        In this study, the calculation of the behavior of gas in the intake and exhaust system of the multi-point injection SI engine has been accomplished under various operating conditions. The generalized method of characteristics including friction, heat transfer, area change and entropy gradients was used to analyze the pipe flow. The simulation models were selected not to depend much on the empirical constants. The constant-pressure model and the constant-volume model were compared to analyze the gas behavior in the plenum chamber in intake system. Through the comparison of predicted results with experiments, the simulation program was verified by showing good prediction of the behavior of IC engine qualitatively and quantitatively over the wide range of operating conditions.

      • 노인틀니 보험급여를 위한 틀니수가 산정법에 관한 조사연구

        The use of insurance to help spread the financial burden attendant upon geriatric populations in Korea is of a governmental issue. Much of recent issues is the effort to apply this principle to current dental expenses. Therefore, the purpose of this study was to provide a guideline for the national oral health care policy of the insurance payment system for denture service. An opinion survey form was mailed to an approximately 10% of general dental practitioners (1,164) and 260 returned survey responses were analysed. Also, conducted were a telephone interview with 64 general dental practitioners who were willingly participated out of initial contact of 80 practitioners concerning all expenses incurring for the maintenance of dental clinics and personnel. Preexisting informations concerning the dental technician's fee schedule and the cost of materials needed for the construction a denture were also compiled from the Korean Dental Technician's Association and the Korean Association for Dental Materials and Instruments for the actuarial cost to determine the total budget and insurance payment for the denture services of geriatric population in Korea. The results were al follow. 1.A 64.6% of dentists agreed to as a carrier of beneficiary of national denture services for a group of more than 70 years of age. A 73.5% of dental practitioners responded that the selection of the carrier of beneficiary of the responses agree to expand the denture service to all prosthodontic patients. 2. As far as the area of national insurance plans for denture services are concerned, a 93.9% and a 77.5% agreed in the resin-based full dentures only for full denture and in the non-precious metal cast for partial denture respectively. Therefore, the results clearly indicated that both types of denture services need to be included for the national oral health care program. 3. As far as indemnity for the prosthodontic coverage is concerned, the survey results indicated a 693,000 won for resin-based full denture and 708,000 won for non-precious metal cast. Dentists are more favor of a monetary (77.3%) than material (20.4%) reimbursement system. 4. When the actuarial cost for the prosthetic appliance only with the time of services rendered, the materials used and the fee for dental technician were calculated without the consideration of difficulty of the prosthetic services, the cost for resin-based full dentures and non-precious metal cast partial dentures were a 560,484 won and 594,160 won respectively. 5. When the actuarial cost for the prosthetic appliance were calculated with the consideration of difficulty of the services which were based on relative service index of 6,567 won per hour, the basic expenses of service rendered, the current insurance payment schedule and the personnel expense for services by a hygienist, the cost for one resin-based full denture and one non-precious metal cast partial denture were a 589,714 won and 610,296 won respectively.

      • 가토 악골에 매식된 Apatite-Wollastonite Glass Ceramic의 생체반응에 관한 실험적 연구

        The authors implanted the Apatite Wollastonite Glass Ceramic(AWGC) powders in the rabbit maxillae to evaluate the possibility of clinical use as an artificial bone substitute material. Firstly we made AWGC and analized powder X-ray diffraction pattern of glass after crystalization. Secondly we made animal experiment with New Zealand white rabbits. The rabbits were sacrificed at the day of opstoperative 2 weeks and 4 weeks after implantation. We took the radiography of the maxillae block specimens, each of which includes the AWGC, at the day of sacrifice. And the block specimens were routinely prepared for the SEM and EDX study. We observed the attitudes at the border between the bone and the material with the SEM, and we searched the components such as Ca or P by use of EDX and compared the periodic changing data. The obtained results are as follows : 1. On SEM findings, silica band-like zone was found between the bone and the implanted materials at 2-week specimen, and this zone was found diffusely mingled between the bone and the implanted meterials at the 4-week specimen. 2. On intraoral X-ray findings, the border line was definitely found between the bone and the the material just after implantation of AWGC, but the border line became difused and fusion was occasionally found at the border area. 3. There was a little change in the natural bone components at the border area, and Ca or P was drawn out from Awgc at the border area. 4. Si of AWGC was increased as the time being, and AWGC was dissolved with leakage of Ca and P. So it is suggested that the intermediate zone of Calcium Phosphate Apatite was formed at the border area by Ca and P from AWGC dissolution, and it makes AWGC further tightly bonded to the bone.

      • Apatite-Wollastonite Glass-Ceramics 材料開發 및 치면열구전색재로의 이용을 위한 동물실험 연구

        The authors has experimented with Apatite - Wollastonite - Glass -Ceramic of MgO-CaO-SiO_2-P_2O_5-CaF_2 as biomaterials in order to find out the possibility for use of pit & fissure sealants. 2-rabbits would be selected with insertion of AWGC powder at lower anterior teeth after flap operation. X-ray taking has been done after 2 weeks and 4 weeks breedings. Extracted tooth had been used as samples for SEM observation after sacrificing every 2 weeks. Every samples had been observed at material portion as well as tooth. Such components as Si, Ca and P had been checked by use of EDX and compared the periodic changing data. The obtained results are as follows; 1. Apatite - Wollastonite glass ceramics were prepared with the component of MgO-CaO-SiO_2-P_2O_5-CaF_2 successfully. 2. On intraoral X-ray findings for teeth, A-W materials had been shown the slightly absorption at 2 weeks breeding sample and A-W material had been remove out by attrition otherwise it had been shown the generally radiolucent area as decalcification at 4 weeks breeding sample. 3. On SEM finding for tooth sample after insertion of A-W, the band like intermadiate zone had been found more remarkably at 2nd and 4th week breeding samples, rather than that of the sample which prepared at right after insertion. And this band zone had been fingured diffuse image by gradual. 4. Si and P might be drawn out from A-W material by disolution of A-W through the time, and it was estimated that calcium phosphate as apatite crystal, newly formed biomaterial, would be made of P component from A-W and Ca component from Tooth. 5. It was estimated that A-W material would be more useful at bone than at teeth and adaptable cement should be needed between tooth surface and A-W in case of using for pit & fissure sealant. So more studies for developing the adaptable cement would be needed.

        A single center experience of adrenalectomy for adrenal tumors

        Purpose: In this study, we reviewed 16 cases of adrenalectomy, focusing specifically on the surgical outcomes and clinical courses. Methods: The data from 16 patients who underwent an adrenalectomy at our hospital between January 2007 to December 2016 were retrospectively analyzed based on their medical records. Data available for each patient included age, sex, tumor location, length of postoperative hospital stay, tumor size, tumor pathology, final diagnosis, operation time, operative blood loss, and type of operation. Results: The study population consisted of eight males and eight females, with a mean age of 58.00±11.34 years (range, 33–76 years). The mean tumor size was 2.78±2.02 cm (range, 0.5–7 cm). Fourteen patients were diagnosed with functional adrenal tumors and two with nonfunctional adrenal tumors. Compared to laparoscopic adrenalectomy, open surgery resulted in a statistically significantly longer operating time, increased operative blood loss, and a longer postoperative hospital stay. Conclusion: If the hormonal activity of adrenal tumors is not confirmed, the tumor should be considered functional when accompanied by related symptoms and an adrenalectomy should be performed. In addition, we suggest that a laparoscopic adrenalectomy is superior to an open adrenalectomy in terms of recovery after surgery, despite the small number of cases evaluated. Open adrenalectomy should be considered following comprehensive consideration of the patient’s condition, such as accompanying surgery or metastatic cancer.

        Effects of Repeated Citalopram Treatments on Chronic Mild Stress- Induced Growth Associated Protein-43 mRNA Expression in Rat Hippocampus

        Although growth associated protein-43 (GAP-43) is known to play a significant role in the regulation of axonal growth and the formation of new neuronal connections in the hippocampus, there is only a few studies on the effects of acute stress on GAP-43 mRNA expression in the hippocampus. Moreover, the effects of repeated citalopram treatment on chronic mild stress (CMS)-induced changes in GAP-43 mRNA expression in the hippocampus have not been explored before. To explore this question, male rats were exposed to acute immobilization stress or CMS. Also, citalopram was given prior to stress everyday during CMS procedures. Acute immobilization stress significantly increased GAP-43 mRNA expression in all subfields of the hippocampus, while CMS significantly decreased GAP-43 mRNA expression in the dentate granule cell layer (GCL). Repeated citalopram treatment decreased GAP-43 mRNA expression in the GCL compared with unstressed controls, but this decrease was not further potentiated by CMS exposure. Similar decreases in GAP-43 mRNA expression were observed in CA1, CA3 and CA4 areas of the hippocampus only after repeated citalopram treatment in CMS-exposed rats. This result indicates that GAP-43 mRNA expression in the hippocampus may differently respond to acute and chronic stress, and that repeated citalopram treatment does not change CMS-induced decreases in GAP-43 mRNA expression in the GCL.

        Response rate to androgen therapy in aplastic anemia has been reported at 20-50%, and patient survival of the treatment group was not different from control group in several randomized trials. We wanted to confirm our hypothesis that the clinical behavior of aplastic anemia in Korea is different from western countries by documenting better response rate and longer survival of aplastic anemia patients who were treated with androgen only. We analyzed retrospectively the data of 51 patients with aplastic anemia who were diagnosed and treated with 100㎎/day of oxymetholone during the period of January 1990 - December 2000. The results are as follows. The sex ratio was 23:28 or 1.21. There were 26 moderate aplastic anemia versus 25 severe aplastic anemia patients. The median age of patients was 42 (18-82) years. 43 (84.3%) patients received oxymetholone as the first-line treatment (Group l), whereas 8 (15.7%) patients received oxymetholone after failure of immunosuppressive therapy (Group 2) Among 43 patients in Group 1, 7 (16%) patients obtained complete response and 16 (37%) patients achieved partial response with an overall response rate of 53%, and among 8 patients in Group 2,1 (13%) patients obtained complete response and 5 (63%) patients partial response with an overall response rate of 75%. There was no significant difference in survival rates between Group 1 and Group 2 (p=0.259). The overall response rate was 57%. Among 29 patients in response group, 21 (72%) patients survived and among 22 patients in Non-response group, 8 (36%) patients survived. There was a significant difference in survival rates between Response group and Non-response group (p=0.020). The overall survival rate was 57%. Kaplan-Meier analysis revealed a longer survival in responders (p=0.0029). The oxymetholone treatment in patients with aplastic anemia increases survival rates and prolongs the survival duration.
