        입원된 정신활성물질 남용자의 사회정신의학적 특징

        This study was attempted to clarify sociopsychiatric characteristics according to the class of psychoactive substances which were classified into 8 groups such and methamphetamin, cocaine, inhalants(bond/gas), dextromethorphan(Romilar), sedatives marijuana, analgesics and etc. The data of the study were collected from 134 psychoactive substance abusers(89men, 45women), who had admitted to SNMH during past 5 years. The results were as follows ; 1) The mean age of patients was 26.5±9.6 years and according to the class of psychoactive substance, inhalant and dextromethorphan abusers were significantly younger than sedatives and analgesics abusers(P〈0.01). 2) 43.1% of patients were grown up in broken family until puberty and 27.9% of married patients showed unstable marital state such as a divorce or seperation. 3) 41.4% of patients had past history of delinquent behavior and especially, inhalants and dextromethorphan abusers had more past history of delinquent behavior than sedatives and analgesics abusers(P〈0.01). 4) The main motive of mitiation of psychoactive substance was peer pressure, relief of psysical symptoms, curiosity in that order. 75% of inhalants abusers were itiated by peer pressure, but most of sedatives and analygesics abusers used it for the purpose of relieving of physical symptoms, and methamphetamine abusers used it mainly due to peer pressure or for the purpose of increasing sexual plesure(P〈0.01). 5) 57.5% of patients reported the experience of unwanted response such as unpleasant hallucinatory state, loss of control, and psychotic state after use of psychoactive substances. Especially the unwanted respose was reported more frequently in methamphetamin and inhalants abusers. 6) 38.5% of patients abused more than two classes of psychoactive substances, and their significant combination types were not found but inhalant-dextromethorphan, analygesics-sedatives and methamphetamine-marijuana combination types were relatively common.

      • Spheroid 조합자극치료 적용이 신체구성에 미치는 영향

        The present study aims to explore the effects of applying spheroid-combined stimulating therapy on body composition. For this aim. 20 persons were selected as subjects and 10 of them each were assigned to the experimental group and control group, respectively. The experimental group was applied a 12-week combined stimulating therapy and changes in physical constitution of the members were measured before application and 4, 8 and 12 weeks after application. From the study, following conclusions could be drawn: First, body weights of the experimental group decreased from 59.9kgs before application of the spheroid-combined stimulating therapy to 57.2kgs 12 weeks after application, no significant but 4.51% change. Second, body fats of the experimental group changed from 18.8kgs before application to 16.4kgs 12 weeks after application, no significant but 12.77% reduction. Third, % fats of the experimental group decreased from 31.2% before application to 28.6% 12 weeks after application, no significant change but 8.33% reduction. Fourth, body mass index(BMI) of the experimental group decreased from 23.23 kg/㎡ before application of the therapy to 21.43kg/㎡ after application, no significant but 8.03% reduction. As seen above, the 12-week application of the spheroid-combined stimulating therapy produced no significant changes in physical constitution as physical exercise did, but improved physical constitution by reducing certain amount of boy weight, body fat, % fat and BMI. In order to detect more positive and significant changes in physical constitutions, continued studies and clinical experiments should be performed to more diverse subjects using technically reinforced equipment for spheroid-combined stimulating therapy.

        Environmental Exposure to Arsenic, Lead, and Cadmium in People Living near Janghang Copper Smelter in Korea

        <P>Concentrations of heavy metals exceed safety thresholds in the soil near Janghang Copper Refinery, a smelter in Korea that operated from 1936 to 1989. This study was conducted to evaluate the level of exposure to toxic metals and the potential effect on health in people living near the smelter. The study included 572 adults living within 4 km of the smelter and compared them with 413 controls group of people living similar lifestyles in a rural area approximately 15 km from the smelter. Urinary arsenic (As) level did not decrease according to the distance from the smelter, regardless of gender and working history in smelters and mines. However, in subjects who had no occupational exposure to toxic metals, blood lead (Pb) and cadmium (Cd) and urinary Cd decreased according to the distance from the smelter, both in men and women. Additionally, the distance from the smelter was a determinant factor for a decrease of As, Pb, and Cd in multiple regression models, respectively. On the other hands, urinary Cd was a risk factor for renal tubular dysfunction in populations living near the smelter. These results suggest that Janghang copper smelter was a main contamination source of As, Pb, and Cd, and populations living near the smelter suffered some adverse health effects as a consequence. The local population should be advised to make efforts to reduce exposure to environmental contaminants, in order to minimize potential health effects, and to pay close attention to any health problems possibly related to toxic metal exposure.</P>

        Online Learning-Based Downlink Transmission Coordination in Ultra-Dense Millimeter Wave Heterogeneous Networks

        <P>In heterogeneous ultra-dense networks with millimeter wave macro cells and small cells, base stations (BSs) and mobile user equipments (UEs) perform beamforming operations to establish highly directional links. In spite of the spatial diversity achieved through directional links, as a number of BSs are densely deployed, inter-cell interference caused by concurrent directional transmissions of adjacent BSs becomes severe, resulting in downlink performance degradation in the network. However, it is very difficult to manage inter-cell interference because of the nature of the time-varying wireless fading environment, the dynamic changes in beam propagation directivity, and unpredictable UEs’ locations. In this paper, we propose an online learning-based transmission coordination algorithm based on the framework of multi-armed bandits to learn the unknown characteristics of inter-BS interference and exploit learned data to derive an optimal policy for maximizing the number of successful downlink transmissions. Through the numerical simulations, we verify the effectiveness of the proposed online learning-based inter-BS interference management scheme.</P>

        A Comparison of Patient Acceptance and Preferences Between CT Colonography and Conventional Colonoscopy in Colorectal Cancer Screening

        Background/Aims: Colorectal cancer, one of the most common cancers in developed countries, is curable when diagnosed at an early stage. However, for better screening, both a test that patients will tolerate and diagnostic accuracy are required. We compared patient experiences and preferences between computed tomographic (CT) colonography and conventional colonoscopy (CC) under conscious sedation. Methods: Patients referred to the gastrointestinal clinic for CC were enrolled to also undergo CT colonography prior to CC. After each procedure, patients completed a questionnaire in which variables, such as abdominal pain, abdominal discomfort, and loss of dignity, were assessed using a 7-point Likert scale, with the highest score representing the worst experience. To verify response stability, a telephone questionnaire followed within 24 h after each procedure. Patients were then asked about their preference for CT colonography or CC. Results: Data were collected from 51 patients who fulfilled all requirements, including CT colonography, CC, the two questionnaires after each procedure, and a follow-up questionnaire. Severity of abdominal pain, abdominal discomfort, and a loss of dignity were reported to be higher in CT colonography than in CC (p<0.01). In addition, the preference for CC was significantly higher than that for CT colonography (p<0.01). Conclusions: Although CT colonography is a safe and noninvasive screening test for colorectal cancer, further study is required to increase patient acceptance. (Korean J Intern Med 2009;24:43-47)

        Functional links between clustered microRNAs: suppression of cell-cycle inhibitors by microRNA clusters in gastric cancer

        <P>microRNAs (miRNAs) play integral roles in diverse processes including tumorigenesis. miRNA gene loci are often found in close conjunction, and such clustered miRNA genes are transcribed from a common promoter to generate polycistronic primary transcript. The primary transcript (pri-miRNA) is then processed by two RNase III proteins to release the mature miRNAs. Although it has been speculated that the miRNAs in the same cluster may play related biological functions, this has not been experimentally addressed. Here we report that the miRNAs in two clusters (miR-106b∼93 ∼ 25 and miR-222 ∼ 221) suppress the Cip/Kip family members of Cdk inhibitors (p57<SUP>Kip2</SUP>, p21<SUP>Cip1</SUP> and p27<SUP>Kip1</SUP>). We show that miR-25 targets p57 through the 3′-UTR. Furthermore, miR-106b and miR-93 control p21 while miR-222 and miR-221 regulate both p27 and p57. Ectopic expression of these miRNAs results in activation of Cdk2 and facilitation of G1/S phase transition. Consistent with these results, both clusters are abnormally upregulated in gastric cancer tissues compared to the corresponding normal tissues. Ectopic expression of miR-222 cluster enhanced tumor growth in the mouse xenograft model. Our study demonstrates the functional associations between clustered miRNAs and further implicates that effective cancer treatment may require a combinatorial approach to target multiple oncogenic miRNA clusters.</P>

        Voltage Drop in a Ferroelectric Single Layer Capacitor by Retarded Domain Nucleation

        <P>Ferroelectric (FE) capacitor is a critical electric component in microelectronic devices. Among many of its intriguing properties, the recent finding of voltage drop (V-drop) across the FE capacitor while the positive charges flow in is especially eye-catching. This finding was claimed to be direct evidence that the FE capacitor is in negative capacitance (NC) state, which must be useful for (infinitely) high capacitance and ultralow voltage operation of field-effect transistors. Nonetheless, the NC state corresponds to the maximum energy state of the FE material, so it has been widely accepted in the community that the material alleviates that state by forming ferroelectric domains. This work reports a similar V-drop effect from the 150 nm thick epitaxial BaTiO3 ferroelectric thin film, but the interpretation was completely disparate; the V-drop can be precisely simulated by the reverse domain nucleation and propagation of which charge effect cannot be fully compensated for by the supplied charge from the external charge source. The disappearance of the V-drop effect was also observed by repeated FE switching only up to 10 cycles, which can hardly be explained by the involvement of the NC effect. The retained reverse domain nuclei even after the subsequent poling can explain such behavior.</P>
