        사망 진단서(시체 검안서) 작성의 문제점

        Background: This study was conducted to analyze the current problems in completing death certificates and to identify the correct method for completing death certificates Methods: We reviewed 262 death certificates in three hospitals from March 1 to April 30, 2000, and 119 death certificates in one hospital from March 1 to 31, 2000. We identified major and minor errors and analyzed and compared them retrospectively. Results: A total of 381 death certificates were reviewed: 59 in Seoul National University Hospital,101 in Ewha Woman's University Hospital, and 102 in Gachon Medical College Hospital, which has no education program for completing death certificates in postgraduate training, and 119 in Samsung Medical Center which has an education program for completing death certificates. 358 certificates(94.0%) had at least one error. There were only 23 death certificates(6.0%) without an error. In 182 cases(47.8%), there was one major error. In 321 death certificates(84.3%), there were more than two errors. A comparison of Samsung Medical Center with the other hospitals showed that the number of total errors was statistically different(p=0.001). Conclusion: There were few death certificates without an error in this study. In a hospital which has postgraduate training in completing death certificates, there are fewer errors than in other hospitals which have no training course. Emergency physicians actually certify many deaths, so they must know the correct method of completing death certificates for statistics on morbidity and mortality.

      • 금강 수서생태계의 구조와 기능에 관한 연구 : 수질 및 미생물

        소백산맥의 주봉인 덕유산에서 발원하여 군산지역에서 황해로 유입되는 금강의 본류는 총 유로연장이 401.4km에 이르는 우리나라 4대강 중의 하나이며, 대전광역시, 충청남북도 및 전라북도 일부지역의 생활용수 및 산업용수의 공급원으로서 대단히 중요한 역할을 담당하고 있다. 1960년대 이후 계속적으로 추진된 경제개발과 공업화 정책에 의해 인구의 도시집중화, 생활수준의 향상등이 이루어지고, 이에 따라 양질의 생활용수 및 산업용수에 대한 수요는 계속적으로 크게 증가하고 있는 실정이다. 그러나 금강의 경우 도시의 생활하수, 공업단지로부터의 공업폐수, 유역의 축산폐수 등에 의해 수질을 비롯한 수환경의 악화가 점차 심화되고 있으며, 이에 따른 수서생태계의 변화도 매우 큰 것으로 보고되고 있다. 특히 최근에 조성된 바 있는 금강 하구둑이 94년 8월부터 가동되고 있고, 또 1995년 이후 금강종합개발이 중류와 하류지역을 중심으로 대대적으로 시행될 예정이어서 금강 본류의 수환경은 장차 많은 변화가 예상된다. 수환경의 변화는 생육하는 생물들의 종조성은 물론 군집의 구조와 기능에 큰 영향을 미치기 때문에 금강 본류의 수서생태계에도 많은 변화가 수반될 것으로 예측되고 있다. 이와 같이 금강본류를 대상으로 하는 사업이 시행된 후 야기되는 수환경의 변화는 장기간에 걸친 조사를 요구하며, 이로부터 얻은 결과를 사업 시행 이전의 조사 결과와 비교함으로써만이 효과적으로 그 경향을 파악할 수 있다. 또한 이러한 비교 분석은 수질관리를 위한 대책 수립과 효율적인 사후 환경관리를 모색하는데 대단히 중요하다. 이에 반하여 지금까지 금강을 대상으로 한 수환경 및 수서생태계의 조사 연구는 단편적인 분야에 대하여 극히 단기간 수행된 것이 대부분이고 정량분석이 아닌 정성분석에만 치중되었기 때문에 금강의 수질 및 생태게보존을 위한 자료로서의 기능을 발휘하지 못하고 있다. 이러한 관점에서 본 환경문제연구소에서는 장기(향후 5년 이상)적인 계획하에 금강본류를 대상으로 수서생태계의 구조와 기능을 조사ㆍ분석하고, 이를 통하여 금강의 수질 및 생태계보존과 수환경관리를 위한 기초자료를 수립하고자 한다. 이와 같이 본 환경문제연구소가 장기적으로 추진하려 하는 금강 수서생태계학술조사는 금강 수서생태계의 현황은 물론 금강을 대상으로 하는 여러가지 형태의 사업에 의한 변화 양상을 정확하게 파악하고, 이에 대한 적절한 대처 방안을 강구하게 할 수 있는 기초자료를 제공하여 줄 것으로 기대된다. 지금까지 금강에 대한 종합적인 생태계 조사가 이루어지지 않고 있음을 고려할 때, 본 학술조사는 장차 금강 수서생태계에 대한 연구의 지침이 될 것으로 기대되며, 이는 본교가 금강 생태계의 보존과 관리를 통하여 지역사회에 기여하는 중추적 학술기관으로서의 위치를 확고히 마련하는 계기가 될 것이다. 본 보고서는 이러한 연구계획의 1차년도 연구결과로서 수질 및 미생물, 수생식물, 어류, 동식물 플랭크톤, 수서곤충을 중심으로 금강 중ㆍ하류 유역 수서생태계를 구성하고 있는 주요 생물군의 구조와 기능을 정량적으로 조사ㆍ분석하는데 그 목적을 두었다. Heterotrophic microbial populations and physicochemical parameters of water samples collected from six stations on the Keum River have been examined from November 1994 to July 1995. The ranges for BOD and COD were 1.5-11.3㎎/L and 2.1-13.7㎎/L, respectively. At the station closest to the estuary, the values of BOD and COD were higher than at other stations, but those of PO₄-P were lower. Metal ions including Cu, Mn, Pb, Cd, and Hg were not detected at all. The population densities of heterotrophic bacteria amounted to 13.7-22.0% of the total bacteria. The densities of coliforms, Salmonella-Shigella, and yeasts were in the range of 49-538 cfu/ml, 16-271 cfu/100ml, and 30-259 cfu/㎖, respectively.

      • 미세절제술과 비교 유전자 보합법에 의한 각종 종양에서의 유전자 변화에 관한 연구

        For the evaluation of oncogenesis, progression and prognosis of cancer, CGH is an important technique, because this technique is economic due to utilization of only one probe and lack of culture, screening mathod of whole genome and possibility of retrospective and prospective study. By the CGH, genornic variation of 20 breast cancer tissues, 23 stomach cancer tissues and 16 bladder cancer tissues were analyzed. The results were as followes ; 1. breast cancers The CGH results showed gains on chromosomes 8q(40%), lq(30%), 17q(15%), 20q(15%), 18q (15%), 5p(15%), and 13q(15%). The Deletions were on chromosomes 17p(45%) and 22q(20%). High-level amplifications(green/red ratio >1.5) were noted on chromosomes 1p31, iq, 3q25-qter, 5p, 7q31-qter, 8q, 9q22-qter, 10p, l1p, 11q22-qter, 12p, 12q24, 14g21-qter, 15q23-qter, 17q, 18p, 18q12-qter, 20p, and 20q. By comparison with infiltrating ductal carcinoma, the two medullary carcinomas showed high-level amplification on chromosomes iq3l, lq, 8q, 10p, 11p and 12p. 2. stomach cancers 1) Usual amplification sites of genome were lq, 13q, 17q, 20p,q. 2) 17p was the most common deletion site. The other sites of the deletion were lq, 4q. 3) In intestinal type of stomach cancer, genomic variation is more common than diffuse type. 4) In the cases of no evidence of lymph node metastasis, deletion of 17p is absent but amplification of 8q is obvious in the case of lymph node metastasis. 3. bladder cancers Common amplification of copy numbers of DNA sequences by CGH were seen at 1q, 3q, 4q, 5p, 6pq, 7p, 8q, 11q, 12q, 13q, 17q, 18q and 20pq(more than 20% of cases). High level amplification was noted at 1p32, 3p2l, 3q24, 4q26, 8q21-ter, 11q14-22, 12q15-21, 12q21-24, 13q 21-31, 17q22, and 18q22. Deletions were noted at 2q21-qter, 4q13-23, 5q, 8p12-22, 9pq, 11p13-15 (more than 20% of cases).

        In capitalist society, businesses tend to use human beings as a resource and tool for them to achieve the goal of maximizing profits of the businesses. As a result, the phenomenon of human alienation and polarization of wealth has been cited as serious social problems. Today, the conventional wisdom that material affluence and prosperity resulting from economic growth will bring happiness to the human society is shaken by the problem of spiritual(consciousness) poverty in material affluence. To play a role as an economic system that contributes to the happiness and co-prosperity of the human society, capitalism must also evolve in keeping with the changes of the times. In this article, to discuss the alternative of capitalism to fit the new era, we pursue a new realm of possibility through the new scientific discipline of MOSEE; Management Of allSelves Enlightenment and Empowerment that Dr. Jae Yoon Rhee, the authur of this article, has founded in 1990, while at the same time announcing the new 5<SUP>th</SUP> dimensional factors of the spirit resource and asset (SRA) of human beings of the MOSEE. This aims to achieve a 5th-dimensional MOSEE revolution for the creation of the 21st century Universe Human Spirit Science Civilization by analyzing and evaluating the inherent structural constraints of the modern business philosophies and theories, such as CSV (Michael Porter), In other words, it will continue to carry out academic research, education, and human society activities that lead to the creation of the Universe Human Spiritual Science Civilization of 21 Century, beyond the Advanced Scientific Informative Civilization of 20 Century. We, the co author of this article, are trying to conciliate the modern management philosophies and theories of the following prominent scholars such as CSV of Michael Porter and Mark Kramer in the U.S.A. in this article because of the required limitation of page numbers by the Association involved now. Thus other prominent scholars’ theories such as the Strategic Intent (SI) of Dr. Gary Hamel and Prahalad in the U.K., the Complexity Economics (CE) of Dr. Brian Arthur in the U.S.A. and the Vertuous-Based Management (VBM) of Ikujiro Nonaka and Gono Novru in Japan are to be left sometime in the future. (*Note: Spirituality may be called High level of Human Consciousness or High level of Spirit, As Spirit is more common in english, it is called Spirit in this article.)

      • an Efficient Key Escrow System in IP Security

        IPSec is a security protocol suite that provides encryption and authentication services for IP messages at the network layer of the internet. Key recovery has been the subject of a lot of discussion, of much controversy and of extensive research. Key recovery, however, might be needed at a corporate level, as a form of key management. The basic observation of the present paper is that cryptographic solutions that have been proposed so far completely ignore the communication context. We propose example to provide key recovery capability by adding key recovery information to an IP datagram. It is possible to take advantage of the communication environment in order to design key recovery protocols that are better suited and more efficient.

      • TCP Connection Management on Client-side for Improving Web Server Performance

        HTTP/1.1 does not define explicitly connection-closing time but specifies a certain fixed holding-time model. This model may induce wasting server's resource when server maintains connection with the idle-state client that requests no data for a certain time. This paper proposes the mechanism of a heuristic connection management supported by the client-side under persistent HTTP, in addition to fixed holding-time model on server-side. We compare them to the fixed holding-time model used in the current implementation of Apache Web server. An experimental evaluation of connection management policies, conducted using Web server logs, shows that our policy achieves 15-20% reduction on busy Web server in cost with respect to the fixed holding-time policy. As a result, the mechanism allows busy Web servers to use server resource more efficiently without additional server load and give services to clients more fairly.

      • Key Recovery Compatible with IP Security

        IPSec is a security protocol suite that provides encryption and authentication services for IP messages at the network layer of the Internet. Key recovery has been the subject of a lot of discussion, of much controversy and of extensive research. Key recovery, however, might be needed at a corporate level, as a form of key management. The basic observation of the present paper is that cryptographic solutions that have been proposed so far completely ignore the communication context. Static systems are put forward for key recovery at network layer and solutions that require connections with a server are proposed at application layer. We propose example to provide key recovery capability by adding key recovery information to an IP datagram. It is possible to take advantage of the communication environment in order to design key recovery protocols that are better suited and more efficient.
