      • Increased Effect-Site Concentration of Propofol Reduces EC50 of Remifentanil for Successful Intubation Using the Shikani Optical Stylet without Neuromuscular Blockade

        Objective: For short-duration surgery, propofol and remifentanil are the drugs of choice for intubation without neuromuscular blockade. The Shikani Optical Stylet (SOS) is a novel semi-rigid type fiberoptic laryngoscope. In this study, we determined the clinically required effect-site concentration of remifentanil for intubation using SOS without neuromuscular blockade depending on propofol effect-site concentration. Methods: We enrolled patients scheduled for elective surgery with general anesthesia, and assigned them into two groups by a randomized, double-blind method: concentration of propofol 3.5 μg/mL (group PRO 3.5) and 7.0 μg/mL (group PRO 7.0). Anesthesia was conducted with target-controlled infusion in predetermined target effect-site concentrations of propofol. The concentration of remifentanil for successful intubation using SOS in 50% of patients (EC50) was determined using a modified Dixon’s up-anddown method. Results: The mean±standard deviation EC50 of remifentanil was 5.07±0.40 ng/mL in group PRO 3.5 and 1.79±0.44 ng/mL in group PRO 7.0. From probit analysis, EC50 and EC95 of remifentanil in group PRO 3.5 were 4.85 ng/mL (95% confidence interval [CI], 4.44– 5.16 ng/mL) and 5.42 ng/mL (95% CI, 5.13–7.47 ng/mL) respectively, and EC50 and EC95 of remifentanil in group PRO 7.0 were 1.68 ng/mL (95% CI, 1.22–2.01 ng/mL) and 2.29 ng/mL (95% CI, 1.98–4.05 ng/mL), respectively. Conclusion: Increased concentration of propofol reduced EC50 of remifentanil for successful intubation using SOS without neuromuscular blockade.

        Detection of Lymphomatous Marrow Infiltration using F-18 FDG PET at Initial Staging and after chemotherapy

        목적 : 악성 림프종 환자에서 골수 침범 여부를 평가하는 데 있어 FDG PET 스캔의 유용성을 장골 골수 생검에 비교하여 평가하고자 한다. 대상 및 방법: 73명의 림프종 환자(남:녀=43:30, 평균 연령 47세)를 대상으로 하였다. 이 중 69명은 비호지킨성 림프종이었으며 4명은 호지킨성 림프종이었다. FDG PET은53명의 환자에서 병기 결정을 목적으로, 20명에서는 치료후 효과 평가를 위해 시행되었다. 골수 침범에 대한 최종 판정은 두 검사의 일치성으로 판단되었으며 서로 다른 경우엔 장골 골수 생검, 방사선 검사 소견, 그리고 추적 검사에 의해 결정되었다. 결과: 73명중 54명(74%)에 있어 FDG PET과 장골 골수 검사 결과가 일치하였다. 두 검사 모두 음성인 환자는 54명중 47명이었으며 모두 양성인 환자는 7명이었다. 두 검사가 불일치 하는 경우는 19명이었었으며 이 중 6명은 골수 검사는 음성이었으나 FDG PET에서 양성 소견을 보였다. 반대로 12명에서는 FDG PET은 음성이었으나 장골 골수 생검상 골수 침범이 확인되었다. 불일치 결과를 보인 19명 중 마지막 1명의 환자에서는 골수 생검상 음성이었으나 FDG PET에서 위양성 소견을 보였다. 결론 : FDG PET은 림프종 환자의 병기 결정에 있어 장골 골수 검사상 발견되지 않은 골수 침윤을 발견하는 데 도움이 되어 골수 생검에 보조적인 역할이 있었다. PET 검사상 횡격막 상하 림파절 침범과 다수의 골수 병변이 있는 환자에 있어 골수 생검의 필요성에 대해서는 치료 방침이나 예후를 변화시킬 수 있는지의 여부를 보는 연구가 필요하리라 생각된다. 치료후 효과를 판정하는 데 있어 FDG PET은 골수의 현미경학적 잔존 질환을 평가하는 데 있어 골수 생검을 대체하기는 어려울 것으로 사료된다. PURPUSE: To assess the ability of FDG PET for the detection of bone marrow infiltration compared to iliac crest biopsy in patients with lymphoma. MATERIALS AND METHODS: Seventy-three patients (females and 43 males, mean age of 47 years old) malignant lymphoma (4 Hodgkin's disease, HD and 69 Non-Hodgkin's lymphoma, NHL) were included. FDG PET was performed for staging in 53 patients and to assess treatment response after the completion of chemotherapy in 20 patients. Final conclusions were based on biopsy, other imaging studies, or clinical follow-up. RESULTS: There were 54 (74%) of the 73 patients in whom FDG PET and iliac crest biopsy were concordant. Forty-seven of the 54 patients showed concordant negative results while the remaining 7 patients had concordant positive results. Of 19 patients with discordant results, FDG PET accurately detected bone marrow infiltraion in 6 patients with negative iliac crest biopsy. On the contrary, iliac crest biopsy identified bone marrow infiltraion in 12 of the 19 patients in remaining one of the 19 patients with discordant results, iliac crest biopsy were true negative but FDG PET was falsely positive. CONCLUSION: FDG PET seems to be an adjunct in detecting marrow infiltraion that may not be revealed by iliac crest biopsy staging. For the assessment of treatment response, it may be less helpful than biopsy in detecting microscopic residual disease in the bone marrow.

        Experimental Research Article : Preconditioning of isoflurane on spinal cord ischemia can increase the number of inducible nitric oxide synthaseexpressing motor neurons in rat

        Background: Spinal cord ischemia with resulting paraplegia remains one of the most common complications after repair of thoracoabdominal aortic aneurysms or dissection. Inducible nitric oxide synthase (iNOS) is known to have both neuroprotective and neurotoxic effects in the central nervous system. We investigated the possible relationship between the effect of pre-ischemic isoflurane exposure on mild spinal cord ischemia and the inducible nitric oxide synthase (iNOS) expression by using iNOS-specific antibody and pyrrolidinedithio carbamate (PDTC), NF-κB inhibitor, in the ventral horn of spinal cord in rats. Methods: The animals were divided into five groups (n = 6 in each group): sham group, control group, PDTC-treated group, isoflurane-treated group, and PDTC/isoflurane-treated group. In the PDTC-treated groups, 2% 100 mg/kg PDTC was administered intraperitoneally at 1 h before operation and at 24 h and 48 h after reperfusion. The rats in the isoflurane-treated groups received 30 min inhalation of 2.8% isoflurane at 24 h before spinal cord ischemia. Immunohistochemistry was performed to detect iNOS expression in the motor neuron of the ventral horn in spinal cord. Results: Preconditioning with isoflurane increased the iNOS expression when compared to the control group (P < 0.05), whereas pre-treatment with both PDTC and isoflurane significantly decreased the iNOS expression compared to isoflurane-treated group (P < 0.05). Conclusions: Pre-ischemic isoflurane exposure was related with increase of the iNOS expression via a pathway modulated by NF-κB. iNOS may act as an important mediator of delayed preconditioning with isoflurane for the protective effect against spinal cord ischemia.       • Synthesis of Centimeter-Scale Monolayer Tungsten Disulfide Film on Gold Foils

        <P>We report the synthesis of centimeter-scale monolayer WS<SUB>2</SUB> on gold foil by chemical vapor deposition. The limited tungsten and sulfur solubility in gold foil allows monolayer WS<SUB>2</SUB> film growth on gold surface. To ensure the coverage uniformity of monolayer WS<SUB>2</SUB> film, the tungsten source-coated substrate was placed in parallel with Au foil under hydrogen sulfide atmosphere. The high growth temperature near 935 °C helps to increase a domain size up to 420 μm. Gold foil is reused for the repeatable growth after bubbling transfer. The WS<SUB>2</SUB>-based field effect transistor reveals an electron mobility of 20 cm<SUP>2</SUP> V<SUP>–1</SUP> s<SUP>–1</SUP> with high on–off ratio of ∼10<SUP>8</SUP> at room temperature, which is the highest reported value from previous reports of CVD-grown WS<SUB>2</SUB> samples.         Recombinant Human Bone Morphogenic Protein-2 (Rhbmp-2) Immobilization onto the Surface of Apatite-Coated Titanium Significantly Promotes Osteoblast Function and Mineralization

        Titanium (Ti) has been widely used in the dental field owing to its good biocompatibility, superior mechanical properties, and excellent corrosion resistance. However, Ti lacks the osteoconductivity and osteoinductivity required to promote mineralization. In the present study, the authors investigated whether apatite-coated Ti and rhBMP-2 treated apatite-coated Ti promote more mineralization than pristine Ti. Characterizations of pristine Ti, apatite-coated Ti, or rhBMP-2/apatite-coated Ti were analyzed using scanning electron microscopy (SEM), energydispersive spectroscopy (EDS), and attenuated total reflection Fourier transform infrared spectroscopy (ATR-FTIR). In addition, the release profile of rhBMP-2 from rhBMP-2/apatite-coated Ti was monitored for 28 days, and the biocompatibility of pristine Ti, apatite-coated Ti, and rhBMP-2/apatite-coated Ti was evaluated by measuring cell proliferation, alkaline phosphatase (ALP) activity, calcium deposition, and real-time PCR using MG-63 cells. SEM, EDS, and ATR-FTIR showed that the apatite on apatite-coated Ti surfaces was similar to that of natural bone. Furthermore, rhBMP-2 appeared to be released steadily over 28 days from rhBMP-2/apatite-coated Ti, and MG-63 cells grown on rhBMP-2/apatite-coated Ti showed significantly higher proliferation activity, ALP activity, and calcium deposition compared to MG-63 cells grown on pristine Ti or apatite-coated Ti. Furthermore, osteocalcin and osteopontin gene expression in MG-63 cells grown on rhBMP-2/apatite-coated Ti was significantly greater than that in MG-63 cells grown on pristine Ti or apatite-coated Ti by real-time PCR. Taken together, rhBMP-2/apatite-coated Ti substrate has enhanced osteoblast function and mineralization. Thus, rhBMP-2/apatite-coated Ti may be a more effective substrate than pristine Ti used in the dental field.

        Spinel(AB_2O_4)is one of the most important compound ceramic materials. The formation reactions have been extensively studied, however, the formation kinetics has not been completed. In this paper, a thorough derivation for the kinetics of MgAl_2O_4 spinel formation via diffusion bonding was performed using flux equations.       • Hierarchically Porous Carbon Nanosheets from Waste Coffee Grounds for Supercapacitors

        <P>The nanostructure design of porous carbon-based electrode materials is key to improving the electrochemical performance of supercapacitors. In this study, hierarchically porous carbon nanosheets (HP-CNSs) were fabricated using waste coffee grounds by in situ carbonization and activation processes using KOH. Despite the simple synthesis process, the HP-CNSs had a high aspect ratio nanostructure (∼20 nm thickness to several micrometers in lateral size), a high specific surface area of 1945.7 m<SUP>2</SUP> g<SUP>–1</SUP>, numerous heteroatoms, and good electrical transport properties, as well as hierarchically porous characteristics (0.5–10 nm in size). HP-CNS-based supercapacitors showed a specific energy of 35.4 Wh kg<SUP>–1</SUP> at 11250 W kg<SUP>–1</SUP> and of 23 Wh kg<SUP>–1</SUP> for a 3 s charge/discharge current rate corresponding to a specific power of 30000 W kg<SUP>–1</SUP>.
