      • A case of eccrine poroma developed from prolonged existing seborrheic keratosis

        A 56-year-old man visited our dermatologic department with intermittent itching and hemorrhage on skin lesions accompanied with hyperpigmentation of the right buttock that lasted 40 years. The patient did not complain of any medical history, and a skin lesion, which was 4.5 x 3.1 cm sized brownish wart-like plaque, was found and two pink nodules of 0.8 x 0.7 cm and 0.4 x 0.3 cm were above the plaque. The brown-colored plaque was said to have existed about forty years, and pink nodules were formed three years ago. Incision biopsy including both lesions demonstrated eccrine poroma in the seborrheic keratosis. Because both diseases are common, the occurrence of eccrine poroma in seborrheic keratosis cannot be ruled out only by chance. However, it is a tumor originating from the upper part of the ridge of the sweat gland and the ductal cells of the lower part of the sweat gland, and some reports have showed that K5 / K14 expression and local K1 / K10 expression are observed in the uppermost layer of the sweat gland ridge. Also, it has been reported that it can be seen to fuse with basal keratinocytes, which suggests that the eccrine poroma may occur in association with changes from keratinocytes of the epidermis in seborrheic keratosis. So far, there is no report that seborrheic keratosis may be converted to eccrine poroma in South Korea.       • Successful treatment of Hailey-Hailey disease with alitretinoin

        We report a case of 50-year old man with 5-year history of erythema, vesicles and erosions with pustular discharge on the axillae and inguinal area. He had been treated with topical corticosteroid, antibiotics and systemic corticosteroids, but there was no improvement of the lesions. A 4-mm punch biopsy was taken from the lesion on axilla. Histology showed intraepidermal clefts, corp ronds in the granular layer and extensive acantholysis throughout the epidermis giving appearance of a ‘dilapidated brick wall’. From these findings, we diagnosed the patient Hailey-Hailey Disease (HHD). After being treated with alitretinoin 30mg once a day and topical steroid for 3 months, skin lesions much improved. HHD, also known as familial benign chronic pemphigus, is a rare autosomal-dominant blistering skin disease. There are various therapeutic options for HHD, but these are often ineffective or limited. Because alitretinoin is well tolerated and has a shorter period of contraception, we suggest alitretinoin as an additional therapeutic option of refractory HHD.

        Changes in the Contents of Prosapogenin in the Red Ginseng (Panax ginseng) Depending on Steaming Batches

        This study compared the contents of ginsenosides depending on steaming conditions of red ginsengs to provide basic information for developing functional foods using red ginsengs. The red ginseng steamed eight times at 98℃ ranked atop the amounts of prosapogenins ever detected in red ginsengs (ginsenoside Rg₂, Rg₃, Rg?, Rg?, Rh₁, Rh₄, Rk₁, Rk₃, F₁, F₄, 1.15%) among red ginsengs steamed more than twice. When steamed eight times at 98℃, 2.7 times as much prosapogenins such as ginsenosides Rg₂, Rg₃, Rg?, Rg?, Rh₁, Rh₄, Rk₁, Rk₃, F₁, and F₄ as those steamed just once at 98℃ was collected. In addition, the red ginsengs steamed eight times at 98℃ contained more amounting ginsenoside Rg₃ (0.28%) than that in the red ginseng steamed several times at random. Accordingly, it is recommendable that red ginsengs steamed 8 times, which proved to be the optimal steaming condition, be used rather than those steamed 9 times (black ginsengs), in order to develop red ginseng products of high prosapogenin concentration and high functions.

        <P>Tuberculosis (TB) caused by Mycobacterium tuberculosis (Mtb) is one of the most deadly infectious diseases, with approximately two million people dying of TB annually. An effective therapeutic method for activating dendritic cells (DCs) and driving Th1 immune responses would improve host defenses and further the development of a TB vaccine. Given the importance of DC maturation in eliciting protective immunity against TB, we investigated whether Rv0315, a newly identified Mtb antigen, can prompt DC maturation. We found that Rv0315 functionally activated DCs by augmenting the expression of the co-stimulatory molecules CD80 and CD86 as well as MHC class I/II molecules. Moreover, it increased DC secretion of the pro-inflammatory cytokines IL-6, IL-1 beta, and TNF-alpha. Unlike LPS, however, Rv0315 induced the secretion of IL-12p70, but not IL-10. In addition, Rv0315-treated DCs accelerated the proliferation of CD4(+) and CD8(+) splenic T cells from Mtb-infected mice, with increased levels of IFN-gamma, in syngeneic and allogeneic mixed lymphocyte reactions, indicating that Rv0315 contributes to Th1 polarization of the immune response. Importantly, both mitogen-activated protein kinases and nuclear factor kappa B signaling mediated the expression of DC surface markers and cytokines. Taken together, our results indicate that Rv0315 is a novel DC maturation-inducing antigen that drives T cell immune responses toward Th1 polarization, suggesting that Rv0315 plays a key role in determining the nature of the immune response to TB.</P>

        Comparison of the Efficacy of Azithromycin Versus Doxycycline in Acne Vulgaris: A Meta-Analysis of Randomized Controlled Trials

        Background: Acne vulgaris is one of the most common disorders of the pilosebaceous unit. Although doxycycline is considered to be a first-line anti-acne antibiotic, various other antibiotics have been tried due to its adverse effects and contraindications. We performed a meta-analysis of randomized controlled trials (RCTs) that compared the efficacy of oral azithromycin pulse therapy with that of oral daily doxycycline in the management of moderate to severe acne vulgaris. Methods: Five scientific databases (MEDLINE, EMBASE, Cochrane Library, SCOPUS, and Web of Science) were searched to identify relevant studies. A review of 1,341 publications produced six RCTs that met our predefined inclusion criteria. The clinical outcome measures were remaining acne lesion counts, patients’ self-assessment of treatment, and the investigators’ assessment of treatment after 12 weeks. Results: We included six studies assessing 906 patients with moderate to severe acne vulgaris. Meta-analyses of clinical outcome measures revealed no significant difference between the two groups regarding remaining acne lesion counts (p=0.27), patients’ self-assessment of treatment (p=0.67), and the investigators’ assessment of treatment (p=0.32). The incidence of severe adverse events leading to the discontinuation of therapy was higher in the doxycycline daily therapy group when compared with the azithromycin pulse therapy group. Conclusion: This study indicates that azithromycin pulse therapy is equivalent to doxycycline at 12 weeks in the efficacy of the treatment for moderate to severe acne vulgaris Therefore, oral azithromycin pulse therapy may be a good alternative to doxycycline in the management of acne for those unable to tolerate doxycycline.       • The roles of Dock1 and Elmo1 in malignant melanoma

        Background: The dedicator of the cytokinesis I (Dock1) superfamily of proteins has been identified as a novel and unconventional guanine nucleotide exchange factor (GEF) for Rho GTPases. Dock1 requires binding to engulfment and cell motility 1 (ELMO1) to achieve nucleotide exchange on Rac1. Objectives: To assess the roles of Dock1 and Elmo1 in melanoma, we investigated effects of Dock1 and Elmo1 in cancer progression using the melanoma cell line G361. Methods: To examine the effects of Dock1 and Elmo1 in melanoma cell, G361 cell line was used. Cell proliferation, apoptosis and migration of G361 cell line were determined using standardized assays. Changes in Dock1, Elmo1 and other protein were analysed by western blots using siRNA of Dock1 and Elmo1. Results: An increased rate of apoptosis was found in the G361 after transfection with those siRNA. Silencing of both Dock1 and Elmo1 significantly decreased the cell migration of G361 cell line and levels of pERK1/2 and pAKT. In western blot, transfection with Dock1-siRNA downregulated expression of Dock1 and significantly attenuated Elmo1 levels without affecting Rac1 in G361. Transfection with Elmo1-siRNA inhibited Elmo1 and downregulated Rac1 without altering Dock1 in the G361. Conclusion: Our results indicate that Dock1 and Elmo1 are functionally related to the pathophysiology of melanoma, suggesting that those might be potential targets for treatment or prevention of melanoma.

        경골 원위부 골절에서 시행한 골수강내 금속성 고정술

        목 적: 경골 원위부 골절에서 폐쇄성 골수강내 금속정 고정술을 이용하여 치료한 18예를 대상으로 그 결과를 분석하여 이의 유용성에 대해 알아보고자 한다. 대상 및 방법: 1999년 5월부터 2001년 6월까지 경골 원위 골간단-간부 골절에서 골수강내 금속정 내고정술을 시행한 18예를 대상으로 하였다. 골절의 형태는 Robinson 등의 분류에 따라 제1형이 7에, 제2A형이 7예, 제2B형이 1예, 제2C형이 3예이었으며, 가장 원위부의 골절선과 족관절 사이의 길이는 평균 2.6±1.1㎝ (0~5)이었다. 전 환자에서 족관절의 기능, 골유합 시기, 합병증 등을 조사하였다. 결 과: 원위 비골 골절이 동반된 10예 중 7예에서 금속판 및 나사못을 이용하여 고정하였다. 4예에서 골수정 삽입 후 부정정열을 보여 수술 중에 각각 1개의 Poller 나사를 사용하여 변형을 고정하였다. 전 예에서 골유합을 이루었고, 골유합 시기는 평균 20주 (12~40주)였다. Blaird의 방법에 의한 족관절의 평균 기능적 점수는 92.5점이었다. 1예에서 9도의 전굴 변형(antecurvatum)을 보였으나 일상생활에 지장을 초래하지는 않았다. 결 론: 경골 원위부 골절에서 골수강내 금속정 고정술은 수술에 의한 연부 조직의 손상을 최소화하고, 조기 관절 운동을 가능하게 하며, 높은 골유합률과 함께 합병증도 적어 좋은 치료 방법의 하나라고 사료된다. Purpose: This study was undertaken in order to evaluate the efficiency of the closed intramedullary nailing 18 cases of distal tibial fractures. Materials and Methods: From May 1999 to June 2001, eighteen patients were treated by closed intramedullary nailing for distal tibial meta-diaphyseal fracture. According to Robinson classification, there were 7 type 1 fractures, 7 type 2A fractures, 1 type 2B fractures, and 2C type fractures. The mean distance between distal end of fracture and tibial plafond was 2.6±1.1㎝ (0~5㎝). We evaluated both clinical and radiographic parameters. Results: Plate fixation of distal fibular fracture was performed in 7 cases. Poller screw was used in 4 cases which showed malalignment after insertion of nail. The mean score was 92.5 point by Blaird ankle scoring system. All patient got the bone union average of 20 weeks (12~40 weeks). In patient had a antecurvatum deformity of 9 degrees. Conclusion: Intramedullary nailing for distal tibial fracture is one of the safe and reliable method for managing these injuries.
