        오행화침법(五行和鍼法)의 이론적 고찰 및 운용

        Objectives: The object of this study is to report on the theory of Ohaeng-Hwa Acupuncture Therapy. Methods: The theory of Ohaeng-Hwa Acupuncture Therapy(OHAT; 五行和針法) is a part of the Five Elements Theory unique to Korea. This research Classic of Difficulty Issues-Nan Jing review Ohaeng-Hwa Acupuncture Therapy. Results: OHAT, created and developed by Jae-hoon Song, integrates the victor-vanquished as well as the son-mother relationship of the Five Elements of breakdown and restoration of balance between yin and yang. And also, it provides resources and data on The seventy fifth Nan(75難), The sixty ninth Nan(69難) of Classic of Difficulty Issues-Nan Jin 75, 69. OHAT establishes objectiveness and accuracy of diagnosis based upon the traditional method and procedure of pulse taking. In OHAT, a person`s state of illness is diagnosed by applying the comparative examination of the palpitation of the pulse. It is the fact that the pulse varies according to the state, and that OHAT treatment has proven the positive results by using the victor-vanquished relationship on The Nan Jin 75. On the basis of this, it is necessary to add the sixty ninth Nan(69難), to research the theory of the generation of the Five Element. Conclusions: Ohaeng-Hwa Acupuncture is very effective in treating the wide range of illness, and thus it has gained an increasing attention of many scholars and practitioners in the field of traditional Korean oriental medicine. However, it is the first theoretical attempt to the clinical research and scientific methodology of Ohaeng(Five) Ohaeng-Hwa Acupuncture, and more active Ohaeng-Hwa Acupuncture R&D is being conducted nationwide.

        Effect of Action Observation Training Using Y-Balance on Balance Capability in Young Adults

        Purpose: This study examined the effects of action-observation training using the Y-Balance on the balance ability of young adults. Methods: Thirty-four healthy adults were randomized into an action-observation group (n=17) or a control group (n=17). All subjects performed the Y-Balance test before and after watching the video. The action observation group watched a video of someone performing a Y-Balance test, and the control group watched a video of scenery unrelated to the training. The subjects were measured through a Y-Balance test for both the length of the legs extended in three directions and the Y-balance composite score. Results: A significant difference in the Y-balance composite score was observed between the two groups. A part of the direction of the extended leg in the action observation group was increased significantly (posteromedial direction of the right leg, posterolateral direction of the right leg, posteromedial direction of left leg) compared to the control group. Conclusion: These results suggest that action observation training only could help improve balance.

      • Metabolic Reprogramming by the Excessive AMPK Activation Exacerbates Antigen-Specific Memory CD8 <sup>+</sup> T Cell Differentiation after Acute Lymphocytic Choriomeningitis Virus Infection

        <P>During virus infection, T cells must be adapted to activation and lineage differentiation states via metabolic reprogramming. Whereas effector CD8<SUP>+</SUP> T cells preferentially use glycolysis for their rapid proliferation, memory CD8<SUP>+</SUP> T cells utilize oxidative phosphorylation for their homeostatic maintenance. Particularly, enhanced AMP-activated protein kinase (AMPK) activity promotes the memory T cell response through different pathways. However, the level of AMPK activation required for optimal memory T cell differentiation remains unclear. A new metformin derivative, IM156, formerly known as HL156A, has been reported to ameliorate various types of fibrosis and inhibit <I>in vitro</I> and <I>in vivo</I> tumors by inducing AMPK activation more potently than metformin. Here, we evaluated the <I>in vivo</I> effects of IM156 on antigen-specific CD8<SUP>+</SUP> T cells during their effector and memory differentiation after acute lymphocytic choriomeningitis virus infection. Unexpectedly, our results showed that <I>in vivo</I> treatment of IM156 exacerbated the memory differentiation of virus-specific CD8<SUP>+</SUP> T cells, resulting in an increase in short-lived effector cells but decrease in memory precursor effector cells. Thus, IM156 treatment impaired the function of virus-specific memory CD8<SUP>+</SUP> T cells, indicating that excessive AMPK activation weakens memory T cell differentiation, thereby suppressing recall immune responses. This study suggests that metabolic reprogramming of antigen-specific CD8<SUP>+</SUP> T cells by regulating the AMPK pathway should be carefully performed and managed to improve the efficacy of T cell vaccine.</P>

        Influence of Extraction Method on Quality and Functionality of Broccoli Juice

        This study was performed to compare the quality and functionality of broccoli juice as affected by extraction method. Broccoli juice was extracted using method I (NUC Kuvings silent juicer), method II (NUC centrifugal juicer), and method III (NUC mixer), and the quality properties of the broccoli juices were analyzed using three different methods. Additionally, the antioxidative, anticancer, and anti-hyperglycemic activities of broccoli juice prepared by the three different methods were investigated in vitro. The broccoli juice made by method I contained the highest polyphenol and flavonoid contents at 1,226.24 ㎎/L and 1,018.32 mg/L, respectively. Particularly, broccoli juice prepared by method I showed higher DPPH and ABTS radical scavenging activities than those of the other samples. Additionally, broccoli juice made by method I showed the highest growth inhibitory effects against HeLa, A549, AGS, and HT-29 cancer cells. Broccoli juice prepared by method I had the highest α-glucosidase inhibitory effects. These results indicate that there are important differences in chemical and functional qualities between juice extraction techniques.

        Prognostic Value of Pretreatment Metabolic PET Parameters in Cervical Cancer Patients With Metabolic Complete Response After Concurrent Chemoradiotherapy

        PURPOSE: Cervical cancer may recur after concurrent chemoradiotherapy (CCRT), even in patients with metabolic complete response (mCR) on post-CCRT F-FDG PET. We retrospectively analyzed the value of metabolic parameters, measured by pretreatment PET, as prognostic factors for predicting recurrence and death in cervical cancer patients with mCR. METHODS: In 61 patients who reached mCR on post-CCRT, metabolic PET parameters including SUVmax, metabolic tumor volume (MTV), and total lesion glycolysis (TLG) of the primary tumors, as well as those of the metastatic lymph nodes, were measured by pre-CCRT PET. Total MTV and total TLG were also measured from primary tumors and metastatic lymph nodes. Survival analyses for disease-free survival (DFS) and overall survival (OS) were performed. RESULTS: Eleven patients (18.0%) presented with recurrence of cervical cancer, and 12 (19.7%) died during follow-up. SUVmax, MTV, TLG of metastatic lymph nodes, and total TLG could significantly predict DFS, and nodal SUVmax could predict OS. Para-aortic lymph node metastasis also significantly correlated with both DFS and OS. In multivariate analysis, nodal SUVmax and para-aortic lymph nodes metastasis were prognostic factors for DFS, and the best predictor was nodal SUVmax. For OS, nodal MTV and para-aortic lymph node metastasis were significant prognostic factors in multivariate analysis, and the best predictor was para-aortic lymph node metastasis. CONCLUSIONS: Metabolic PET parameters, SUVmax or MTV values of metastatic lymph nodes, may be surrogate prognostic markers for recurrence or death in patients with locally advanced cervical cancer who reached mCR after definitive CCRT.

        Differential Expression Profiling of Salivary Exosomal microRNAs in a Single Case of Periodontitis - A Pilot Study

        Exosomes are Nano-sized lipid vesicles secreted from mammalian cells containing diverse cellular materials such as proteins, lipids, and nucleotides. Multiple lines of evidence indicate that in saliva, exosomes and their contents such as microRNAs (miRNAs) mediate numerous cellular responses upon delivery to recipient cells. The objective of this study was to characterize the different expression profile of exosomal miRNAs in saliva samples, periodically isolated from a single periodontitis patient. Unstimulated saliva was collected from a single patient over time periods for managing periodontitis. MicroRNAs extracted from each phase were investigated for the expression of exosomal miRNAs. Salivary exosomal miRNAs were analyzed using Affymetrix miRNA arrays and prediction of target genes and pathways for its different expression performed using DIANA-mirPath, a web-based, computational tool. Following the delivery of miRNA mimics (hsa-miR-4487, -4532, and -7108-5p) into human gingival fibroblasts, the expression of pro-inflammatory cytokines and activation of the MAPK pathway were evaluated through RT-PCR and western blotting. In each phase, 13 and 43 miRNAs were found to be differently expressed (|FC| ≥ 2). Among these, hsa-miR-4487 (|FC|=9.292005) and has- miR-4532 (|FC|=18.322697) were highly up-regulated in the clinically severe phase, whereas hsa-miR-7108-5p (|FC|= 12.20601) was strongly up-regulated in the clinically mild phase. In addition, the overexpression of miRNA mimics in human gingival fibroblasts resulted in a significant induction of IL-6 mRNA expression and p38 phosphorylation. The findings of this study established alterations in salivary exosomal miRNAs which are dependent on the severity of periodontitis and may act as potential candidates for the treatment of oral inflammatory diseases.

      • Tannin-Titanium Oxide Multilayer as a Photochemically Suppressed Ultraviolet Filter

        <P>UV filters can initiate redox reactions of oxygen and water when exposed to sunlight, generating reactive oxygen species (ROS) that deteriorate the products containing them and cause biological damages. This photochemical reactivity originates from the high chemical potential of UV filters, which also determines the optical properties desirable for sunscreen applications. We hypothesize that this dilemma can be alleviated if the photochemical pathway of UV filters is altered to coupling with redox active molecules. Here, we employ tannic acid (TA) as a key molecule for controlling the photochemical properties of titanium dioxide nanoparticles (TiO<SUB>2</SUB> NPs). TA provides an unusual way for layer-by-layer assembly of TiO<SUB>2</SUB> NPs by the formation of a ligand-to-metal charge transfer complex that alters the nature of UV absorption of TiO<SUB>2</SUB> NPs. The galloyl moieties of TA efficiently scavenge ROS due to the stabilization of ROS by intramolecular hydrogen bonding while facilitating UV screening through direct charge injection from TA to the conduction band of TiO<SUB>2</SUB>. The TiO<SUB>2</SUB>-TA multilayers assembled in open porous polymer microspheres substantially increased sun protection while dramatically reducing ROS under UV exposure. The assembled structure exhibits excellent in vivo anti-UV skin protection against epidermal hyperplasia, inflammation, and keratinocyte apoptosis without long-term toxicity.</P> [FIG OMISSION]</BR>

        Combined Open Door Laminoplasty with Unilateral Screw Fixation for Unstable Multi-Level Cervical Stenosis : A Preliminary Report

        Objective : The authors reviewed their experiences of combined surgery (open door laminoplasty with unilateral screw fixation) for unstable multi-level cervical stenosis, to clarify the situation regarding the surgical approach most appropriate for the treatment of diffuse unstable multi-level cervical stenosis. Methods : From January 2011 to January 2012, combined surgery was performed for unstable multi-level cervical stenosis by one surgeon at our institution. The subjects of this study were 6 men of mean age 53.7 years (range, 48-71) with a mean follow-up of 9.3 (range, 3-14) months. All imaging studies showed severe multi-level cervical stenosis with spinal cord signal change, and instability or kyphotic deformity. A retrospective review of clinical, radiological, and surgical data was conducted. Results : Average laminoplasty level was 4.8 and the average screw fixation level was 5.0. Japanese Orthopedic Association score improved from an average of 5.2 to 11.2 points. According to Nurick's grades and Odom's criteria, symptom improvement was statistically significant. On the other hand, Cobb's angle changes were not significant. Average operation time was 5.86 hours with an average blood loss of 460 mL. No significant surgical complication was encountered. Conclusion : Despite the small cohort and the short follow-up duration, the present study demonstrates that laminoplasty with unilateral screw fixation is a safe and effective treatment for unstable multi-level cervical stenosis.
