Ki-67 labeling index as a prognostic marker in advanced stomach cancer

Purpose: Proliferation marker Ki-67 is widely used in cancer prognosis prediction. We tried to investigate the role of Ki-67 as a prognostic factor in stomach cancer after surgery in this study. Methods: We retrospectively evaluated 251 patients who underwent curative resection for gastric cancer from 2010 to 2015. In pathologic examination, Ki-67 labeling index was defined as the percentage of Ki-67 antigen positive cells. Prognostic significance of Ki-67 for gastric cancer was evaluated. Disease-free survival (DFS) was assessed as a primary end-point. Results: The median follow-up period was 28.0 months. Thirty-one patients (12.4%) showed Ki-67 labeling index (LI) lower than 25%. Sixty-eight patients (26.6%) showed recurrence during follow-up period. Recurrence was associated with Ki- 67 LI level (≤25%, P = 0.016), and lymph node metastasis status (P = 0.002). High Ki-67 LI level (>25%) was also related to p53 positivity (P < 0.001) and poorly cohesive type (P = 0.002). The 3-year DFS was 69.4%. Low Ki-67 LI level (≤25%) was related with low DFS (47.6% vs. 72.6%, P = 0.016). T stage (P < 0.001), N stage (P = 0.006), lymphovascular invasion (P = 0.010), and neuronal invasion (P = 0.001) also affected the DFS. In addition, T stage (P = 0.03) and Ki-67 LI (P = 0.035) were independent prognostic factors for DFS. In patients treated with adjuvant chemotherapy (n = 239, 93.4%), low Ki-67 (≤25%) was a poor prognostic factor for DFS (P = 0.013). Conclusion: Low Ki-67 LI predicts high rate of progression and low DFS of stomach cancer. Ki-67 LI can be a predictive marker in resected stomach cancer treated with surgery and adjuvant chemotherapy.
Adiponectin Attenuates the Inflammation in Atopic Dermatitis-Like Reconstructed Human Epidermis

Background: Atopic dermatitis (AD) is a chronic disorder, with a vicious cycle of repetitive inflammation and deterioration of the epidermal barrier function. Adiponectin, an adipokine, has anti-inflammatory effects on various metabolic and inflammatory disorders. Recently, its level was found to be reduced in serum and tissue samples from AD patients. Objective: We aimed to investigate the effects of adiponectin on epidermal inflammation and barrier structures in AD skin. Methods: A three-dimensional in vitro epidermal equivalent model mimicking AD was obtained by adding an inflammatory substance cocktail to normal human epidermal equivalents (HEEs). The expression of epidermal differentiation markers, primary inflammatory mediators, and lipid biosynthetic enzymes was compared between adiponectintreated AD-HEEs, untreated control AD-HEEs, and normal HEEs. Results: Adiponectin co-treatment 1) inhibited the increase in mRNA expression of major inflammatory mediators (carbonic anhydrase II, neuron-specific NEL-like protein 2, thymic stromal lymphopoietin, interleukin-8, tumor necrosis factor-alpha, and human beta-defensin-2) from keratinocytes in AD-inflammatory HEEs, 2) enhanced the expression of lipid biosynthetic enzymes (fatty acid synthase, HMG CoA reductase, and serine-palmitoyl transferase), and 3) promoted the expression of differentiation factors, especially filaggrin. We also found that the expression of adiponectin receptor-1 and -2 decreased in the epidermis of chronic AD lesion. Conclusion: Activation of the adiponectin pathway is expected to enhance epidermal differentiation and barrier function as well as attenuate inflammatory response to AD as a therapeutic approach. Background: Focal adhesion kinase (FAK) and Src are non-receptor tyrosine kinases. FAK and Src play a critical role in inducing malignant transformation in tumor cells. Objectives: We performed immunohistochemical staining for total and phosphorylated forms of FAK and Src, to evaluate the role of FAK and Src in the development of premalignant and malignant skin lesions. Methods: A total of 59 facial skin samples (30 actinic keratoses, 10 Bowen``s diseases, 13 squamous cell carcinomas and six perilesional skins) were immunohistochemically stained for Ki-67, total (t) and phosphorylated (p) form of FAK and Src. Results: Cells positive for t-Src, p-Src-y530, t-FAK and pFAK-s722 were detected in premalignant intra-epithelial lesions (PELs) and squamous cell carcinomas (SCCs), but not in the perilesional skin. There was a tendency towards high correlation between Ki-67 and t-FAK or pFAK-s722, suggestive of the active role of FAK in cell proliferation. Conclusion: Our findings of higher t-Src and p-Src-y530 positive cells in PELs, as compared to SCCs (with higher Ki-67 level), are suggestive of the other role of Src in tumor formation and progression, which requires further investigation.

An eleven year old castrated male Maltese was presented to the Veterinary Medical Teaching Hospital of Chungnam National University. In physical examination, approximately 0.5 cm round mass was observed on tail base. This small elevated mass has also shown alopecic and firm configuration. It was differentially diagnosed as sebaceous adenoma and hyperplasia by fine needle aspiration. After surgical removal, the sample was diagnosed as sebaceous hyperplasia by histopathological examination. It has not yet shown any signs of recurrence and prognosis has teen good.
무증상으로 발견된 담낭-담관 누공을 동반한 Mirizzi 증후군 1예

Mirizzi's syndrome is a rare complication of the gallstone disease, characterized by narrowing of the common bile duct (CBD) due to chronic extrinsic compression of an impacted gallstone in the cystic duct or the neck of the gallbladder. The impacted gallstone may erode into the bile duct, causing cholecystocholedochal fistula. The patients generally have history of repeated attacks of jaundice and abdominal pain. But there is no report about asymptomatic Mirizzi syndrome with cholecystocholedochal fistula which were found incidentally. So herein we present our clinical experience with a case of asymptomatic Mirizzi syndrome with cholecystocholedochal fistula in a young female.

생리활성을 지닌 신경펩타이드의 구조와 활성간의 관계를 규명하기 위하여 고상법으로 합성한 세종류의 neuropeptide γ(mammalian-,trout- 그리고 goldfish-neuropeptide γ)를 사용하여 연구하였다. Circular dichroism spectra에 의하면 mammalian-,trout- 와 goldfish-neuropeptide γ는 완충액 조건하에 모두 random한 구조를 나타내었다. 중성 및 산성 지질 존재 하에서, mammalian-과 trout- neuropeptide γ는 여전히 random한 구조를 취하였다. 그러나, goldfish-neuropeptide γ는 중성 및 산성지질하에서 부분적으로 α-helix구조를 나타내었다. 장관 수축활성에 있어서는 carp 장관, guinea-pig 회장 그리고 rat십이지장에 대하여 비교하였다. Carp에 대해서는 goldfish-neuropeptide γ= trout- neuropeptide γ>mammalian-neuropeptide γ순으로 활서이 나타났다. 그러나, guinea-pig 회장과 rat십이지장에 대해서 mammalian-neuropeptide γ는 어류 유래성 neuropeptide g들 보다 높은 수축활성을 나타내었다. 이러한 결과들은 neuropeptide γ들이 종-특이적인 활성을 나타낸다는 것을 제시한다. The relationship between structure and biological activity was studied on the three neuropeptides(mammalian-,trout- and goldfish-neuropeptide γ)that were syntheized by the solid-phase method. Circular dichroism spectra showed that mammalian-,trout- and goldfish-neuropeptide γadopted an undered structure in buffer solution. In the presence od neutral and acidic liposomes, mammalian-and trout- neuropeptide γ also took a random structure. However, goldfish-neuropeptide γ took an α-helical structure in acidic liposomes. The intestinal motility response was investigated with carp interstines, guinea-pig ileums and rat duodenums. In case of carp intestine, contractine activity was as follows:goldfish-neuropeptide γ= trout- neuropeptide γ> mammalian-neuropeptide γ. On the other hand, the contracile activity of mammalian-neuropeptide γ was more potent than trout- and goldfish-neuropeptide γ in the guinea-pig ileums and rat duodenums. these results suggest that neuropeptide γs showed the species-specific activity.
