조선업 도장작업자의 유사노출군과 노출변이; 크실렌 노출을 중심으로

목적 : 조선업 도장 작업자의 크실렌 노출을 평가하기 위하여, 수행직무 및 밀폐특성에 따라 유사노출군으로 구분하여 노출 변이 및 동질성을 파악하고자 하였다. 방법 : 3회 이상 반복측정이 이루어진 근로자 중동일공정에서 직무상 변동이 없는 근로자 총 28명을 대상으로 하였다. 노출농도 값을 대수전환(lOgtransformation)하여 근로자간, 근로자내 노출 분산요소을 추정하기 위하여, nested randomeffects ANOVA model을 활용하였다. 선정된 유사노출군 내 구성원의 측정 평균이 동일하다고 할수 있는지를 검정하기 위하여 분산요인(9f)을 산출하였고, 쿨즈0.031일 경우 동일하다고 생각하였다.또한 노출 값을 대수전환하여 95 %의 근로자(b-R9s)가 2배수 범위 내에 측정값을 가질 경우 동일하다고 생각하였다. bwRSS의 값을 유사노출군내의 근로자들간(between worker)의 기하표준편차(bwsy)를 구한 후 exp(3.92 bwsy)로 계산하여 산출하였고, 그 값이 2이하이면 동일하다고 생각하였다. 결과 : 일반적으로 유사노출군은 노출농도가 비슷한 것이라는 생각과는 달리, 직무에 근거한 유사노출군은 동질성 검정상 이질적이었다. 분무도장과 붓도장 모두 bwRss은 2를 초과하였고, 룰 또한 0.031를 초과하여 변이 폭이 컸다. 그러나 밀폐특성에 따른 노출 분포의 변이성은 직무만으로 구분했을 때의 변이지표 보다 그 수치가 낮아 밀폐특성으로 직무의 변이성을 설명 할 수 있었다. 결론 : 직무에 근거한 유사노출군이 동질적이지알다는 결과는 유사노출군으로 구분된 근로자의 업무특성이 하나의 유사노출군으로 설정할 수 없다는 점을 시사한다. 따라서 향후 작업환경 측정 시 직무뿐만 아니라 특정 업무(specific task) 또는 세부작업현황까지 파악할 필요성을 제시하고 있다. Objectives : This study was conducted to assess the concentration of xylene exposure and exposure variability of a similar exposure group (SEG) in accordance with job title and confined space. Method : The study subjects included 28 workers working in the painting process in the ship-building industry. Measurement of subjects were repeated three times. Within and between-worker exposure valiance components were estimated from log-transformed exposure concentrations employing a one-way nested random effects analysis of variance model. A uniformly exposed group was defined as one where the between-worker variance(σB2) was less than or equal to 0.031 or bwR95 ≤2, where bwR95 =exp(3.92 bwSy) , representing the radio of the mean exposures of the 97.5th and 2.5th percentile groups of the workers sampled. Result : The results indicated that, contrary to popular belief, similar exposed groups based on job title were heterogeneous. The between-worker variance(σB2) is large(≥0.031) and the ratios of the 97.5th and 2.5th percentile workers of the log normally distributed exposures of each group of worriers was more than 2. However dividing job title into confined space (in-block, out-block, in/out-block) decreased the between-worker exposure variability. Conclusion : This study focused on the quantitative aspects of exposure in painters in the ship-building industry. We cannot accept the assumption that SEG are uniformly exposed, as a major component of variation in exposure relates to differences in tacks or other factors. Therefore, the observational schemes can be improved by investigating Job-specific task and practice.
Nucleotome을 이용한 요추간판탈출증의 경피적 수핵절제술

The authors performed 19 cases of percutaneous automated discectomy using Nucleotome between Feb, 1988 and Jan, 1993. The mean age was 33 years ranging from 20 to 56 years. Eleven of the patients were men .and eight were women. One level nucleotomy was done in 14 cases, two level in 4 cases and three level in 1 case. Overall success rate was 63%. No intraoperative and postoperative complications occurred except transient back pain in 3 cases.
요추 Y-각 협착증 : 요추간 협착증의 새로운 개념 New Concept of Lumbar Stenosis

Authors measured the Y-shaped angle made by both yellow ligaments and both laminae named Y-angle at the spinal CT scan in normal and lumbar stenosis group. The normal range of Y-angle is between 60˚and 95˚. Lumbar stenosis is classified into narrow Y-angle stenosis(<60°) and wide Y-angle stenosis(>95°) by Y-angle. Narrow Y-angle stenosis is most common in degenerative spondylotic stenosis. Wide Y-angle is most common in congenital stenosis. The Y-angle is a simple and useful diagnostic indicator in the diagnosis of lumbar spinal stenosis on CT scan and MEU scan.

Early repolarization is a common electrocardiographic (ECG) feature found in young adults, men and athletes, and has been considered to be a benign feature for the last several decades. But recent studies suggest that early repolarization may be related to idiopathic ventricular fibrillation and sudden cardiac death. We report a young man, 35 years old, who had life threatening ventricular fibrillation and sudden cardiac arrest. He was evaluated for cardiac causes of ventricular fibrillation. There was no explanation other than that his ECG showed an early repolarization pattern so we treated him with implantable cardioverter defibrillator. Thus, we suggest that early repolarization may be related with life threatening ventricular arrhythmia.

The effects of IH901, a ginseng intestinal metabolite, on the antitumor activity and cardiotoxic adverse effect of doxorubicin were investigated in sarcoma-180 ascitic tumor-bearing mice. To evaluate the enhancing effect on the antitumor activity of doxorubicin, IH901 (50 ㎎/㎏) was orally administered for 28 days, in combination with intraperitoneal injection of doxorubicin (3 ㎎/㎏) on days 5, 12, 19 and 24, to mice intraperitoneally inoculated with 1 × 10⁵ sarcoma-180 cells. The body weights of tumor-bearing mice dramatically increased from 10 days following tumor inoculation, leading to a mean survival time of 17.3 days. In contrast, such an increase in body weights induced by the ascitic tumor growth was markedly attenuated by doxorubicin (3 ㎎/㎏) administration, resulting in a long lifespan of 34.9 days. Interestingly, the body weight gain was further suppressed by the combination of IH901 (50 ㎎/㎏), leading to a normal feature. In addition, the mean survival time was extended by 129.3%, reaching 39.7 days, although IH901 was inactive alone. Separately, for the evaluation of protective effect on the cardiotoxicity of doxorubicin, IH901 (50 ㎎/㎏) was orally administered for 14 days, in combination with intraperitoneal injection of doxorubicin (5 ㎎/㎏) on days 5 and 9 to tumor-bearing mice. Although the heart weight of tumor-inoculated mice decreased to about 75% of normal, such an atrophy was remarkably recovered by coadministration of IH901. Furthermore, IH901 substantially reversed the dramatic decrease in mRNA of myocytic phospholipid hydroperoxide glutathione peroxidase, an antioxidant enzyme, as well as histopathological changes such as cytoplasmic vacuolization, mitochondrial swelling, and loss of myofibrils and intercalated disc induced by doxorubicin. Taken together, it is suggested that IH901 could be a potential adjunct for the potentiation of antitumor activity and reduction of cardiotoxicity of chemotherapeutic agents including doxorubicin.
Sequence and Gene Organization of Spodoptera litura Granulovirus

The genome of a granulovirus isolated from the tobacco cutworm, Spodoptera litura, was completely sequenced. The nucleotide sequence of the Spodoptera litura granulovirus (SlGV) genome was 124,121 bp long, with a 61.2% A+T content and contained 133 putative open reading frames (ORFs) of 150 nucleotides or larger. The 133 putative ORFs covered 86.3% of the genome. Among these, 29 ORFs were conserved in most completely sequenced baculovirus genomes, 35 were granuloviruses (GVs)-specific, and 60 were present in some NPVs and/or GVs. Especially, we proved that there were 9 SlGV-specific ORFs by RT-PCR. When the phylogenic relationship was analyzed using the nucleotide sequence of granulin gene, SlGV was most closely related to Trichoplusia ni granulovirus (TnGV) and Xestiac-nigrum granulovirus (XcGV) which were belonged to Type-I granulovirus. Comparative analysis of gene organization of the SlGV genome with those of other baculoviruses were carried out using blast matrix and gene order diagram.
