보신 관광에서 감염된 수입성 Sparganosis 1예 : Intake of Thailand Snake during Trip

Human sparganosois was reported more than 100 cases in Korean population. However no case reported yet in Korea as a patient was infected by sparganum from other geographical area, such as Thailand. The case we are reported here is a 38 year-old Korean male who had been suffered from mass being a small, round and soft consistance in upper portion of his right thigh. The patients stated that he had an opportunity to eat living snake in Bangkok, Thailand during trip. As therapy praziquantel (75㎎/㎏/day) was given p.o. for 14 days, but in vain to cure. Following medical treatment, surgical exicision was performed to remove a alive, soft, whitish-milky, mobile and elongated sparganum. Serlogical test by micro-ELISA for sparganum-specific antibody revealed a high serum antibody level of absorbance 0.67 (positive criterion: absorbance 0.22).

Through the Bucherer-Bergs method, new 5-alkylthio (sulfonyl)-methyl-p-x-phenylhydantoins were prepared. Hydantoin-3-acetates were synthesized by the reaction of the hydantoins and ethyl chloroacetate and through the subsequent hydrolysis of the 3-acetates with dilute sodium hydroxide hydantoin-3-acetic acids were obtained. These products are expected to exhibit antiinflammatory and analgetic activities.

This study has been carried out to prevent and decrease the grinding burr under various working conditions; number of grinding passes, depth of cut, hardness of the materials and work speed. Grinding wheel is WA grinding wheel and workpieces are SM45C, SS41, SCM23 and STS 304. The results of tained from this study are as follows; The length of gringing burr increased with increasing grinding passes and it has nothing to do with materials and heat treatment. The size of grinding burr increased with increasing grinding depth and feed speed. The size of grinding burr decreased with increased cross setion and of workpiece. According as the hardness of material was increased, the width of grinding burr decreased conspicuously owing to suppression of plastic zone of burr root part.

An algorithm to automatically extract coordinate and time information from optical observation data of geostationary orbit satellites (GEO satellites) or geosynchronous orbit satellites (GOS satellites) is developed. The optical wide-field patrol system is capable of automatic observation using a pre-arranged schedule. Therefore, if this type of automatic analysis algorithm is available, daily unmanned monitoring of GEO satellites can be possible. For data acquisition for development, the COMS1 satellite was observed with 1-s exposure time and 1-m interval. The images were grouped and processed in terms of "action", and each action was composed of six or nine successive images. First, a reference image with the best quality in one action was selected. Next, the rest of the images in the action were geometrically transformed to fit in the horizontal coordinate system (expressed in azimuthal angle and elevation) of the reference image. Then, these images were median-combined to retain only the possible non-moving GEO candidates. By reverting the coordinate transformation of the positions of these GEO satellite candidates, the final coordinates could be calculated.

The detector subsystem of the Optical Wide-field Patrol (OWL) network efficiently acquires the position and time information of moving objects such as artificial satellites through its chopper system, which consists of 4 blades in front of the CCD camera. Using this system, it is possible to get more position data with the same exposure time by changing the streaks of the moving objects into many pieces with the fast rotating blades during sidereal tracking. At the same time, the time data from the rotating chopper can be acquired by the time tagger connected to the photo diode. To analyze the orbits of the targets detected in the image data of such a system, a sequential procedure of determining the positions of separated streak lines was developed that involved calculating the World Coordinate System (WCS) solution to transform the positions into equatorial coordinate systems, and finally combining the time log records from the time tagger with the transformed position data. We introduce this procedure and the preliminary results of the application of this procedure to the test observation images.

<P>Oxidative stress in the retinal pigment epithelium (RPE) can lead to the pathological causes of age-related macular degeneration (AMD). Hypoxia induces oxidative damage in retinal pigment epithelial cells (RPE cells). In this study, we investigated the capacity of 3,3-diindolyl-methane (DIM) to reduce the expression of vascular endothelial growth factor (VEGF) under hypoxic conditions, as well as the molecular mechanisms involved. Human RPE cells (ARPE-19 cells) were treated with cobalt chloride (CoCl2, 200 mu M) and/or DIM (10 and 20 mu M). The production of VEGF was measured by enzyme-linked immunosorbent assay. The trans-location of hypoxia-inducible factor-1 (HIF-1) and nuclear factor-B (NF-B) was determined by western blot analysis. The binding activity of HIF-1 and NF-B was analyzed by electrophoretic mobility shift assay. The phosphorylation levels of mitogen-activated protein kinases (MAPKs) were measured by western blot analysis. The levels of mitochondrial reactive oxygen species (ROS) were detected by fluorescence microplate assay. The results revealed that DIM significantly attenuated the CoCl2-induced expression of VEGF in the ARPE-19 cells. The CoCl2-induced translocation and activation of HIF-1 and NF-B were also attenuated by treatment with DIM. In addition, DIM inhibited the CoCl2-induced activation of p38 MAPK in the ARPE-19 cells. Pre-treatment with YCG063, a mitochondrial ROS inhibitor, led to the downregulation of the CoCl2-induced production of VEGF by suppressing HIF-1 and NF-B activity. Taken together, the findings of our study demonstrate that DIM inhibits the CoCl2-induced production of VEGF by suppressing mitochondrial ROS production, thus attenuating the activation of HIF-1 and p38 MAPK/NF-B.</P>

금속 대 금속 인공 고관절 치환술에서 제거된 거칠기 처리 시멘트형 스템의 표면 분석

목적: 금속 대 금속 인공고관절 치환술 시행 후 실패한 대퇴스템의 표면을 분석하여 스템의 이완과 골용해의 원인, 임상적 요인 등을 문헌고찰과 함께 조사하고자 한다. 대상 및 방법: 관찰대상은 1996년 1월부터 1998년 12월까지 41명(남자 30명, 여자 11명) 43 고관절을 대상으로 하였으며, 거칠기 처리(Ra=1.6 μm) 된 시멘트형이 18례, 무시멘트형이 25례였다. 평균 10.15년 추적 조사 후 임상결과는 Harris hip score (HHS), 운동량은 Devane 지수를, 방사선적 결과는 Engh 방법으로 조사하였으며, 제거된 대퇴스템은 SEM (scanning electron microscopy) 방법으로 조사 되었다. 결과: 임상결과로 HHS는 수술 전 56.2에서 92.6로 향상 되었으며, 방사선상 전 례에서 무시멘트 비구컵과 무시멘트 대퇴스템은 골형성이 관찰되었다. 재치환술은 8례 시멘트 스템에서 시행되었으며 이중 1례는 반복된 탈구로 인한 재치환술로 나머지 7례에서는 조기 골용해와 이완소견이 발견되어 재치환술이 시행되었다. 통계학적으로 거칠기 처리된 시멘트스템과 최종추시 생존률(p=0.001)은 통계학적 관련이 있었으며, 직업등과 관련된 운동량과 생존률 (p=0.031)도 통계학적 의미를 보였다. 결론: 본 논문의 후향적 연구 결과, 금속-금속 인공 고관절 전치환술에서, 시멘트 스템을 사용하는 경우에 발생된 조기 골용해와 이완의 높은 발생률을 볼 때, 활동량이 높은 환자에서 표면 처리가 거칠기 처리된 시멘트 스템 (≥Ra=1.6 μm)의 사용은 고려되어야 할 것으로 보인다. Purpose: This study investigated the possible etiologic role of stem loosening and osteolysis by examining the surface of a rough blast retrieval cement stem and physical activity in second generation metal-on-metal (MOM) total hip arthroplasty (THA) Materials and Methods: Forty one patients (43 hips) were analyzed retrospectively from Jan. 1996 to Dec.1998. Of the femoral stem, a rough blasted (Ra=1.6 um) cement stem and a cementless Ti-alloy stem was used in 18 and 25 hips, respectively. The mean follow-up duration was 10.15 years. Each patient was evaluated clinically by the HSS and radiographically by Engh`s method. Scanning electron microscopy was used to observe the retrieved femoral stem and histology studies were performed. Results: The mean Harris hip score improved from 56.2 points before surgery to 92.6 points after surgery. All the cementless stems and cups showed excellent results at the last follow-up. Eight of the rough blast cement stems were revised. Seven showed loosening and one was a recurrent dislocation. Cross-validation evaluation revealed osteolysis to be associated with a rough surface cement stem of metal on metal THAs (p=0.001). In addition, patients who underwent strenuous activity had a higher incidence of osteolysis (p=0.031). Conclusion: These findings suggest that a rough blast surface cement stem is associated with early osteolysis and loosening in patients with a metal-on-metal hip replacement. A prospective study with a large group of patients and multiple diagnostic methods will be needed to determine if there is a causal relationship between metal hypersensitivity and osteolysis. These results suggest that a cement stem with a rough blasted surface should be considered in metal-on-metal total hip arthroplasty.
