        Purpose:The objective of this study was conducted to find out solution that physical therapist can get own clinic. Method:This is a literature study with books and reviewing the dismiss a suit of the court of constitutional law. Results:Physical therapist can have own clinic with legislature by the legislation and the Executive activities. Physical therapist is not only head of rehabilitation but all so controller. Physical therapist has different roll for the patient than medical doctor. Conclusion:Physical therapist was educated for the patient`s activities. Physical therapist can increase task performance of the patient and life of quality.

        The Effect of Task-oriented Training on Mobility Function, Postural Stability in Children with Cerebral Palsy

        PURPOSE: The purpose of this study is to examine how task-oriented training focused on lower extremity strengthening can affect mobility function and postural stability. METHODS: The study`s subjects included 10 children with cerebral palsy: 7 girls and 3 boys between the ages of 4 and 9 whose Gross Motor Functional Classification System (GMFCS) level was I or II. Their functional mobility was gauged using the Gross Motor Function Measurement (GMFM), and their postural stability was evaluated using a force platform. Participants received task-oriented training focused on lower extremity strengthening for 5 weeks. The study used a paired t-test to investigate the difference in mobility function and postural stability of children with cerebral palsy before and after the lower extremity strengthening exercise. RESULTS: The GMFM dimensions D (standing) (p<.02) and E (walking) (p<.001) improved significantly between the pre-test and post-test. A significant increase in the posturographic center of pressure (CoP) shift and surface area of the CoP were found overall between the pre-test and post-test (p<.001). CONCLUSION: The present study provides evidence that an 8-week task-oriented training focused on strengthening the lower extremities is an effective and feasible strategy for improving the mobility function and postural stability of children with cerebral palsy.

        관찰형태에 따른 동작관찰 훈련이 뇌졸중 환자의 안정성 한계와 동적보행능력에 미치는 영향

        PURPOSE: The purpose of this study was to determine the effect of action observation with observation type on the limits of stability and dynamic gait ability in stroke patients. METHODS: The 20 stroke patients who participated in this study were randomly divided into two experimental groups who underwent training three times a week for 4 weeks. Their balance was tested as the limit of stability with Biorescue. Their Dynamic gait ability was tested with the Dynamic Gait Index (DGI) before the intervention, and after 4 weeks. Independent and paired t-tests were used to analyze the results. RESULTS: The results confirmed the limit of stability on the moving areas of the paralyzed and non-paralyzed sides. The limit of stability and dynamic gait index measurements confirmed that the moving area showed a significant difference after the intervention in the whole movement observation group (p<.05), but the partial movement observation group showed no significant difference (p>.05). A significant difference was also noted for the comparison between the both groups after the interventions (p<.05). The functional walking ability showed a significant difference when compared to the ability before the intervention, as determined by the changes in scores obtained for the dynamic gait index (p<.05). CONCLUSION: Interventions utilizing whole movement confirm that training improves stability and functional walking ability in stroke patients with disabilities in balance and walking ability.

        치매 진단도구로서 치매노인의 보행능력 평가에 대한 연구

        PURPOSE: The purpose of this study was to compare the gait of elderly patients with and without dementia to investigate the possibility of an ambulation assessment test as a diagnostic tool for dementia. METHODS: A total of 96 subjects were included with 60 participants without dementia (control group) and 36 patients with dementia (dementia group). To compare the walking ability of the two groups, a 4-m walking test (4MWT) and Groningen Meander Walking Test (GMWT) were conducted. The GMWT is graded by amount of time in seconds and by number of oversteps outside the track. Mann-Whitney U test was used to compare the gait between the groups and the area under the curve (AUC) with Received Operating Characteristic (ROC) curve was analyzed. Statistical significance was considered at a p<.05, with a 95% confidence interval. RESULTS: There were statistically significant differences (p<.05) between the dementia group and the control group for the 4MWT, GMWTSEC, and GMWTSTEP scores. The AUC was .95 for 4MWT, .92 for GMWTSEC, and .96 for GMWTSTEP with the 95% confidence interval. The cut-off values of the ROC curve were 1.03m/s for 4MWT, 10.8 second for GMWTSEC, and 3.75 steps for GMSTEP. CONCLUSION: In our study, we investigated the utility of ambulatory assessment tools to predict dementia. The results of this study suggest that the 4MWT and the GMWT used in this study are appropriate assessment tools for dementia prediction.

        흉곽확장운동이 뇌졸중 환자의 폐기능에 미치는 효과

        Purpose The Purpose of this study was on determine whether thoracic expension exercise might increase the pulmonary function of the patients with stroke, Methods Fourty paients with stroke were randomly assigned to experimental(n=20) and control group(n=20). During four weeks, each group participated thirty minutes for five times per week. Subjects were assessed using pre-value and post-value measurement pulmonary function(Forced vital capacity, Forced expiratory volume at one second, FEV1/FVC, Peak expiratory flow, Tidal volume, vital capacity, Inspiratory capacity, Expiratory reserve volume, Inspiratory reserve volume), Results These finding suggest that experimental group was significant increase in FVC, FEV1, PEF, TV, IC, IRV, ERV (p<.05), In comparison of two group, experimental group was high pulmonary function than control group, Conclusion This study showed experimental group can be used to improve pulmonary function than control group. Thus it indicates that the thoracic expension exercise will be more improved through the continued respiratory exercise program.

        팔 흔들기가 뇌졸중으로 인한 편마비 환자의 보행에 미치는 영향

        Purpose:The purpose of this study was to investigate the variations in gait parameters according to arm swing use in post stroke hemiparesis. Methods:Sixteen patients participated in this study and walked at self-selected speeds on a Rs-scan systems. The were randomly assigned conditions: self-selected arm swing, constraint arm swing, emphasis arm swing. Results:In the comparison of parameters in each trial, both affected step length, non affected step length, affected stride length, non affected stride length, affected single support time, and non affected single support time were significantly increased and double support time was significantly decrease in emphasis arm swing when compared with both self-selected arm swing and constraint arm swing(p<.05). However, Asymmetrical ratio was significantly increased in both emphasis arm swing and self-selected arm swing when compared constraint arm swing(p<.05). Conclusion:Therefore, In this study, gait rehabilitation of patients with hemiplegia depending on what you need to apply the arm swing is considered.

        고유수용성신경근촉진법의 결합패턴과 공 운동을 통한 요부안정화운동이 만성 요통 환자의 통증 및 정적 균형에 미치는 영향

        Purpose:This study aimed to compare the effect of two modes (proprioceptive neuromuscular facilitation combination patterns and ball exercise) of low back stability for chronic low back patients. Methods:The subjects were recruited fourty patients who had low back pain. All subjects were randomly assigned to PNF combination patterns group, ball exercise group. Measurements were performed fourtimes: pre test, 2weeks, 4weeks, and 6weeks. Main outcome measures comprised the visual analogue scale(VAS), balance performance monitor(BPM). Results:The results were as follows. In the comparison of VAS score, sway area, sway path, and sway velocity according treatment period, score was significantly reduced in both PNF combination pattern group and ball exercise group. In the comparison of the both VAS and sway area between groups, there were significant. PNF combination pattern group significantly more decrease than ball exercise group at 6 weeks. However, both sway path and sway max velocity between group, there were not significantly. Conclusion:These results of this study indicated that PNF combination pattern which performed for six weeks had a significant influence than ball exercise group on low back pain.

        시트의 각도 및 높이가 앉고 서기 시 하지 및 몸통의 근활성도에 미치는 영향

        PURPOSE: This study investigated the effects of lower limb and trunk muscles activation on seat angle and height during sit to stand and stand to sit. METHODS: Subjects were instructed to sit and stand on different angles and heights; 0˚(43cm), 5˚(48cm), 10˚(51cm) and 15˚(58cm). Measurements were conducted from sit to stand and stand to sit for external oblique, transversus abdominis, rectus femoris, vastus medialis, vastus lateralis, tibialis anterior and gastrocnemius muscle activation by surface EMG. The muscle activation was measured three times for five seconds in each posture, and the %RVC value was calculated after the exclusion of one second before and after this measurement. This calculation standardized the data from each participant so it could be comparatively analyzed. RESULTS: The external oblique, tibialis anterior, and erector spinae muscles showed significant difference in 10˚(51cm), 15˚(58cm) and 0˚(43cm), rectus femoris, in 15˚(58cm) and 0˚(43cm)(p <.05) during sit to stand. The external oblique, tibialis anterior, and rectus femoris muscles showed significant difference in 0˚(43cm) and 15˚(58cm), erector spinae and vastus medialis muscles showed significant difference in 10˚(51cm), 15˚(58cm) and 0˚(43cm)(P<.05) during stand to sit. CONCLUSION: The present study findings indicate that the toilet seat angle and height could affect muscle activities of the lower limb and trunk.

        복식호흡을 병행한 척추 안정화운동이 요통환자의 균형능력과 오스웨스트리 장애지수에 미치는 영향

        PURPOSE: This study was to investigate the effect of stabilization exercise with abdominal breath on Balance and Oswestry Disability Index for Low Back Pain Patients. METHODS: The subjects were 18 low back pain patients in their twenties. The subjects were randomly assigned into experimental group and control group. Control group and experimental group both participated in 30 minutes of stabilization exercise for 8 weeks(5 times/week), and experimental group participated in stabilization exercise with abdominal breath. The subjects were tested balance and pain using BioRescue and Oswestry Diability Index respectively, before and after intervention. RESULTS: The experimental group showed a statistical significance in movement areas of the body``s center(p<.05), but there was no significant differences between groups (p>.05). The experimental group showed a statistical significance in movement distances of the body``s center with eyes open(p<.05), and there was significant differences between groups(p<.05). Both groups showed a statistical significance in ODI between pre and post test(p<.05) but there was no significant differences between groups(p>.05). CONCLUSION: As a result, the group participating in stabilization exercise with abdominal breath had a more effective improvement than the control group. Therefore, the stabilization exercise with abdominal breath may be used improving balance and pain in low back pain patients.

        탄력 밴드를 결합한 고유수용성신경근촉진법 상지 패턴이 편마비 환자의 신체정렬과 체중지지에 미치는 영향

        Purpose:The purpose of this study was to investigate how upper-extremity-movement of PNF(proprioceptive neuromuscular facilitation) with elastic band has effected on arrangement of body and weight-bearing. Methods:The object has worked with 20 patients with hemiplegia inmated and admitted a hospital in Pusan. At random they have divided into two groups - 10 experimental group who were applied typical movement therapy for stability of trunk, and 10 control group who were carried out upper-extremity-movement of PNF with elastic band. This study has carried out 30 minute a day in six weeks from 21 September 2009 to 31 October 2009. The object has used electric weighing scales to compare weight-bearing of two groups, and used full-spine X-ray to make comparison of arrangement of body. It has used Wilcoxson Signed-Rank Test Approach to analyse data of comparison how each groups was changed through this experiment and Kolmogorov-Smirnov Test has operated for verification of valence. Results:A comparison of the percentage weight-bearing, Cobb`s angle, pelvis height, and shoulder heights obtained before and after the 6-week exercise revealed statistically significant differences for the experimental group; however, there was no such difference in the case of the control group. It hasn`t appeared the statistically significant difference of valence in the comparison of each groups after this experiment. Conclusion:These results of this study is that upper-extremity-movement of PNF with elastic band improves the arrangement of body and weight-bearing of patients with hemiplegia. It will be a base to be used to objective data for more aid in clinical therapy.
