Endoscopic Management of Staple Line Leak after Bariatric Surgery: Surgeon’s Perspective

Laparoscopic sleeve gastrectomy (LSG) has become a standalone primary procedure as a bariatric metabolic surgery since the early2000s. The overall complication rate of LSG is reported to range from 2% to 15%. Staple line leakage (SLL) remains a major adverseevent and occurs in approximately 1–6% of patients. Choosing the optimal treatment modality is a complex process. Clinicians mustunderstand that nutritional support and drainage of fluid collection are essential for initial management. Conservative endoscopicmanagement and sufficient drainage can resolve approximately 70% of SLLs. Endoscopic management of bariatric complicationshas been rapidly evolving in recent years and can be considered in all patients who are hemodynamically stable. We will reviewthe available endoscopic management techniques, including stent placement (self-expanding stents and bariatric-specific stents),clipping, tissue sealant application, and internal drainage (double-pigtail stents [DPS] placement, endoscopic vacuum therapy, andseptotomy). Stent placement remains the mainstream treatment for SLLs. However, healing with stents requires multiple sessions/stents and a long course of recovery. Endoscopic internal drainage is gaining popularity and has the potential to be a superiormethod. The importance of early intervention and combined endoscopic methods should be recognized.
부모교육 참여정도와 기관만족도 및 교육공동체 인식 비교 연구: 유아 부모와 초등 부모를 대상으로

본 연구는 유아 자녀를 둔 부모와 초등 자녀를 둔 부모를 대상으로 교육공동체로서 유아교육기관 및 초등학교에 대한 인식, 부모교육 참여정도, 기관만족도를 살펴 보고자 한 것으로, B시와 K지역의 유아 자녀를 둔 부모와 초등 자녀를 둔 부모를 대상으로 설문조사를 실시하였다. 분석에 활용된 설문지는 유아 부모 대상이 총 544부, 초등 부모 대상이 총 422부이며, 빈도분석, t검증, 일원배치 분산분석 및 Pearson의 적률상관분석을 실시하였다. 그 결과 첫째, 유아 부모는 부모의 학력과 월수입에 따라 교육공동체 인식에 차이가 있었고, 월수입에 따라 부모교육 참여정도에 차이가 있었다. 둘째, 초등 부모는 학력과 월수입에 따라서 교육공동체 인식에 유의미한 차이가 있었다. 셋째, 교육공동체 인식과 기관만족도는 유아 부모가 초등 부모보다 더높은 것으로 나타났고, 부모교육 참여정도는 유아 부모보다 초등 부모가 더 높았다. 넷째, 유아 부모는 교육공동체 인식과 부모교육 참여정도, 기관만족도 사이에 상관 관계가 없었으나, 초등 부모는 부모교육 참여정도와 기관만족도가 높을수록 교육공 동체 인식이 높은 것으로 나타났다. 이러한 결과를 토대로 본 연구의 시사점을 논의 하였다. The study aims to examine the perception of the educational community, parent’s educational involvement and institutional satisfaction between early childhoods’s parents and elementary school student’s parents. It conducted surveys targeting early childhood’s parents and elementary school student’s parents in B city and K area. The results of this study are as follows: First, it finds differences in perception of the educational community according to the parents’ education and monthly income, and parent’s educational involvement according to their monthly income, which figures are from early childhood’s parents. Second, it shows meaningful figure about the perception of the educational community according to the parents’ education and monthly income. Third, early childhood’s parents have higher perception of the educational community and institutional satisfaction than elementary school student’s parents. Forth, it shows correlations which are the higher perception of the educational community, the higher parent’s educational involvement and institutional satisfaction. The implications, limitations, and future suggestions were also discussed.
