        어린이에서 함치성 낭과 연관된 매복 소구치와 대구치의 치료

        치아 매복은 어린이 환자 진료시에 자주 관찰되는 맹출 장애이다. 어린 환자에서 매복된 치아가 존재하는 경우,함치성 낭과 연관이 있을 가능성이 높다. 함치성 낭은 제3대구치를 제외하고는 상악 견치,하악 소구치 부위에서 가장 호발하며,점진적인 증식으로 악골이 팽윤되어 안모를 변화시키며,주위 악골의 파괴와 치근의 흡수를 야기하거나 침범된 치아의 변위를 유발할 수 있으므로,조기 진단과 적절한 치료가 무엇보다도 중요하다. 함치성 낭과 연관된 치아가 과잉치나 지치라면,치아의 발거를 포함한 완전한 낭종 적출술이 적절한 치료라 할 수 있지만,그렇지 않은 경우에는 환자의 심리적,정선적 외상을 예방하기 위해 원인 치아의 보존이 고려되어야 할 것이다. 이뿐만 아니라,치아의 변위 정도,골 파괴 정도,치근의 성숙도,주위 치아와의 관계,환자의 교합과 구강 악안면 영역의 성장 양상 등도 같이 고려되어야 할 것으로 생각된다. 본 증례에서는 위와 같은 사항들을 고려하여,함치성 낭과 연관된 매복 소구치와 대구치블 낭종 적출술 후 공간 유지,외과적 수술과 교정적 견인,외과작 발거 후 교정적 배열 등의 방법을 통해 양호한 치료 결과를 얻었기에 보고하는 바이다. Tooth impaction is a frequently observed eruption anomaly in pediatric dental practice. Young patients with impacted or unerupted teeth have more prediction for dentigerous cyst formation. Dentigerous cyst presents radiographic features. unilocular or multilocular radioluscency. Cysts occur most frequently in the premolar region except third molar. Dentigerous cysts can grow to a considerable size, and large cysts may be associated with a painless expansion of the bone in the involved area. Extensive lesions may result in facial asymmetry, osseous destruction, root resorption of proximal teeth and displacement of associated tooth. The nature of the causative tooth influences the type of surgical treatrnent required for the dentigerous cyst. If the cyst is associated with a supernumerary or wisdom tooth. complete enucleation of the cyst along with extraction of tooth may be the first treatment choice. Otherwise. preservation of the associated teeth should be considered to prevent a young patient from psychological and mental trauma because of the loss of tooth. We should consider the degree of tooth displacement. osseous destruction and growth pattern of oromaxillofacial area when planning treatment. Thus a proper and logical treatment planning can help a proper growth and development of oromaxillofacial area and can save the patient from a psychological and mental trauma. This report describes 4 cases of the management of impacted premolars and molars associated with dentigerous cysts in children.

      • S-483 Albumin Level Modifies Paradoxical Effect between BMI and Mortality in AKI Patients Undergoing CRRT

        Background: Nutritional status is generally accepted as a factor to explain the paradoxical association between high body mass index (BMI) and survival benefit in critical ill patients. Patients with acute kidney injury (AKI), particularly those receiving continuous renal replacement therapy (CRRT), are highly catabolic and more susceptible to loss of energy. Thus, we aimed to investigate the relationship between albumin level, BMI, and mortality in these patients. Methods: We conducted an observational study in 573 patients who had undergone CRRT owing to various causes of AKI between 2010 and 2014. Patients were divided into quartiles according to BMI ; Q1, 12.7 - 20.9; Q2, 21.0 - 23.5; Q3, 23.6 - 26.5; Q4, 26.6 - 45.3 and also classified into two groups by albumin (albumin <3.0g/dL or ≥ 3.0g/dL) in each quartile. The study end point was defined as death that occurred within 30 days after the initiation of CRRT. Results: The mean age was 61.5 years and 355 (63.1%) patients were male. The mean levels of BMI and albumin were 23.9±4.3 kg/m2 and 2.6±0.6 g/dL, respectively. Kidney function, comorbidity index, and sepsis-related organ failure assessment (SOFA) score were not significantly different among the BMI quartile groups. During 30 days after the initiation of CRRT, 85 patients (59.4%) died in the highest quartile as compared with 106 patients (73.6%) in the lowest quartile (p=0.012). In a multivariable analysis adjusted for sex, blood pressure, estimated glomerular filtration rate, sepsis, age adjusted Charlson comorbidity score, SOFA score, albumin and white blood cell count, CRRT prescription, the highest quartile of BMI was associated with a decreased risk of death [hazard ratio (HR), 0.61; 95% confidence interval (CI), 0.46-0.82; p=0.001]. This association remained unaltered in low albumin group (HR, 0.58; 95% CI, 0.41-0.80; p=0.001), whereas no such relationship was seen in high albumin group (HR, 0.72; 95% CI, 0.35-1.48; p=0.37).?Conclusions: This study showed that a protective effect of high BMI was observed in AKI patients undergoing CRRT only in patients with low albumin levels, and this effect was lost in patients with higher albumin levels.

        현재 생물학 연구실에서 시퀀싱 실험을 통해 생성되거나 또는 공개용 생물 데이터베이스로부터 획득된 유전체 및 단백질 정보는 각각 이질적인 데이터형식을 사용하고 있다. 이 때문에, 생물정보를 분석하여 상호 간의 정보를 효율적으로 사용하기 위해서는 공통된 형식의 데이터 표준화작업이 필수적이다. 그리고 이러한 이질적 데이터 형식에 대한 표준화 연구의 미비로 인하여 플랫 파일간의 정보공유에 어려움을 겪고 있다. 따라서, 이 논문에서는 다양한 유전체 및 단백질 정보를 관리 공유하기 위해 이질적인 포맷간의 맵핑 과정을 통하여 BSML(Bioinformatic Sequence Markup Language) 형태로 변환하고 이를 객체관계형 데이터베이스 (Object Relational DataBase)에 저장하는 시스템을 개방하였다. 그리고, 개방된 시스템은 생물정보 데이터의 표준화를 위해 개방된 XML(Extend Markup Language) 기반의 BSML을 이용함으로써 효율적으로 생물학 데이터들 간의 정보를 공유할 수 있으며, 개인 생물학 데이터베이스 구축이나 다양한 생물학적 데이터를 통합관리하는 시스템에서 유용하게 쓰일 수 있다.

      • S-536 The Effect of PTD-mediated BMP-7 on EMT in Peritoneal Mesothelial Cells

        Objective:?We investigated the effect of PTD-mediated BMP-7 (tissue-regeneration polypeptide 2, TRP2) on TGF-β1-induced EMT in cultured human peritoneal mesothelial cells (HPMCs). In addition, we investigated how to deliver the drugs to peritoneum in vivo models. Design and Method:?In vitro, HPMCs were cultured in M199 media containing 5.6 mM glucose (normal glucose, NG), NG + TGF-β1 (2 ng/ml) with or without TRP2 (100 ng/ml). After 72 hours, cells were harvested. In vivo, PD rat-O-port inserted into 11 Sprague-Dawley rats, and saline (control group, n=3), 4.25 % PD solution (PD group, n=3) or 4.25 % PD solution + TRP2 (PD + TRP2 group, n=5) were infused for 4 weeks. After 4 weeks, rats were sacrificed and the peritoneal tissues were removed. E-cadherin, ZO-1, α-smooth muscle actin (α-SMA), snail, vimentin, type I collagen, and fibronectin protein expression in HPMCs and the peritoneum were estimated by western blot analysis, and fibronectin expression was evaluated by immunohistochemistry staining. PF was assessed by Masson’s trichrome (MT) staining. Results:?In vitro, protein expression of E-cadherin and ZO-1 (epithelial marker) were significantly decreased, while α-SMA, snail, vimentin (mesenchymal marker), type I collagen and fibronectin were significantly increased in TGF-β1-stimulated HPMCs compared to control group, and these changes were significantly improved by TRP2 treatment. In vivo, peritoneal EMT and PF were significantly increased in PD rats compared to control rats. The thickness of mesothelial layer and the intensity of MT staining in the peritoneum of PD rats were also significantly higher compared to control rats. These changes of the peritoneum in PD rats were significantly ameliorated by the administration of TRP2. Conclusions:?This study suggests that TRP2 directly inhibits the process of TGF-β1-induced PF via peritoneal EMT in HPMCs. In addition, TRP2 mitigates PF in PD rats. The effect of PTD-mediated recombinant protein delivery system may be a potential therapeutic strategy for prevention of PF in PD patients.

StrokePortal is a comprehensive resource for information on stroke that integrates and provides essential findings regarding stroke pathology, diagnostics, and treatments, based on Oriental and Western medicine. The stroke information was collected from various sources, such as journal articles, books, websites, and news stories, and it was refined, classified, and stored into a relational database system by automatic classification and manual curation. To provide the stored information effectively to users, a specialized retrieval system, based on web interfaces, was implemented. StrokePortal provides cutting-edge information to experts; interested people, including patients and their families; and investigators to broaden their knowledge of effective treatments for patients and offer many preventive measures.         Patients with infiammatory bowel disease (IBD) have an elevated risk of venous thromboembolism compared with the general population. The most common sites of venous thromboembolism in IBD patients are the deep veins of the legs and pulmonary system, followed by the portal and mesenteric veins. Cerebral venous thrombosis is extremely rare in IBD-related venous thromboembolism. This report describes a case of Crohn`s disease complicated by cerebral venous thrombosis. A 17-year-old female, diagnosed with Crohn`s disease 4 years ago, hospitalized with headache, anorexia, vomiting for 6 weeks. Brain MRI& venography showed venous thrombosis in cortical veins, superior sagittal sinus, right transverse sinus, and right internal jugular vein. The patient immediately started anticoagulation therapy with intravenous heparin infusion followed by oral rivaroxaban 10mg qd. After anticoagulation, her headache was improved. At 2-weeks later, Brain MRI & venography showed resolved venous thrombosis with recanalization of cortical veins, superior sagittal sinus, right transverse sinus, and right internal jugular vein. She had taken anticoagulation with rivaroxaban for 6 months, remains well during the outpatient follow-up.
