정신분열병 환자에서 Risperidone에 의한 Cytokine의 변화

기존의 항정신병약뮬인 haloperidol이나 clozapine과는 약물학적 특성이 다른 대표적인 약물인 risperidone을 사용하여 정신분열병 환자가 치료 전과 4주 치료 후에 어떤 면역학적 차이를 보이는지 6가지 cytokine의 혈장 농도를 측정하여 알아보았다. 방법: 1999년 2월부터 1999년 5월까지 카톨릭대학교 대전성모병원 정신과에 입원한 환자 중 DSM-IV 진단 기준에 의해 정신분열병으로 진단된 환자 25명을 대상으로 하였다. 입원 다음날 아침 8시 risperidone 투여전의 기본적인 cytokine농도를 측정하기 위해 혈액을 채취하였으며 제 2병일에 risperidone 2mg, 제 4병일에 4mg, 제 6병일에 6mg으로 증량하는 방식을 항정신병약물 치료를 하였다. 임상평가는 risperidone 투여 전과 투여 4주 후 정신과 전공의가 각각 PANSS 검사를 시행하였다. 혈장에서 IL-1β,IL-2, IL-6, IL-12, INF-□, TNF-α의 정량 측정은 ELISA kit (Endogen Inc., Woburn, MA, USA)을 사용하여 quantitative ELISA법으로 하였다. 결과: risperidone을 4주간 투여 후 환자 혈장 IL-12 농도가 유의하게 증가되었고 나머지 cytokine 농도는 유의한 변화가 없었다. 결론: 저자들은 연구에서 risperidone 치료 후 IL-12가 증가된 것은 이러한 항정신병약물치료가 면역반응 을 활성화시키는 역할을 한 것으로 시사된다. Objectives: This study was carried out to evaluate immunologic difference between baseline and after 4 weeks drug treatment with atypical antipsychotics(rispreidone) by measurement of serum concentration of 6 cytokines. Methods: The subjets were composed of 25 patients who are admitted at Dajeon St's Marys hospital of psychiatry department and diagnosed as schizophrenia by DSM-IV diagnositc criteria. We measured serum IL-1β, IL-2, IL-6, IL-12, INF-□, TNF-α concenatrations by quantitative ELISA method using ELISA kit(Endogen In., Woburn, MA, USA). The two psyciatrists per-formed PANSS examination between baseline and after 4 weeks risperidone treatments. Results: The serum level of IL-12 was increased significantly after medication of 4 weeks and the serum concentration of IFN-□ showed the tendency of decreasement but not significant. The serum level of the other cytokines showed no significant change. Conclusion: We spectulate that the increasement of IL-12 may contribute to role of activa-tion of immune response by treatment of antipsychotic medication(risperidone). This study is first trial of IL-12 study in neuropsychiatric field and IL-12 which play impor-tant role of immune response becomes interesting subjects in immune research.

<B>Abstract</B><P>To describe a new device for the transcervical collection of amniotic fluid (AF) in patients with ruptured membranes, and to compare the concentration of proteins in fluid retrieved by transabdominal amniocentesis and the transcervical AF collector.</P><P>Paired AF samples were collected in patients with preterm prelabor rupture of membranes (PROM) (n=11) by transabdominal amniocentesis and with the transcervical AF collector (Yoon’s AF Collector™). Three proteins known to have high concentrations in AF [α-fetoprotein (AFP), β-human chorionic gonadotrophin (β-hCG), and prolactin] were measured.</P><P>(1) There was a significant correlation between the concentrations of analytes in AF obtained by transabdominal amniocentesis and by the transcervical AF collector (r=0.94, P<0.001 for AFP; r=0.96, P<0.001 for β-hCG; r=0.72, P<0.05 for prolactin); (2) Bland-Altman plots showed no evidence of heteroscedasticity between transabdominal or transcervical AF concentrations of these markers.</P><P>There was a strong correlation between the concentrations of proteins in AF collected by amniocentesis or with the transcervical device.</P>

The insecticidal activity of 120 plant essential oils and control efficacy of six experimental spray formulations (SF) containing the oils (SF-0.25, 0.5, 1, 2.5, 5 and 10% sprays) against both nymph and adult of citrus flatid planthopper, Metcalfa pruinosa, was examined using a direct contact application. Reponses varied according to dose (1000 mg litre-1 and 500 mg litre-1). Based on 24 h exposure in leaf dipping assay at 1000 mg litre-1 strong mortality was observed in more than 19 essential oils among 130 was screened. At 500 mg litre-1 very strong mortality (100%) was observed in cinnamon technical, cinnamon green leaf, cinnamon #500, cassia tree, citronella java and penny royal followed by origanum, thyme white, grape fruit, savory, fennel sweet, aniseed and cinnamon bark (93.3- 80%) showed considerable moratality against nymphs of M. pruinosa. The moderate mortality was found in thyme red, tagette, calamus, lemoneucalptus and geranium (73.3-60%). The other oil has low or very low mortality against M. pruinosa. The oil applied as SF-10% sprays provided 100% mortality against adult M. pruinosa. In particular cinnamon technical showed very strong (>SF-0.5= 100%) effect followed by cinnamon #500 (>SF-2.5=100%), cinnamon green leaf (>SF-2.5=100%) and penny royal (>SF-2.5=100%) respectively. Global efforts to reduce the level of highly toxic synthetic insecticides in the agricultural environment justify further studies on the active essential oils as potential larvicides for the control of M. pruinosa populations as direct spray with contact action.

Backgrounds/Aims: Backgrounds/Aims: In Korea, hepatocellular carcinoma (HCC) is the sixth most common cancer and results in the second-highest cancer death rate among all cancers. We aimed to describe the characteristics of patients who were newly diagnosed with HCC in Korea between 2008 and 2011. Methods: The Korean Primary Liver Cancer Registry (KPLCR) is a random sample consisting of approximately 15% of patients with newly diagnosed primary liver cancer registered in the Korean Central Cancer Registry. We investigated the baseline characteristics, treatment modalities, and overall survival (OS) of patients with HCC registered in the KPLCR between 2008 and 2011. Results: A total of 6,083 patients were histologically or radiologically diagnosed with HCC. The hepatitis B virus was the predominant HCC etiology (72.0%). According to the Barcelona Clinic Liver Cancer (BCLC) staging system, stages 0, A, B, C, and D accounted for 8.6%, 39.7%, 11.5%, 33.8%, and 6.9%, respectively. Transarterial therapy (41.7%) was the most commonly performed initial treatment, followed by best supportive care (21.7%), surgical resection (16.7%), and local ablation therapies (10.6%). The overall rate of adherence to the BCLC treatment guideline was only 37.7%. The 1-, 3-, and 5-year OS rates were 65.6%, 46.2%, and 36.8%, respectively. Conclusions: Between 2008 and 2011, approximately half of patients with HCC (48.3%) were candidates for curative treatment (BCLC stage 0 or A), but one-third of patients (33.8%) had advanced HCC (BCLC stage C). Transarterial therapy was the most commonly conducted initial treatment and the 5-year OS rate was 36.8% in this period. (J Liver Cancer 2020;20:41-52)

The insecticidal activity of cinnamon essential oils, cinnamon bark, cinnamon technical, cinnamon green leaf oils and their constituents and structurally related compounds against citrus flatid planthopper, Metcalfa pruinosa, was examined using a direct contact application. At 500 mg litre-1 very strong mortality (100%) was observed in cinnamon technical, cinnamon bark, cinnamon green leaf, mortality against flatid planthopper M. pruinosa. The cinnamon oils constituents were identified by GC-MS. The active principles were determined to be hydrocinnamic acid (24 h LC50, 30.66 mg/L), geranic acid (24 h LC50, 31.23 mg/L), cinnamaldehyde (24 h LC50, 32.65 mg/L), hydrocinnamaldehyde (24 h LC50, 39.11 mg/L) and trans-cinnamaldehyede (24 h LC50, 39.54 mg/L) were the most toxic against both nymph and adult of citrus flatid planthopper, M. pruinosa. The moderate activity was observed with cinnamyl acetate, dibutyl pthalate, anethole, α -cyano cinnamic acid, cinnamyl alcohol, methyl cinnamaldehyde, eugenol, 2.4-dihydrocyl cinnamic acid, bornyl acetate (24 h LC50, 53.35- 97.17 mg/L) respectively. The other constituents were showed less or no activity against adult of M. pruinosa. Global efforts to reduce the level of highly toxic synthetic insecticides in the agricultural environment justify further studies on the active cinnamon oils active principles act as potential insecticides for the control of M. pruinosa populations as direct spray with contact action.
