<P>Macrolide antibiotics are mainstays in the treatment of lung disease due to the Mycobacterium abscessus complex. Although previous studies have reported development of acquired macrolide resistance in this species, limited data are available on the outcomes of lung disease due to macrolide-resistant Mycobacterium abscessus subsp. abscessus. This study evaluated the clinical features, treatment outcomes, and molecular characteristics of macrolide-resistant isolates of M. abscessus subsp. abscessus. We performed a retrospective review of medical records and genetic analysis of clinical isolates from 13 patients who had acquired macrolideresistant M. abscessus subsp. abscessus lung disease between November 2006 and March 2016. Eleven (85%) patients had the nodular bronchiectatic form of the disease, and two (15%) patients had the fibrocavitary form. When acquired macrolide resistance was detected, 10 (77%) patients were on antibiotic therapy for M. abscessus subsp. abscessus, and three (23%) patients were on therapy for lung disease due to other nontuberculous mycobacteria. The median treatment duration after detecting resistance was 24.0 months (interquartile range, 16.0 to 43.0 months). Treatment outcomes were poor, and final sputum culture conversion was achieved in only one (8%) patient, after resectional surgery. All 13 clinical isolates demonstrated point mutations at position 2058 (n = 10) or 2059 (n = 3) of the 235 rRNA gene, which resulted in acquired macrolide resistance. This study indicates that treatment outcomes are very poor after the development of acquired macrolide resistance in patients with M. abscessus subsp. abscessus lung disease. Thus, more effective measures are needed to prevent development and effectively treat macrolide-resistant M. abscessus subsp. abscessus lung disease.</P>
Size-dependent toxicity of silver nanoparticles to Glyptotendipes tokunagai

Objectives This study aims to evaluate the size-dependent toxicity of spherical silver nanoparticles (Ag NPs) to an endemic benthic organism, Glyptotendipes tokunagai. Methods Ag nanoparticles of three nominal sizes (50, 100, and 150 nm) capped with polyvinyl pyrrolidone (PVP-Ag NPs) were used. Their physicochemical properties, acute toxicity (48 hours), and bioaccumulation were measured using third instar larvae of G. tokunagai. Results The aggregation and dissolution of PVP-Ag NPs increased with exposure time and concentration, respectively, particularly for 50 nm PVP-Ag NPs. However, the dissolved concentration of Ag ions was not significant compared with the median lethal concentration value for $AgNO_3$ (3.51 mg/L). The acute toxicity of PVP-Ag NPs was highest for the smallest particles (50 nm), whereas bioaccumulation was greatest for the largest particles (150 nm). However, larger PVP-Ag NPs were absorbed and excreted rapidly, resulting in shorter stays in G. tokunagai than the smaller ones. Conclusions The size of PVP-Ag NPs significantly affects their acute toxicity to G. tokunagai. In particular, smaller PVP-Ag NPs have a higher solubility and stay longer in the body of G. tokunagai, resulting in higher toxicity than larger PVP-Ag NPs.

정신건강복지법 개정에 따른 입원적합성심사와 관련된 이해관계자의 경험

2016년, 구 정신보건법이 「정신건강증진 및 정신질환자 복지서비스 지원에 관한 법률」로 개정되면서, 입원적합성심사위원회(이하 입적심) 제도가 신설되고 비자의입원 절차가 강화되었다. 본 연구는 입적심의 목표인 정신질환자의 인권증진 여부를 평가하기 위해 이해관계자들의 참여경험을 질적으로 분석하고, 향후 나아갈 방안을 모색하였다. 이에 전국 5개 국립정신병원에서 입적심과 관련된 총 27명의 전문의, 법조인, 정신건강전문요원, 가족, 당사자, 조사원, 정신질환자를 심층 면담하였다. 주제분석 결과, 4개의 주제와 16개의 하위주제가 도출되었다. 입적심 제도는 부당한 비자의입원 및 강압 이송행위에 대한 안전장치를 마련하여 인권을 향상시켰지만, 대면조사보다 서류심사가 주를 이루고 의사결정지원이 미흡하여 자기결정권이 제한된다는 한계를 드러냈다. 추후 자기결정권을 보장하기 위한 대면조사와 절차보조사업을 확충하고, 의료기관과 지역사회서비스를 연결하는 협력시스템이 구축되어야 한다. The revision of the “Mental Health Act” into the “Act on the Improvement of Mental Health and the Support for Welfare Services for Mental Patients” in 2016 led to the tightening of procedures for involuntary hospitalization through the implementation of the “Committee for Review as to Legitimacy of Admission” (hereinafter “Admission Review Committee”). This study evaluated whether the Admission Review Committee has achieved its goal to promote the human rights of people with a mental illness through qualitative analysis of stakeholder experiences. The evaluation is based on interviews with a total of 27 subcommittee members from all the five national psychiatric hospitals which have an Admission Review Committee in place. Those interviewed were psychiatrists, lawyers, mental health professionals, patients and their families, and investigators. As a result, a total of four categories and 16 themes emerged from the thematic analysis. Our study shows that the Admission Review Committee has promoted human rights by functioning as a minimum safeguard to prevent unjust involuntary hospitalization and forced transfer to psychiatric hospitals. However, insufficient procedural assistance has limited the right to self-determination, and the committee heavily relied on paper review rather than the face-to-face investigation with people with a mental illness. For a better supportive committee for the human rights of people with a mental illness, we suggest that the committee adopt the face-to-face review process, procedural assistance services, and a governance system closely connecting medical facilities with local communities.
중증정신질환자의 정신의료기관 이용과 관련된 동료지원 서비스에 대한 체계적 문헌고찰

Purpose: The purpose of this study was to systematically review peer support services related to mental health services utilization by people with severe mental illness and to identify core strategies essential for effective peer support services. Methods: A systematic search was conducted using keywords in English, 4 electronic databases, including CINAHL, EMBASE, MEDLINE, and PubMed. Of the 1,305 publications identified, 14 studies were finally included in this systematic review. The Scottish Intercollegiate Guidelines Network (SIGN) was used for the quality assessment. Results: The 14 studies consisted of 8 randomized controlled trials (RCTs), 4 cohort studies, and 2 quasi-experimental studies. Overall, peer support services were found to be effective in reducing hospitalization rates, hospitalization days, number of emergency room visits, and total medical costs. There was inconsistent evidence that the services were associated with positive effects on measures of psychiatric symptoms, quality of life, and social support. Conclusion: Peer support services for people with severe mental illness can be a cost-effective way to increase crisis stabilization and continuity of treatment. The core strategies of the services is the development of structured programs to cultivate professional peer providers and the establishment of a cooperative system between mental health professionals and peer providers.

<P>Protein-cage nanoparticles are promising multifunctional platforms for targeted delivery of imaging and therapeutic agents owing to their biocompatibility, biodegradability, and low toxicity. The major advantage of protein-cage nanoparticles is the ability to decorate their surfaces with multiple functionalities through genetic and chemical modification to achieve desired properties for therapeutic and/or diagnostic purposes. Specific peptides identified by phage display can be genetically fused onto the surface of cage proteins to promote the association of nanoparticles with a particular cell type or tissue. Upon symmetrical assembly of the cage, peptides are clustered on the surface of the cage protein in bunches. The resulting PBNC (peptide bunches on nanocage) offers the potential of synergistically increasing the avidity of the peptide ligands, thereby enhancing their blocking ability for therapeutic purposes. Here, we demonstrated a proof-of-principle of PBNCs, fusing the interleukin-4 receptor (IL-4R)-targeting peptide, AP-1, identified previously by phage display, with ferritin-L-chain (FTL), which undergoes 24-subunit assembly to form highly stable AP-1-containing nanocage proteins (AP1-PBNCs). AP1-PBNCs bound specifically to the IL-4R-expressing cell line, A549, and their binding and internalization were specifically blocked by anti-IL-4R antibody. AP1-PBNCs exhibited dramatically enhanced binding avidity to IL-4R compared with AP-1 peptide, measured by surface plasmon resonance spectroscopy. Furthermore, treatment with AP1-PBNCs in a murine model of experimental asthma diminished airway hyper-responsiveness and eosinophilic airway inflammation along with decreased mucus hyperproduction.
