        좌측 전대뇌동맥 영역의 뇌경색 후 발생한 초피질성 혼합 실어증 1예

        We present a case of transcortical mixed aphasia (TMA) presumably caused by left anterior cerebral artery infarction. A 53-year-old, right-handed woman suddenly developed speech disturbance and abnormal behavior. Her spontaneous speech was remarkably reduced to almost mutistic state and objective naming, comprehension, reading and writing were severely impaired. However, repetition of phonemes and sentences were fully preserved. She showed echolalia and completion phenomenon, which prompted us to make a diagnosis of TMA. Although the lesion was confined to extrasylvian area on MRI, SPECT demonstrated diminished blood flow left perisylvian cortices suggestive of functional isolation of speech area. In spite of her echolalic repetition, she couldn't repeat affective prosody, presumably, because of the left anterior corpus callosal lesion. Treatment with bromocriptine, a dopamine agonist helped her recovery of behavioral changes, namely abulia or akinetic mutism consisitent with mesial frontal syndrome.

      • Adipocyte-Specific Deficiency of De Novo Sphingolipid Biosynthesis Leads to Lipodystrophy and Insulin Resistance

        <P>Sphingolipids have been implicated in the etiology of chronic metabolic diseases. Here, we investigated whether sphingolipid biosynthesis is associated with the development of adipose tissues and metabolic diseases. SPTLC2, a subunit of serine palmitoyltransferase, was transcriptionally upregulated in the adipose tissues of obese mice and in differentiating adipocytes. Adipocyte-specific SPTLC2-deficient (aSPTLC2 KO) mice had markedly reduced adipose tissue mass. Fatty acids that were destined for the adipose tissue were instead shunted to liver and caused hepatosteatosis. This impaired fat distribution caused systemic insulin resistance and hyperglycemia, indicating severe lipodystrophy. Mechanistically, sphingosine 1-phosphate (S1P) was reduced in the adipose tissues of aSPTLC2 KO mice, and this inhibited adipocyte proliferation and differentiation via the downregulation of S1P receptor 1 and decreased activity of the peroxisome proliferator-activator receptor gamma. In addition, downregulation of SREBP (sterol regulatory element-binding protein)-1c prevented adipogenesis of aSPTLC2 KO adipocytes. Collectively, our observations suggest that the tight regulation of de novo sphingolipid biosynthesis and S1P signaling plays an important role in adipogenesis and hepatosteatosis.</P>

        Nicotine and tyrosine detection in blood and urine samples using taurine/reactive blue-immobilized conducting polymer composite

        <P><B>Abstract</B></P> <P>A sensor for the nicotine (Nic) and L-tyrosine (Tyr) detection was developed with a taurine (Tau) and reactive blue 4 (RB4) bonded-conducting polymer ((poly (2,2´:5´,5″-Terthiophene-3´-<I>p</I>-benzoic acid) (pTBA)) layer formed on AuNPs doped-glassy carbon. The sensing material facilitated a selective electron transfer for Nic and Tyr oxidations and effectively blocked the interferences of negatively charged species by the sulfonate groups and carboxylic acid groups present on the Tau/RB4-pTBA/AuNPs layer. Each layer was characterized employing electrochemical and surface analysis methods. The experimental parameters affecting the sensor response were optimized in terms of amounts of Tau/RB4, pH, temperature, and interference effects. The modified electrode demonstrated excellent performance for the detection of Nic and Tyr in the concentration ranges of 0.02–2.0 and 2.0–300.0 μM and 0.02–5.0 and 5.0–300.0 μM, respectively, with the detection limit (<I>k = </I>3) of 0.93 ± 0.08 and 0.88 ± 0.06 nM. The reliability of sensor was evaluated by the simultaneous detection of Nic and Tyr in urine and whole blood samples. In addition, the cigarette smoking effect onto the concentrations of target molecules in blood were investigated, where the sensor probe confirms that decrease of Tyr and increase of Nic level by smoking.</P> <P><B>Highlights</B></P> <P> <UL> <LI> Functionalized conductive polymer was used to covalently immobilize Taurine and Reactive Blue 4. </LI> <LI> The interferences of negatively charged species was blocked by the sulfonate groups and carboxylic acid groups. </LI> <LI> The modified electrode demonstrated an excellent performance for the detection of Nic and Tyr in blood and urine samples. </LI> <LI> The effect of cigarette smoking onto the concentrations of Nic and Tyr was investigated using human whole blood samples. </LI> </UL> </P>

        최근 디지털 동영상의 사용이 증가하면서 자동으로 구조를 분석하는 기술이 필요하게 되었다. 특히 뉴스 동영상은 다른 동영상에 비해 그 구조가 비교적 정형화 되어 있다는 특징 때문에 많은 연구에서 이용되어졌다. 뉴스 동영상을 이용하는 이전의 연구에서 문제점으로 제시될 수 있는 사항은 서로 다른 자료 구조와 시스템 구조로 인한 호환성의 부족을 들 수 있다. 본 논문에서는 이와 같은 호환성 부족을 해결하기 위해 멀티미디어 데이터를 기술하는 표준인 MPEG-7을 기반으로 한 뉴스 동영상 스카마를 제안하였다. 특히 전체 뉴스를 보지 않고 효율적으로 뉴스 동영상을 이해할 수 있도록 요약하는 방법을 제시하였으며 MPEG-7의 MDS를 이용하여 기술하였다. 본 논문에서 제안한 방법은 디지털 비디오 라이브러리와 같은 응용 분야에서 유용하게 이용될 수 있을 것이다.

        MOF-on-MOF Architectures: Applications in Separation, Catalysis, and Sensing

        Metal–organic frameworks (MOFs) are porous crystalline materials with a high tunability. To improve the functionality of the original frameworks, several strategies, such as the use of different metal cations and organic ligands and post-synthetic modification, have been developed, enabling the use of MOFs in numerous practical applications in various fields. Recently, another approach, i.e., MOF-on-MOF architecturing, has been actively studied by combining two or more MOFs into a composite. MOF-on- MOF materials not only possess the intrinsic properties of each MOF but also exhibit unprecedented synergism within a single system, resulting in a considerable potential for various applications. This review summarizes the interesting areas of application of MOF-on-MOF architectures into three categories: separation, catalysis, and sensing. In particular, the synergism occurring within such MOF-on-MOF architectures is discussed.

        디스크 전지를 삼킨 소아에서 내시경적 이물 제거술의 비교

        목 적: 소아에서 디스크 전지를 삼키는 사고가 증가하고 있다. 식도에 걸린 경우 식도 궤양, 천공, 누공, 종격동염 등 심각한 합병증을 유발할 수 있으므로 즉각 제거해야 한다. 본 연구에서는 디스크 전지의 내시경적 제거에 있어서 기구에 따른 제거 속도의 차이와 안전성을 비교해 봄으로써 가장 빠르고 효율적으로 제거할 수 있는 방법을 찾아보고자 하였다. 방 법: 1997년 7월부터 2007년 6월까지 디스크 전지를 삼키고 경북대학교병원 소아과에 내원하여 내시경적 적출술을 시도한 24명의 환아를 대상으로 하였다. 평균 나이는 1.5세였고 남아가 9명이었다. 내시경 보조기구로써 magnetic extractor와 풍선을 이용하거나 retrieval net을 사용한 경우를 별도로 비교하여 제거에 걸린 시간과 안전성에 차이가 있는지 검토하였다. 결 과: 전지의 직경과 두께가 $20{\times}2$ mm (또는 2 mm) 인 것이 5건이었고 이 중 3예(60%)에서 식도에 걸려 식도 점막 괴사 또는 궤양을 초래하였다. 직경 20 mm 디스크 전지는 직경 10 mm 전지에 비해 식도에 걸려 손상을 초래하는 경우가 유의하게 많았다(p<0.01). 내시경적 제거방법으로 retrieval net을 사용한 경우가 10명으로 평균 1.5분이 소요되었으며 magnetic extractor와 풍선(또는 condom)을 이용한 경우가 6명이었는데 평균 소요 시간이 3.3분 걸려 retrieval net을 사용할 경우 더욱 신속한 제거가 가능하였다(p<0.05). 결 론: 식도의 디스크 전지는 내시경적 응급 상황으로 즉시 내시경으로 신속하고 안전하게 제거할 수 있도록 조치하여야 한다. 저자 등의 경험에 의하면 retrieval net을 이용한 위장내 이물 제거가 더 효과적이었으나, 시술자의 숙련도와 선호도에 따라 magnetic extractor와 풍선을 이용한 방법 또는 retrieval net을 이용한 제거 등을 선택하여 사용할 수 있을 것이다. Purpose: Any battery impacted in the esophagus must be removed urgently because of the possibility of serious complications such as perforation, fistula, and mediastinitis. The use of endoscopic procedures was compared to find the most rapid, effective, and safe method to remove disk batteries. Methods: The cases of 24 children (9 males, mean age, 1.5 years) that had accidentally ingested disk batteries were reviewed. The patients had visited the Department of Pediatrics, Kyungpook National University Hospital from July 1997 to June 2007 for the removal of disk batteries. Endoscopic removal was attempted using a retrieval net or a magnetic extractor with a balloon (condom). The procedure times for removing the ingested battery were compared. Results: Children that ingested batteries with a larger diameter (20 mm) had a greater chance to have esophageal impaction with serious injury, such as esophageal mucosal necrosis or ulcers, as compared to the ingestion of smaller diameter batteries (10 mm) (p<0.01). Endoscopic removal of disk batteries was attempted either using a retrieval net in 10 children or by using a magnetic extractor with a balloon in 6 children. Endoscopic removal using a retrieval net was more effective (mean procedure time: 1.5 min) as compared to using a magnetic extractor with a balloon (mean procedure time: 3.3 min) (p<0.05). Conclusion: Both procedures, either using a retrieval net or a magnetic extractor with a balloon were effective in removing ingested disk batteries. Based on our experience, an endoscopic procedure using a retrieval net was a more effective method for foreign body removal.

        면역 저하 환자에서 발생한 파종성 Mycobacterium intracellulare 감염

        We report a case of disseminated Mycobacterium intracellulare infection in a 31-year-old man who had been diagnosed as having dermatomyositis and systemic lupus erythematosus 3-years prior. The patient developed a left pleural effusion M. intracellulare was repeatedly isolated from the pleural fluid. After antimycobacterial treatment, the patient`s pleural effusion resolved, but a left knee joint effusion developed newly and M. intracellulare was cultured from the joint fluid. At present, the patient has been taking antimycobacterial medication for 15 months but his left knee joint fluid remains positive for M. intracellulare. To our knowledge, this is the second reported case of disseminated NTM infection in a non-HIV infected patient in Korea.
