        정맥주입 전문간호사가 삽입한 말초삽입형 중심정맥관(PICC) 사용 결과에 대한 후향적 분석

        Background : Intravenous(Ⅳ) access is becoming an increasingly important part of health care today. The current drive for clinical effectiveness and cost-effective health care serves to increase the need for reliable vascular access. Venous access devices were developed to overcome problems associated with limited peripheral access and frequent venipuncture in patients with long-term therapy. Although the peripherally inserted central catheter(PICC) have become popular during recent years in USA, its procedure is rare in Korea. Purpose : The goal of this study was to analyze the PICC inserted patient data by Ⅳ CNS intervention. Method : A Total of 62 PICCs were inserted into 51 patients by the Ⅳ CNS during a 10-month period form November, 14, 2002, to October 2, 2W2. Data was obtained retrospectively through chart review. Result : The patient population included 34(54.8%) men and 28(45.2%) women, with a mean age 50.6 years. The main indication for PICC placement was to access vein in poor peripheral venous status(40.3%). The mean served interval for PICC insertions was 16.7 days(range, 2~61 days). The reasons for removal were completed therapy in 18 cases(29.0%), patient death in 13 cases(21.0%), and mechanical or functional PICC problem in 10cases(16.1%). The three PICCs removed for presumed infection, and one had only positive tip cultures(0.2%). Conclusion : PICCs are rapidly growing popularity and required an extended course of Ⅳ therapy.

        <P>Euphorbia maculata (EM) is a traditionally used antidiarrheal, antibacterial, antifungal and antioxidant agent. However, the effects of EM on platelet activity remain to be elucidated. Therefore, the present study investigated the antiplatelet effect of various EM extract fractions on platelet aggregation in rats. The antiplatelet activity of the EM fractions on collagen or adenosine diphosphate (ADP)?induced platelet aggregation was evaluated in?vitro and ex?vivo. Thromboxane?B2 (TXB2) formation, rat?tail bleeding time and coagulation time were also measured. Among the fractions, the chloroform fraction of EM (CFEM) significantly inhibited ADP?induced platelet aggregation in?vitro. Furthermore, oral administration of 50?mg/kg CFEM to rats significantly reduced ADP?induced platelet aggregation without increasing the tail bleeding time or coagulation time. In addition, EM significantly inhibited the level of TXB2 formation in a dose?dependent manner. These results suggest that CFEM exhibits antiplatelet activity, without causing bleeding, via the suppression of TXB2 formation. CFEM may be a type of food which has the potential for preventing cardiovascular disease.</P>

        Antithrombotic activity of Vitis labrusca extract on rat platelet aggregation

        <P>Vitis labrusca is a grapevine that has antioxidant, neuroprotective, hepatoprotective, and anticarcinogenic activity. However, the antithrombotic effect of Vitis labrusca leaves on platelets is yet to be ascertained. We investigated the inhibitory effect of V. labrusca leaf extract (VLE) on platelet aggregation in vitro and ex vivo. The thromboxane B-2 (TXB2) and serotonin concentrations were measured by ELISA. The flavonoids content was measured by ultraperformance liquid chromatography (UPLC). The antithrombotic activity of VLE was evaluated using various agonists in vitro. VLE strongly inhibited adenosine diphosphate (ADP)-induced platelet aggregation. In rats, VLE treatment (100mg/kg) reduced ADP-stimulated platelet aggregation, without affecting tail bleeding and coagulation time. Moreover, VLE significantly suppressed TXB2 and serotonin secretion. UPLC analysis indicated that VLE contains quercetin, isorhamnetin, and rutin. Our results indicate that VLE possesses antiplatelet activity via the suppression of TXB2 and serotonin, without affecting bleeding. Further, we identified the flavonoids present in VLE.         Quantitative and rapid analysis of transglutaminase activity using protein arrays in mammalian cells

        <P>We developed a novel on-chip activity assay using protein arrays for quantitative and rapid analysis of transglutami-nase activity in mammalian cells. Transglutaminases are a family of Ca2+-dependent enzymes involved in cell regulation as well as human diseases such as neurodegenerative disorders, inflammatory diseases and tumor progression. We fabricated the protein arrays by immobilizing N,N'-dimethylcasein (a substrate) on the amine surface of the arrays. We initiated transamidating reaction on the protein arrays and determined the transglutaminase activity by analyzing the fluorescence intensity of biotinylated casein. The on-chip transglutaminase activity assay was proved to be much more sensitive than the [3H]putrescine-incorporation assay. We successfully applied the on-chip assay to a rapid and quantitative analysis of the transgluta-minase activity in all-trans retinoic acid-treated NIH 3T3 and SH-SY5Y cells. In addition, the on-chip transglutaminase activity assay was sufficiently sensitive to determine the transglutaminase activity in eleven mammalian cell lines. Thus, this novel on-chip transglutaminase activity assay was confirmed to be a sensitive and high-throughput approach to investigating the roles of transglutaminase in cellular signaling, and, moreover, it is likely to have a strong potential for monitoring human diseases.</P>

      • Simultaneous Activity Assay of Two Transglutaminase Isozymes, Blood Coagulation Factor XIII and Transglutaminase 2, by Use of Fibrinogen Arrays

        <P>We developed an on-chip activity assay system to simultaneously determine the transamidating activities of blood coagulation factor XIII (FXIII) and transglutaminase 2 (TG2) by use of fibrinogen arrays. FXIII and TG2 are transglutaminase family members that are involved in various physiological functions, including vascular pathophysiology, bone development, and cancer progression. However, investigation of their differential functions is limited by the lack of high-throughput and isozyme-specific activity assays. For the on-chip activity assay, we fabricated protein arrays by immobilizing fibrinogen onto the 3-aminopropyltrimethoxysilane surface of well-type arrays, and we determined transamidating activity by probing biotinylated fibrinogen with Cy3-conjugated streptavidin on arrays. We optimized assay conditions, such as buffer pH, concentrations of dithiothreitol and 5-(biotinamido)pentylamine, and incubation time, and we created equations to determine specific FXIII and TG2 activities in samples. We successfully applied this assay system to monitor changes in FXIII and TG2 activities in THP-1 monocytic cells differentiated with phorbol 12-myristate13-acetate and interleukin-4.         인공호흡기 적용환자에서 폐쇄형 기도흡인 전·후 과환기와 과산소 적용의 효과

        Purpose: The purpose of this study was to identify the effects of hyperventilation and hyperoxygenation before and after endotracheal suctioning using closed-suction system on mechanically ventilated patients. Method: We selected 20 patients who were mechanically ventilated and randomly assigned into 4 groups according to the type of treatments using repeated measure. We applied 3 types of treatment, hyperventilation, hyperoxygenation, hyperventilation+hyperoxygenation, and no treatment and measured physiologic effects-SpO_(2), HR and, mena BP. Result: There were no significant differences between the group with no treatment and the group with hyperventilation, before and after endotracheal suctioning using closed-suction system(SpO_(2)(p=.693), HR(p=.970) and mean BP(p=.638)). There were no significant differences between the group with no treatment and the group with hyperoxygenation before and after endotracheal suctioning using closed-suction system(SpO_(2)(p=.693), HR(p=.970) and mean BP(p=.638)). There were no significant differences between the group with no treatment and the group with hyperventilation hyperoxygenation before and after endotracheal suctioning using closed-suction system(SpO_(2)(p=.693), HR(P=.970) and mean BP(P=.638)). Conclusion: As the results, all of the methods, hyperventilation, hyperoxygenation, and hyperventilation hyperoxygenation can be used when nurses apply endotracheal suctioning using closed-suction systems. In case of hypoxemic patients, hyperoxygenation is recommended for prevention of deoxygenation due to endotracheal suctioning using closed-suction systems.

        Effect of Chitosan Oligosaccharides on Polyamine Metabolism for Chemopreventive Activity

        N/A Salted-fermented Alaska pollack, Theragra chalcogramma, tripe is one of the traditional fishery fermented foods in Korea and has played an important role in Korean dietary life. But, salted-fermented Alaska pollack tripe has about 30 days of shelf-life. In order to extend the shelf-life of salted-fermented Alaska pollack tripe, chitosan and oligochitosan were added and their effect on the property changes of salted Alaska pollack tripe during fermentation were investigated. There was no significant difference in proximate compositions among samples. pH of all samples decreased a little as fermentation Progressed and showed a higher value in salted-fermented Alaska pollack tripe with chitosan and oligochitosan than control. Lactic acid content increased up to 1 month very rapidly and slowly thereafter. The content amino-nitrogen and VBN of salted-fermented Alaska pollack tripe fomented at 5℃ increased as fermentation progressed lower with chitosan and oligochitosan than control after 2 weeks of fermentation.

      • Electromagnetic Wave Absorption Film Incorporated with FeCo Nano-chained Particles

        The wireless communication has revolutionized to meet the consumer demands in communication and mobility where the fifth generation (5G) telecommunications are rapidly developing for reliable data transmission and better connection in users. However, serious health hazards and malfunctioning in devices has become one of the issues when operating at high frequency bands around 26-30 GHz. Notable attempts have been made to solve these problems through recent breakthroughs in electromagnetic wave absorbing materials (EWAMs), composed of dielectric and magnetic materials for eliminating or attenuating electromagnetic waves. From this point of view, FeCo-based soft magnetic alloys is particularly attractive EWAMs due to their performances, including high saturation magnetization (Ms) of 2.45 T, small coercivity (Hc), and complex permeability. In addition, a structural modulation of the soft magnetic materials provides the magnetic anisotropy, resulting in enhancing the limit of ferromagnetic resonance (fmr) to achieve high initial permeability. Here, we prepare the FeCo nanochains using a highly-productive thermal plasma synthesis to be utilized as an electromagnetic absorber with exceptionally low reflection loss in the high frequency bands. The composition of FeCo nanochains ranging from 7:3 to 3:7 shows high saturation magnetization of 151 – 227 emu g<SUP>-1</SUP>. Subsequently, the planetary ball milling is also implemented for a shape modulation in order to enhance the complex permeability and the reflection loss performance. The shape-modulated FeCo nanochains offer the enhancement in both the real and the imaginary part of the complex permeability. The shape modulation is one of the technological advancements in improving the complex permeability of FeCo alloys.
