        2022 Taiwan clinical multicenter expert consensus and recommendations for thyroid radiofrequency ablation

        Radiofrequency ablation (RFA) is a minimally invasive management strategy that has been widely applied for benign and recurrent malignant thyroid lesions as an alternative to surgery in Taiwan. Members of academic societies for specialists in interventional radiology, endocrinology, and endocrine surgery collaborated to develop the first consensus regarding thyroid RFA in Taiwan. The modified Delphi method was used to reach a consensus. Based on a comprehensive review of recent and valuable literature and expert opinions, the recommendations included indications, pre-procedural evaluations, procedural techniques, post-procedural monitoring, efficacy, and safety, providing a comprehensive review of the application of RFA. The consensus effectively consolidates advice regarding thyroid RFA in clinical practice for local experts.

      • Development of Novel Pineapple Peel Juice Through Lactic Acid Fermentation for Enhancing Antioxidant Capacity and Stability

        Pineapples are tropical fruit that are rich in vitamins, enzymes and fibers with many health benefits not only in pulps but also peels. However, the peels of pineapples are normally removed and dropped. The pulps and peels of four pineapple cultivars, Milk, Honey Fragrance, Smooth Cayenne and Golden Diamond pineapples, were juiced, filtered and analyzed, and the Golden Diamond pineapple was found to have highest antioxidant capacity. The pulp and peel juices of Golden Diamond pineapple were inoculated with Enterococcus faecium LYC91 and various concentration calcium carbonate to ferment at 37°C for 24 hours follow by bacterial and antioxidant capacity analysis. The results indicated that the fermented pineapple peel juice with calcium carbonate had highest bacterial number, total polyphenol content, DPPH radical scavenging ability, Fe2 + chelating ability and reducing power activity than pulp juice and unfermented pineapple juice. The viable bacterial number of fermented pineapple pulp and peel juices with calcium carbonate remain 8.0 log CFU/mL after stored at 4°C for 1 month. The freeze-dried powder of the fermented pineapple pulp and peel juices with calcium carbonate retained the viable cell number of 9.0 log CFU/g after stored at 4°C for three months. These results showed that the fermented pineapple pulp and peel juices with calcium carbonate were novel pineapple lactic acid fermented drinks with growth potential.

      • Experimental Study on Inhibition Effects of the XAF1 Gene against Lung Cancer Cell Proliferation

        Objective: To investigate the effect of high expression of XAF1 in vivo or in vitro on lung cancer cell growth and apoptosis. Methods: 1. The A549 human lung cancer cell line was transfected with Ad5/F35 - XAF1, or Ad5/F35 - Null at the same multiplicity of infection (MOI); (hereinafter referred to as transient transfected cell strain); XAF1 gene mRNA and protein expression was detected by reverse transcription polymerase chain reaction (RT-PCR) and Western blotting respectively. 2. Methyl thiazolyl tetrazolium (MTT) and annexin V-FITC/PI double staining were used to detect cell proliferation and apoptosis before and after infection of Ad5/F35 - XAF1 with Western blotting for apoptosis related proteins, caspase 3, caspase - 8 and PARP. 3. After the XAF1 gene was transfected into lung cancer A549 cells by lentiviral vectors, and selected by screening with Blasticidin, reverse transcription polymerase chain reaction (RT-PCR) and Western blotting were applied to detect mRNA and protein expression, to establish a line with a stable high expression of XAF1 (hereinafter referred to as stable expression cell strain). Twenty nude mice were randomly divided into groups A and B, 10 in each group: A549/XAF1 stable expression cell strain was subcutaneously injected in group A, and A549/Ctrl stable cell line stable expression cell strain in group B (control group), to observe transplanted tumor growth in nude mice. Results: The mRNA and protein expression of XAF1 in A549 cells transfected by Ad5/F35 - XAF1 was significantly higher than in the control group. XAF1 mediated by adenovirus vector demonstrated a dose dependent inhibition of lung cancer cell proliferation and induction of apoptosis. This was accompanied by cleavage of caspase -3, -8, -9 and PARP, suggesting activation of intrinsic or extrinsic apoptotic pathways. A cell strain of lung cancer highly expressing XAF1 was established, and this demonstrated delayed tumor growth after transplantation in vivo. Conclusion: Adenovirus mediated XAF1 gene expression could inhibit proliferation and induce apoptosis in lung cancer cells in vitro; highly stable expression of XAF1 could also significantly inhibit the growth of transplanted tumors in nude mouse, with no obvious adverse reactions observed. Therefore, the XAF1 gene could become a new target for lung cancer treatment.

        Treatment outcomes of patients with stage II pure endometrioid-type endometrial cancer: a Taiwanese Gynecologic Oncology Group (TGOG-2006) retrospective cohort study

        Objective: Choice of hysterectomy and adjuvant treatment for International Federation of Gynecology and Obstetrics (FIGO) 2009 stage II endometrioid endometrial cancer (EEC) is still controversial. Aims of this study were to evaluate survival benefits and adverse effects of different hysterectomies with or without adjuvant radiotherapy (RT), and to identify prognostic factors. Methods: The patients at 14 member hospitals of the Taiwanese Gynecologic Oncology Group from 1992 to 2013 were retrospectively investigated. Patients were divided into simple hysterectomy (SH) alone, SH with RT, radical hysterectomy (RH) alone, and RH with RT groups. Endpoints were recurrence-free survival (RFS), overall survival (OS), disease-specific survival (DSS), adverse effects and prognostic factors for survival. Results: Total of 246 patients were enrolled. The 5-year RFS, OS, DSS and recurrence rates for the entire cohort were 89.5%, 94.3%, 96.2% and 10.2%, respectively. Patients receiving RH had more adverse effects including blood loss (p<0.001), recurrent urinary tract infections (p=0.013), and leg lymphedema (p=0.038). Age over 50-year (HR=9.2; 95% confidence interval [CI], 1.2–70.9) and grade 3 histology (HR=7.28; 95% CI, 1.45–36.6) were independent predictors of OS. Grade 3 histology was an independent predictor of RFS (HR=5.13; 95% CI, 1.38–19.1) and DSS (HR=5.97; 95% CI, 1.06–58.7). Patients receiving adjuvant RT had lower locoregional recurrence (p=0.046), but no impact on survival. Conclusion: Different treatment modalities yield similar survival outcomes. Patients receiving SH with RT had lower locoregional recurrent with acceptable morbidity. Age and tumor grading remained significant predictors for survival among patients with FIGO 2009 stage II EEC.

        Exercise dosage is proven to be an important factor in the physical treatment. Robot assistive approach can improve the rehabilitation quality and evaluate patient’s recovery quantitatively. This paper presents kinematic structure, assistive control system, and integrated F/T sensor for an upper limb rehabilitation robot. By using the human arm dynamic, there are three rehabilitation modes presented in this paper: active mode, assistive mode, and passive mode. In assistive mode, we have two strategies to implement it. One is to amplify interactive torque, and the other is to apply assist-as-needed concept. The goal of this mode is to assist patients to finish motion tasks. The rehabilitation robot under investigation has 7 degree of freedom (DOF) actuated by DC motors, which are programmed to drive the robot arm in the 3D space. To validate our control design, some realistic experiments are conducted and its satisfactory performance is demonstrated. This work is approved clinical testing by the Department of Health, Executive Yuan, R.O.C. So far, we are demonstrating the effect of our controller.

        A FACILE ONE-STEP HYDROTHERMAL METHOD TO PRODUCE GRAPHENE OXIDE/Fe2O3-NANOTUBES COMPOSITES AND ITS ELECTROCHEMICAL PROPERTIES

        The composites of graphene oxide/Fe2O3-nanotubes (GO/Fe2O3-NTs) have been fabricated through a facile one-step hydrothermal method. Fe2O3-NTs crystallines were homogeneously distributed on GO, which have been con¯rmed by transmission electron microscopy analysis (TEM). The structure, composition and electrochemical properties of GO/Fe2O3-NTs composites were investigated by means of selective-area electron di®raction (SAED), Fourier transform infrared spectroscopy (FT-IR), X-ray di®raction (XRD), Raman spectra, cyclic voltammogram (CV) curves and galvanostatic charge?discharge curves. GO/Fe2O3-NTs composites exhibit a high speci¯c capacitance of 133.2 Fg?1 in 1M Li2SO4 electrolyte. In addition, the GO/Fe2O3-NTs composites electrode shows good long-term cycle stability (only 9% decrease of the speci¯c capacitance is observed after 1000 charge?discharge cycles).

      • Knowledge, Perceptions and Acceptability of HPV Vaccination among Medical Students in Chongqing, China

        Objectives: To evaluate medical students' knowledge of HPV and HPV related diseases and assess their attitudes towards HPV vaccination. Methods: A total of 605 medical undergraduates from Chongqing Medical University in China were surveyed using a structured and pretested questionnaire on HPV related knowledge. Results: Some 68.9% of the medical students were females, and mean age was 21.6 (${\pm}1.00$) years. Only 10.6% correctly answered more than 11 out of 14 questions on HPV related knowledge, 71.8% being willing to receive/advise on HPV vaccination. Female students (OR: 2.69; 95% CI: 1.53-4.72) and students desiring more HPV education (OR: 4.24; 95% CI: 1.67-10.8) were more willing to accept HPV vaccination. HPV vaccination acceptability was observed to show a positive association with HPV related knowledge. Conclusions: Our survey found low levels of HPV related knowledge and HPV vaccination acceptability among participating medical students. HPV education should be systematically incorporated into medical education to increase awareness of HPV vaccination.

      • Knowledge of Human Papillomavirus (HPV) Infection, Cervical Cancer, and HPV Vaccine and its Correlates among Medical Students in Southwest China: a Multi-center Cross-sectional Survey

        Background: Since cervical cancer can be prevented and controlled through human papillomavirus (HPV) vaccination, it is important to train health care providers and provide them with appropriate knowledge. This study aimed to understand the level of HPV related knowledge among medical students and correlates in Southwest China in order to address any potential gap in their knowledge base. Methods: We conducted a cross-sectional survey among medical students in six selected universities across Southwest China based on a pretested questionnaire regarding HPV infection, cervical cancer, and HPV vaccines. Results: We successfully surveyed 1, 878 medical students, of whom 32.1% were males and 67.9% were females. Their mean age was 20.8 (standard deviation: 1.3) years. 91.8% of them were ethnic Han Chinese, and 43.8% were students in clinical medicine. While 76.5% had heard of HPV only 48.8% knew that the prevention of cervical cancer was possible through HPV vaccination. Only 42.9% of the male and 49.2% of the female students correctly answered over 10 out of 22 questions on HPV related knowledge. Female students appeared to know more about HPV and HPV vaccination (OR: 1.39; 95% CI: 1.11-1.75). In addition, the student knowledge improved with the grade (p<0.001). University courses were the most selected source of knowledge about HPV vaccination (66.4%). 83.6% of males and 91.4% of females were willing to have more HPV related education by experts (p<0.001). Only 10.1% of the students acknowledged that people had asked for their advice regarding HPV vaccination. Conclusions: Our survey indicates that medical students from Southwest China have poor knowledge of HPV and HPV vaccination, but are willing to receive more relevant information. Targeted education should be incorporated into school courses to inform these future health care providers and ensure success of programs for cervical cancer control and prevention.

        On the extended period of a frequency domain method to analyze transient responses

        Transient response analysis can be conducted either in the time domain, or via the frequency domain. Sometimes a frequency domain method (FDM) has advantages over a time domain method. A practical issue in the FDM is to find out an appropriate extended period, which may be affected by several factors, such as the excitation duration, the system damping, the artificial damping, the period of interest, etc. In this report, the extended period of the FDM based on the Duhamel's integral is investigated. This Duhamel's integral based FDM does not involve the unit impulse response function (UIRF) beyond the period of interest. Due to this fact, the ever-lasting UIRF can be simply set as zero beyond the period of interest to shorten the extended period. As a result, the preferred extended period is the summation of the period of interest and the excitation duration. This conclusion is validated by numerical examples. If the extended period is too short, then the front portion of the period of interest is more prone to errors than the rear portion, but the free vibration segment is free of the wraparound error.
