        Association between Heart Rate Variability Indices and Depressed Mood in Patients with Panic Disorder

        Objective: Heart rate variability (HRV) reflects the regulation of the autonomic nervous system. Panic disorder is highly associated with autonomic dysfunction, and is often accompanied by depression. The aim of this study is to determine the association between depression and HRV indices in patients with panic disorder. Methods: A total of 110 outpatients diagnosed with panic disorder participated in this study. The medical records of patients with panic disorder who visited the outpatient clinic of Konkuk University Hospital between December 2018 and March 2020 were retrospectively reviewed. Measurements used in this study include the Panic Disorder Severity Scale-Self Report, Beck Depression Inventory (BDI-II), Insomnia Severity Index, and HRV. Patients were divided into depressive and non-depressive groups based on their BDI-II scores. The association between HRV indices and depressive symptoms was statistically analyzed. Results: The low frequency/high frequency (LF/HF) ratio was reduced in patients with depression (mean = −0.095, p = 0.004 in the above moderate depressive group, mean = −0.120, p = 0.020 in the severe depressive group). Significant correlations were found between depressive symptoms and standard deviation of NN interval (SDNN) (ms) (−0.19, p = 0.044), very low frequency (VLF) (ms2/Hz) (−0.22, p = 0.021), LF (−0.25, p = 0.008), HF (−0.19, p = 0.043), and LF/HF (−0.25, p = 0.009). Multiple linear regression analysis showed that BDI predicted SDNN (ms), VLF (ms2/Hz), LF, HF, and LF/HF. Conclusion: We confirmed that the LF/HF ratio decreases when depression is accompanied by panic disorder. HRV indices may be useful markers for detecting depressive symptoms in patients with panic disorder.

        부모교육 프로그램 실태 비교에 관한 질적 연구

        본 연구는 교육정책변화 속에서 서로 다른 교육철학과 프로그램을 갖고 있는 유아교육기관이 현장에서 부모교육을 어떻게 실시하고 있는지 그 실태를 파악하는 것을 목적으로 하였다. 이를 위해 부산시내 소재의 생태유아교육기관, 레지오 교육기관, 몬테소리 교육기관과 국가수준의 교육과정을 오래전부터 수행하고 있는 국공립 유아교육기관, 10곳을 중심으로 부모교육 연간계획안 및 관련 문서자료들을 수집하고 원장과 교사를 중심으로 면담을 실시하였다. 계획, 내용, 실시, 평가 네 부분으로 범주화하여 설문지와 면담을 실시하였다. 부모교육은 각 기관의 원장의 교육신념과 철학에 큰 영향을 받고 있었으며 이와 함께 표준화된 평가기준과 누리과정이 부모교육 프로그램의 다양성과 각 기관이 갖는 철학적 고유성에 다소 부정적 영향을 미치고 있는 것으로 나타났다. 부모교육의 실시에서 교사들의 과중한 업무무담으로 부모교육은 또 하나의 행사로 자리매김하고 있었으며 부모들에게 현실적이면서 실질적인 도움을 줄 수 있는 부모교육이 이루어지기 어려운 실정이었다. 따라서 대부분의 기관에서 연간계획안의 문서상 프로그램과 현장에서의 구현은 상당히 달랐다. 현 실정에 맞는 부모교육에 대한 정책적 이해 및 실행이 절실히 요구되며 기관의 철학적 다양성과 지역적 특수성이 반영될 수 있는 방안이 함께 고려되기를 희망한다. This study compared parent education programs (PEP) and their actual implementations in 10 kindergartens and child care centers. We focused on four parts: 1)the planning of PEP, 2)the contents of PEP, 3)the implementation of PEP, and 4)the evaluation of PEP. For data collection, annual parent education plans and interview data were collected. The results showed that PEP was affected by the Nuri curriculum. The PEP in public education sectors was regarded as one of the administrative works. However Reggio and Early Childhood Eco Education programs showed us different story. They encouraged parent participation through PEP. PEP was regarded as important way of philosophy reflection. Through this study, we could see the pressure of the Nuri curriculum. Some child care centers come to lose their autonomy of application of their own educational perspectives. Therefore PEP was gradually considered as one of the administrative works that teachers have to write, not implement. We suggest that the Ministry of Education need to think deeply what will be going on in the education fields when everything is standardized.

        A Study on the Framework Development for Context Analysis in Smart Home Environment

        현대 사회는 컴퓨터가 우리 주변 곳곳에 내재되어 있고, 그들 간의 네트워크로 연결되어 있는 유비쿼터스 환경으로 변화되고 있다. 이러한 환경 변화에 따라 스마트 홈에 관한 연구가 활발히 진행되고 있으며, 스마트 홈 환경에서는 사용자와 주변 환경, 정보가전에 이르기까지 매우 다양한 상황 정보와 사용자의 행동패턴이 존재한다. 이에따라 유비쿼터스 환경에서의 스마트 홈 디자인은 사람들의 생활(life)과 서비스(service)를 디자인 하는 개념 으로 정의 내릴 수 있는데, 이는 사용자의 컨텍스트(context:정황, 배경, 환경)파악이 중요하다는 것을 보여 준다. 그럼에도 불구하고 컨텍스트 관련 연구 중 사용자를 중심으로 하는 연구는 상대적으로 부족한 실정이며, 대부분 기술 분야에 많은 초점이 맞춰져 있다. 본 연구는 사용자 행동과 의도를 중심으로 컨텍스트를 파악하여 스마트 홈 디자인을 진행할 때 보다 명확한 니즈 도출을 유도할 수 있는 프레임웍을 개발하는 것을 최종 목적으로 한다. 최종적인 프레임웍은 시각화되어 나타내어지며, 이를 활용하면 사용자에게 적절한 서비스를 제공할 수 있을 뿐 아니라 다양한 응용 서비스에 공동으로 사용할 수 있으며, 이는 차후 스마트 홈 디자인 연구의 활용 자료로 쓰일 수 있을 것이다. 본 연구는 다음과 같은 방법으로 진행되었다. 첫째, 문헌연구를 통한 스마트 홈 이해를 바탕으로 컨텍스트 개념과 요소를 파악하였다. 그 중 Dey의 사용자와 응용 서비스 사이의 상호작용을 강조한 개념과 우운택의 5W1H(Who, What, Where, When, Why, How)개념을 스마트 홈 환경에 맞게 재해석하였다. 둘째, 요소를 유형화하는 단계로서, 추출된 기본 요소를 바탕으로 관찰 조사를 바탕으로 추출된 집안에서의 사용자 행동을 분석하여 행동과 의도, 그러한 의도가 나오게 된 컨텍스트 요소를 분류하여 유형화시켰다. 셋째, 프레임웍을 개발하는 단계로서, 유형화된 요소를 바탕으로 사용자 컨텍스트 중심의 프레임웍을 개발하였으며, 이를 시각화하였다. 본 연구 결과 다음과 같은 점을 발견할 수 있었다. 첫째, 문헌 연구를 통한 스마트 홈 이해를 바탕으로 추출한 요소를 유형화하고 사용자 행동을 의도에 따라 분석함으로서, 컨텍스트 파악을 용이하게 하였다. 둘째, 유형화된 요소를 바탕으로 사용자 컨텍스트 중심의 프레임웍을 개발하였으며, 이를 시각화하였다. 이는 사용자와 응용서비스간의 복잡한 인터랙션을 단순화 된 시각화로 보여줌으로 요소간의 관계파악이 용이해졌다. 셋째, 시각화 된 자료를 사용자의 행동유형 분류와 의도분석에 따라 웹상에 구현함으로 다른 컨텍스트 연구와는 차별화 되도록 하였다. 이는 프레임웍을 시각화 하는 기존의 연구보다 체계적이며 사용자 행동에 따라 의도를 파악하고 의도분석을 통해 컨텍스트를 파악하는 등 새로운 관점에서 볼 수 있어 매우 유용하게 활용될 수 있을 것이다. Designing smart home means designing system, object, and services for the future homes to improve user’s life. To design a smart home to support user’s domestic life, understanding the context of the life is crucial. This purpose of this study is to analyze domestic context and develop a visualized framework of it so that a designer can apply the analytical framework for future smart home research and design. The analysis and the framework in this study are particularly focused on user’s intention and behaviour. The framework is visualized to provide clear understanding on complex domestic context and dynamic context. We expect that the visualized analytical framework would help the future domestic design by assisting in selection of appropriate service for certain context. This study was proceeding as following: First, we understand the definition and elements of the context based on the prior research in context and smart home. We reselected the elements of the context to fit the domestic environment based on the concepts of Dey and Woo’s 5W1H (Who, What, Where, When, Why, How) concept. Then we classified the elements into physical and user context. Home members, intention and behaviour are Included in the user context. Object, home space, and time are included in physical context. Interaction associates with physical context and user context. Second, we analyzed user behaviour and context elements in the house on basis of observation research on real domestic environments in three Korean homes. Next, we materialized them after classifying the elements of context that make such intention, intention and behaviour come out. Third, we developed user context-centered framework on the basis of element taken a concrete form of and visualized it. In the visualized framework related behaviours came out with emphasis on user’s intention. The main elements of context were able to have access to intention nearly. After approaching the intention of the user, we were able to conclude needs quickly and correctly from an overalleye. On this study, we were in progress of work to visualize it, setting 4 events of visitor invitation, preparation of going-out, family reunion, and caring for babies among various intentions As a result, the visualized data are realized on the web according to intention analysis and classification of types of user behaviour. The element in the center is a person’s intention and invitation. The factors around center are the person’s behaviour according to the intention. The factors very outside the web are context elements. From the context web, we found that the picture of the context is different depending on user’s intention and relationship between the elements of the context. This results are similar to Dourish’s (2004, 23) opinion that context is not stable but an occasioned property. The visualized context analysis of this study suggests that it can be applied to design Smart Home System in the future for revealing dynamic and occasional relationship between user intention, behaviour, and other context elements.

        Network Analysis Revealed the Role of Helplessness as a Central Feature Among Late-Life Depressive Symptoms in Patients With Mild Cognitive Impairment and Early Stage Dementia

        Objective It has been reported that depressive symptoms in older adults are different from those in younger adults, especially when accompanied by cognitive decline. However, few studies have investigated the network structure of depressive symptoms in this population.Methods The participants consisted of 627 older adults (>60 yr) who were diagnosed with mild cognitive impairment (MCI) or early stage dementia. Among them, 36.7% were male and the mean age was 76.20±7.71 years. The Korean form of Geriatric Depression Scale (KGDS) was used to evaluate their depressive symptoms and network analyses were performed using bootnet R-package to identify the central features among depressive symptoms.Results Of all the KGDS items, we found that KGDS 2 (often feel helpless) had the highest node strength followed by KGDS 21 (in good spirits), KGDS 14 (not confident at all), and KGDS 15 (cheerful and happy). In terms of node betweenness, KGDS 2 also showed the highest value. The edge weights of edges connected to node KGDS 2 were strongest in KGDS 3 (restless and fidgety) and KGDS 28 (easily get tired).Conclusion In this study, we presented which symptoms are central among the elderly with MCI and early stage dementia. This result not only increases the understanding of depressive symptoms in this group but would also help determine target symptoms in the treatment program.

        The Mediating Effect of Insomnia on the Relationship between Panic Symptoms and Depression in Patients with Panic Disorder

        Background: This study aimed to determine if sleep disturbances may mediate the relationship between panic symptoms and depression in patients with panic disorder (PD). Methods: Electronic medical records were retrospectively reviewed for 110 consecutive patients with diagnosed PD in an outpatient clinic between October 2018 and December 2019. Measurements include the PD Severity Scale, Beck Depression Inventory-II (BDI-II) and Insomnia Severity Index (ISI). Statistical analyses were performed to assess any potential relationship between PD, insomnia and depression. Results: Of the PD patients, 88 (80%) and 89 (80.9%) had comorbid depression (BDI-II ≥ 14) and insomnia (Korean version of the ISI ≥ 8), respectively. In a mediation model using insomnia as the mediating variable, the total effect of panic symptom severity on depression was significant (t = 7.23, P < 0.001). There were significant effects of panic symptoms on insomnia (t = 4.62, P < 0.001) and of insomnia on depression (t = 6.69, P < 0.001). The main effect of panic symptom severity on depression was also significant, after controlling for the effect of insomnia (t = 5.10, P < 0.001), suggesting partial mediation. Conclusion: Both depressive symptoms and insomnia are common in patients with PD and depression was partially mediated by insomnia in these patients. These results suggest that an intervention for insomnia in patients with PD might help prevent the development of depression.

        Impact of Body Mass Index on Postoperative Urinary Symptoms after Miduretheral Sling Surgery in Female Patients

        Objectives: This study aims to examine the clinical outcomes of women who underwent a midurethral sling surgery for stress urinary incontinence and compare postoperative urinary symptoms among different body mass index (BMI) groups. Methods: A retrospective cohort study on results after midurethral sling surgery according to BMI was conducted at the institution of the current study from January 2010 to December 2019. The study population was classified into three groups according to patients’ BMI (in kg/m2) during surgery: normal weight (BMI < 23.0 kg/㎡), overweight (BMI, 23.0-24.9 kg/㎡), and obese (BMI ≥ 25.0 kg/㎡). The primary outcome was the recurrence of urinary symptoms after surgery. The secondary outcomes were operation time, estimated blood loss, length of hospital stay, and postoperative complications. Results: This study included 376 patients (normal weight, 148; overweight, 74; and obese women, 154) who underwent midurethral sling surgery. No significant difference was noted in urinary symptom recurrence after midurethral sling surgery. Of the patients, 6.8% (n = 10), 9.5% (n = 7), and 7.8% (n = 12) were normal weight, overweight, and obese women, respectively (P = 0.775). Moreover, operation time (P = 0.589), blood loss (P = 0.138), and complication rate (P = 0.865) showed no significant difference. Conclusions: Midurethral sling surgery is effective regardless of BMI. Even when midurethral sling surgery was performed as a concomitant surgery, no significant difference in urinary symptom recurrence, operation time, intraoperative blood loss, and complication rate was noted among different BMI groups.

        Temperament Clusters in Patients With Panic Disorder in Relation to Character Maturity

        Objective This study explored whether temperament profiles are associated with psychological functioning and whether character maturity affects this association in patients with panic disorders (PD).Methods A total of 270 patients with PD were enrolled in this study. Measurements included the Temperament and Character Inventory-revised-short (TCI-RS), a self-report version of the Panic Disorder Severity Scale (PDSS-SR), Beck Depression Inventory-II (BDI-II), and Spielberger State-Trait Anxiety Inventory (STAI). Cluster analysis was used to define the patients’ temperament profiles, and the differences in discrete variables among temperament clusters were calculated using a one-way analysis of variance. An analysis of covariance was conducted to control for the impact of character maturity on psychological functioning among clusters.Results We identified four temperament clusters of patients with PD. Significant differences in the PDSS-SR, BDI-II, STAI-state, and STAI-trait scores among the four clusters were detected [F(3, 262)=9.16, p<0.001; F(3, 266)=33.78, p<0.001; F(3, 266)=19.12, p<0.001; F(3, 266)=39.46, p<0.001]. However, after controlling for the effect of character maturity, the effect of cluster type was either eliminated or reduced ([STAI-state] cluster type: F(3, 262)=0.94, p>0.05; SD+CO: F(1, 262)=65.95, p<0.001, η<sub>p</sub><sup>2</sup> =0.20).Conclusion This study enabled a more comprehensive and integrated understanding of patients by exploring the configuration of all temperament dimensions together rather than each temperament separately. Furthermore, we revealed that depending on the degree of character maturity, the psychological functioning might differ even within the same temperament cluster. These results imply that character maturity can complement inherently vulnerable temperament expression.

        <P>Electromagnetic radiation at the plasma frequency and/or its second harmonic, the so-called plasma emission, is widely accepted as the fundamental process responsible for solar type II and III radio bursts. There have also been occasional observations of higher-harmonic plasma emissions in the solar-terrestrial environment. This paper presents the first demonstration of multiple harmonic emission by means of two-dimensional electromagnetic particle-in-cell simulation. This finding indicates that under certain circumstances the traditional mechanism of fundamental-harmonic pair emission might also be accompanied by higher-harmonic components. Consequently, the present findings are highly relevant to in situ observations of third- and/or higher-harmonic plasma emission in astrophysical and solar-terrestrial environments.</P>
