Purpose: Senior nursing and medical interns’ lack of familiarity and confidence with respect to practicing universal precaution for theprevention of occupational needlestick or sharp injuries may harm themselves. Trainees’ self-reported needlestick or sharp injury ratewas known to be especially high during the first 2 months of internship in Taiwan. This prospective cohort study aimed to assess the effect of newly developed virtual reality (VR) game, which uses Gagne’s learning model to improve universal precaution for needlestickor sharp injury prevention and decrease the rates of needle stick or sharp injuries in new-coming medical and nursing interns in Taiwan. Methods: From 2017 to 2019, the VR system was developed and applied in training of 59 new-coming nursing and 50 medical interns. Occupational needlestick or sharp injury prevention was sought to be achieved through a game of right and wrong choices for safe orunsafe universal precaution behaviors. Results: In comparison with medical interns, a higher proportion of nursing interns had past experiences of deep occupational needlestick or sharp injury. Before VR training, the familiarity and confidence for needlestick or sharp injury prevention were higher amongnursing interns than medical interns. Trainees with past experiences of deep needlestick or sharp injury exhibited better performanceon the accuracy rate and time needed to complete 20 decisions than those without past experiences in VR practice. All trainees showedan improved performance after VR training. A high proportion of trainees reported that the VR-based training significantly decreasedtheir anxiety about needlestick or sharp injury prevention. Conclusion: This self-developed VR game system using Gagne’s flow improved universal precaution for needlestick or sharp injuryprevention and reduced the needlestick or sharp injury rates in the first 2 months of nursing and medical internship.
Glutathione S-Transferase Expression in Upper Urinary Tract Urothelial Carcinomas: a Taiwan Study
Chen, Szu-Han,Wu, Wen-Jeng,Tu, Hung-Pin,Li, Wei-Ming,Huang, Chun-Nung,Li, Ching-Chia,Lin, Hui-Hui,Ke, Hung-Lung Asian Pacific Journal of Cancer Prevention 2013 Asian Pacific journal of cancer prevention Vol.14 No.11
Objectives: Glutathione S-transferase (GST) isoenzymes play important roles in resistance to cell apoptosis and carcinogenesis. We aimed to establish the relationship between GST expression and the prognosis of upper urinary tract urothelial carcinoma (UTT-UC) in Taiwan. Methods: This study retrospectively reviewed 46 patients with pathologically confirmed UUT-UC at Kaohsiung Medical University Hospital. In each patient, expression of GSTT1 and GSTP1 was compared between urothelial carcinoma and normal urothelial cells by Western blotting. Results: GSTP1 expression in the UUT-UC cells was significantly higher than that in normal urothelial cells (1.6 fold, p<0.001). Expression of GSTT1 was significantly associated with the invasiveness of the carcinoma (p=0.006). Conclusions: In UUT-UC, GSTP1 might be a potential tumor marker, whereas high GSTT1 expression could be used as an indicator of cancer progression. This study is the first to demonstrate potential applications of different GST isoenzymes for biomolecular analysis of UUT-UCs in Taiwan.
New Single-Phase Power Converter Topology for Frequency Changing of AC Voltage
Jou, Hurng-Liahng,Wu, Jinn-Chang,Wu, Kuen-Der,Huang, Ting-Feng,Wei, Szu-Hsiang The Korean Institute of Power Electronics 2018 JOURNAL OF POWER ELECTRONICS Vol.18 No.3
This paper proposes a new single-phase power converter topology for changing the frequency of AC voltage. The proposed single-phase frequency converter (SFC) includes a T-type multi-level power converter (TMPC), a frequency decoupling transformer (FDT) and a digital signal processor (DSP). The TMPC can convert a 60 Hz AC voltage to a DC voltage and then convert the DC voltage to a 50 Hz AC voltage. Therefore, the output currents of the two T-type power switch arms have 50 Hz and 60 Hz components. The FDT is used to decouple the 50 Hz and 60 Hz components. The salient feature of the proposed SFC is that only one power electronic converter stage is used since the functions of the AC-DC and DC-AC power conversions are integrated into the TMPC. Therefore, the proposed SFC can simplify both the power circuit and the control circuit. In order to verify the functions of the proposed SFC, a hardware prototype is established. Experimental results verify that the performance of the proposed SFC is as expected.

Bipolar disorder (BD) is a major psychiatric disorder that is easily misdiagnosed. Patient adherence to a treatment regimen is of utmost importance for successful outcomes in BD. Several trials of antipsychotics suggested that depot antipsychotics, including long-acting first- and second-generation agents, are effective in preventing non-adherence, partial adherence, and in reducing relapse in BD. Various long-acting injectable (LAI) antipsychotics are available, including fluphenazine decanoate, haloperidol decanoate, olanzapine pamoate, risperidone microspheres, paliperidone palmitate, and aripiprazole monohydrate. Due to the increasing number of BD patients receiving LAI antipsychotics, treatment guidelines have been developed. However, the clinical applicability of LAI antipsychotics remains a global cause for concern, particularly in Asian countries. Expert physicians from Taiwan participated in a consensus meeting, which was held to review key areas based on both current literature and clinical practice. The purpose of this meeting was to generate a practical and implementable set of recommendations for LAI antipsychotic use to treat BD; target patient groups, dosage, administration, and adverse effects were considered. Experts recommended using LAI antipsychotics in patients with schizophrenia, rapid cycling BD, BD I, and bipolar-type schizoaffective disorder. LAI antipsychotic use was recommended in BD patients with the following characteristics: multiple episodes and low adherence; seldom yet serious episodes; low adherence potential per a physician’s clinical judgment; preference for injectable agents over oral agents; and multiple oral agent users still experiencing residual symptoms.
