Creep-fatigue crack growth behavior was experimentally measured particularly when a crack was located in the heat affected region of 1Cr-0.5Mo steel. Load hold times of the tests for trapezoidal fatigue waveshapes were varied among 0, 30, 300 and 3,600 seconds. Time-dependent crack growth rates were characterized by the C_t-parameter. It was found that the crack growth rates were the highest when the crack path was located along the fine-grained heat affected zone(FGHAZ). Cracks located in other heat affected regions had a tendency to change the crack path eventually to FGHAZ. Creep-fatigue crack growth law of the studied case is suggested in terms of (da/dt)_avg vs. (C_t)_avg for residual life assessment.

The aim of present study was to examine the inhibitory effects of hydroxamic acid derivatives on the melanogenesis. We found that hydroxamic acid moiety was important for anti-melanogenic activity. Compounds 1a and 1b strongly inhibited melanin synthesis via deactivation of tyrosinase. Hydroxamic acid has metal ion chelating ability which is similar to that kojic acid, however, anti-tyrosinase mechanism of compounds 1a and 1b was different from that of kojic acid. Objective: Avoiding a catastrophic outcome may be a more realistic goal than achieving functional independence in the treatment of acute stroke in octogenarians. This study aimed to investigate predictors of catastrophic outcome in elderly patients after an endovascular thrombectomy with an acute anterior circulation large vessel occlusion (LVO). Materials and Methods: Data from 82 patients aged ≥ 80 years, who were treated with thrombectomy for acute anterior circulation LVO, were analyzed. The association between clinical/imaging variables and catastrophic outcomes was assessed. A catastrophic outcome was defined as a modified Rankin Scale score of 4–6 at 90 days. Results: Successful reperfusion was achieved in 61 patients (74.4%), while 47 patients (57.3%) had a catastrophic outcome. The 90-day mortality rate of the treated patients was 15.9% (13/82). The catastrophic outcome group had a significantly lower baseline diffusion-weighted imaging-Alberta stroke program early CT score (DWI-ASPECTS) (7 vs. 8, p = 0.014) and a longer procedure time (42 minutes vs. 29 minutes, p = 0.031) compared to the non-catastrophic outcome group. Successful reperfusion was significantly less frequent in the catastrophic outcome group (63.8% vs. 88.6%, p = 0.011) compared to the non-catastrophic outcome group. In a binary logistic regression analysis, DWI-ASPECTS (odds ratio [OR], 0.709; 95% confidence interval [CI], 0.524–0.960; p = 0.026) and successful reperfusion (OR, 0.242; 95% CI, 0.071–0.822; p = 0.023) were independent predictors of a catastrophic outcome. Conclusion: Baseline infarct size and reperfusion status were independently associated with a catastrophic outcome after endovascular thrombectomy in elderly patients aged ≥ 80 years with acute anterior circulation LVO.
Association between urinary sodium levels and iodine status in Korea

Background/Aims: To evaluate the association between the urinary sodium concentration and iodine status in different age groups in Korea. Methods: This nationwide, population-based, cross-sectional study used data from the Korean National Health and Nutrition Examination Survey (VI 2-3, 2014 to 2015). We included 3,645 subjects aged 10 to 75 years with normal kidney function and without a history of thyroid disease. Adequate iodine intake was defined as a urinary iodine/creatinine (I/Cr) ratio of 85 to 220 μg/g. The urinary sodium/ creatinine (Na/Cr) ratios were classified as low (< 47 mmol/g), intermediate (47 to 114 mmol/g), or high (> 114 mmol/g). Results: The median urinary iodine concentration (UIC) was 292 μg/L (interquartile range [IQR], 157 to 672), and the median urinary I/Cr ratio was 195 μg/g (IQR, 104 to 478). Iodine deficiency (< 100 μg/L) and iodine excess (> 300 μg/L) were observed in 11.3% and 49.0% of subjects, respectively. The UIC was significantly associated with the urinary sodium concentration, and the urinary I/Cr ratio was significantly correlated with the urinary Na/Cr ratio (both p < 0.001). The distributions of UIC, urinary I/Cr ratio, and Na/Cr ratio varied among age groups. Low urinary I/Cr and Na/Cr ratios were most common in young adults (age, 19 to 29 years), while high urinary I/Cr and Na/Cr ratios were most common in elderly people (age, 60 to 75 years). Conclusions: Iodine intake was significantly associated with sodium intake in the Korean population. Our study suggested that an adequately low salt intake might be helpful for preventing iodine excess in Korea.

Rice is the most important staple food in Korea. Rice blast, caused by Magnaporthe oryzae, is one of the most devastating diseasesin the rice-growing world and in Korea. Eighteen major blast resistance genes were screened in 33 leading Korean japonica varieties using44 DNA markers. The 33 leading japonica varieties are cultivated in 659,642 ha (90.4%), and a variety of Samkwang is cultivated widelyon a nation-wide scale. Four resistance genes, Pia, Pib, Pi19, and Pb1, were detected in 10-21 varieties, and the R-genes of Pii, Pish, Pita/Pita-2,Piz/Piz-t, Pi35, and Pi65(t), were present in 2-5 varieties. Five R-genes, Pit, Pi5, Pi9, pi21, and Pi40, were not detected in the Korean japonicarice varieties. A total of six varieties, two mid-late flowering varieties, Ilmi and Saeilmi, a medium flowering variety Gopum; and three earlyflowering varieties Jopyeong, Haedamssal, and Haedeul; were resistant to blast nursery screening over 12 years. A variety of Gopum harboredalleles for Piz/Piz-t, and three early flowering varieties, Jopyeong, Haedamssal, and Haedeul, had extensive loci of the multiple-gene familyPiz/Piz-t, Pi9, and Pi40 on chromosome 6. Two mid-late flowering varieties, Ilmi and Saeilmi, harbored the Pita-2 gene, and Gopum hadPita. In the japonica rice breeding program, an effective means for enhancing stable resistance to blast will be to introduce resistance genesPit, Pi5, Pi9, pi21 and Pi40 which were not identified in Korean japonica rice varieties, and resistance genes Pita/Pita-2, Piz-t/Piz-5, to 2-3R genes among and Pi20 that showed stable resistance in monogenic IRBL lines.

<P><B>Background</B></P> <P>Several electronic alert systems for acute kidney injury (AKI) have been introduced. However, their clinical benefits require further investigation.</P> <P><B>Study Design</B></P> <P>Before-and-after quality improvement study.</P> <P><B>Setting & Participants</B></P> <P>A tertiary teaching hospital in Korea, which adopted an AKI alert system on June 1, 2014. Before and after launch of the alert system, 1,884 and 1,309 patients with AKI were included in the usual-care and alert groups, respectively.</P> <P><B>Quality Improvement Plan</B></P> <P>Implementation of an AKI alert system through which clinicians could generate automated consultations to the nephrology division for all hospitalized patients.</P> <P><B>Outcomes</B></P> <P>Primary outcomes included overlooked AKI events, defined as not measuring the follow-up creatinine value, and the consultation pattern of clinicians. Secondary outcomes were severe AKI events; AKI recovery, defined based on the creatinine-based criterion; and patient mortality.</P> <P><B>Measurements</B></P> <P>ORs for events of overlooked AKI, early consultation, and severe AKI were calculated with logistic regression. AKI recovery rate and patient mortality were assessed using Cox regression.</P> <P><B>Results</B></P> <P>After introduction of the alert system, the odds of overlooked AKI events were significantly lower (adjusted OR, 0.40; 95% CI, 0.30-0.52), and the odds of an early consultation with a nephrologist were greater (adjusted OR, 6.13; 95% CI, 4.80-7.82). The odds of a severe AKI event was reduced after implementation of the alerts (adjusted OR, 0.75; 95% CI, 0.64-0.89). Furthermore, the likelihood of AKI recovery was improved in the alert group (adjusted HR, 1.70; 95% CI, 1.53-1.88). Mortality was not affected by the AKI alert system (adjusted HR, 1.07; 95% CI, 0.68-1.68).</P> <P><B>Limitations</B></P> <P>Possible unreported differences between the alert and usual-care groups.</P> <P><B>Conclusions</B></P> <P>Implementation of the AKI alert system was associated with beneficial effects in terms of an improved rate of recovery from AKI. Therefore, widespread adoption of such systems could be considered in general hospitals.</P>

Background: Onychomycosis, especially toenail onychomycosis has become one of the common fungal infection and has historically been regarded as a cosmetic rather than medical problem by many patients, even by physicians. Recently, however, there are several reports that is a refractory disease which may cause a deleterious effect on patiets quality of life (QOL). Objective: The purpose of this study was to investigate the impact of toenail onychomycosis on QOL in Korea and to assess the chandes of QOL after treatment. Methods: Total 1004 patients with toenail onychomycosis which was confirmed by clinical findings and KOH preparation were were enrolled at 47 dermatologic centers in Korea, and interviewed with standardized QOL questionnaire before and after sytemic antifungal treatment. Responses to the questionnaire were scored by 5-point scale (1∼4) and averaged, and were analyzed for 5 dimensions of emotional impact, social impact, symptorn and functional impact, patients views concerning treatment, and relationship with doctor. Results: 1. Before and after treatment, the most serious impact was emotional dimension showing 1.90 and 1.30 in average score (AS), and social (AS: 1.14 and 0.83) and symptom and functional impact (AS: 1.05 and 0.92) was also affected. 2. In female rather than male, statistically more significant impact on patients QOL was observed in all dimensions. 3. After treatment, 3 of 5 dimensions were improved significantly - emotional dimension (AS : from 1.90 to 1.30), social dimension (AS: from 1.14 to 0.83), patients view concerning treatment (AS: from 1.34 to 1.02). 4. The drgree of patients satisfaction at the therapeutic effect was very high- 62.4% (immediately after treatment) and 65.8% (9 months affer initiation of treatment) of patients answered excellent or good. Conclusion: This study confirms that toenail onychomycosis has significant impact on the overall QOL of patients. Also the effect of antifungal therapy on patients QOL were watisfactory. Therefore, both doctor and patient should pay more attention to the treatment of onychomycosis. [Kor J Med Mycol 3(1): 115∼124]
철도시스템의 효과적 RCM접근을 위한 신뢰성 기반 검사(RBI)적용

In the current railway industry in Korea, a lot of researches and studies have been progressed to establish RAMS process and construction of RCM system in accordance with international standards. On the point of view of applying the RCM to the railway system, many problems (i.e., reliability of the existing field data, the lack and incorrectness of failure information, etc) are described. To solve these problems, it is need to study and effort the RCM, practically. In this paper, lots of problems and solutions for methods determining the preventive maintenance interval of application of the RCM will be described. To do this, the method using the theory of RBI (Reliability Based Inspection) which is applied to the existing chemical plants and power plants is described. From this study, it is expected that in this situation that the quantitative data is not enough, the results that an analyst quantifies the qualitative factors and/or items will be a good reference that can effectively approach, on the side of determination about each maintenance task and inspection interval.
