Sesquiterpene Components from the Flower Buds of Magnolia fargesii

From the Chinese crude drug shin-i, the flower buds of Magnolia fargesii, four sesquiterpene, oplopanoe (1), oplodiol (2), homalomenol A (3) and $1{\beta},4{\beta},7{\alpha}$-trihydroxyeudesmane (4) were isolated. These structures were elucidated and the ${13}^C-NMR$ chemical shifts of these compounds were revised by means of various 2D-NMR techniques.

중증 재생불량성빈혈에서 타인 혹은 HLA 불일치 혈연간 골수이식을 위한 cyclophosphamide와 antithymocyte globulin전처치 요법

배경: Cyclophosphamide와 ATG의 전처치 요법은 HLA가 일치하는 형제간 골수이식에서는 매우 효과적인 것으로 알려져 있으나, 타인 혹은 HLA 불일치 혈연간 골수이식에서는 전처치 요법으로서 불충분한 것으로 보고되었는데, 주요 문제는 착상 부전이었다. 우리나라를 포함하는 극동아시아 지역의 국가들에서 발표되는 보고에 의하면 타인이식을 포함하여 골수이식 후 착상 부전과 이식편대숙주질환의 발생이 서구 국가들로부터의 보고에서보다 적은 것으로 알려져 있다. 저자 등은 6예의 중증 재생불량성빈혈 환자들에서 질병 초기에 Cyclophosphamide와 ATG만을 이용하여 전처치를 한 후에 타인 혹은 HLA 불일치 혈연간 골수이식을 시행하였다. 방법: 1999년 5월부터 2000년 4월까지 서울중앙병원에서 타인 혹은 HLA 불일치 혈연간 골수이식을 시행 받은 성인 중증 재생불량성빈혈 환자들을 대상으로 하였다. 골수이식을 위한 전처치 요법으로 cyclophosphamide (50 mg/kg/d x 4)와 ATG (30 mg/kg/d x 3)를 투여하였는데, 골수이식 전에 ATG에 대하여 과민 반응을 보였던 한 예 (UPN 120)에서는 ATG 대신에 fludarabine (30 mg/m2/d x 3)을 투여하였다. 이식편대숙주질환의 예방을 위하여 cyclosporine과 methotrexate를 투여하였다. 결과: 연구 기간 중에 모두 6예가 타인 혹은 HLA 불일치 혈연간 골수이식을 시행 받았는데, 5예는 타인으로부터 골수를 공여 받았으며, 1예는 HLA의 표현형이 하나의 유전자좌에서 일치하지 않는 형제로부터 골수를 공여 받았다. 타인 골수이식을 받은 5예 중에서 3예의 공여자는 HLA의 표현형이 일치하였으며, 1예는 하나의 유전자좌가 major mismatch였고, 1예는 하나의 유전자좌가 minor mismatch였다. 추적기간의 중앙값은 406일 (범위, 328-643일)인데, 6예 모두에서 착상된 상태를 유지하면서 생존해있다. 2예에서 3도의 급성 이식편대숙주질환과 전신성 만성 이식편대숙주질환이 발생하였다. 결론: Cyclophosphamide와 ATG의 전처치 요법은 재생불량성빈혈의 초기에 타인 혹은 HLA 불일치 혈연간 골수이식을 시행 받은 한국인 환자들에서 충분히 착상을 가져올 수 있는 요법으로 생각된다. 비록 본 연구의 대상 환자 수가 적지만 결과는 매우 고무적이다. HLA가 일치하는 형제가 없는 중증 재생불량성빈혈 환자의 경우에 타인 골수 공여자에 대한 검색을 조기에 시행해야 하며, 타인 혹은 HLA 불일치 혈연간 골수이식을 질병 초기에 적극적으로 고려해야 할 것으로 생각된다. Background: Cyclophosphamide (CY) and ATG regimen was reported to be insufficient for alternative donor BMT in SAA patients due to high incidence of graft rejection. The rates of graft rejection and GVHD after allogeneic BMT including unrelated donor transplants seemed to be lower in reports from Asian countries of Far East. We applied CY/ATG regimen to six patients with early stage of SAA transplanted with marrow from alternative donors other than HLA-identical siblings. Methods: Three patients were transplanted from HLA-phenotypically identical unrelated donors, two from one-locus mismatched unrelated donors, and one from an one-locus mismatched sibling donor. Four patients received no previous therapy for SAA, and two received one course of immunosuppressive therapy with ATG. Preparative regimen was CY (200 mg/kg) plus ATG (90 mg/kg) in all patients except one who had had an anaphylactic reaction to ATG prior to BMT and received fludarabine (90 mg/m2) in place of ATG. Cyclosporine plus methotrexate were given for GVHD prophylaxis. Results: All six patients engrafted and all are alive with durable engraftment at a median follow-up of 406 days (range, 328 to 643). Two patients developed grade III acute GVHD and extensive chronic GVHD. Conclusions: CY/ATG regimen may be sufficient in Korean patients with early stage of SAA transplanted with marrow from alternative donors. Although the number of patients in present study is small, results are encouraging. Stem cell transplantation using alternative donors may be considered at early stage of aplastic anemia.

Systematic Review of Heavy Metal Concentrations in Fish and Shellfish in Korea

Objectives: Heavy metals ingested through the consumption of aquatic products can accumulate in the human body over the long-term and cause various health problems. This study aims to present comprehensive data on the amount of heavy metals found in fish and shellfish in Korea using a systematic review of studies that report on that issue. Methods: The study used the following databases: PubMed, Korean Studies Information Service System, and Research Information Sharing Service. The search terms for PubMed included fish OR shellfish OR seafood AND mercury OR cadmium OR lead OR heavy metal AND Korea. The search terms for Korean Studies Information Service System and Research Information Sharing Service included eoryu sueun, eoryu kadeumyum, eoryu nab, eoryu jung-geumsog, paeryu sueun, paeryu kadeumyum, paeryu nab, paeryu jung-geumsog, eopaeryu sueun, eopaeryu kadeumyum, eopaeryu nab, and eopaeryu jung-geumsog. Results: A total of 32 articles were selected for review. The total mercury, lead, and cadmium concentrations in fish and shellfish reported in each of the articles are summarized, as are the species of fish and shellfish with relatively high concentrations of heavy metals. Total mercury concentrations tended to be higher in predatory fish species, such as sharks, billfishes, and tuna, while lead and cadmium concentrations tended to be higher in shellfish. Conclusion: This paper is the first to report a comprehensive summary of the concentrations of heavy metals in fish and shellfish. This data could be used as evidence to protect Koreans from exposure to heavy metals due to the consumption of highly polluted aquatic products.

조혈모세포이식 후 발생한 주폐포자층 폐렴에 대한 고찰

Background : Pneumocytitis cainii pneumonia (PCP) can occur in immunocompromised hosts especially such as AIDS or cancer patients. Although recent research had focused on PCP in AIDS patients, few studies have described the clinical presentations of PCP in recipients of stem cell transplantation (SCT). We evaluated the clinical manifestations of PCP in SCT patients admitted at St. Mary's hospital, Seoul, Korea. Methods : The medical records of 17 PCP patients undergoing SCT between Feb. 1998 and Feb. 2000 were reviewed. The diagnosis of PCP was confirmed through the demonstration of Pneumocytitis cainii via either cytology of brochoalveolar lavage (BAL) or histological technique of lung biopsy. CMV disease and CMV infection were confirmed by BAL culture and antigenemia respectively . Results : Seventeen patients were all recipients of allogeneic SCT and 7 of 17 patients were performed non-sibling SCT. Patients presented with symptoms including brief period (4 ∼23 days) of fever (76%), dyspnea (70%), cough (64%), and signs such as rare(58.8%), Sixteen patients (94%) had been receiving immunosuppressive agent such as cyclosporine A (64%) or Fk506 (35%) without PCP prophylaxis. Eleven patients (64%) were treated with corticosteroid with mean dose of 16 mg/day prednisolone and mean duration of 4.6 months after post-SCT period. Twelve patients were co-infected with CMV. Another co-infected miCroorganisms were Pseudomonas aeruginosa, Mycobacterium tuberculosis, herpes simplex virus, parainfluenza virus, Average duration of treatment with trimethoprim-sulfamethoxazole (TMP/SMX) was 21 ±9 days. Four patients died, and three of them were related with PCP. Conclusion : PCP developed frequently in patients who were taking immunosuppressive drug due to graft versus host disease or were not taking TMP/SMX prophylaxis. High risk patients showing fever, cough, or dyspnea should be considered to take early bronchoscopic intervention for detection of PCP. When treat for PCP, it also be considered to the possibility of coinfection such as CMV. (Korean J Infect Dis 33:273∼279, 2001)

발레뤼스(1909-1929)의 작품 성향 연구

This paper studied the art character and the choreographic inclination of choreographers be led by Serge Diaghilev who newly made the whole artistic forms of the Ballets Russes(1909-1929). Therefore, his achievement was acknowledged as the thoughtful and aesthetic base and was much affect on the representation field of modern ballet. Michel Fokin, the choreographer of the Ballets Russes, had a plenty of knowledge on music and folk dances, and made an integration of both a fine art designer and an artistic work With a strong will about the new innovation, he represented a dramatic dance and brought a male dancer into relief in his works. Also, he emphasized that the style of ballet is fitted for the subject, the step always has to be new, and the dance has to be represented by the whole body. It was an inclination of his works that he tried on the condensed ballet into one act, showed to be unified completely both the subject and the emotional representation, and particularly, the stage fine art and the costume by Leon Bakst in <Scheherazade> were regarded to an epoch making stage for Fokin. Vaslav Nijinsky, as well a born dancer as the choreographer, sought after the new representation of modern ballet through his four works at period of the Ballets Russes. In <L'Apres-midi d'un faune> and <La Sacre du printemps>, he showed remarkably the primitive inclination with the Stravinsky's music. Also, he emphasized the primitive character by the individual choreography which made a trial of the new experimental motions both using a straight line and an acute angle and getting out of the tradition. Leonid Massine, the choreographer, who was excellent and choreographed the ballet on the basis of characteristic dances and folk dances, he represented the rhythmical melody which put emphasis on the importance of both music and dance, emphasized the dramatic representation which took much more of the interchange between dancers and spectators. In <Parade>, he tried the typical modernism and made a new motion vocabularies by using of both the music and the various acoustic effects which were mixed with the fine art of the Picasso's geometrical cubism style and the theories of dances. And he had an originative choreographic inclination. Therefore, he was able to make not only a symphonic ballet but also a story ballet to the comic satires. Bronislava Nijinska, the choreographer composed an actual choreograph with the characteristic motions of animals which represented the fabled contents on the basis of <Le Renard>. Also, in <Le Train Blue>, she tried to compose the classical ballet with the dynamic and acrobatic motions which was related to the sport games, and she showed the choreographic inclination of new modern senses. The <Les Noces>, which used the constructive composition with her excellent musical talent, was represented through the rhythmical character of Stravinsky and rhythmic movements. The various representations of folk, comedy, fable, and etc. were included in her works. So, she expanded the representative extent of arts. George Balancine, as the last choreographer of the Ballets Russes, accepted the thought of Diaghilev and made his own art character with the artistic aesthetics. In <Apollon Musagete>, he represented the excellence of the tradition on the logical techniques as the purity and the classical balance, created newly the elegant and angled motions with the combination of between music and choreograph. The inclination of works of the Ballets Russes showed the modern characters with the subject, the form, and the pursuit of new as an imagination which were totally included the realism, the primitivism, the expressionism, the new classicism, and so on. And Diaghilev as an art planner, five choreographers, and the famous artists at that time created together the new values which were developed the change of needs and responses in those days. Therefore, their works became wide spread around the world by a lots of choreographers and dancers who acted In the Ballets Russes. Finally, the Ballets Russes made a great contribution periodically and culturally to the history of modern Ballet.

한국의 전문무용교육의 현황과 발전방향 모색

The goal of this study is to examine the present situation and system of professional dance education of Korea and grope for the development direction for training the dancers whose level si high in the future. In the followings, this researcher is to make some kinds of suggestions about the points at issues of professional dance education of Korea and about its desirable development direction. 1) Only in case that there should be the teacher in change of dance subject in primary school for the necessity of early education as well as expansion of base of dance and that professional dance school or ballet school should be founded newly for the excavation of gifted talents, phased dance is made. 2) As for the subject of practical talent of major study of art school, 8--12 hours per a week is very insufficient. And, the hours of practical talent for major study should be expanded, and the basic subjects for major study theory should be added. And, the change of number of hours of general subjects is the most urgent business. 3) While the graduates of art school(middle school0 are about 145 persons every year, the number limit of students to enter the department of dance in art high school amounts to 8-00 persons or so. So the lack phenomenon which is due to the unbalance of number of students to enter the department of dance is generating the situation of insufficiency. Thus, the increase of art middle school is being required keenly. 4) The number of general subjects at the department of dance in art high school should be reduced, and more hours for professional subjects should framed. to get to reduce the burden of general subjects which is caused by the examination for the study ability in university may develop and improve the ability to be able to show the talent as a dance through more practical talents of major study. Thus, raising the reflection rate of practical talent in university examination may bring the certainty about the training of specialists. 5) There is the tendency that the department of dance in high education institutions will increase all the more, And, as for the scheme for providing for insufficiency state about it, specialization and differentiation should be made at every department in each university. Thus, through the change and correction of composition system of subjects, is should ? the students to have the professional nature as dancers, educators, and theorists ?, and the system for a certificate of qualification should be established and ?. 6) The subjects and the certificate of qualification of art specialists at department of dance in General Art School are fulfilling its function as national Education Institution to ? able to train the professional dancers. And, as the division of department of practical ? department of creation and department of theory is clear, the subject system to be able to train dance specialists is being composed well. ut the officially approved attached school is necessary, so that the phased professional dance education from early education may be executed. 7) As for the private professional education institution for dance, the management system that the certificate of qualification is given with the ability to be able to be subject ? professional dance group though the composition of program of diverse systems is necessary. For the training of specialists which is suitable for education goal that the department of dance in each art middle and higher education institution seeks as above, ? that the development will be made only in case of enabling the gifted talents to ? fostered by enabling the phased and systematic dance education to be started at early period and of educating with the connection nature with higher education institution.

A planar chromel-alumel multijunction ac-dc converter was fabricated on a LPCVD Si_(3)N_(4)/SiO_(2)/Si_(3)N_(4)-diaphragm, prepared by silicon bulk micromachining, which thermally isolated a bifilar evanohm-heater and the hot junctions of a chromel-alumel thermopile from the silicon substrate. The voltage responsivity, the ac-dc transfer error, and the fluctuation of the output thermoelectric voltage of the converter were investigated. The respective voltage responsivities in air and in a vacuum of the converter were about 3.16 mV/mW and 9.41 mV/mW. The ac-dc voltage and the current transfer errors in air were about ±1.5 ppm and ±0.7 ppm in a frequency range from 10 Hz to 10 kHz, respectively. The fluctuation of the output thermoelectric voltage from 5 seconds to 120 seconds after preheating in air for 5 seconds was about 0.06 % for a heater input of dc 1 V.

삼차신경통으로 발현된 소뇌�u각 지방종 : 증례보고 A Case Report

Intracranial lipomas are rare lesions that constitute 0. 1% of cerebral tumors. Moreover, the cerebellopontine(CP) angle is one of the most unusual sites of such hamartomas with only 30 reported cases in the literature. We report a case of CP angle lipoma presenting with trigeminal neuralgia on a 39-year-old male. Magnetic resonance imaging revealed a high signal mass at right CP angle on T_(1)-weighted images before contrast administration. This mass appears hypointense on T_(2)-weighted images. We operated through right suboccipital craniectomy and excised the mass subtotally. Total removal may be obtained with significant cranial nerve deficits. Therefore, the treatment of CP angle lipomas should be conservative. Histopathologic studies demonstrated lipoma.

정신분열병에 대한 Risperidone의 효과 및 안전성에 관하여

In order to evaluate the efficacy and safety of risperidone, 10 patients with chronic schizophrenia were examined for 8 weeks in a silgle-open study. After the wash-out period of 1 week, risperidone was administered. Efficacy was evaluated by means of Positive and Negative syndrome Scale for Schizophernia(PANSS) and Clinical Global Impression(CGI). Safety assessment included the Extrapyramidal Symptom Rating Scale(ESRS), vital signs, body weight, ECG, and laboratory tests. The results were as follows 1) On the PANSS total score, there was significant improvement of total score after 1 week (P<.05) of administratiom. 2) On the PANSS positive and negative subscal, there were significant improvement of posotive and negative scores after 2 weeks (P<.05) of administration. 3) On the PANSS general psychopatholgy subscals, there were significant improvement of general psychopathologyscores after 1 week (P<.05) of administratiom. 4) On the CGI, there was significant decreasement of clinical impression of severity of schizophrenia score after 2 week (P<.05) of administration. 5) A statistically significant increase in body weight (P<.05) was observed after 8 weeks of administration. 6) EPS reached the peak at the end of the 1st week of administration of risperidone (11.8± 24.25) but they were easily relieved by addition of benztropine and clonazepam. There was not significant change in laboratory tests, vital sign, ECG after 8 weeks of administration. These results suggest that Risperidone is an effective antipsychotics and clinically safe except for increased body weight in chronic schizophrenia.
