        일 종합병원에서 정신분열병과 분열정동장애를 진단받은 환자에서 항정신병약물의 1차 선택약물의 비율 및 1년 유지율에 관한 연구

        Objectives: This study was designed to evaluate prescription rate and indirectly the tolerability of antiosychotics using the maintenance rate for 1 year. The study was done in a situation where the patients who were diagnosed with either schizophrenia or schizoaffective disorder were given the antipsychotics and were looked into the prescription rate and the state of use of drugs at first regardless of kind of drugs. Methods: This was a retrospective, naturalistic study using the data from medical record conducted in both inpatients and outpatients from January 1st in 2002 and June 30th in 2004 with a diagnosis of schizophrenia and schizoaffective disorder as defined by the Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition. Patients were evaluated at 3, 6, 9, and 12th months after the onset of medication for the first-drug prescription rate and maintenance rate. Results: In total, 105 patients were recruited, and 40 out of 105 were male and 65 out of 105 were female. The mean age was 34.71 years old. The number of patients with schizophrenia was 94 and that of schizoaffective disorder was 11. For the prescription rate of first drugs, of total risperidone was 56.2% ; likely, 25.7% for olanzapine ; 10.5% for haloperidol ; 4.8% for quetiapine ; and 2.9% for chlorpromazine. For 1-year maintenance rate, of total, haloperidol was 45.5% ; 33.3% for chlorpromazine ; 22.0% for risperidone ; 14.8% for olanzapine ; and 20.0% for quetiapine. Conclusion: Overall, atypical antipsychotics were far more prescribed than typical ones(86.7% and 13.3% for atypical and typical anti psychotics respectively). Although atypical antipsychotics were more prescribed than typical ones, the one-year maintenance rate was not higher than typical anti psychotics unlike our expectation that atypical antipsychotics would have better compliance and higher maintenance rate for 1 year under the circumstances with uncontrolled naturalistic conditions. There should be more statistical data available with larger sample size in the future.

        Effects of Long-acting Injectable 3-Monthly Paliperidone Palmitate on the Clinical and Social Performance of Patients with Schizophrenia

        Objective: To investigate the effects of long-acting injectable 3-monthly paliperidone palmitate on the clinical and social functioning of patients with schizophrenia. Methods: This study enrolled patients with schizophrenia receiving long-acting injectable 1-monthly paliperidone palmitate for at least 4 months and who subsequently received 3-monthly paliperidone palmitate. Accordingly, 418 patients were followed up for 24 weeks. Their clinical symptoms and social functioning were measured using the Clinical Global Impression-Severity of Illness and Personal and Social Performance scales. Results: The Personal and Social Performance total score was significantly higher after 3-monthly paliperidone palmitate treatment than at baseline (baseline vs. week 24: 54.3 ± 18.0 vs. 61.0 ± 14.5 [mean ± standard deviation]; p < 0.001; Wilcoxon signed-rank test); the proportion of patients in the mildly ill group (scores 71−100) also increased significantly (baseline vs. week 24: 16.5% vs. 20.6%; p < 0.001; McNemar-Bowker test). The mean Clinical Global Impression-Severity of Illness score decreased significantly (baseline vs. week 24: 3.7 ± 1.0 vs. 3.4 ± 0.9; p < 0.001; Wilcoxon signed-rank test), as did the proportion of patients in the severely ill group (baseline vs. week 24: 4.1% vs. 2.1%; p < 0.001; McNemar-Bowker test). Conclusion: Continuous 3-monthly paliperidone palmitate treatment significantly enhances the personal and social performance of patients with schizophrenia and reduces the proportion of those with severe illness. These findings suggest that long-acting injectable antipsychotic administration at intervals longer than 1 month might improve the social functioning of and promote return to activities of daily living in patients with schizophrenia.

      • Short-Selling with a Short Wait : Trade- and Account-Level Analyses in Korean Stock Market

        We examine in this paper the short-selling trades that are initiated and closed on the same day using the proprietary dataset, which covers the comprehensive trading records at the trade- as well as account- level in Korean stock market. Our results show that day trading short-selling is pervasive, accounting for 42% of total shorted shares, and that the individual day traders in general make positive profits from shortselling even when transaction costs are considered. We further investigate the timing of short-selling and the covering transactions and find that traders who short stocks in the morning and hold the position longer, and those who short stocks with high intraday volatility earn higher profit. Lastly, we find that day traders make profits by executing numerous trades to exploit short-term small price fluctuations.

        Comparison of outcomes of laparotomic and minimally invasive radical hysterectomy in women with early-stage cervical cancer

        Objective: This study compared the outcomes of laparotomic radical hysterectomy (LRH) and minimally invasive radical hysterectomy (MISRH) in patients with early-stage cervical cancer. Methods: The clinical data of patients with early-stage cervical cancer who underwent LRH or MISRH (laparoscopic/robotic) at Chang Gung Memorial Hospital, Linkou Branch, from 2002 to 2017 were retrospectively reviewed. The surgical safety (operation time, blood loss, blood transfusion rate, length of postoperative stay, and perioperative complications), overall survival (OS), disease-free survival (DFS), and recurrence pattern were analyzed. Propensity score matching (PSM) at a 3:1 ratio was performed to balance prognostic variables. Results: Of the 760 patients (entire cohort), 614 underwent LRH and 146 underwent MISRH. After PSM, 394 and 140 patients were included in the LRH and MISRH groups, respectively. The 5-year OS rate was significantly lower in the MISRH group than in the LRH group (85.6% vs. 93.2%, p=0.043), and the 5-year DFS rate (p=0.21) did not differ significantly. After PSM, the 5-year OS rates did not differ significantly between the MISRH and LRH groups (87.1% vs. 92.1%, p=0.393). The MISRH group had a significantly shorter operation time (p<0.001), lower intraoperative blood loss (p<0.001), lower blood transfusion rate (p<0.001), and shorter postoperative stay (p<0.001) but a significantly higher rate of intraoperative bladder injury (p<0.001) than the LRH group. Conclusion: After PSM, MISRH is associated with nonsignificantly lower OS but a significantly higher risk of intraoperative urological complications than LRH.

        Can Liquidity be a Risk Factor that Predicts Economic Growth?: The U.S. and International Evidence

        We examine whether illiquidity premium, constructed as a difference in returns between the most illiquid and the least illiquid (or the most liquid) portfolios, predict future economic growth, measured by GDP growth rate. We find supporting evidence that the premium significantly predict future GDP growth rate in the U.S. even with various controls such as HML, SMB, dividend yield, industrial production, term premium, and default premium. Our empirical evidence provides new rational for why liquidity, a stock characteristic, is priced - since it has information about future real economy. However, contrast to the U.S. results, empirical evidence is mixed and generally weaker in the global sample of stocks from 35 countries.

        Microcantilever biosensor: sensing platform, surface characterization and multiscale modeling

        The microcantilever (MCL) sensor is one of the most promising platforms for next-generation label-free biosensing applications. It outperforms conventional label-free detection methods in terms of portability and parallelization. In this paper, an overview of recent advances in our understanding of the coupling between biomolecular interactions and MCL responses is given. A dual compact optical MCL sensing platform was built to enable biosensing experiments both in gas-phase environments and in solutions. The thermal bimorph effect was found to be an effective nanomanipulator for the MCL platform calibration. The study of the alkanethiol self-assembly monolayer (SAM) chain length effect revealed that 1-octanethiol (C8H17SH) induced a larger deflection than that from 1-dodecanethiol (C12H25SH) in solutions. Using the clinically relevant biomarker Creactive protein (CRP), we revealed that the analytical sensitivity of the MCL reached a diagnostic level of 1~500 μg/ml within a 7% coefficient of variation. Using grazing incident x-ray diffractometer (GIXRD) analysis, we found that the gold surface was dominated by the (111) crystalline plane. Moreover, using X-ray photoelectron spectroscopy (XPS) analysis, we confirmed that the Au-S covalent bonds occurred in SAM adsorption whereas CRP molecular bindings occurred in protein analysis. First principles density functional theory (DFT) simulations were also used to examine biomolecular adsorption mechanisms. Multiscale modeling was then developed to connect the interactions at the molecular level with the MCL mechanical response. The alkanethiol SAM chain length effect in air was successfully predicted using the multiscale scheme.

        Remarkably efficient hydrolysis of cinnamaldehyde to natural benzaldehyde in amino acid ionic liquids

        The hydrolysis of cinnamaldehyde to natural benzaldehyde was investigated systematically using tetramethylammonium- based amino acid ionic liquids as homogeneous catalysts. The results indicated that tetramethylammonium prolinate ([N1111][Pro]) can be a powerful catalyst for the highly efficient hydrolysis of cinnamaldehyde, in which natural benzaldehyde was obtained with almost 94% yield and over 99% selectivity in 1 h. Moreover, kinetic study showed that compared with other catalysts, the catalytic system of [N1111][Pro] has a lower activation energy of 38.30 kJ·mol−1 in the hydrolysis reaction, indicating superior catalytic performance of [N1111][Pro]. Quantum-mechanical calculations further manifested that such high performance originates from the cooperative catalysis of the secondary amino and carboxyl group in the anion [Pro].
