Hepatic glucose production by glucagon is crucial for glucose homeostasis during fasting, yet the underlying mechanisms remain incompletely delineated. Although CD38 has been detected in the nucleus, its function in this compartment is unknown. Here, we demonstrate that nuclear CD38 (nCD38) controls glucagon-induced gluconeogenesis in primary hepatocytes and liver in a manner distinct from CD38 occurring in the cytoplasm and lysosomal compartments. We found that the localization of CD38 in the nucleus is required for glucose production by glucagon and that nCD38 activation requires NAD+ supplied by PKCδ-phosphorylated connexin 43. In fasting and diabetes, nCD38 promotes sustained Ca2+ signals via transient receptor potential melastatin 2 (TRPM2) activation by ADP-ribose, which enhances the transcription of glucose-6 phosphatase and phosphoenolpyruvate carboxykinase 1. These findings shed light on the role of nCD38 in glucagon-induced gluconeogenesis and provide insight into nuclear Ca2+ signals that mediate the transcription of key genes in gluconeogenesis under physiological conditions.

<P>LPS has been shown to induce hepatocyte autophagy, but little is known about how LPS is able to do this during acute toxic liver injury. Our aim was to determine the existence of any selective Ca<SUP>2+</SUP> signaling coupling to hepatocyte autophagy in response to LPS. LPS increased the autophagic process in hepatocytes, and CD38 knockdown prevented this response. Ned19, a specific inhibitor for nicotinic acid adenine dinucleotide phosphate (NAADP), prevented LPS-mediated Ca<SUP>2+</SUP> signaling and autophagosome formation in hepatocytes. CD38 overexpression protected the liver from LPS/<SMALL>D</SMALL>-galactosamine (GalN)-induced injury, and NAADP administration promoted autophagosome formation and protected hepatocytes from injury induced by LPS/GalN. Autophagy was promoted by the up-regulation of autophagy-related gene expression <I>via</I> NAADP-mediated Ca<SUP>2+</SUP> signaling in response to LPS. However, CD38-knockout mice displayed down-regulation in hepatocyte gene expression. Ned19 also inhibited the NAADP-stimulated induction of gene expression by inhibiting the LPS-induced nuclear translocation of transcription factor EB (TFEB). Hepatocyte autophagy protects against LPS-induced liver injury <I>via</I> the CD38/NAADP/Ca<SUP>2+</SUP>/TFEB pathway. The role of NAADP-mediated Ca<SUP>2+</SUP> signaling in the autophagic process will help elucidate the complexities of autophagy regulation, which is essential toward the discovery of new therapeutic tools against acute liver injury.—Rah, S.-Y., Lee, Y.-H., Kim, U.-H. 본 연구는 지역사회 내에 거주하는 재가노인의 신체적, 심리적, 사회ㆍ환경적 상황을 종합적으로 평가하고 그에 적합한 서비스를 제공하기 위한 욕구 중심의 사정도구를 개발하는데 목적이 있다. 본 연구를 통해 개발된 사정도구는 총 200문항으로 구성되어 있으며, 재가노인의 신체, 심리, 사회ㆍ환경적 상황을 통합적으로 사정한다는 장점을 가지고 있다. 또한 노인이 현재 생활하며 느끼는 실제적인 욕구를 적극적으로 반영할 수 있는 주관적 호소를 포함시킴으로써 전문가적 입장에서 관찰되는 객관적 측면뿐만 아니라 노인의 주관적인 삶의 모습을 표현하게 하였다. 본 도구의 개발은 2004년 7월부터 2006년 3월까지 21개월에 걸쳐 이루어졌으며, 크게 그 과정을 세 단계로 나누어 볼 수 있다. 1단계에서는 문헌고찰, 외국자료 조사 및 분석을 통해 욕구 중심의 통합적 사정을 위한 문항들을 수집하였으며, 2단계에서는 1차적으로 개발된 도구를 노인장기요양보험제도 시범사업에 적용하였다. 이 과정에서는 시범사업에서 선정된 250명의 재가노인과 본 연구팀이 자체적으로 선정한 재가노인 200명의 대상자에게 본 도구를 적용한 후, 도구의 미흡한 점을 수정, 보완하였다. 3단계에서는 개발된 도구의 신뢰도와 타당도를 확보하기 위해 포커스 집단 활용과 전문가 집단의 자문을 활용하여 최종적으로 도구를 완성하였다. 본 도구는 13개의 하위영역에 총 200문항이 구성되었다. 그 하위영역은 기본정보, 주관적 호소내용, 주수발자의 기본정보, 현재 이용하고 있는 서비스, 주거환경, 건강상태, 재활상태, 일상생활(ADL, IADL, 배변, 배설, 조력), 사회적 지지, 행동장애, 의료건강, 생활습관, 강점으로 구성되었다. This research has its purpose of developing a tool to assess the needs of the Korean elderly at home population and to provide adequate services by evaluating their physical, psychological, and socio-environmental aspects. This developed tool is composed of two hundred questions and has the advantage of combined physical, psychological and social environmental situation assessment of the elderly at home. The tool also contains not only the objective view of the professionals, but also the subjective appeals of the elderly at home population so that it can reflect their substantial desires. The assessment tool was developed over 21 months from July, 2004 to March, 2006 and this period can be divided into three different stages. In the first stage, collecting of questions for the desire-focused assessment of the elderly by literature investigation and researching foreign source materials was carried out, and this ultimately developed assessment tool was applied to the long-term care insurance pilot project in the second stage. In this process, we revised some insufficiencies of this tool after we applied to elderly of 250 from the pilot project and other 200 elderly from this research team. For the last stage, the tool was completed by using inquiries of the focused group and the group of professionals to ensure its reliability and validity. In the process of developing the tool, the total of 200 questions under 13 subcategories was selected. The 13 subcategories are basic information, subjective appeals, information of the main helper, use of services, house environment, condition of health, condition of rehabilitation, daily living(ADL, IADL, defecation, assistance), social maintenance, behavioral disability, medical health, living habits, and strength. This tool is on the purpose to assess thoroughly the desires that the elderly at home population has and to provide the best service they need.
Ginsenosides That Show Antinociception in Writhing and Formalin Tests

We demonstrated in previous study that protopanaxadiol and protopanxatriol saponins show antinociceptive activity in acetic acid induced writhing test and in the second phase (11-40 min) of formalin test but not tail-flick test. To identify further which ginsenoside has antinociceptive activity among various ginseng saponins, we have investigated antinociceptive effects of several ginsenosides using writhing and formalin test. Ginsenoside Rc, Rd, Re, and Rf induced antinociception in writhing test. These four ginsenosides also induced antinociception in the second phase of formalin (11-40 min) test but these ginsenosides showed a slight antinociception in the first phase (010 min) of formalin test except ginsenoside Rf. The antinociceptive effects induced by the ginsenosides were dose dependent and were not blocked by an opioid receptor antagonist, naloxone. The order of antinociceptive potency was Rd > Rc > Re > Rf in the formalin test. However, these ginsenosides did not show any significant analgesic effects in a tail-flick test. These results suggest that ginsenosides such as Rc, Rd, Re, and Rf inhibit tonic pain rather than acute pain induced by noxious heat. These results also indicate that the antinociceptive activity. Induced by ginsenosides may be one of the actions for pharmacological effects of Panax ginseng.
Anti-inflammatory effects of a traditional Korean medicine: Ojayeonjonghwan

<P><B>Abstract</B></P><P><B>Objective:</B> To study the anti-inflammatory properties of OJ.</P><P><B>Context:</B> Ojayeonjonghwan (OJ) is a traditional Korean prescription, which has been widely used for the treatment of prostatitis. However, no scientific study has been performed of the anti-inflammatory effects of OJ.</P><P><B>Materials and methods:</B> Peritoneal macrophages were isolated 3–4 days after injecting a C57BL/6J mouse with thioglycollate. They were then treated with OJ water extract (0.01, 0.1, and 1 mg/mL) for 1 h and stimulated with lipopolysaccharide (LPS) for different times. Nitric oxide (NO), inducible nitric oxide synthase (iNOS) and cyclooxygenase (COX)-2, and proinflammatory cytokine levels were determined by NO assay, Western blotting, RT-PCR and ELISA.</P><P><B>Results:</B> NO generation and iNOS induction were increased in the LPS-activated mouse peritoneal macrophages. However, NO generation and iNOS induction by LPS were suppressed by treatment with OJ for the first time. The IC<SUB>50</SUB> value of OJ with respect to NO production was 0.09 mg/mL. OJ did not influence LPS-stimulated COX-2 induction, but did significantly decrease LPS-stimulated secretions and mRNA expressions of tumour necrosis factor (TNF)-α, interleukin (IL)-6, and IL-1β. Inhibition rates of TNF-α, IL-6, and IL-1β at an OJ concentration of 1 mg/mL were 77%, 88%, and 50%, respectively. OJ also suppressed the LPS-induced nuclear translocation of NF-κB. High-performance liquid chromatography showed schizandrin and gomisin A are major components of OJ.</P><P><B>Conclusions:</B> OJ reduces inflammatory response, and this probably explains its positive impact on the prostatitis associated inflammation.</P>
