는 비교적 서술하기가 간편하고 안전하여 고령자에서도 쉽게 시행할수 있는 방법이며 반복하여 시행하기가 용이하다. 안면 및 경부암으로 인한 동통에도 Cervical rhizotomy와 같이 시행하므로서 좋은 결과를 얻을 수 있다. 수술중 세심한 주의를 하므로서 각막반사 소실과 심한 이상 감각증의 발생율을 낮출수 있다. 2) 삼차 신경통에 대한 Neurovascular Decompression은 각막 반사의 소실이나 이상 감각증이 없이 통증을 치료할 수 있는 좋은 방법이며 비교적 젊은 환자에게 적용할 수 있다. Percutaneous radiofrenquency rhizotomy와 더불어 적용 대상을 잘 선택하여 사용할 수 있는 방법이다. 3) Percutaneous radiofrenquency cordotomy는 비교적 간편하고, X-선 조정이나 생리학적 조정등의 보조 조사등으로 정확하게 통각 신경로를 차단할 수 있으며 고령이나 일반 상태가 좋지 못한 말기 암환자에서도 적용이 가능하여 Open cordotomy를 대치할 것으로 생각된다. 호흡장애는 사망의 원인이 될수 있으므로 수술전 호흡 기능 검사를 하여햐 할 것이며, 양측 수술이 필요할때는 일측은 Lower cervical cordotomy를 시행하는 것이 안전하다. 4) Electrical neurostimulation은 신경조직을 파괴하지 않고 동통을 치료할수 있다는 장점이 있다. Transcutaneous electrical stimulation은 사용하기가 간편하고 합병증이 거의없고, 기구값이 저렴하므로 동통 환자에서 일차 시도해볼 만한 방법이다. 말초신경, 척수 혹은 뇌에 대한 전기자극은 적응 대상을 잘 선택하면 좋은 효과를 얻을수 있다고 하나 기구가 고가이므로 우리 형편으로는 시술하는데 어려움이 있다.

일상생활에서 피폭될 수 있는 전자기파로 뇌세포 손상 여부를 측정하기 위하여 2.45GHz 대역의 주파수를 이용하여 피폭량(10분-2.4joule, 20분-4.8joule, 30분-7.2joule)과 피폭후 시간경과(피폭 즉시, 1주, 2주, 4주후)에 따라서, 가토 36마리에서 전자기파를 조사한 후 가토의 뇌조직 손상정도를 측정한 바, 전자기파 조사후 신경 세포체, 수초신경섬유 및 무수초신경섬유에 일어난 변화는 사립체의 종창, 신경세포체와 신경섬유의 종창 및 공포형성이 나타났으며 이들 변화는 피폭량에 따라 비례하여 조직 변성이 심하였으나, 피폭후 시간이 흐름에 따라 대부분 가역성 변화를 나타내었다. 그러나 무수초신경섬유에서는 조사량에 따른 조직의 변화가 더욱 현저하게 나타났으며, 시간경과에 따라 가역성 변화 즉 회복의 정도는 역시 가장 미미하였다. 본 실험에서 전자기파의 피폭으로 신경세포의 손상을 확인 할 수 있었고, 이는 전자기파의 피폭양에 따라 뇌실질의 변화가 심하게 나타났으며, 특히 무수초신경섬유에서 손상의 정도가 심한 것을 밝혀졌다. 이들 변화는 대부분 시간경과에 따라서 세포자체의 가역성으로 회복되는 것을 증명할 수 있었다. The author measured the serverity of brain damage of 36 rabbits irradiated 2.45 GHz microwave for 10 minutes, 20 minutes and 30 minutes, respectively. Electromicroscopic and light microscopic findings of just after, Lweek, 2 weeks and 4 weeks after irradiated brain were compared in each time. Swelling and vacualization of nerve cell and mitochondria were noted. Also these changes tend to increased seventy along amount of radiation, but revealed reversible changes with time. Myelinated and unmyelinated nerve fibers were examined as same manner. These nerve fibers also revealed swelling and vascuolization, these findings also tend to increase severity with irradiation power and more prominent at unmyelinated nerve fibers. Reversability of these findings after each time period were more prominent in myelinated nerve fibers than unmyelinated nerve fibers.

Glossopharyngeal neuralgia (GPN) is a cranial nerve rhizopathic disorder that presents as extreme neuralgic pain in the ipsilateral deep throat, tongue base, or ear. Attacks of pain are usually intense and electric shock-like, occurring either without warning or triggered by swallowing. GPN is roughly estimated to account for 1% of trigeminal neuralgia cases. Microvascular decompression (MVD) surgery for GPN is still challenging, but it can be a safe and efficient option, with an over 90% cure rate for neuralgic pain. However, the glossopharyngeal or vagus nerves are composed of thin and vulnerable bundles of fine fibers, making them sensitive to surgical manipulation, such as traction or compression. Disastrous side effects such as permanent swallowing difficulty, respiration difficulty, or fatal cardiac arrhythmia could occur during or after surgery. Furthermore, MVD may not be a suitable treatment option for elderly patients or those with serious medical conditions. Gamma Knife radiosurgery for patients with GPN has recently been covered by national medical insurance, in Korea.
