        부산·울산·경남지역 직업병 감시체계

        Objective: Occupational medicine specialists in the Busan, Ulsan and Kyung-Nam areas established an area-based occupational disease surveillance system and used this system to collected case information for the purpose of preventing occupational diseases Methods: l l hospital participated in this system. The authors selected five main diseases, which were hand-arm vibration syndrome (HAVS), work-related musculoskeletal disorder (WRMSD), occupational asthma, occupational skin disease and occupational and occupational lung cancer and established their case definitions. All cases were reported on the web, and real time analysis was conducted. Results: Between April 2001 and April 2003, 192 cases of HAVS, 118 cases of WRMSD, 33 cases of occupational asthma, 17 cases of occupational skin disease and 17 cases of occupational lung cancer (for a total of 377 cases of these five main diseases) were reported. most of the HAVS cases came from shipbuilding, and 172 of them (92.7%) were associated with grinding. Of the four main types of WRMSD, carpal tunnel syndrome (CTS) was the most prevalent with 46 cases and shipbuilding was also the main industry involved (83.9%). The ergonomic risk factors involved mainly associated with the hand. In 19 (57.6%) and 4 (12.1%) cases of occupational asthma, the agents involved. The causative agents of occupational lung cancer included Cr, welding fume, PAH, Ni, etc. 10(58.8%) of the cases were diagnosed as squamous cell carcinoma and 5(29.4%) as adenoma. Conclusion: This result showed that an area-based occupational disease surveillance system might provide an effective method of evaluating the prevalence of such diseases, however the Busan, Ulsan and Kyung-Nam provinces are too large to be treated as individual areas. Therefore, the authors suggest that each province should establish its own surveillance system.

        응급 의료 센터내 사망 환자의 분석

        We need the constitute of Emergency medical system that connect prehospital care of inhospital care effectively for optimal treatment of emergency patient. The analysis of actual condition of our emergency medical system, through study of mortality case in emergency medical center will be a aid to the constitute. The authors performed a review on the records of 60 cases who died in Ajou university Emergency medical center during management, since June 1994 to september 1995. Among the 60 patient, 72% were male and 28% were female. In nontrauma patient, 6th decade was many, in trauma patients, 3rd and 5th decade was many. Among the 60 patient, 33% were traumatic cases, 66% were nontraumatic cases. Among the traumatic patients, more than half of the cases were due to motor vehicle accidents. The most frequent time interval from onset of emergent situation to arreving at emergency center was more than 2 hours. The most common transfer method was by 119 ambulance. In most cases, prehospital treatments were not taken. In most cases, initial mental status was comatose, and systolic blood pressure was less than 60 mmHg. Most cardiovascular resuscitation was done in 30 min. to an hour. In cases as traffic accident of ischemic heart disease, where rapid transportation of the patient is critical for the patient's survival, however, in most cases it took more than two hours to bring such patients to hospital, and first aid treatment before arrival to hospital was rare. No prehospital treatment was done to nearly all patient. In many cases the patient was already in under coma or shock state. The inhospital care at emergency center was made relatively quickly and adequately. The mean duration of cardiopulmonary resustation was 30min to one hour. From this study, we could notice the poor quality of prehospital care in the region ,near Ajou university hospital, and we came to know that the improvement of quality of prehospital care was the most important factor to reduce the motality of emergency department patient. that is, in the treatment of emergency patients, weak points has been revealed in the pre-hospital treatment, the improvement of which is important factor for the survival of emergency patients.

        The debate over the change in the Mandibular position during the cervical headgear traction has continued for decades. The purpose of this report is to assess the generalized rotation of the Mandibule occured due to the cervical traction by a graphic method that Isaacson proposed, and illustrate the mechanism by which the vertical independent variables affect the sagittal position of the mandible. The consequences is in accordance with the Baumrind's report that the use of cervical traction is associated with condyle growth, and its effect is to keep the Mandibular plane angle relatively constant.

        Most of debris flow occurred in Korea have been known to be caused by the heavy rainfall at the soil deposits on the mother rock, affected by conditions of rainfall, topography and geology, especially terrain deposits. A study on debris flow behavior should be carried out by investigating various types of debris flow systematically and analyzing their complicate characteristics in the engineering view points. Tremendous debris flows occurred at Duksan-ri in Inje-gun of Gangwon province during summer in 2006. These sites are selected to study the characteristics of debris flow by investigating the influencing factors on it and analyzing their correlations between them.       • Functional Shift of Mandible 에 대한 임상적 연구

        Anterior Bite Plate has been used as an auxillary appliance in orthodontic therapy for a long time. It's main use has been as a bite opening appliance for deep bite cases. Since it's main mode of action was allowing vertical extrusion of molars, Anterior Bite Plate was contraindicated for so called "high angle" cases or long face syndrome cases. For cases showing functional shift of mandible, traditional treatment mechanics involved the use of functional appliances or use of heavy elastics towards the end of fixed appliance therapy. In present report, I have shown the applying Anterior Bite Plate at beginnging of therapy for a short time can aid in the correction of functional shift of mandible by elimination of oral reflexes that caused antero-posterior or lateral shift of mandible from premature occlusal contacts and by reeducation of closing muscles which made the mandible to close into more physiologically stable position. All three cases presented are either vertical growing facial type or back ward rotating facial types, which according to traditional concepts are contraindicated to the use of Anterior Bite Plate , but since the use of Anterior Bite Plate in these csaes were limited to very short time, no adverse reaction on facial growth were observed.

      • Al-Li과 Al-Li-Cu 合金의 高溫變形擧動에 관한 硏究

        In this study, Al-2.0wt%Li and Al-2.0wt%Li-1.2wt%Cu alloys heat treated in aging condition with the highest value of hardness are tested in tension at various temperature(10, 100, 200 and 300℃) and their hot deformation behaviors was observed. In case of Al-2.0wt%Li alloy, the amount of reduction of tensile strength is gradually increased with increasing tension temperature, but elongation rapidly increase at 300℃. The dislocations in Al-Li alloy tested tension at 100℃ after aging at 190℃ for 16 hours move as superlattice dislocation and the between them was wide. In case of tension at 200 and 300℃ the dislocation forms loop with increasing temperature by being bow-out, the interaction mode between dislocations and precipitates changes from cutting mode to bypassing mode. In case of Al-2.0wt%Li-1.2wt%Cu alloy, its tensile strength is more increased than that of Al-Li alloy owing to solid solution hardening of Cu and precipitation of T_1 phase, but its elongation decreases and shows great reduction at 300℃ tension. The distance between two dislocations with superlattice dislocation increase with increasing tensile temperature, and finally dislocation moves as single one. The dislocation in tension at 300℃ forms Orowan loop and by-pass the precipitates.

      • 전위성 3급 부정교합 환자에서의 교합평면의 Decompensation에 대한 임상적 고찰

        Transitional Class Ⅲ is mandibular prognathism that functional factor is transitive to skeletal factor. Considerations for the treatment of transitional Class Ⅲ with deep bite are functional factor, LFH & overbite, anterior component of force, inclination of maxillary incisor, occlusal plane angle and residual growth. The purpose of this report is attempt to decompensate transitional class Ⅲ with inadequate compensation of occlusal plane due to functional factor and then compensate residual protrusive causing anatomic factor correctly. Because treatment range that orthodontists can control is limitted to lower facial area, this report deal with occlusal plane angle and functional occlusal plane. In these 3 cases, after assessment of skeletal analysis, adequate compensation pattern of each case was reevaluated and harmonious skeletal pattern was established.
