        당뇨교육경험이 제2형 당뇨환자의 지식, 자가간호행위, 당화혈색소에 미치는 영향

        Purpose: This study was to identify the effect of the experience of diabetes education on knowledge, self-care behavior and glycosylated hemoglobin (HbA1C). Further, this study was held to examine about patient’s preferred methods of education and re-education frequency. Methods: 166 type 2 diabetes patients from two hospitals in Incheon participated in this study. Data were analyzed by using descriptive analysis, t-test, ANOVA, Scheffes test and multiple regression analysis. Results: 72.3% patients needed re-education and the average interval of re-education was 8.53 months. Patients preferred education methods were lectures, practical training, and studying from pamphlet. Depending on the frequency of diabetes education, there were significant differences in the level of diabetes knowledge (F=10.88, p<.001) and self-care behaviors (F=4.59, p=.012), but there was not significant difference with HbA1C (F=1.53, p=.220). As to how much the diabetes education helped managing diabetes, there was a significant difference in the level of self-care behaviors (t=2.01, p=.049), but there were not significant differences in level of knowledge (t=1.10, p=.275) and HbA1C (t=-.33, p=.746). The experience of diabetes education was a significant factor which influenced patients knowledge (t=3.93, p<.001) and self-care behaviors (t=2.21, p<.001). But HbA1C was not influenced by the experience of diabetes education (t=-1.68, p=.096). Conclusion: It is necessary to provide diabetes education with appropriate interval and methods and subjects that reflect the needs of patient through the study results.

      • Wedge shaped polydioxanone threads in a folded configuration (“solid fillers”): a treatment option for deep static wrinkles on the upper face

        Background: Facial wrinkles are key signs of aging which affect one’s quality of life and psychological well-being. Absorbable wedge shaped polydioxanone (PDO) sutures are popular in the aesthetic cl. Objectives: To describe a novel technique applicable to deep static wrinkles on the upper face and to assess its safety and efficacy. Methods: A retrospective chart review was performed on the static wrinkle cases which were treated with folded, wedge shaped PDO threads. A total of 33 Korean patients with glabellar and forehead wrinkles were included. All participants underwent a single treatment session. The results were assessed objectively using serial photography and subjectively based on the patients’ satisfaction scores. Complications were also recorded. Results: Our patients displayed moderate (15.2%), severe (39.4%), and very severe (45.4%) static wrinkles at baseline. The majority of patients (81.8%) considered the results satisfactory. Consensus ratings by two independent dermatologists showed that the objective outcomes at 2-month follow-up were very-much improved (30.3%), much improved (30.3%) and improved (24.2%). The incidence of complications was low and the complications were minor. Conclusion: Wedge shaped PDO sutures in a folded configuration (“solid fillers”) are a promising treatment option for deep static wrinkles on the upper face. They may be applied as an adjunct, or as an alternative to conventional therapies such as botulinum toxin and soft tissue filler injection.

        A Structural Equation Model Linking Health Literacy to Self-efficacy, Self-care Activities, and Health-related Quality of Life in Patients with Type 2 Diabetes

        Purpose: Health literacy has been attracting increasing attention because low health literacy is considered an important predictor of adverse health outcomes in many chronic conditions, including diabetes. However, it is unclear how health literacy is associated with health outcomes. The purpose of this study was to formulate a hypothetical structural equation model linking health literacy to self-efficacy, selfcare activities, and health-related quality of life (HRQOL) in patients with type 2 diabetes. Methods: A cross-sectional survey design was employed, and 459 patients with type 2 diabetes were recruited from outpatient clinics in two university hospitals. The patients completed a pack of questionnaires. The hypothetical model was tested using structural equation modeling analysis. Results: The values of multiple fit indices indicated that the proposed model provided a good fit to the data. Health literacy exerted not only a direct effect on self-care activities, but also an indirect effect on self-care activities via self-efficacy. However, health literacy exerted only an indirect effect on HRQOL. This structural model was invariant across hemoglobin-A1c-controlled and hemoglobin-A1c uncontrolled groups. Based on R2 values, the final model accounted for 20.0% of the variance in selfefficacy, 61.0% of the variance in self-care activities, and 16.0% of the variance in HRQOL. Conclusions: This study suggests that self-care activities are crucial to the link between health literacy and HRQOL. Both health literacy and self-efficacy need to be considered in clinical practice for enhancing self-care activities in patients with type 2 diabetes. This approach may ultimately improve HRQOL in these patients.

      • Practical Prevalence of Sexual Transmitted Diseases in Patients who Visited Out-patient Clinic for Sexual Transmitted Diseases Examination

        Present study intend to show practical prevalence of sexual transmitted disease (STD) in patients who visited private clinic for STD examination. The present study investigates the types of STD tests and infection rates among the patients, who visited a private clinic and underwent vaginal examination for STD from October 2007 to April 2011, retrospectively. Regarding test results, 479 (56%) out of the 845 patients were found to be infected with an STD, and 92 patients (11%) were afflicted with more than two STDs. Gardnella vaginalis (n=103, 21.5%), Ureaplasma urealyticum (n=102, 21.3%), Mycoplasma hominis (n=101, 21.%), Chlamydia trachomatis (n=60, 12.5%), Herpes simplex virus type Ⅱ (n=51, 10.6%), Tricomonas vaginalis (n=40, 8.5%), Mycoplasma gentialium (n=11, 2.3%) and Neisseria gonorrhea (n=11, 2.3%) were found in the order named. Surprisingly, many of these patients had actually infected with STDs. To make conclusive results, the more organized and randomized study might be needed. However, STDs are likely to affect others or even newborns as well as patients themselves with various range of complications. Therefore early diagnosis and treatment are very important. To patients who suspect their STD infection, clinicians should conduct STD test for early diagnosis and proper treatment.

      • Psychometric Properties of the Diabetes Management Self-Efficacy Scale in Korean Patients with Type 2 Diabetes

        <P><I>Objectives</I>. The aims of this study were to perform a cultural translation of the DMSES and evaluate the psychometric properties of the translated scale in a Korean population with type 2 diabetics. <I>Methods</I>. This study was conducted in patients with diabetes recruited from university hospitals. The first stage of this study involved translating the DMSES into Korean using a forward- and backward-translation technique. The content validity was assessed by an expert group. In the second stage, the psychometric properties of the Korean version of the DMSES (K-DMSES) were evaluated. <I>Results</I>. The content validity of the K-DMSES was satisfactory. Sixteen-items clustered into four-subscales were extracted by exploratory factor analysis, and supported by confirmatory factor analysis. The construct validity of the K-DMSES with the Summary of Diabetes Self-Care Activities scale was satisfactory (<I>r</I> = 0.50, <I>P</I><0.001). The Cronbach's alpha and intraclass correlation coefficient were 0.92 and 0.85 (<I>P</I><0.001; 95% CI = 0.75–0.91), respectively, which indicate excellent internal consistency reliability and test-retest reliability. <I>Conclusions</I>. The K-DMSES is a brief instrument that has demonstrated good psychometric properties. It is therefore feasible to use in practice, and is ready for use in clinical research involving Korean patients with type 2 diabetes.</P>
