      • Patient- and family-centered care interventions for improving the quality of health care: A review of systematic reviews

        <P><B>Abstract</B></P> <P><B>Background</B></P> <P>Patient- and family-centered care interventions are increasingly being implemented in various settings for improving the quality of health care. However, the huge amounts of information coming from both primary studies and reviews on patient- and family-centered care interventions have made it difficult to identify and use the available evidence effectively.</P> <P><B>Objectives</B></P> <P>This review aimed to synthesize and evaluate the evidence from published systematic reviews on the effects of patient- and family-centered care interventions. It also aimed to assess the quality of the systematic reviews in order to formulate recommendations for improving the quality of future systematic reviews.</P> <P><B>Design</B></P> <P>Review of systematic reviews.</P> <P><B>Data sources</B></P> <P>Six databases were searched for relevant published reviews that assessed patient- and family-centered care interventions and were reported on in English in peer-reviewed journals up to September 18, 2017. The reference lists of all selected publications were also used to identify additional eligible studies.</P> <P><B>Review methods</B></P> <P>Reviewers independently selected reviews, extracted data, and assessed the methodological quality of the included reviews using A MeaSurement Tool to Assess Systematic Reviews (AMSTAR) checklist. These results were presented and discussed among researchers to resolve disagreements and reach a consensus. A narrative approach was adopted to pool the constituent elements of interventions. The review protocol was registered with PROSPERO (registration number CRD42017080427).</P> <P><B>Results</B></P> <P>Twenty-eight reviews published between 2011 and 2017 met the inclusion criteria. The interventions targeted the patients, their family members, and the health-care. The interventions involved the following core outcomes: Regarding patients, they were improving knowledge about their health, increasing skills to manage self-care behaviors, enhancing satisfaction, increasing quality of life, and reducing admissions, readmissions, and length of the hospital stay. Regarding family members, they were reducing the intensity of stress, anxiety, depression, and increasing the satisfaction and relationship with health-care providers. Regarding health-care providers, the interventions could improve job satisfaction and confidence, quality of care, and reduce stress and burnout. The overall methodological quality of the 28 reviews was moderate, with a mean AMSTAR score of 6.79 (SD 1.45).</P> <P><B>Conclusion</B></P> <P>This review has provided evidence for the effects of patient- and family-centered care interventions applied to diverse patients, family members, and health-care providers. The evidence indicates that patient- and family-centered care could be a critical approach for improving the quality of health care. Additionally, the quality of future reviews needs to be improved in order to produce reliable evidence in the current era of evidence-based practice.</P>

      • The roles of unmet needs and formal support in the caregiving satisfaction and caregiving burden of family caregivers for persons with dementia

        <B>ABSTRACT</B><B>Background:</B><P>A growing number of studies are emphasizing the importance of positive and negative appraisals of caregiving and the utilization of social resources to buffer the negative effects of caring for persons with dementia. By assessing the roles of unmet needs and formal support, this study tested a hypothesized model for Korean family caregivers’ satisfaction and burden in providing care for persons with dementia.</P><B>Methods:</B><P>The stress process model and a two-factor model were used as the conceptual framework for this study. Data for 320 family caregivers from a large cross-sectional survey, the Seoul Dementia Management study, were analyzed using structural equation modeling. In the hypothesized model, the exogenous variables were patient symptoms, including cognitive impairment, behavioral problems, and dependency on others to help with activities of daily living and with instrumental activities of daily living. The endogenous variables were the caregiver's perception of the unmet needs of the patient, formal support, caregiving satisfaction, and caregiving burden.</P><B>Results:</B><P>The adjusted model explained the mediating effect of unmet needs on the relationship between patient symptoms or formal support and caregiving satisfaction. Formal support also had a mediating effect on the relationship between patient symptoms and unmet needs. Patient symptoms and caregiving satisfaction had a significant direct effect on caregiving burden.</P><B>Conclusion:</B><P>The level of unmet needs of persons with dementia and their family caregivers must be considered in the development of support programs focused on improving caregiving satisfaction.</P>

        모바일 앱 기반 간호대학생 핵심간호술 자가학습 프로그램 개발

        본 연구는 간호대학생을 대상으로 핵심수기술 자가학습용 모바일 앱을 개발하고, 유용성을 평가하기 위해 실시된 융합적 연구이다. 모바일 앱 평가도구(MARS)와 7개의 서술형 문항을 사용하여 22명의 간호대학생을 대상으로 모바일 앱의 유용성 평가하였다. 정보제공성 영역이 4.19(SD 0.79)점으로 가장 높았으며, 주관적 앱의 질영역이 3.08(SD 0.87)점으로 가장 낮은 평가를 받았다. 대상자의 핵심수기술 수행자신감은 8.23(SD 1.60)점 이었으며, 학습만족도는 7.89(SD 0.87)점이었다. 또한, 서술형 문항에서 학생들은 시공간의 제약이 없는 편리성과 반복적인 자가학습이 가능하다는 점을 가장 큰 강점으로 꼽았으며, 디자인과 기술적인 보완, 교수자 피드백의 필요성을 언급하였다. 본 융합연구의 결과를 활용하여 핵심수기술 자가학습의 효과적인 학습 촉매제로 모바일 앱의 사용을 도모할 수 있을 것이며, 향후 간호대학생의 핵심수기술 수행역량에 대한 모바일 앱의 효과연구가 필요하다. A convergence study was conducted to develop a smartphone application for self-practice of core nursing skills and evaluate its usefulness for undergraduate nursing students. Mobile Application Rating Scale and seven essay questionnaire were used to for usability evaluation among 22 undergraduate nursing students. The score of the information domain was the highest with 4.19(SD 0.79). The subjective quality domain showed the lowest score of 3.08(SD 0.87). Participants' performance confidence score was 8.23(SD 1.60), and learning satisfaction score was 7.89(SD 0.87). Participants reported that the convenience and repetitive self-learning were the strengths of the app. In addition, design and technical supplementation, and lecturer-feedback would improve effectiveness of the current educational app. Findings of this convergent study would be helpful to promote the application of mobile apps for effective self-learning of core nursing skills in undergraduate nursing education. Future resesarch is needed to examine effectiveness study of mobile app on the performance of core nursing skills.

        응급처치 원격지도용 스마트글래스 사용에 대한 한국 해양경찰의 인지된 유용성 및 태도

        본 연구는 서남해안 해양경찰이 이송하는 응급환자 유형과 이와 관련된 교육 및 원격의료지도의 필요성, 통신수단으로서 스마트글래스에 대한 인식과 태도를 알아보고자 31명의 해양경찰을 대상으로 수행되었다. 응급환자 유형은 출혈, 자상, 복통에서 상대적으로 높은 빈도와 훈련요구도가 확인되었다. 반면에 약물사용과 중증도 분류에 대한 원격의료지도 요구가 높았으며, 통신도구로서 스마트글래스의 활용에 대한 인지된 유용성과 태도점수는 각각 3.76±0.61점, 3.64±0.45점이었다. 스마트글래스에 대한 인지된 유용성과 태도 간 중간 정도의 상관관계가 확인되었다(r=.630, p<.01). 기술의 발전과 함께 해양경찰의 응급구조 활동의 효율성과 안전성을 높이기 위해 스마트글래스와 같은 새로운 장비의 적극적 도입과 다양한 노력이 필요한 시점이다. This study was to investigate the types of emergencies transported by the Southwestern Coast Guard, the need for telemedicine guidance, and the perception and attitude of smart glasses as a communication method targeting 31 coast guards. A relatively high frequency and training requirement were confirmed for bleeding, abrasion, and abdominal pain. The demand for telemedicine guidance on medication and triage was higher, and the perceived usefulness and attitude scores for the use of smart glasses were 3.76±0.61 and 3.64±0.45, respectively. A moderate correlation between perceived usefulness and attitude toward smart glasses was confirmed (r=.630, p<.01). With the development of technology, it is time to actively introduce new devices such as smart glasses.
