We have applied a haptic device to the rehabilitation tool and made efforts to integrate the motion and sensory therapy, in order to avoid boredom that the patients usually feel. This rehabilitation tool system consists of a haptic device, a display, a computer, and software for training. When the user moves the grip, the haptic device provides a virtual force and it can either assists the movement of the arm or work against it. We have carried out a preliminary experiment with young and middle age group and elderly group. It aims to confirm the functionality of this system and to collect the reference data for the evaluation of the upper-limb disability. The results of our experiment show that the system could be used to evaluate the patient"s ability of upper-limb motion.
Two Dimensional Measurement of Electron Density in LHD plasmas by Sheet-Shaped Thermal Lithium-Beam

In the Large Helical Device (LHD), of which the magnetic surfaces are surrounded by a complicated ergodic magnetic field layer, two dimensional measurement of electron density was carried out by using a thermal lithium beam shaped into a sheet extended along the major radius. The thermal lithium beam having a sheet shape was produced in a simple way, the lithium vapor was passed through three orifices placed in front of an aperture of an oven, and was injected vertically into the LHD plasma. Two dimensional images of LiI emission were obtained by using a CCD camera with an interference filter for the LiI spectral line. A two dimensional profile of electron density near the plasma edge region was successfully derived from the LiI emission image.

Aging is a natural process where changes occur in multiple tissues, resulting in aesthetic and functional effects. Aging eyelids lead to a tired or sleepy appearance. Progressive changes continue throughout life; these may eventually affect the ocular surface and cause visual impairment. Objective is to report the pathophysiology of the common age-related eyelid disorders and the current management concepts. This article presents the pathophysiology and the surgical procedures of eyelid disorders, such as blepharoptosis, dermatochalasis, entropion, ectropion, baggy eyelids, epicanthus, and lateral canthal aging. In general, most agerelated eyelid changes are corrected due to its to its associated cosmetic and functional problems. Management is based on anatomic factors contributing to the eyelid disorder. Conclusion is the choice of surgical procedure is based on the exact localisation of the eyelid pathology.

Study Design: Retrospective observational study. Purpose: We examined change in lumbrosacral spine alignment and low back pain (LBP) following total hip arthroplasty (THA) in patients with severe hip osteoarthritis (OA). Overview of Literature: Severe hip osteoarthritis has been reported to cause spine alignment abnormalities and low back pain, and it has been reported that low back pain is improved following THA. Methods: Our target population included 30 patients (29 female, mean age 63.5 years) with hip OA who underwent direct anterior approach THA. There were 12 cases with bilateral hip disease and 18 cases with unilateral osteoarthritis. Visual analogue scale (VAS) scores for LBP and coxalgia, the Roland-Morris Disability Questionnaire (RDQ), and the Japanese Orthopaedic Association Back Pain Evaluation Questionnaire (JOABPEQ) were assessed before and after surgery. Spinal alignment metrics were measured before and after surgery. Results: VAS for LBP change from preoperative to final postoperative observation was significantly improved (p <0.05), as was VAS for hip pain (p <0.001). RDQ improved significantly (p <0.01). All five domains of JOABPEQ were significantly improved (p <0.05). In terms of coronal alignment, lumbar scoliosis change from preoperative to last observation was significantly reduced (p <0.05). There were no significant changes in the sagittal alignment metrics. In addition, there was a correlation between before and after RDQ difference and before and after lumbar scoliosis difference (p <0.05). VAS for LBP (p <0.05) as well as RDQ (p <0.05) were significantly improved only in unilateral OA. Lumbar scoliosis was significantly improved in cases of unilateral OA (p <0.05), but alignment did not improve in cases of bilateral OA (p =0.29). Conclusions: The present study demonstrates improvements in VAS for LBP, RDQ, and all domains of JOABPEQ. There were also significant reductions in lumbar scoliosis and an observed correlation of RDQ improvement with lumbar scoliosis improvement. We were able to observe improvements in lumbar scoliosis and low back pain only in cases of unilateral OA. It has been suggested that the mechanism of low back pain improvement following THA is related to compensatory lumbar scoliosis improvement.

Study DesignRetrospective cohort study.PurposeThis study aimed to evaluate aggravated lateral recess stenosis and clarify the indirect decompression threshold by combined lateral interbody fusion and percutaneous pedicle screw fixation (LIF/PPS).Overview of LiteratureNo previous reports have described an effective radiographic indicator for determining the surgical indication for LIF/PPS.MethodsA retrospective review of 185 consecutive patients, who underwent 1- or 2-level lumbar fusion surgery for degenerative spondylolisthesis (DS). According to their symptomatic improvement, they were placed into either the “recovery” or “no-recovery” group. Preoperative computed tomography (CT) images were evaluated for the position of the superior articular processes at the slipping level, followed by a graded classification (grades 0–3) using the impingement line (I line), a new radiographic indicator. All 432 superior articular facets in 216 slipped levels were classified, and both groups’ characteristics were compared.ResultsThere were 171 patients (92.4%) in the recovery group and 14 patients in the no-recovery group (7.6%). All patients in the no-recovery group were diagnosed with symptoms associated with deteriorated bony lateral recess stenosis. All superior articular processes of the lower vertebral body in affected levels reached and exceeded the I line (I line-; grade 2 and 3) on preoperative sagittal CT images. In the recovery group, most superior articular processes did not reach the I line (I line+; grade 0 and 1; p=0.0233).ConclusionsIn DS cases that are classified as grade 2 or greater, the risk of aggravated bony lateral recess stenosis due to corrective surgery is high; therefore, indirect decompression by LIF/PPS is, in principle, contraindicated.
햅틱 장치를 이용한 상지 운동기능 장애인의 재활치료에 관한 기초 실험

상지 운동기능 장애인의 재활치료에 대한 지속적인 관심과 훈련의 정량적인 평가가 요구되고 있다. 이러한 문제를 해결하기 위해서 햅틱 장치를 개발했다. 피험자가 햅틱 장치의 손잡이를 움직일 때, 가상의 힘을 제시하여 그들의 동작에 대해 어시스트력이나 항력을 제시할 수 있다. 개발한 상지 운동훈련 프로그램과 시스템의 유효성을 검토하기 위해, 5 명의 운동기능 장애인을 대상으로 직선동작 훈련프로그램을 수행할 때의 손잡이 위치, 수행 속도, 손잡이의 오차 및 상지의 EMG 신호를 측정하였다. 5 명의 피험자의 손잡이의 궤적 및 EMG 활동이 유사함을 보였다. 이러한 결과는 가상 현실의 힘을 손잡이에 제시함으로써 EMG 활동이 증가하였다고 볼 수 있다. 본 실험의 결과, 객관적이며 정량적인 상지 재활치료 시스템의 개발 및 평가 방법이 시사되었다. Rehabilitation exercises must maintain a patient’s interest and permit a quantitative evaluation of the rehabilitation. We have developed a haptic-device system. When users move a grip, the haptic device provides a virtual force that either assists the movement of their arm or working against it. To investigate the functional effect of this system in a rehabilitation program, we used for five subjects with motor-function disorders and measured the grip position, velocity, force exerted on the grip, and EMG activities during a reaching task of one subject. The accuracy of the grip position, velocity and trajectories patterns were similar for all the subjects. The results suggested that the EMG activities were improved by applying the virtual force to the grip. These results can be used for the development of rehabilitation programs and evaluation methods.

Objective: Models for genomic selection assume that the reference population is an unselected population. However, in practice, genotyped individuals, such as progeny-tested bulls, are highly selected, and the reference population is created after preselection. In dairy cattle, the intensity of selection is higher in males than in females, suggesting that cows can be added to the reference population with less bias and loss of accuracy. The objective is to develop formulas applied to any genomic prediction studies or practice with preselected animals as reference population. Methods: We developed formulas for calculating the reliability and bias of genomically enhanced breeding values (GEBV) in the reference population where individuals are preselected on estimated breeding values. Based on the formulas presented, deterministic simulation was conducted by varying heritability, preselection percentage, and the reference population size. Results: The number of bulls equal to a cow regarding the reliability of GEBV was expressed through a simple formula for the reference population consisting of preselected animals. The bull population was vastly superior to the cow population regarding the reliability of GEBV for low-heritability traits. However, the superiority of reliability from the bull reference population over the cow population decreased as heritability increased. Bias was greater for bulls than cows. Bias and reduction in reliability of GEBV due to preselection was alleviated by expanding reference population. Conclusion: Cows are easier in expanding reference population size compared with bulls and alleviate bias and reduction in reliability of GEBV of bulls which are highly preselected than cows by expanding the cow reference population.
