      • A case of oral fibroma

        Oral fibroma is a tumor-like mass of fibrous scar tissue caused by chronic trauma, typically a bite injury. It is also known as traumatic fibroma or irritation fibroma. The patient notices a painless (unless recently bitten) nodule that is located at sites easily bitten, such as the lower labial mucosa, lateral tongue, and buccal mucosa. It will be suspected clinically when it presents with the usual history and examination findings. Other differential diagnosis such as lipoma, oral papilloma, granular cell tumor, salivary gland tumor, mucocele, etc. should have to be considered. Histology shows typical dense fibrous tissue and overlying epithelium may be ulcerated, thinned or thickened. Surgical excision is only option of treatment. A 34-year-old man presented with dome shaped pinkish firm nodule on lower lip. The lesion was a 0.7x0.5 cm sized and its onset was 1 year ago. Punch biopsy specimen showed acanthosis, rete ridge elongation and proliferation of collagen fiber and fibroblasts in the dermis. We diagnosed it as oral fibroma and consulted to department of otorhinolaryngology for surgical excision.

      • Fucoxanthin inhibits the inflammatory response by suppressing the activation of NF-κB and MAPKs in lipopolysaccharide-induced RAW 264.7 macrophages

        <P><B>Abstract</B></P><P>It has been previously determined that pro-inflammatory mediators including nitric oxide (NO), prostaglandin E<SUB>2</SUB> (PGE<SUB>2</SUB>), tumor necrosis factor (TNF)-α, and interleukin (IL)-1β and IL-6 contribute to the courses of a variety of inflammatory diseases. In this study, we evaluated the anti-inflammatory effects of fucoxanthin (FX), a natural biologically active substance isolated from <I>Ishige okamurae</I>, by determining its inhibitory effects on pro-inflammatory mediators in lipopolysaccharide (LPS)-stimulated murine macrophage RAW 264.7 cells. FX induced dose-dependent reductions in the levels of inducible nitric oxide synthase (iNOS) and cyclooxygenase-2 (COX-2) proteins and concomitant reductions in the production of NO and PGE<SUB>2</SUB>. Additionally, FX was shown to suppress the production of inflammatory cytokines including IL-1β, TNF-α, and IL-6. Furthermore, FX inhibited the cytoplasmic degradation of inhibitors of B (IκB)-α and the nuclear translocation of p50 and p65 proteins, resulting in lower levels of nuclear factor (NF)-κB transactivation. Additionally, FX was shown to induce a dose-dependent inhibition of the phosphorylation of mitogen-activated protein kinases (MAPKs; JNK, ERK and p38). Collectively, the results of this study demonstrate that FX reduces the levels of pro-inflammatory mediators including NO, PGE<SUB>2</SUB>, IL-1β, TNF-α, and IL-6 via the inhibition of NF-κB activation and the suppression of MAPK phosphorylation in RAW 264.7 cells. These findings reveal, in part, the molecular basis underlying the anti-inflammatory properties of FX.</P>

      • The effect of a bioactive tissue‐engineered sling in a rat of stress incontinence model

        <P><B>Abstract</B></P><P>In this study, we attempt to examine the feasibility of the bioactive tissue‐engineered sling by using muscle precursor cells (MPCs)‐seeded Poly(ε‐caprolactone) (PCL) nanofiber sheet in a rat model of stress urinary incontinence (SUI). <I>In vitro</I>, MPCs were cultured on a PCL nanofiber sheet for one week, where the MPCs‐seeded PCL nanofiber sheet showed constant twitching contraction by electrical field stimulation in an organ bath. <I>In vivo</I>, MPCs‐seeded PCL nanofiber sheet was placed under the female rat's urethra after pudendal nerve denervation (animal model of SUI). The leak point pressure (LPP) was evaluated with the vertical tilt table after the operation for four weeks. The resulting LPP of MPCs‐seeded PCL nanofiber sheet group was observed to be significantly higher than the denervation‐only group's. Furthermore, PKH‐26‐labeled MPCs were observed under the urethral sphincter by immunohistochemistry. These results indicated that, the MPCs‐seeded PCL nanofiber sheet have not only provided support for the deficient sphincter, but also actively improved the sphincter's function overall.         Wafer Burn-in Method of SRAM for Multi Chip Package

        This paper presents the improved burn-in method for the reliability of SRAM in Multi Chip Package (MCP). Semiconductor reliability is commonly improved through the burn-in process. Reliability problem is more significant in MCP that includes over two chips in a package, because the failure of one chip (SRAM) has a large influence on the yield and quality of the other chips - Flash Memory, DRAM, etc. Therefore, the quality of SRAM must be guaranteed. To improve the quality of SRAM, we applied the improved wafer level burn-in process using multi cells selection method in addition to the previously used methods. That method is effective in detecting special failure. Finally, with the composition of some kind of methods, we could achieve the high quality of SRAM in Multi Chip Package.

        Preclinical development of a humanized neutralizing antibody targeting HGF

        <P>Hepatocyte growth factor (HGF) and its receptor, cMET, play critical roles in cell proliferation, angiogenesis and invasion in a wide variety of cancers. We therefore examined the anti-tumor activity of the humanized monoclonal anti-HGF antibody, YYB-101, in nude mice bearing human glioblastoma xenografts as a single agent or in combination with temozolomide. HGF neutralization, The extracellular signal-related kinases 1 and 2 (ERK1/2) phosphorylation, and HGF-induced scattering were assessed in HGF-expressing cell lines treated with YYB-101. To support clinical development, we also evaluated the preclinical pharmacokinetics and toxicokinetics in cynomolgus monkeys, and human and cynomolgus monkey tissue was stained with YYB-101 to test tissue cross-reactivity. We found that YYB-101 inhibited cMET activation <I>in vitro</I> and suppressed tumor growth in the orthotopic mouse model of human glioblastoma. Combination treatment with YYB-101 and temozolomide decreased tumor growth and increased overall survival compared with the effects of either agent alone. Five cancer-related genes (TMEM119, FST, RSPO3, ROS1 and NBL1) were overexpressed in YYB-101-treated mice that showed tumor regrowth. In the tissue cross-reactivity assay, critical cross-reactivity was not observed. The terminal elimination half-life was 21.7 days. Taken together, the <I>in vitro</I> and <I>in vivo</I> data demonstrated the anti-tumor efficacy of YYB-101, which appeared to be mediated by blocking the HGF/cMET interaction. The preclinical pharmacokinetics, toxicokinetics and tissue cross-reactivity data support the clinical development of YYB-101 for advanced cancer.</P>

        Non-recurrent Laryngeal Nerve Identified During Robotic Thyroidectomy in a Patient with Papillary Thyroid Carcinoma

        A non-recurrent laryngeal nerve is a rare anatomical variant that entails considerable riskfor iatrogenic injury during thyroid surgery. We encountered a patient with a non-recurrentlaryngeal nerve that went unnoticed on preoperative imaging but was discoveredincidentally during robotic thyroidectomy. A 44 year old woman presented at ourdepartment with papillary thyroid microcarcinoma, diagnosed by ultrasonography-guidedfine needle aspiration cytology. During robotic right thyroidectomy and central lymphnode dissection, we could not detect any structure resembling the recurrent laryngeal nervearound the inferior thyroid artery. Thus, we suspected the existence of a non-recurrentlaryngeal nerve, and successfully identified a nerve entering the larynx directly from thevagus nerve without recurring. A three-dimensional high magnification view via a roboticendoscope can aid thyroid surgeons to safely identify and preserve a non-recurrentlaryngeal nerve.

      • A case of generalized bullous fixed drug eruption

        Generalized bullous fixed drug eruption is bullous type of fixed drug eruption characterized by defined bullae at the same site following administration of medication. It is characterized by multiple, sharply defined, red macules and blisters of various sizes, bilaterally often in symmetric distribution. It occurs abruptly within a few hours to days following exposure to offending drug or sometimes the drug of similar group. Common medications that cause generalized bullous fixed drug eruption are antibiotics such as tetracycline, penicillin, quinolones and dapsone. A 76-year-old female patient visited our hospital with well-defined multiple erythematous patches and bullae on nearly whole body that appeared 5 hours ago after taking a second-generation cephalosporin used for periodontitis. Other mucosal invasion was not found. She visited our hospital 3 years ago with well-defined erythema without bullae on same with as the current one after taking cephalosporin. At the time of the biopsy, there was a favorable findings of fixed drug eruption. After stopping all other medications, we treated the patient with daily foam dressing and started methylprednisolone 60 mg a day and subsequently tapered once the lesions started healing. After 2 weeks, the lesions gradually recovered.

      • A case of hobnail hemangioma

        Hobnail hemangioma, also known as targetoid hemosiderotic hemangioma, benign vascular tumor, characterized histologically by a biphasic growth pattern of dilated vascular structures in the superficial dermis lined by prominent hobnail endothelial cells, and collagen dissecting, rather narrow neoplastic vessels in deeper parts of the lesion. The lesion has a characteristic ‘targetoid’ appearance, in which violaceous papule is usually surrounded by thin, pale rim with peripheral ecchymotic halo, occurring commonly on the trunk and extremities. A 12-year-old male patient visited our hospital with a 3mm sized, well-demarcated violaceous, irregularly bordered papule with ecchymotic halo on right chest that appeared 3 years ago. A 3mm skin biopsy was performed from the lesion to rule out granuloma pyogenicum, patch stage of Kaposi‘s sarcoma. Histologic examination showed dilated vascular channels with hobnail endothelial cells protruding into the lumen and occasional intraluminal papillary projections in the upper dermis. Deeper in the dermis, the vascular channels were thinner and seemed to dissect the collagen bundles. Other findings included numerous extravasated erythrocytes and mild perivascular lymphocyte infiltrates. In correlation with clinical findings, we diagnosed it as a hobnail hemangioma. The lesion disappeared completely following punch biopsy.
